Measuring Thresholds of Serum Lipid Level related to Hypertension and Age Using Receiver Operating Characteristic Curve in Korean Adult

  • PURPOSE: The purpose of this study was to determine the optimal thresholds of serum lipid level related to hypertension according to age in Korean adults. METHODS: In total, 564 adults who visited the health examine center in general hospital were included. The blood pressure and lipid profiles of low density lipoprotein cholesterol(LDL), high density lipoprotein cholesterol (HDL), triglyceride(TG) were measured. Receiver operating characteristic (ROC) curve were used to establish optimal thresholds between blood pressure and lipid profiles. RESULTS: The optimal TG cutoff value were 110.50(mg/dl) in the 18-39 and 81.50(mg/dl) in the 40-59 age group, and optimal cutoff value of LDL were 126.50(mg/dl) in the 40-59 age group and 111.00(mg/dl) in the 60 and over age group. There was a negative correlation between HDL and hypertension, a higher HDL decreased hypertension. The optimal cutoff value of HDL was 49.50(mg/dl) in the 18-39 age group. CONCLUSION: The thresholds of hypertension were lower in LDL and TG with aging. This result indicated that elderly people needs to concern more about their lipid profiles to maintain healthy cardiovascular function.

Comparison of Predictive Value of Obesity and Lipid Related Variables for Metabolic Syndrome and Insulin Resistance in Obese Adults

  • In this study, obese adults were compared for their ability to predict obesity and lipid related variables and their optimal cutoff values to predict metabolic syndrome and insulin resistance. In this study, 9,256 adults aged 20 years or older and less than 80 years old, who were in the Gyeonggi region from January 2014 to December 2016 and who were examined at a general hospital, were enrolled. The diagnostic criteria for obesity were WHO (World Health Organization), and BMI $25kg/m^2$ or more presented in the Asia-Pacific region. Metabolic syndrome was diagnosed based on the criteria of American Heart Association / National Heart, Lung, and Blood Institute (AHA / NHLBI). According to the results of receiver operating characteristic curve (ROC) analysis, Triglyceride / HDL-cholesterol (TG / HDL-C), Triglyceride and Glucose (TyG) index, lipid accumulation product (LAP) and visceral adiposity index (VAI) showed high predictive power for diagnosing metabolic syndrome. The diagnostic accuracy of LAP (AUC: 0.854) for males and VAI (0.888) for females was the highest. The optimal cutoff value of LAP was 42.71 for male and 35.44 for female, and the cutoff value of VAI was 1.92 for male and 2.15 for female. In addition, WHtR (waist to height ratio), TyG index, and LAP were used as predictors of insulin resistance in obese adults. Therefore, LAP and VAI were superior to other indicators in predicting metabolic syndrome in obese adults.

A Study on the Reliability and Validity of the Five-Visceral Weak Children Questionnaire for Children with Anorexia (식욕부진 소아 대상 오장허약아 설문지의 신뢰도 및 타당도 연구)

  • Objectives This study aimed to assess the reliability and validity of the Five-Visceral Weak Children Questionnaire (FWCQ) in childhood anorexia and to propose optimal cutoff values. Methods We conducted two surveys, spaced one month apart, targeting the parents of 366 children aged 2-9 years who had been diagnosed with anorexia for at least one month. To evaluate the reliability of the FWCQ, we calculated Cronbach's alpha coefficient and employed the test-retest method. Additionally, correlation analysis was performed between the FWCQ and each visceral question, and discriminant validity was assessed by comparing responses from the anorexic and normal groups. Furthermore, we determined the cutoff value of the spleen weak score, which is closely associated with appetite, for the anorexic group. Results The first survey yielded responses from 336 participants, followed by 171 responses in the second survey conducted one month later. The FWCQ demonstrated high internal consistency (Cronbach's alpha = 0.776), and test-retest results were consistent. The correlation coefficients between the FWCQ and each visceral question were all statistically significant. Moreover, significant discriminant validity was observed between the anorexic and normal groups in the FWCQ score and the heart, spleen, and kidney weak scores. The optimal cutoff value for the spleen weak score in the anorexic group was found to be 5.5 points (with a sensitivity of 73.44% and specificity of 66.47%). Conclusions The FWCQ exhibited strong reliability and validity in children with anorexia.

The Cutoff Value of HbA1c in Predicting Diabetes and Impaired Fasting Glucose (당뇨병 및 공복혈당장애 예측을 위한 당화혈색소 값)

  • There have been many studies to develop methods for predicting diabetes and to prevent diabetes. The validity of glycated hemoglobin (HbA1c), one of the commonly known tools in predicting diabetes, has been verified by many previous studies. In this study, we examined the cutoff value of HbA1c for diabetes and impaired fasting glucose (IFG). Based on this study, we proposed a proper clinical guideline and evaluated the validation of the guideline. Excluding those without blood glucose and HbA1c data, we used the data of 5,161 subjects (2,281 men and 2,880 women) over the age of 20 years from the 2015 Korean National Health and Nutrition Examination Survey. The correlation efficient of fasting plasma glucose (FPG) and HbA1c was 0.79, indicating a strong relationship. Howeve, the correlation efficient of FPG and HbA1c was low, showing 0.27 in non-diabetes, 0.39 in IFG, and 0.66 in diabetes, showing a strong relationship. The cutoff value of HbA1c for predicting diabetes using ROC curve was 6.05% (sensitivity 84.6%, and specificity 92.0%), and AUC was 0.941 (0.937 in men, and 0.946 in women). The cutoff value of HbA1c for predicting IFG using ROC curve was 5.55% (sensitivity 64.5%, and specificity 70.0%), and AUC was 0.733 (0.708 in men, and 0.764 in women). Therefore, it may not be appropriate to apply the guidelines for diagnosing IFG since sensitivity and specificity were below 70%. For future studies retarding the cutoff value of HbA1c in predicting IFG, high sensitivity and specificity are expected if we segment the reference range of IFG.

  • Optimum screening procedures using prior information are presented. An optimal cutoff value on the screening variable X minimizing the expected total cost is obtained for the normal model; it is assumed that a continuous screening variable X given a dichotomous performance variable T is normally distributed and that costs are incurred by screening inspection and misclassification errors. Methods for finding optimal cutoff values based on the prior distributions for unknown parameters are presented.

Forced Resonant Type Cutoff Cavity-Backed Aperture Antennas Loaded with a Single External Reactance

  • This paper presents the basic characteristics of a cutoff cavity-backed aperture antenna with a feed post and a parasitic post inserted parallel to the aperture. It is shown that this type of antenna forcibly resonates the cutoff cavity by adding a single external reactance to the parasitic post. The Galerkin's method of moments is used to analyze integral equations for the unknown electric current on each post and the aperture electric field on the aperture. The value of an external reactance for forced resonance is analytically obtained by deriving a determining equation. Also the current distribution on each post, aperture electric field distributions, and the radiation patterns are discussed. The theoretical analysis is verified by the measured return loss and radiation patterns.

  • Singular Value Decomposition of 2-D Finite Impulse Response was used to design a variable cutoff 2-D FIR filters. Basically this approach allows to use 1-D techniques while dealing with 2-D filters. In this paper, our aim is to provide away of easily tuning 2-D FIR filters. For example, one would like to adjust interactively the cutoff frequency of a 2-D Circular Symmetric low-pass filter on an image processing system. The decomposition of 2-D FIR filters along with techniques available for tuning 1-D FIR filters allows such adjustment. Several design examples are shown.

Usefulness of serum procalcitonin test for the diagnosis of upper urinary tract infection in children (소아 상부 요로감염의 진단을 위한 혈청 procalcitonin 검사의 유용성)

  • Purpose : It is difficult to make a distinction between lower urinary tract infection(UTI) and acute pyelonephritis(APN) during the acute phase of febrile UTI due to nonspecific clinical symptoms and laboratory findings, especially among young children. We measured the serum procalcitonin(PCT) in children with UTI to distinguish between acute pyelonephritis and lower UTI, and to determine the accuracy of PCT measurement compared with other inflammatory markers. Methods : Serum samples were taken from children who admitted with unexplained fever or were suspected of having UTI. 51 children(mean $12.2{\pm}11.4$ months) were enrolled in this study. Leukocyte counts, erythrocyte sedimentation rates(ESR) and C-reactive protein(CRP) were also measured. Renal parenchymal involvement was assessed by $^{99m}Tc$ DMSA scintigraphy in the first 7 days after admission. PCT was measured by immunoluminometric assay. Results : PCT values were significantly correlated with the presence of renal defects in children with UTI(n=16)($5.06{\pm}12.97{\mu}g/L$, P<0.05). However, PCT values were not significantly different between children with UTI without renal damage(n=18) and children without UTI(n=17). Using a cutoff of $0.5{\mu}g/L$ for PCT and 20 mm/hr for ESR, 20 mg/L for CRP, sensitivity and specificity in distinguishing between UTI with and without renal involvement were 81.3 percent and 88.9 percent for PCT 87.5 percent and 72.2 percent for ESR, and 87.5 percent and 55.6 percent for CRP, respectively. Positive and negative predictive values were 86.7 percent and 84.2 percent for PCT and 60.9 percent and 81.8 percent for CRP, respectively. Conclusion : In febrile UTI, PCT values were more specific than CRP, ESR and leukocyte count for the identification of patients who might develop renal defects.

Diagnostic value of eosinopenia and neutrophil to lymphocyte ratio on early onset neonatal sepsis

  • Purpose: To determine the diagnostic value of eosinopenia and the neutrophil-to-lymphocyte ratio (NLR) in the diagnosis of early onset neonatal sepsis (EONS). Methods: This cross-sectional study was conducted in the Neonatology Ward of R.D. Kandou General Hospital Manado between July and October 2017. Samples were obtained from all neonates meeting the inclusion criteria for EONS. Data were encoded using logistic regression analysis, the point-biserial correlation coefficient, chi-square test, and receiver operating characteristic curve analysis, with a P value <0.05 considered significant. Results: Of 120 neonates who met the inclusion criteria, 73 (60.8%) were males and 47 (39.2%) were females. Ninety (75%) were included in the sepsis group and 30 (25%) in the nonsepsis group. The mean eosinophil count in EONS and non-EONS groups was $169.8{\pm}197.1cells/mm^3$ and $405.7{\pm}288.9cells/mm^3$, respectively, with statistically significant difference (P<0.001). The diagnostic value of eosinopenia in the EONS group (cutoff point: $140cells/mm^3$) showed 60.0% sensitivity and 90.0% specificity. The mean NLR in EONS and non-EONS groups was $2.82{\pm}2.29$ and $0.82{\pm}0.32$, respectively, with statistically significant difference (P<0.001). The diagnostic value of NLR in the EONS group (cutoff point, 1.24) showed 83.3% sensitivity and 93.3% specificity. Conclusion: Eosinopenia has high specificity as a diagnostic marker for EONS and an increased NLR has high sensitivity and specificity as a diagnostic marker for EONS.

A case study on the selection process of cutoff wall for ground-water using VE/LCC analysis (VE/LCC 기법을 활용한 차수공법 선정사례 연구)

  • This study shows decision-making process for selection of cutoff wall on a wastewater treatment project. There are 10 different cut-off wall methods So, we examine the site to gather information for find appropriate methods. After using that information, 10 cutoff wall methods are reviewed for analysis. Through brainstorming, four alternatives are selected for design VE item. Following the standard VE process, we established performance criteria and evaluated function score(F) using questionnaire. The questionnaires, brainstorming and AHP method for weighting on performance criteria and evaluate function score increased the reliability of this selection process. Water Jet method, one of four methods, has the best function score(F=92.71) and the lease construction cost(as cost index 1,000). The value score also highest as 92.7, so we select the method. The result is value innovation type In addition, the authors try to calculate the environmental burden in selection process using LCA. We cannot conduct the full LCA as defined ISO, so perform Simple LCA In LCA result, the cut-off grouting has the least environmental burden as index 9.09E+01 and Water Jet method has following as the second. To selection best method to specific area and purpose, design VE/LCG process used as useful tool and it is needed to develop integrated method that evaluate VEILCC and LCA as one-set process.
