Purpose: The diagnosis of pelvic fractures pattern has become to be essential in the decision making of treatment modality and reducing morbidity and mortality in multiple trauma patients. Sacroiliac joint (SIJ) disruption can cause life-threatening massive arterial bleeding. This study aimed to determine a method of predicting the prognosis and treatment direction with pelvis X-ray alone in the emergency room. We investigated whether SIJ disruption can be used alone as a poor prognostic factor. Methods: We analyzed the medical records and radiologic examination results of 167 patients with pelvic fractures from January 1, 2015 to December 31, 2016 retrospectively. Patients with pathologic fractures, thoraco-abdominal bleeding, and acetabulum fractures and pediatric patients (n=63) were excluded. Factors related to the clinical manifestations and treatments, such as transfusion and surgery, were statistically compared. Results: The cross-sectional analysis showed that there was no correlation between SIJ injury and sex; there were statistically significant relationships between occurrences of shock, conjoined fractures, transfusion, and surgeries. The hospitalization period and partial thromboplastin time and prothrombin time values increased. The logistic regression analysis showed that when an SIJ injury occurred, blood transfusion and hypotension possibilities increased. Conclusions: When pelvic fractures occur near the SIJ, blood transfusion and shock possibilities increase. Physicians must be aware of the high severity and poor prognosis of such fractures when these are diagnosed in the emergency room. And furthermore, the physician has to predict and prepare the intensive care and multidisciplinary approaches.
This study was done to identify awareness of human rights in nursing students, and to evaluate the factors affecting human rights for nursing students. A cross-sectional design was used, with a convenience sample of 440 nursing students from two university in Incheon and Kangwon province. The score for right consciousness was much higher than duty consciousness inherent in human rights. The predicting factor for human rights was mother's rearing attitude in terms of autonomy and this factor explained 7.6% of human rights for nursing students. There was a significant difference on human rights for nursing students according to age and awareness of human rights were related to perceived parents' rearing attitude, self-esteem, psychological home environment.
Partial enteral nutrition (PEN) supplemented with insulin-like growth factor-I (IGF-I) to neonatal piglets receiving parenteral nutrition increases lactase-phlorizin hydrolase (LPH) activity, but not LPH mRNA. The goal of the current study was to investigate the mechanism by which IGF-I up-regulates LPH activity. We hypothesized that IGF-I regulates LPH synthesis post-transcriptionally. Methods: Newborn piglets (n=15) received 100% parenteral nutrition (TPN), 80% parenteral nutrition + 20% PEN (PEN), or PEN + IGF-I (1.0mg/kg/d). On day 7, two stable isotopes of leucine, [$^2 H_3$]-leucine and [$^{13}C_1$]-L-leucine were intravenously administered to measure mucosal protein and brush LPH (BB LPH) synthesis. Results: Weight gain, nutrient intake and jejunal weight and length were similar among the treatment groups. PEN increased mucosal weight, villus width and cross-sectional area, LPH activity, mRNA expression and the abundance of proLPHh compared to 100% TPN (p<0.05). IGF-I further increased mucosal weight, LPH activity and LPH activity per unit BB LPH ~2-fold over PEN alone (p<0.05), but did not affect LPH mRNA or the abundance of proLPHh or mature LPH. Isotopic enrichment of [$^2 H_3$]-leucine and [$^{13}C_1$]-L-leucine in plasma, mucosal protein and LPH precursors, and the fractional and absolute synthesis rates of mucosal protein and LPH were similar among the treatment groups. Total mucosal protein synthesis was increased 60% (p<0.05) and LPH synthesis tended (p=0.14) to be greater in the IGF-I treated animals compared to the other two groups. Conclusions: The primary mechanism by which IGF-I up-regulates LPH may be post-translational, either via reducing LPH turnover, or by specifically altering LPH activity.

Background: Smoking is a learnt behavior during adolescence and understanding the factor/s associated with smoking will assist in identifying suitable measures in combating the rising prevalence of smoking among adolescents. This research aimed to identify the factor/s associated with smoking among form four students in Kota Tinggi, Johor. Multistage sampling was used to select a representative sample of students in 2008 and data were collected using a self-administered validated questionnaire. This study revealed that the overall smoking prevalence was 19.0% with a significantly higher proportion of male smokers (35.8%) as compared to females (3.15%). Adolescents who were male (aOR 6.6, 95%CI 2.61-16.4), those who had peer/s who smoked (aOR 4.03, 95% CI 1.31-12.4), and those who studied in rural areas and Felda Settlements ( aOR 4.59, 95 CI 1.11-18.0; aOR 9.42, 95%CI 3.91-29.1) were more likely to smoke in the past one week. On the other hand, adolescents with better knowledge on the hazards of smoking and negative attitudes towards smoking were less likely to smoke (aOR 0.51, 95%CI 0.37-0.72; aOR 0.67, 95%CI 0.46-0.99). Future promotional and interventional programmes on smoking should be considered and the above identified risk factors integrated to reduce smoking prevalence among students of school-going ages in Kota Tinggi. Purpose: This study is to identify how the meaning in life, depression, and social support influences the suicidal ideation of old adult hemodialysis patients. Methods: A cross-sectional design was used. A descriptive correlative study was conducted through an organized and structured self-administrated questionnaire survey for 120 sampled old adult renal dialysis patients. The collected data were analyzed by t-tests, ANOVA, Pearson's correlation coefficient, and multiple linear regression analysis using the Stata 13.0 program. Results: The suicidal ideation of participants was positively correlated with the meaning in life, and social support, and the depression was negatively correlated with the suicidal ideation. Significant factors influencing the suicidal ideation included the meaning in life, depression, social support, age, caregiver, monthly income, and smoking. These factors explained 68% of the variance. Conclusion: The depression of old adult hemodialysis patients was a major risk factor that increased suicidal ideation, and the meaning in life and social support was a protective factor that reduced the suicidal ideation. These results suggest that health professionals should provide old adult hemodialysis patients with proper management of suicidal ideation, and depression as well as its meaning in life, and social support.
Background: The risk of tuberculosis (TB) infection among health care workers (HCWs) is higher than as noted among workers in the general population. The prevalence and risk factors of TB infection among HCWs were assessed in a tertiary hospital in South Korea, resulting in a conclusion of an intermediate TB burden within the country. Methods: This cross-sectional study enrolled HCWs who underwent a QuantiFERON-TB Gold In-Tube (QFT-GIT) test to detect the presence of a latent TB infection (LTBI), in patients admitted to a tertiary hospital in South Korea in 2017. The departments of the hospital were divided into TB-related and TB-unrelated departments, which were based on the risk of exposure to TB patients. In this sense, the risk factors for LTBI, including current working in the TB-related departments, were analyzed. Results: In this case, a total of 499 HCWs (54 doctors, 365 nurses and 80 paramedical personnel) were enrolled in this study. The median age of the subjects was 31 years (range, 20-67 years), 428 (85.8%) were female, and 208 (41.7%) were working in the TB-related departments. The prevalence of LTBI was 15.8% based on the QFT-GIT. Additionally, the prevalence of experience of exposure to pre-treatment TB patents was higher among HCWs working in the TB-related departments, than among HCWs working in the TB-unrelated departments (78.8% vs. 61.9%, p<0.001). However, there was no significant difference in the prevalence of LTBI between the two groups (17.3% vs. 14.8%, p=0.458). On a review of the multivariate analysis, only the factor of age was independently associated with an increased risk of LTBI (p=0.006). Conclusion: Broadly speaking, the factor of age was associated with an increased risk of LTBI among the HCWs in South Korea. However, those workers current working in the TB-related departments was not associated with an increased risk of LTBI.
The purpose of this paper is to identify factors affecting the optimum mix of required inputs and other relevant factors which account for the variation in physician's productivity in general hospitals, and to find out their implications for the efficient health planning and management. An extended version of Cobb-Douglas production function and cross sectional data of one day patient census from all general hospitals in Korea in 1988 were used in the analysis. Main results of the analysis and their implications could be summarized as follows : (1) The production function for physician's inpatient service shows the evidence of economies of scale, but the production function for physician's outpatient and adjusted-patient service, which combines both out- and in-patient service, shows that of dis-economies of scale. (2) The physician's role for production for all service is smaller than auxiliary personnel's, which imply that more intensive utilization of nurses, nursing aides and other auxiliary personnel is desirable for improving general hospital productivity (3) In case of physician's inpatient and adjusted-patient service, nurses' role is greater than nursing aides'. Therefore, more extensive utilization of nurses is recommended for the efficient operation of general hospitals. (4) The factor of hospital beds plays the leading role among required inputs in the production for physician's in- and adjusted-patient service. (5) The physician's productivity of general hospitals in rural area is lower than that in urban area. And the productivity of teaching hospitals is lower than that of the other hospitals. Further analysis was made in physician production function based upon the size of hospitals, namely those hospitals below 250 beds and those above. Explained variances by the factor of hospital beds was significantly increased in the case of those hospitals above 250. A more detailed and thorough investigation is needed for verifying factors influencing physician's productivity in general hospitals in Korea.
Objectives: Pedestrian traffic injuries have been an important cause of childhood mortality and morbidity for decades. The aim of this study was to describe the epidemiological characteristics of child pedestrian traffic injuries that occurred during 2000 in one metropolitan city and its school-zones, and to determine the factors associated with those accidents. Methods: A cross-sectional study was performed in 2001. Police records were used to identify the cases of pedestrian injury. Children aged between 6 and 15 years, injured during road walking, were included in this study. A direct survey of the environmental factors within the school-zones in study area (n=116) was also performed. Self-administered questionnaires, via mail and telephone surveys, were used to assess the safety education programmes. The schools were divided into two groups according to the occurrence of pedestrian traffic injuries in their school-zone. Results: Pedestrian injuries (n=597) were found to account for 3.2% of all traffic injuries in the subject area. The epidemiological characteristics were not significantly different between genders. There were some significant risk factors within the environmental factors, such as local road (OR: 2.3, 95% CI=1.05-5.35), heavy traffic volume (OR: 2.2, 95% CI=1.00-5.04), poor visibility of speed-limit signs (OR: 2.8, 95% CI=1.25-6.42), no separation of pedestrian routes from cars (OR: 2.6, 95% CI=1.02-6.75) and barriers on the pedestrian routes (OR: 2.2, 95% CI=1.01-5.08). Only one factor, that of education in a safety-park (OR: 0.3, 95% CI=0.09-0.96), was significantly associated in the traffic and pedestrian safety education factors. Conclusion: Significant associations with pedestrian injury risk were identified in some of the modifiable environmental factors than in the educational factors.
The flexural behaviour of steel beams significantly affects the structural performance of the steel frame structures. In particular, the flexural overstrength (namely the ratio between the maximum bending moment and the plastic bending strength) that steel beams may experience is the key parameter affecting the seismic design of non-dissipative members in moment resisting frames. The aim of this study is to present a new formulation of flexural overstrength factor for steel beams by means of artificial neural network (NN). To achieve this purpose, a total of 141 experimental data samples from available literature have been collected in order to cover different cross-sectional typologies, namely I-H sections, rectangular and square hollow sections (RHS-SHS). Thus, two different data sets for I-H and RHS-SHS steel beams were formed. Nine critical prediction parameters were selected for the former while eight parameters were considered for the latter. These input variables used for the development of the prediction models are representative of the geometric properties of the sections, the mechanical properties of the material and the shear length of the steel beams. The prediction performance of the proposed NN model was also compared with the results obtained using an existing formulation derived from the gene expression modeling. The analysis of the results indicated that the proposed formulation provided a more reliable and accurate prediction capability of beam overstrength.

Background: Colorectal cancer (CRC) is the third most common cancer in Iranian women and fifth in men. The aims of this study were to investigate the relation of dietary factors and public health indicators to its development. Materials and Methods: The required information (2001-2006) about risk factors was obtained from the Non-Communicable Disease Surveillance Centre (NCDSC) of Iran. Risk factor data (RFD) from 89,404 individuals (15-64 years old) were gathered by questionnaire and laboratory examinations through a cross sectional study in all provinces by systematic clustering sampling method. CRC incidence segregated by age and gender was obtained from Cancer Registry Ministry of Health (CRMH) of Iran. First, correlation coefficients were used for data analysis and then multiple regression analysis was performed to control for confounding factors. Results: Colorectal cancer incidence showed a positive relationship with diabetes mellitus, hypertension, lacking or low physical activity, high education, high intake of dairy products, and non-consumption of vegetables and fruits. Conclusions: We concluded that many dietary factors and public health indicators have positive relationships with CRC and might therefore be targets of preliminary prevention. However, since this is an ecological study limited by potential ecological fallacy the results must be interpreted with caution.
