The Effect of CYP Polymorphism on Resistance against Praziquantel in Clonorchis Sinensis-infected Patients

  • Currently praziquantel is used for treatment of not only clonorchiasis but also other trematodes and cestodes. But cure rate of praziquantel is just 60-80% for most trematodes. It needs for the treatment-failed patients to have more drugs. The cause of failure of treatment is not studied. We just know that the blood level of praziquantel is severely different among the people. We guess that this factor may influence the results of treatment. In an endemic area of human clonorchiasis in Heilongjiang Providence, China, 78 subjects were selected for the study. Three doses of 25 mg/kg (total 75 mg/kg) of praziquantel were administered to 78 clonorchiasis patients. After three weeks of treatment, stool examination was undertaken once again to confirm the cured and uncured subjects. To analyze SNP (single nucleotide polymorphism) of CYP3A5 PS2-1, CYP3A5 PS2-2, and CYP3A5*6, PCR method was done with specifically designed primers. The mutation rates of all sites were not significant statistically. The number of subjects was too small, so we need more subjects and other delivery proteins of bile ducts (ex. MRP etc.) were also considered for effects of praziquantel. We analyzed, for the first time, the entire CYP3A5 gene in a French population, using a polymerase chain reaction- single strand conformational polymorphism (PCR-SSCP) strategy.

A Randomized Active Controlled Clinical Trial to Evaluate Safety and Efficacy of a Topical Unani Formulation Marham Kharish Jadeed in the Management of Qūbā (Tinea Corporis)

  • Introduction: Qūbā (Tinea Corporis) is a very common disease widely prevalent worldwide. 20 - 25 % individuals suffer for this stubborn disease. Unani System of Medicine offers its treatment. There are many pharmacopoeial formulations indicated for various types of dermatophytic infections. In this study clinical efficacy and safety of the topical Unani formulation Marham Kharish Jadeed (a compound drug in the dosage form of an ointment) was assessed and compared with a standard conventional medicine. Materials and methods: A clinical study was conducted on 60 participants of qūbā randomized into test and control groups (n=30 in each group). The participants were clinically diagnosed and confirmed by microscopy of skin scrapings. The efficacy of the Unani formulation was assessed in terms of TSS score and elimination of fungal elements from the skin lesions. The data collected were analyzed statistically. Results and discussion: The study showed that the Unani formulation had comparatively better efficacy clinically than conventional medicine Terbinafine hydrochloride 1% cream in terms of reduction of itching, erythema, scaling, peripheral raised margins of the lesion comparing to baseline. In this study, 27 participants in test group and 18 participants in control group were completely cured (≥75% reduction in TSS Score with Mycological Cure) after 4 weeks of treatment. The efficacy of the Unani formulation was found significant statistically. The individual drugs of the formulations having analgesic (Musakkin), blood purifier (Muṣaffi-i-Dam), demulcent (Mulaṭṭif), antifungal (Qātil-i-fafūndῑ), detergent (Jālῑ), refrigerant (Mubarrid) and antiseptic (Dāfi'-i-'Ufūnat) properties might be responsible for the efficacy of Unani formulation. Conclusion: The findings of the study suggested that the Unani formulation was found effective and safe in the management of qūbā. No local and systemic adverse effect was reported during the study.

  • The incidence of inflammatory bowel disease(IBD) has increased continuously in worldwide, but no cure have been discovered. The etiology of IBD may include various factors such as genetics, epigenetic, environment as well as host immune system. Among the environmental factors of IBD, diet heavily influences gut health, especially non-digestible dietary fiber can have a great impact on selective growth of beneficial gut microbiota called probiotics. Seaweeds have been consumed in Asia countries and are a rich source for dietary fiber. Accumulated data have suggested the possibility of utilizing seaweed derived oligosaccharides as prebiotics to prevent IBD and its recurrence. In this review, seaweed derived oligosaccharides such as fucoidan and laminarin regarding gut health and potential therapeutic tools for IBD will be discussed based on studies conducted in vitro and in vivo models.

  • Plants produce a wide range of active principles, making them a rich source of different types of medicines. Without any knowledge of the phytoconstituents or active principles the medicinal plants itself or in the form of polyherbal formulations, were used by all society of human being from ancient times for prevention and cure of disease, but most of the traditional formulations including the formulation of Ayurveda and Unani have not been scientifically validated in order to establish the pharmacopoeial standards to improve the efficacy. Globally, the people become conscious that uses of synthetic drugs for a long period is not safe; the trend of medical society at large is looking at alternatives from natural, safe sources to combat diseases. Due to this comprehension, it has been increased the demand for plant-derived medicine, and on the other side, it is extremely important to standardize the polyherbal formulations and validate them scientifically to improve their safety and efficacy. The polyherbal Unani formulation Safuf-e-Muallif is widely used and recommended in Unani system of medicine (USM) as a spermatogenic agent, semen thickening agent, etc. to treat sexual disorders viz. premature ejaculation, nocturnal emission, etc. The study mainly deals with phytochemical screening for the detection of nature of phytoconstituents and analytical technique like High-performance thin-layer chromatography (HPTLC) for developing fingerprint of Safuf-e-Muallif revealing specific identities of the drug. The phytochemical screening and HPTLC fingerprint profile for SM reported here may be used as a reference standard for quality control purpose in future.

General Survey on the Necessity of Establishment School of Oriental Medicine at the National University Level (국립한의대 설치 필요성에 대한 일반인의 인식도 조사)

  • Subjects in this study comprised of general public (907), high school students (772), individuals associated with Oriental medicine (660), and 60 majoring in western medicine, totalling 2,413 individuals. Survey was conducted on the necessity of establishing Oriental medicine school at the national university level and the following result were obtained: - 78.3% (1847 individuals) were in favor of establishing Oriental medicine school at the national university level. - For the validity of establishment, responders expressed opinions of standard and virtuous education, higher quality education, standardized practice, research on difficult to cure diseases, and obtaining competitiveness in the world market. - One to three schools were considered as an appropriate number of schools with less than 80 students per class, Class size may be adjusted from existing schools (52.5% favored decrease in size) and (46.3% favored increase in size). - Educational and research facilities must be accopanied with schools of Oriental medicine as well as clinical training facilities, herbal pharmaceutical research centers, and fundamental medical centers. - Many favored 6 year curriculum as the most appropriate system and the school of Oriental medicine should be established within the university. Based on the information gathered in this survey, we may recognize the limitations of Oriental medicine schools at the private institutional level and support the establishment of Oriental medicine schools at th national university level. This establishment may play as a steeping stone for advancement in education, standardization of research and treatment, and commercialization of Oriental medicine of benefit the general public.

Design Analysis/Manufacturing /Performance Evaluation of Curved Unsymmetrical Piezoelectric Composite Actuator LIPCA (곡면형 비대칭 압전복합재료 작동기 LIPCA의 설계해석/제작/성능평가)

  • This paper is concerned with design, manufacturing and performance test of LIPCA ( Lightweight Piezo- composite Curved Actuator) using a top carbon fiber composite layer with near -zero CTE(coefficient of thermal expansion), a middle PZT ceramic wafer and a bottom glass/epoxy layer with high CTE. The main point of this design is to replace the heavy metal layers of THUNDER by thigh tweight fiber reinforced plastic layers without losing capabilities to generate high force and large displacement. It is possible to save weight up to about 30% if we replace the metallic backing material by the light fiber composite layer. We can also have design flexibility by selecting the fiber direction and the size of prepreg layers. In addition to the lightweight advantage and design flexibility, the proposed device can be manufactured without adhesive layers when we use epoxy resin prepreg system. Glass/epoxy prepregs, a ceramic wafer with electrode surfaces, and a graphite/epoxy prepreg were simply stacked and cured at an elevated temperature (177 $^{circ}C$ after following an autoclave bagging process. It was found that the manufactured composite laminate device had a sufficient curvature after detached from a flat mold. The analysis method of the cure curvature of LIPCA using the classical lamination theory is presented. The predicted curvatures are fairly in agreement with the experimental ones. In order to investigate the merits of LIPCA, a performance test of both LIPCA and THUNDE$^{TM}$ were conducted under the same boundary conditions. From the experimental actuation tests, it was observed that the developed actuator could generate larger actuation displacement than THUNDERT$^{TM}$.

Effect of Sopung-san on Contact Hypersensitivity induced by Repeat Elicitation of DNCB (소풍산물추출물이 DNCB로 유발된 접촉성피부염에 미치는 영향)

  • Sopung-san(SPS; prescription No. DO27), a herbal medicine prescribed by oriental hospital of Daegu fanny University, has been used as cure for pruritus and psoriasis. To evaluate anti-inflammatory effect of Sopung-san, we treated Sopung-san extract in animal model system induced contact hypersensitivity. Contact hypersensitivity, a local inflammatory response of the skin, was induced by 1% DNCB on the right ear of BALB/C mouse. Sopung-san was prepared as water extract and administrated everyday for 2 weeks per oral. A right ear of mouse was potently swelled by 1% of DNCB treatment, but a mouse ear thickness was significantly reduced by Sopung-san after 2 weeks. treatment. Sopung-san reduced IgG in serum obtained from blood of 1 % DNCB-treated mouse. IgE in serum was not changed by DNCB and/or Sopung-san treatment. $IL-1{\beta}$ and IL-6 from ear tissue were significantly increased by DNCB, Dut were not reduced by Sopung-san. From these results, anti-inflammatory effect of Sopung-san, especially reduction of ear swelling, might be partly due to reduction of IgG in serum.

The Perception for Management of School Foodservice Using of Environmentally Friendly Agricultural Products of Elementary School Children's Mothers in Gunsan (군산지역 초등학생 어머니의 학교급식 운영과 친환경농산물 사용에 대한 인식)

  • The purpose of this study was to compare the degree of understanding about school foodservice management and environmentally friendly agricultural products between urban and rural elementary school children's mothers. The subjects were 280 elementary school children's mothers who lived in Gunsan city and the nearby countryside. The results are summarized as follows. The mothers in urban schools were higher on the degree of participation (p < 0.01) and interest (p < 0.01), but were lower on the degree of satisfaction (p < 0.001) for school foodservice than rural ones. The best improvement of school foodservice management was improving food tastes and qualities of the foodstuffs in urban schools and sanitation at the service area in the rural schools (p < 0.001). The school foodservice program contributed to cure the unbalanced diets and developing of bodies and minds; there was no difference of urban and rural schools. But eating habits in rural schools were more improved than urban schools (p < 0.001). The primary reason for using environmentally friendly agricultural products was to improve their health and in securing safe foods, there was no difference of urban and rural schools, but generating the farmer's income from the products in rural schools was higher than urban schools (p < 0.001). There are conflicting views between urban and rural schools for the additional costs brought by using the environmentally friendly agricultural products (p < 0.001). The order of preference on using environmentally friendly agricultural products was rice and various grains, vegetables, fruits, livestock, seasoning, etc. In conclusion, our central and local governments should change their roles in financially positive ways and reflect the issues in making the policy effective. Responsible administrators of school food suppliers run the system more faithfully with the above government support.

Comparison between C.M.R.T. and acupuncture in the treatment of liver dysfunction (간 기능 이상 치료에 대한 C.M.R.T. 치료 부위(T8 횡돌기)와 경혈과의 비교)

  • Chiropractic is very similar to Oriental Medicine in philosophy on the cause of diseases and in utilization of spinal articulations for diagnosis and treatment. In this paper the spinal area used to treat liver dysfunction in S.O.T. technique, one of chiropractic techniques, was compared to the acupncture points used to cure the same conditions. Because both Oriental medicine and Chiropractic are dealing with autonomic nervous system in regulating abnormal conditions, also the innervation of spinal nerves to those areas was checked. The spinal area that S.O.T. technique utilizes to correct liver dysfunction is transverse processes of T8, which corresponds to B16. Acupncture points from this level down to T12/L1, which are B16, B17, B18, B19, B20, B21, B45, B46, B47, B48, B49, B50, GV6, GV7, GV8 and GV9, all have been applied to control liver function. Apparent discrepency exists in therapeutic areas for liver malfunction between the two natural healing arts. According to the neurology texts, liver is innervated by sympathetic fibers from the 7th-10th thoracic segments and by parasympathetic fibers from vagus nerve. Sympathetic afferent nerves from the liver reach the 7th-12th thoracic spinal cord segments. It can be said all the 7th-12th thoracic spinal cord segments are related to liver function. Therefore the areas used for liver dysfunction in both natural medicine are appropriately selected. However, B16, the Oriental medical equivalent of the main spinal area which is used for lowered liver function in C.M.R.T. Technique, is not utilized as frequent as in Oriental medicine.

A Novel Simple Method to Purify Recombinant Soluble Human Complement Receptor Type 1 (sCR 1) from CHO Cell Culture

  • The human complement receptor type 1 (CR 1, C3 b/C4b receptor) is a polymorphic membrane glycoprotein expressed on human erythrocytes, peripheral leukocytes, plasma and renal glomerular podocytes, which consists of transmembrane and cytoplasmic domains with 30 repeating homologous protein domains known as short consensus repeats (SCR). CR1 has been used as an inhibitor for inflammatory and immune system for the past several years. Recently; it is reported that CRl was found to suppress the hyper-acute rejection in xeno-transplantation and can be used to cure autoimmune diseases. A soluble form of CRl, called sCRl, is a recombinant CRl by cleaving the transmembrane domain at C-terminus and has been expressed in Chinese Hamster Ovary (CHO) cells. Several purification methods for sCR1 from CHO cells have been reported, but most of them require complicated steps at high cost. Moreover, such methods are mostly performed under the pH condition apt to denaturing sCR1 and causes sCRl losing its activity. We here report a rapid and efficient method to purify sCR1 from CHO cell. The new method consists of a two-stage of cell culture by cultivating cells in serum medium followed by serum-free medium, and a two-stage of column purification by means of heparin and gel filtration column chromatography. By using this novel method, sCR1 can be purified in a simple and effective way with high yield and purity, furthermore, the purified sCR1 was confirmed to retain its activity to suppress the complement activation in vivo and ex vivo.
