Analysis of influencing factors on hospital-employed physician's income (병원근무 전문의 소득에 영향을 미치는 요인분석)

  • This study reviews the literature of influencing factor on hospital-employed physician's income, and it describes general distribution of hospital-employed physician's income, and analyzes influencing factor of hospital-employed physician's income. A total of 1.795 persons responded to the mail survey. through stratified sampling by 23 branches of medical society in Korean RBRVS study. The design of the study is cross sectional study. and the unit of analysis is a physician. To examine the change of average income per month. multiple regression was used to test the change according to physician's characteristics. demographic characteristics. scale of hospital. average intensity of ordinary work. and specialty. The major findings of this study are as follows; 1. As for physicians working in first referral hospital. the average income of neurosurgeon per month was the largest. being 1.34 times larger than that of the family physician, and that of the emergency physician was the smallest, being 0.78 times smaller than that of the family physician, but that of the ophthalmic and Orthopaedic physician was significantly larger than that of the family physician under the control of control variables. And average income per month was significantly larger for physicians who worked in Seoul metropolitan area than physicians who worked in rural area. 2. The year of physician's career, number of average out-patients per month significantly positively associated, but the number of hospital beds and average intensity of therapy significantly negatively associated with average income per month. 3. As for physicians working in second referral hospital. the average income of the psychiatric physician per month was the largest, being 1.33 times larger than that of the family physician, and that of the emergency physician was smallest, being 0.74 times smaller than that of the family physician., but no significant difference was seen under the control of control variables, and average income per month was significantly larger for physicians who worked in Seoul metropolitan area than physicians who worked in large municipal area. 4. The year of physician's career and number of hospital beds significantly positively associated, but average working hours per month significantly negatively associated with average income per month. In conclusion, the year of hospital-employed physician's career is the largest influencing factor on hospital-employed physicians. But the difference of average income per month according to working regions and to number of hospital beds existed in employed physicians under the control of control variables. So this study has implementation that we must consider the influence of working regions and the number of hospital beds on the income of hospital-employed physicians in making policy for hospital. Being a cross-sectional study, this study can not suggest causal explanations. In the future, experiment or cohort study is needed for causal explanations.

Three-dimensional Capsular Volume Measurements in Multidirectional Shoulder Instability

  • Background: In a previous study undertaken to quantify capsular volume in rotator cuff interval or axillary pouch, significant differences were found between controls and patients with instability. However, the results obtained were derived from two-dimensional cross sectional areas. In our study, we sought correlation between three-dimensional (3D) capsular volumes, as measured by magnetic resonance arthrography (MRA), and multidirectional instability (MDI) of the shoulder. Methods: The MRAs of 21 patients with MDI of the shoulder and 16 control cases with no instability were retrospectively reviewed. Capsular areas determined by MRA were translated into 3D volumes using 3D software Mimics ver. 16 (Materilise, Leuven, Belgium), and glenoid surface area was measured in axial and coronal MRA views. Then, the ratio between capsular volume and glenoid surface area was calculated, and evaluated with control group. Results: The ratio between 3D capsular volume and glenoid surface area was significantly increased in the MDI group ($3.59{\pm}0.83cm^3/cm^2$) compared to the control group ($2.53{\pm}0.62cm^3/cm^2$) (p<0.01). Conclusions: From these results, we could support that capsular volume enlargement play an important role in MDI of the shoulder using volume measurement.

Effects of Gagamdokhwalgisang-Tang(GD;加減獨活奇生湯) on the Morphometric Changes of Femur and the Factors Related with Bone Metabolism in Ovariectomized Rats (가감독활지생탕(加減獨活奇生湯)이 난소적출 흰쥐 대퇴골의 형태계측학적 변화 및 골대사 관련인자에 미치는 영향)

  • Purpose : This study was carried out to investigate the effects of Gagamdokhwalgisang-Tang on the morphometric changes of femur, and on the hormones and cytokines associated with bone metabolism in overiectomized rats. Methods : Twenty-four female Sprague-Dawley rats were divided into sham operated group(normal) ovariectomized group(control), and treated with extract of GD group(treated). Each group was evaluated the changes of body weight at 0, 3, 6, 8 weeks after ovariectomy. Morphometric analysis(femur weight, femur/body weight ratio, femur ash weight femur ash/body weight ratio cross sectional area of compact bone and concellous bone of femur) and histopathological examination were performed at 8 weeks after ovariectomy. Estrogen, Alkaline Phosphatase(ALP) and cytokine(Tumor necrosis $factor-{\alpha}$, $Interleukin-l{\beta}$, Inerleukin-6) assay were performed at 8 weeks after ovariectomy. Results : 1. The body weight of control and treated group was significantly increased(p<<0.001) compared with the normal group at 8 weeks. 2. The femur weight and femur/body weight ratio of treated group were significantly increased(p<<0.05, p<<0.01) compared with the control group at 8 weeks. 3. The femur ash weight showed no significantly different changes, but femur ash/body weight ratio of treated group was significantly increased(p<<0.05) compared with the control group at 8 weeks. 4. In the cross sectional area of cancellous bone of femoral body, the treated group was significantly increased(p<<0.001) compared with the control group at 8 weeks. 5. The serum estrogen level of treated group showed no significantly different changes compared with the control group at 8 weeks. 6. The serum ALP activity of treated group was significantly decreased(p<<0.01) compared with the control group at 8 weeks. 7. The serum Tumor necrosis $factor-{\alpha}(TNF-{\alpha})$ level of treated group was significantly decreased(p<<0.05) compared with the control group at 8 weeks. 8. The serum $Interleukin-l{\beta}(IL-1{\beta})$ level of treated group was significantly decreased(n<<0.001) compared with the control group at 8 weeks. 9. The serum Interleukin-6(IL-6) level of treated group was significantly decreased(P<<0.01)compared with the control group at 8 weeks. Conclusion : These results indicate that GD inhibits bone resorption in ovariectomized rats. And the major inhibitory mechanism may be related to the inhibitory effects of GD on the secretion of $TNF-{\alpha}$, $IL-1{\beta}$ and IL-6 in the pathogenesis of osteoporosis in estrogen deficient rats.

  • Purpose : This study was carried out to investigated the effects of Saenghyeolbosu-Tang on the morphometric changes of femur, and on the hormones and cytokines associated with bone metabolism in ovariectomized rats. The femur/body weight(g) of treated group(5.27${\pm}$0.36) was significantly increased(p<0.05) compared with the control group(4.78${\pm}$0.37) at 8 weeks. 3. In the cross sectional area of cancellous bone of femur(${mm^2}$) the treated group(1.60${\pm}$0.20)was significantly increased(p<0.01) compared with the control group(1.19${\pm}$0.19) at 8 weeks. 4. The serum estrogen level(pg/ml) of treated(83.67${\pm}$27.44) and control group(800.49${\pm}$22.27)showed no significantly different changes at 8 weeks. 5. The serum $TNF-{\alpha}$ level(pg/ml) of treated(33.71${\pm}$6.43) and control group(41.35${\pm}$8.47) showed no significantly different changes at 8 weeks. 6. The serum $IL-1{\beta}$ level(pg/ml) of treated group(53.78${\pm}$10.83) was significantly decreased(p<0.001) compared with the control group(79.80${\pm}$11.40) at 8 weeks. 7. The serum IL-6 level(pg/ml) of treated group(55.52${\pm}$9.02) was significantly decreased(p<0.01) compared with the control group(69.69${\pm}$7.95) at 8 weeks. Conclusion : This study shows that Saenghyeolbosu-Tang inhibits bone resolution in ovariectomized rats. These results may be related to the inhibitory effect of Saenghyeolbosu-Tang on the secretion of $IL-1{\beta}$ and IL-6 in the Pathogenesis of osteoporosis.

Effects of Six-week Resistance Exercise using an Outdoor Knee Extension Machine on Function and Structure of the Knee Extensor Muscles (6주간 야외용 운동기구를 사용한 저항운동이 노인 여성의 무릎신전근 기능 및 구조에 미치는 영향)

  • The purpose of this study was to investigate the effect of leg extension exercises performed on outdoor resistance exercise machines on knee extension muscle strength and quadriceps muscle group cross sectional area (CSA) in elderly women. Two groups were recruited for this study, including an exercise group (EG: n=13, $71.38{\pm}2.79$ yrs) and a control group (CG: n=5, $73.4{\pm}5.94$), In all subjects, maximum isometric and isokinetic muscle strength of knee flexion and extension were measured using an isokinetic dynamometer (Cybex(R) Humac Norm Testing & Rehabilitation System, USA). Quadriceps muscle group CSA were measured using MRI (Philps, Intera 1.5 T, NE Netherlands). The results of this study showed that post-intervention isometric knee extension peak torque value were higher than pre-intervention measures in the EG. However, the EG did not show improvement in quadriceps muscle group CSA, Also, no differences in the shift of optimal knee joint angle were observed between pre and post-intervention exercise. Outdoor leg extension exercise showed small increases in muscle strength in comparison to other resistance training exercises. The results of this study suggest that because outdoor leg extension exercise machines lack a progressive loading mechanism, significant increases in muscle strength may not be obtained.

Optimization of a Single-Channel Pump Impeller for Wastewater Treatment

  • As a single-channel pump is used for wastewater treatment, this particular pump type can prevent performance reduction or damage caused by foreign substances. However, the design methods for single-channel pumps are different and more difficult than those for general pumps. In this study, a design optimization method to improve the hydrodynamic performance of a single-channel pump impeller is implemented. Numerical analysis was carried out by solving three-dimensional steady-state incompressible Reynolds-averaged Navier-Stokes equations using the shear stress transport turbulence model. As a state-of-the-art impeller design method, two design variables related to controlling the internal cross-sectional flow area of a single-channel pump impeller were selected for optimization. Efficiency was used as the objective function and was numerically assessed at twelve design points selected by Latin hypercube sampling in the design space. An optimization process based on a radial basis neural network model was conducted systematically, and the performance of the optimum model was finally evaluated through an experimental test. Consequently, the optimum model showed improved performance compared with the base model, and the unstable flow components previously observed in the base model were suppressed remarkably well.

The Suppressive Effects of Daebangfung-tang against Disuse Muscle Atrophy in Gastrocnemius of Rats (대방풍탕의 흰 쥐 비복근 불용성 근위축에 대한 억제 효과)

  • Objectives : The aim of this study is to evaluate the effect of Daebangfung-tang (DBFT) on suppressing muscle atrophy followed by immobilization. Methods : Twenty young male Sprague-Dawley rats were randomized into two groups. The rats in experimental group (DBFT group) were orally administrated DBFT once a day for 2 weeks, and the rats in control group were given normal saline in the same manner. The immobilization on left leg was carried out using casting tape. The morphologies of gastrocnemius in both groups were assessed. The cross sectional area of them was measured by image analyzer. Immunohistochemical staining was implemented and the numbers of immunopositive cells implying the expression of TNF-α were measured and quantified. Results : DBFT represented the significant suppressive effects against the weight loss of the left gastrocnemius muscle and the reduction in cross sectional area of the left gastrocnemius muscle compared with normal saline. Also the treatment with DBFT significantly reduced the expression of TNF-α in gastrocnemius compared with normal saline. Conclusions : DBFT is thought to have the significant suppressive effect against muscle atrophy followed by immobilization.

A Radiological Study on the Morphology of Labial Alveolar Bone in the Mandibular Incisor Area of Mandibular Prognathism Patients (하악전돌증 환자의 하악전치부 순측 치조골 형태에 관한 방사선학적 연구)

  • This study was concerned with comparing the measured values of labial alveolar bone through the lateral cephalometric radiography and mandibular incisor cross-sectional tomogram between two groups, one group of mandibular prognathism patients who needed an orthognathic surgery as an experimental group and the other group who had normal molar relationships as a control group. The purpose of the study was to find out the predisposing factor of bone resorption and gingival recession before orthodontic treatment. The results were as follows: 1. The cross-sectional area of labial alveolar bony plate in mandibular prognathism was significantly smaller than that of control group. 2. In mandibular prognathism, the distance between cementoenamel junction and alveolar crest was significantly greater than control group. 3. There were negative correlations between area of labial alveolar bony plate and distance from cementoenamel junction to alveolar crest, and positive correlations between area of labial alveolar bony plate and distance from alveolar crest to root apex. 4. In mandibular prognathism, there were positive correlations between IMPA and thickness of symphysis, and negative correlations between IMPA and the alveolar bony height. The results of the present study suggest the mandibular prognathism patients are prone to the gingival recession due to the small amount of labial alveolar bone around lower incisors.

Field Emission from Selectively-patterned ZnO Nanorods Synthesized by Solution Chemistry Route

  • An effective wet-chemical approach is demonstrated for growing large-area, selectively-patterned, and low-temperature-synthesized ZnO nanorods (ZNRs). The growth of ZNRs was enhanced on a Co layer. The selectivity and density were readily controlled by the control of the temperature when the substrate transfers into aqueous solution. The cross-sectional transmission electron microscopy image shows that single crystalline ZNRs grown along [0001] have good adhesion at interface between ZNRs/substrate. The turn-on field was 4 $V/{\mu}m$ at the emission current density of 1 ${\mu}A/cm^2$. The stable emission was obtained at 0.11 $mA/cm^2$ under 7.2 $V/{\mu}m$ over 10 hr. These results suggest that selectively-patterned ZNRs have the potential for use as field emitters in large-area field emission displays.

Apical root canal cleaning after preparation with endodontic instruments: a randomized trial in vivo analysis

  • Objectives: This study aimed to evaluate vital pulp tissue removal from different endodontic instrumentation systems from root canal apical third in vivo. Materials and Methods: Thirty mandibular molars were selected and randomly divided into 2 test groups and one control group. Inclusion criteria were a positive response to cold sensibility test, curvature angle between 10 and 20 degrees, and curvature radius lower than 10 mm. Root canals prepared with Hero 642 system (size 45/0.02) (n = 10) and Reciproc R40 (size 40/0.06) (n = 10) and control (n = 10) without instrumentation. Canals were irrigated only with saline solution during root canal preparation. The apical third was evaluated considering the touched/untouched perimeter and area to evaluate the efficacy of root canal wall debridement. Statistical analysis used t-test for comparisons. Results: Untouched root canal at cross-section perimeter, the Hero 642 system showed 41.44% ± 5.62% and Reciproc R40 58.67% ± 12.39% without contact with instruments. Regarding the untouched area, Hero 642 system showed 22.78% ± 6.42% and Reciproc R40 34.35% ± 8.52%. Neither instrument achieved complete cross-sectional root canal debridement. Hero 642 system rotary taper 0.02 instruments achieved significant greater wall contact perimeter and area compared to reciprocate the Reciproc R40 taper 0.06 instrument. Conclusions: Hero 642 achieved higher wall contact perimeter and area but, regardless of instrument size and taper, vital pulp during in vivo instrumentation is not entirely removed.
