사이버나이프를 이용한 수술 불가능한 재발성 구강암의 치험례 (CYBERKNIFE RADIOSURGERY FOR INOPERABLE RECURRED ORAL CANCER)

  • CyberKnife is a stereotactic radiosurgery system which could be used to treat many tumors and lesions. It provides the surgeon unparalleled flexibility in targeting using a compact light linear accelerator mounted on a robotic arm. Advanced image guidance technology tracks patient and target position during treatment, ensuring accuracy without the use of an invasive head frame. CyberKnife with Dynamic Tracking Software is cleared to provide radiosurgery for lesions anywhere in the body when radiation treatment is indicated. It has often been used to radiosurgically treat otherwise untreatable tumors and malformations. Moreover, this instrument treats tumors at body sites, most of which are unreachable by other stereotactic systems. Compared with conventional radiotherapy, it is fundamentally different that using non-invasive, frameless, no excessive radiation exposure to normal tissue. In oral malignant neoplasm, surgical excision and radiation therapy should be tried first, additionally chemotherapy could be considered. However, after failure of conventional therapies, patients had poor systemic condition and surgical limitation. So, CyberKnife could be a suitable therapy. A 49 years man was referred in recurred mandibular cancer treated by radiotherapy. The tumor was considered inoperable, because of extensive invasion and was not expected to good response to conventional therapies. We experienced a case of CyberKnife after 4 cycle chemotherapies, so we report it with review of literature.

Dosimetric comparison of volumetric modulated arc therapy with robotic stereotactic radiation therapy in hepatocellular carcinoma

  • Purpose: To compare volumetric modulated arc therapy of RapidArc with robotic stereotactic body radiation therapy (SBRT) of CyberKnife in the planning and delivery of SBRT for hepatocellular carcinoma (HCC) treatment by analyzing dosimetric parameters. Materials and Methods: Two radiation treatment plans were generated for 29 HCC patients, one using Eclipse for the RapidArc plan and the other using Multiplan for the CyberKnife plan. The prescription dose was 60 Gy in 3 fractions. The dosimetric parameters of planning target volume (PTV) coverage and normal tissue sparing in the RapidArc and the CyberKnife plans were analyzed. Results: The conformity index was $1.05{\pm}0.02$ for the CyberKnife plan, and $1.13{\pm}0.10$ for the RapidArc plan. The homogeneity index was $1.23{\pm}0.01$ for the CyberKnife plan, and $1.10{\pm}0.03$ for the RapidArc plan. For the normal liver, there were significant differences between the two plans in the low-dose regions of $V_1$ and $V_3$. The normalized volumes of $V_{60}$ for the normal liver in the RapidArc plan were drastically increased when the mean dose of the PTVs in RapidArc plan is equivalent to the mean dose of the PTVs in the CyberKnife plan. Conclusion: CyberKnife plans show greater dose conformity, especially in small-sized tumors, while RapidArc plans show good dosimetric distribution of low dose sparing in the normal liver and body.

로봇사이버나이프를 이용한 간 종양의 실시간 움직임 측정과 방사선수술 시 호흡추적장치의 정확성 평가 (Evaluation of Real-time Measurement Liver Tumor's Movement and $Synchrony^{TM}$ System's Accuracy of Radiosurgery using a Robot CyberKnife)

  • 목 적: 본 연구에서는 로봇 사이버나이프의 호흡추적장치($Synchrony^{TM}$ Respiratory motion tracking system)을 이용하여 방사선수술을 시행한 간 종양환자를 대상으로 치료 중 실시간 종양의 움직임을 정량적으로 측정하고 방사선 수술시 호흡추적장치의 정확성을 평가하고자 한다. 대상 및 방법: 사이버나이프 치료를 시행한 간 종양 환자 24명을 대상으로 호흡추적 장치를 이용하여 총 64회의 시술을 시행하였다. 모든 환자에서 초음파를 이용하여 간 종양 근처에 $4{\sim}6$개의 금침을 삽입하였고 치료계획용 컴퓨터 단층촬영 영상을 이용하여 치료계획을 세웠다. 매 치료 시 금침의 위치는 치료계획 시 만들어진 디지털 재구성 방사선 영상(Digitally Reconstructed Radiography; DRR)과 실시간으로 촬영되어진 방사선영상(X-ray image)으로 확인하고, 이 결과를 MTS (Motion Tracking System)을 통해 Mtsmain.log 치료파일 형식으로 저장하여 종양의 움직임을 측정하였다 또한 사이버나이프를 이용한 방사선 수술 시 호흡추적장치의 정확성은 실시간 금침의 위치와 미리 예측된 좌표 사이의 상관관계 오차(Correlation Error)로 평가하였다. 결 과: 간 종양의 직선형태 움직임은 SI (Superior-Inferior)방향으로 최대 23.5 mm, 평균 $13.9{\pm}5.5\;mm$, LR (Left-Right)방향으로 최대 3.9 mm, 평균 $1.9{\pm}0.9\;mm$, AP (Anterior-Posterior)방향으로 최대 8.3 mm, 평균 $4.9{\pm}1.9\;mm$였으며 간 종양의 회전 운동 정도는 X (Left-Right)축 회전은 최대 $3.3^{\circ}$, 평균 $2.6{\pm}1.3^{\circ}$, Y (Cranio-Caudal)축회전은 최대 $4.8^{\circ}$, 평균 $2.3{\pm}1.0^{\circ}$, Z (Anterior-Posterior)축 회전은 최대 $3.9^{\circ}$, 평균 $2.8{\pm}1.1^{\circ}$로 측정되었다. 또한 치료의 정확성을 평가하는 상관관계 오차는 평균 $1.1{\pm}0.7\;mm$였다. 결 론: 본 연구에서 방사선 수술 중 간 종양의 실시간 움직임을 정량적으로 확인할 수 있었고 로봇 사이버나이프의 호흡추적 장치를 이용한 방사선 수술의 정확성을 평가할 수 있었다 이를 토대로 간 종양의 방사선 수술이나 일반적인 방사선치료에 있어서 치료용적의 결정과 움직임에 대한 유용한 정보를 제공할 것이라 생각된다.

사이버나이프 $Synchrony^{TM}$ 호흡 추적 장치를 이용한 폐종양 방사선수술의 유용성 평가 (Evaluation on the Usefulness of Lung Tumor Stereotactic Radiosurgery Applying the CyberKnife $Synchrony^{TM}$ Respiratory Tracking System)

  • 본 연구는 사이버나이프 Synchrony 호흡추적 장치를 이용하여 방사선 수술을 시행한 폐종양 환자 48명을 대상으로 전 치료기간 중 종양의 움직임과 방사선수술의 정확성을 평가하였다. 폐종양의 움직임은 종양이나 종양주변에 삽입된 금침의 좌표를 사이버나이프 영상유도 장치로 측정하였으며, 방사선수술의 정확성은 움직임 추적 컴퓨터(MTS)로 계산된 상관관계 오차로 평가하였다. 폐종양의 움직임은 두미방향으로 평균 $2.63{\pm}1.87\;mm$며, 좌우방향 $1.13{\pm}0.71\;mm$, 전후방향 $1.74{\pm}1.16\;mm$의 움직임을 보였으며, 회전 움직임 정도는 X축 $1.66{\pm}1.66^{\circ}$, Y축 $1.20{\pm}0.97^{\circ}$, Z축 $1.18{\pm}0.73^{\circ}$로 측정되었다. 직선 움직임의 벡터 값은 평균 $3.78{\pm}2.00\;mm$값을 나타냈다. 연구 결과에서 두미방향(p<0.001)과 전후방향(p<0.029), 3차원 벡터 값(p<0.002)들은 종양의 위치가 상부보다 하부의 움직임이 크게 나타나 통계적 유의성을 보였다. 사이버나이프 Synchrony 호흡추적 장치를 이용한 폐종양의 방사선 수술시 상관관계 오차는 전체 평균 $0.95{\pm}0.62\;mm$로 매우 정확한 조사로 종양의 움직임을 보상하여 방사선 수술이 이루어졌으며 그 유용성을 확인할 수 있었다.

Stereotactic Radiosurgery with the CyberKnife for Pituitary Adenomas

  • Objective : In recent years, CyberKnife has emerged as an important treatment modality in the management of pituitary adenomas. Treatment results after performing CyberKnife and the complications of this procedure are reviewed. Methods : Twenty-six patients with pituitary adenomas received stereotactic radiosurgery with the CyberKnife (CKRS). The follow-up periods ranged from 7 months to 47 months (mean$\pm$SD : $30{\pm}12.7$ months). The patients consisted of 17 with non-functioning adenomas, 3 with prolactinomas and 6 with acromegaly. The change in the tumor volume, visual acuity, hormonal function, and complications by this therapy were analyzed in each case. Results : The tumor control rate was 92.3%. Hormonal function was improved in all of the 9 (100%) functioning adenomas. Hormonal normalization was observed in 4 of the 9 (44%) patients with a mean duration of 16 months. In two patients (7.6%), visual acuity worsened due to cystic enlargement of the tumor after CKRS. No other complications were observed. Conclusion : CyberKnife is considered safe and effective in selected patients with pituitary adenomas. However, longer follow-up is required for a more complete assessment of late toxicity and treatment efficacy.

Radiosurgery for Recurrent Brain Metastases after Whole-Brain Radiotherapy : Factors Affecting Radiation-Induced Neurological Dysfunction

  • Objective : We retrospectively analyzed survival, local control rate, and incidence of radiation toxicities after radiosurgery for recurrent metastatic brain lesions whose initial metastases were treated with whole-brain radiotherapy. Various radiotherapeutical indices were examined to suggest predictors of radiation-related neurological dysfunction. Methods : In 46 patients, total 100 of recurrent metastases (mean 2.2, ranged 1-10) were treated by CyberKnife radiosurgery at average dose of 23.1 Gy in 1 to 3 fractions. The median prior radiation dose was 32.7 Gy, the median time since radiation was 5.0 months, and the mean tumor volume was $12.4cm^3$. Side effects were expressed in terms of radiation therapy oncology group (RTOG) neurotoxicity criteria. Results : Mass reduction was observed in 30 patients (65%) on MRI. After the salvage treatment, one-year progression-free survival rate was 57% and median survival was 10 months. Age(<60 years) and tumor volume affected survival rate(p=0.03, each). Acute (${\leq}$1 month) toxicity was observed in 22% of patients, subacute and chronic (>6 months) toxicity occurred in 21 %, respectively. Less acute toxicity was observed with small tumors (<$10cm^3$. p=0.03), and less chronic toxicity occurred at lower cumulative doses (<100 Gy, p=0.004). "Radiation toxicity factor" (cumulative dose times tumor volume of <1,000 Gy${\times}cm^3$) was a significant predictor of both acute and chronic CNS toxicities. Conclusion: Salvage CyberKnife radiosurgery is effective for recurrent brain metastases in previously irradiated patients, but careful evaluation is advised in patients with large tumors and high cumulative radiation doses to avoid toxicity.

사이버나이프를 이용한 무고정틀 두개 방사선 수술 중 발생한 환자의 치료 중 움직임 분석 (An Analysis of Intra-Fractional Movement during Image-Guided Frameless Radiosurgery for Brain Tumor Using CyberKnife)

  • 무고정틀 두개 방사선 시술은 두개 고정틀 방식에 비해 환자 친화적이며 다중 분할 치료의 적용이 용이하다는 장점을 가진다. 그러나, 환자의 움직임을 완전히 제어할 수 없기 때문에 치료 중 움직임으로 인해 시술의 정확도가 영향을 받을 수 있다. 본 연구에서는 종양 추적 방식의 무고정틀 방사선 치료를 시행 받은 환자의 실제 치료 기록을 분석하여 무고정틀 방사선 치료 시 발생할 수 있는 치료 중 환자의 움직임을 분석하였다. 사이버나이프(CyberKnife, Accuray Inc, CA)를 이용한 294회의 뇌종양 방사선 수술 기록을 분석하였으며, 이를 토대로 치료시간에 따른 치료 중 움직임의 크기를 통계적으로 제시하였다. 본 연구의 결과는 무고정틀 방사선 수술 시 고려되어야 할 치료 중 움직임에 대한 기본 지표로 활용될 수 있을 것으로 생각된다.
