Acquired tracheal dilatation is a state of abnormal tracheal dilatation developing from various causes. Tracheomalacia and tracheal dilatation can develop in respiratory distress patients with prolonged endotracheal intubation with assisted positive-pressure ventilation due to positive airway pressure and high cuff pressure. The authors have recently experienced one case of respiratory failure, cardiac arrest, and whole body emphysema after tracheostomy and portex tube insertion were performed to patient with the endotracheal intubation with assisted positive-pressure ventilation for two weeks in the septic shock resulted from colon perforation, who developed tracheal dilatation. We summarize diagnostic and therapeutic strategies of acquired tracheal dilatation for the prevention of emergency status and the management for that patients.
Purpose: The purpose of this study was to investigate the effect of strength training combined with blood flow restriction on leg muscle thickness in children with cerebral palsy. Methods: Nineteen children with cerebral palsy, aged between five and 10 years of age, living in area N, were recruited. Ten participants were classified into a blood flow restriction group and nine into a strength exercise group. The experimental group performed strength training using a blood flow restriction cuff on the leg, and the control group performed strength training without blood flow restriction. A paired t-test was performed to confirm intragroup changes before and after five weeks of the experiment, and an independent t-test was performed to confirm intergroup changes, and the significance level was α=0.05. Results: The rectus femoris, gastrocnemius and gluteus medius muscles showed significant differences in the groups after five weeks (p<0.05). There was a significant difference between the groups in the rectus femoris and gastrocnemius after five weeks (p<0.05). Conclusion: As a result of this study, it was found that strength training combined with blood flow restriction had a positive effect on the changes in leg muscle thickness in children with cerebral palsy. This suggests the possibility of using it in the future as basic data for strength training methods and blood flow restriction exercises for children with cerebral palsy.
This study is to report the effectivenes of Meridian-Test (M-test) in 2 cases on shoulder diseases. The M-test was applied to one patient diagnosed with as impingement syndrome and one patient diagnosed with tear of supraspinatus tendon. The changes in numeral rating scale and range of motion were identified to assess the progress of the treatment. After three times treatments, In example 1, the pain decreased from 6 to 0.5 and the range of flexion and abduction increased from $150^{\circ}$ to $180^{\circ}$. In example 2, the pain decreased from 6.5 to 1, and the range of shoulder flexion increased from $120^{\circ}$ to $170^{\circ}$, while the range of abduction increased from $90^{\circ}$ to $170^{\circ}$. Based on the above results, it would be effective to apply M-test to treat shoulder diseases in these two cases.

Objectives: The purpose of this study was to identify the distribution of diagnostic disease among applicants for shoulder musculoskeletal disease. Methods: In 2020, 47 diagnostic disease applicants were investigated for sociodemographic, health, work, job, and diagnostic disease characteristics. The data were corrected through on-site visits and analyzed using descriptive statistics with SPSS WIN23.0. Results: Most of the applicants were male and elderly. They had high blood pressure(38.3%) and diabetes (21.3%), and the drinking rate and smoking rate were also high. The most common type of employment was daily workers, and it was confirmed that the working condition was poor due to excessive working hours and short rest times. Most of the applicants for shoulder diagnostic diseases were in the construction industry, and the most common diagnostic disease was a rotator cuff tear. Conclusions: It is necessary to develop a musculoskeletal disease prevention program suitable for construction workers to reduce their work-related disease. When establishing a program, business type, task, and diagnostic disease must be considered.

The purpose of this study is to investigate the actual status of antithrombotic management before and after the procedure or surgery, the difference between the duration of medication suspension by clinical and demographic characteristics, and the patient's understanding and satisfaction after medication management by a dedicated nurse. The results were as follows. The most commonly used antithrombotic agents were aspirin and flavitol. The drug discontinuation period according to antithrombotic, procedures, and underlying diseases, there was a significant difference in duration for each variables(p<.000). In the case of antiplatelet drugs, 5-day suspension was the most frequent, and anticoagulants 2-day suspension was the most frequent. Depending on the procedure,colonoscope,nucleoplasty,rotator cuff repair,and total knee arthroplasty commonly showed more than 80% of 5-day discontinuation. The differences according to underlying diseases are as follows. 64.7% of all diseases discontinued on the 5th. The patient's understanding of the nurse's medication management performed before and after the procedure was found to be lower in Angina patients than those with other diseases. In terms of age, those in their 50s showed higher understanding than other age groups. There were no differences in understanding and satisfaction with the remaining characteristics.

Isolated deltoid paralysis is a rare pathology that can occur after axillary nerve injury due to shoulder trauma or infection. This condition leads to loss of deltoid function that can cause glenohumeral instability and inferior subluxation, resulting in rotator cuff muscle fatigue and pain. To establish dynamic glenohumeral stability, a novel technique was invented. Humeral suspension is achieved using a double button implant with non-resorbable high strength cords between the acromion and humeral head. This novel technique was used in two patients with isolated deltoid paralysis due to axillary nerve injury. The results indicate that the humeral suspension technique is a method that supports centralizing the humeral head and simultaneously dynamically stabilizes the glenohumeral joint. This approach yielded high patient satisfaction and reduced pain. Glenohumeral alignment was improved and remained intact 5 years postoperative. The humeral suspension technique is a promising surgical method for subluxated glenohumeral joint instability due to isolated deltoid paralysis.
The objective of this study is to report the improved case of Korean medicine treatment with Aconitum ciliare decaisne pharmacopuncture for supraspinatus tendon full-thickness tear. We used Aconitum ciliare Decaisne pharmacopuncture with other Korean medicine treatments (acupuncture, electroacupuncture, and cupping) for this patient. This case is measured and assessed by the numerical rating scale (NRS), shoulder range of motion (ROM), shoulder pain and disability index (SPADI) and shoulder physical examination. As the result, we found the improvement of NRS, SPADI, ROM, and shoulder physical examination. This study suggested that Korean medicine treatment with Aconitum ciliare Decaisne pharmacopuncture is proved to be helpful to relieve pain and recover function for supraspinatus tendon full-thickness tear. But further clinical studies are needed to clarify the effect of Aconitum ciliare Decaisne pharmacopuncture therapy for patients with supraspinatus tendon full-thickness tear.
Background: The Friends and Family Test (FFT) developed by the UK National Health Service evaluates whether patients are satisfied with a service provided, where improvements are needed, and how likely patients are to recommend the intervention. Calculated from the FFT, the Net Promoter Score (NPS) creates a recommendation metric for treatment. The primary aim of this prospective study is to evaluate NPS for arthroscopic subacromial decompression (ASD) and rotator cuff repair (RCR). Secondary aims are to postoperatively evaluate 1-year changes in patients' Oxford Shoulder Scores (OSSs) in terms of the proportion of patients satisfied with their surgery and correlation with FFT. Methods: During a 2-year period, all patients undergoing ASD or RCR completed questionnaires prospectively. Collected preoperatively and postoperatively at 1 year. Results: NPSs were 31 for ASD (n=32) and 52 for RCR (n=39). OSSs increased by 4.3 and 6.9 for ASD and RCR, respectively (P<0.001). Overall, 75% of ASD and 77% of RCR patients were either "satisfied" or "very satisfied," respectively, with procedure outcomes. Scores from FFT had a positive correlation with improvement in OSS and satisfaction scores among patients undergoing arthroscopic shoulder surgeries (P<0.001). Conclusions: The current study shows positive NPS outcomes in patients with ASD and RCR. Scores from FFT correlate well with both satisfaction and OSS among patients. NPS can be an adjunct to traditional patient-reported outcome measures to provide global evaluation of patient experiences to aid in determining the clinical value of common procedures in shoulder orthopaedics. Background: Muscular forces drive proximal humeral fracture deformity, yet it is unknown if arm position can help mitigate such forces. Our hypothesis was that glenohumeral abduction and humeral internal rotation decrease the pull of the supraspinatus and subscapularis muscles, minimizing varus fracture deformity. Methods: A medial wedge osteotomy was performed in eight cadaveric shoulders to simulate a two-part fracture. The specimens were tested on a custom shoulder testing system. Humeral head varus was measured following physiologic muscle loading at neutral and 20° humeral internal rotation at both 0° and 20° glenohumeral abduction. Results: There was a significant decrease in varus deformity caused by the subscapularis (p<0.05) at 20° abduction. Significantly increasing humeral internal rotation decreased varus deformity caused by the subscapularis (p<0.05) at both abduction angles and that caused by the supraspinatus (p<0.05) and infraspinatus (p<0.05) at 0° abduction only. Conclusions: Postoperative shoulder abduction and internal rotation can be protective against varus failure following proximal humeral fracture fixation as these positions decrease tension on the supraspinatus and subscapularis muscles. Use of a resting sling that places the shoulder in this position should be considered.
Objective: The purpose of this studyaimed to investigate the effects of scapular stabilization training using the different exercise prop for impingement syndrome of factory workers. Design: A randomized controlled trial Methods: Twenty shoulder impingement syndrome subjects were randomly divided into two groups of ten subjects each. Inclusion criteria was presence of positive sign in at least two of Neer test, Hawkins test, Yocum test, Jobe test and Speeds test. Andexclusion criteria, those with surgical history of rotator cuff, those with disease in upper limb other than shoulder impingement syndrome, those who took anti-inflammatory drugs over the past 12 months were excluded from the sample. One group was performed scapula stabilization exercise using elastic bands, while dumbbell group performed the exercise using dumbbell. Both groups performed the exercise after applying the general physical therapy. After applying five days a week for a total of six weeks, range of motion of shoulder joint, muscle strength and pain and function level were evaluated. Results: After the intervention, in both groups, all dependent variables at the 6-week post-test significantly improved compared with the pre-test (p<0.05). In addition, elastic band exercise group showed a significant differences in SPADI function score and SPADI total score compared to the dumbbell group(p<0.05). Conclusions: The elastic band training may be effective methodology using low-to-moderate intensity resistance for SPADI scores.
