중국 상하이·허쩌 중·고등학생의 식습관과 비만도 및 영양지식과의 관련성 연구 (Associations of Eating Habits with Obesity and Nutrition Knowledge for Middle and High School Adolescents in Shanghai and Heze China)

  • The aim of this study was to investigate the relationships between eating habits and health among adolescents in Shanghai and Heze, China. A cross-sectional study was conducted in 2013 on 2,089 adolescents; 1,089 students were from Shanghai and 999 students from Heze region. Eating habits, weight, height, and nutritional knowledge were assessed using a self-administered questionnaire. Eating habits score was classified into two categories: healthy eating habits and unhealthy eating habits, based on "Korean Youth Risk Behavior Web-based Survey", for statistical data analysis. Associations between eating habits, BMI, and nutritional knowledge were examined using a general linear model with adjustment of potential confounding factors such as region, gender, age, parents' education level, and pocket money. Statistical analyses were performed using the SAS (version 9.3) program. Proportions of healthy eating habits group were 90.0% for breakfast (3-7 times/wk), 29.1% for fruit (${\geq}once/d$), 12.5% for vegetable (${\geq}3times/d$), 7.3% for milk (${\geq}2times/d$), 90.0% for fast food (<3 times/wk) consumption, respectively. The average BMI score was 20.1 (Shanghai 20.5 Heze 19.6), which is in the range of normal weight. Rates of obesity and overweight were 16.5% and 8.3% in Shanghai and Heze, respectively. There were significant negative correlations between intake frequencies of breakfast, fast food, biscuits, sugar, chocolate, and BMI score. Eating habits and nutritional knowledge score showed a significant positive correlation. These results showed better eating habits regarding eating regularity and consumption of fruits and soft drinks in Chinese adolescents compared with Korean adolescents, although cultural differences were not fully considered. This study demonstrated significant associations of BMI and nutritional knowledge with dietary behavior in Chinese adolescents in two regions of China. Further studies on Chinese adolescents from other regions in China should be considered.

유치원 급식의 위생관리 실태조사 및 미생물적 품질평가 (Evaluation of Sanitation Management Practices and Microbiological Quality of Foods in Kindergarten Foodservice Settings)

  • This research aims to audit foodservice sanitation management practices and to assess microbiological quality of foods and their food contact environments in kindergartens. Sanitation auditing was conducted in 10 kindergartens in Seoul, Gyeonggi, and Incheon areas to assess the levels of safety practices. Results revealed that the surveyed kindergartens scored 41.4 out of 100 points, on average. The average scores of each category were 6.4/11 (58.1%) for facilities sanitation, 4.2/12 (35.0%) for equipment sanitation, 2.4/10 (24.0%) for personal hygiene, 5.1/10 (51.0%) for food ingredients management, 6.0/17 (35.3%) for production process, 5.4/10 (54.0%) for environmental sanitation, 2.0/6 (33.3%) for kitchen utensils sanitation, and 2.2/6 (96.7%) for safety management. Microbiological quality of raw, prepared foods, personal sanitation (hands), environmental sanitation, and drinking water were assessed. Total plate counts (TPC) of the following menus exceeded the critical limit: seasoned leek (5 log CFU/g), cucumber (5.0 log CFU/g), panbroiled fish paste (TNTC at $10^4$), tangpyeongchae (5.3 log CFU/g), egg rolls (6.1 log CFU/g), panbroiled sausage (TNTC at $10^4$), and soft tofu pot stew (TNTC at $10^4$). Coliform which exceeded the standard limit were detected from seasoned leek (2 log CFU/g), cucumber (2.5 log CFU/g), panbroiled fish paste (2.0 log CFU/g), egg roll (3.8 log CFU/g), tangpyeongchae (4.0 log CFU/g), panbroiled sausage (2.3 log CFU/g), and soft tofu pot stew (3.7 log CFU/g). For seasoned foods (muchim), S. aureus ranged 2.2~2.9 log CFU/g. In food workers' hands, microbial profiles ranged 3.8~7.9 log CFU/hand for TPC, ND~4.5 log CFU/hand for coliforms, ND~4.7 log CFU/hand for S. aureus, and ND~5.3 log CFU/hand for Enterobacteriaceae. Microbiological profiles of food contact surface of knives, cutting boards, dish-clothes, and trays showed possibilities of cross-contamination. General bacteria were 2.1~4.5 logCFU/ml in 4 purified water samples and E. coli were found in the kitchen of one kindergarten. These results suggested that environmental sanitation management practices need more strict improvement: effective sanitation education methods and practices were strongly required, and more strict sanitation management for cooking utensils and equipment were required.

The Combined Effect of Subjective Body Image and Body Mass Index (Distorted Body Weight Perception) on Suicidal Ideation

  • Objectives: Mental health disorders and suicide are an important and growing public health concern in Korea. Evidence has shown that both globally and in Korea, obesity is associated with an increased risk of developing some psychiatric disorders. Therefore, we examined the association between distorted body weight perception (BWP) and suicidal ideation. Methods: Data were obtained from the 2007-2012 Korea National Health and Nutritional Evaluation Survey (KNHANES), an annual cross-sectional nationwide survey that included 14 276 men and 19 428 women. Multiple logistic regression analyses were conducted to investigate the associations between nine BWP categories, which combined body image (BI) and body mass index (BMI) categories, and suicidal ideation. Moreover, the fitness of our models was verified using the Akaike information criterion. Results: Consistent with previous studies, suicidal ideation was associated with marital status, household income, education level, and perceived health status in both genders. Only women were significantly more likely to have distorted BWP; there was no relationship among men. In category B1 (low BMI and normal BI), women (odds ratio [OR], 2.25; 95% confidence interval [CI], 1.48 to 3.42) were more likely to express suicidal ideation than women in category B2 (normal BMI and normal BI) were. Women in overweight BWP category C2 (normal BMI and fat BI) also had an increased OR for suicidal ideation (OR, 2.25; 95% CI, 1.48 to 3.42). Those in normal BWP categories were not likely to have suicidal ideation. Among women in the underweight BWP categories, only the OR for those in category A2 (normal BMI and thin BI) was significant (OR, 1.34; 95% CI, 1.13 to 1.59). Conclusions: Distorted BWP should be considered an important factor in the prevention of suicide and for the improvement of mental health among Korean adults, especially Korean women with distorted BWPs.

성인둔상환자에서 평균동맥압과 위해사건발생의 관련성:단면 조사 연구 (Relationship of Mean Arterial Pressure with the Adverse Outcomes in Adult Blunt Trauma Patients: Cross-sectional Study)

  • Purpose: Non-invasive blood pressure measurement is widely used as a pre-hospital triage tool for blunt trauma patients. However, scant data exits for using the mean arterial pressure (MAP), compared to the systolic blood pressure, as a guiding index. The aim of this study was to determine the association between adverse outcomes and mean arterial pressure (MAP) and to exhibit the therapeutic range of the MAP in adult blunt trauma patients. Methods: The electronic medical records for all trauma patients in a single hospital from January 2010 to September 2012 were retrospectively reviewed. Patients below 17 years of age, patients with penetrating injuries, and patients with serious head trauma (injuries containing any skull fractures or any intracranial hemorrhages) were excluded. Adverse outcomes were defined as one of the following: death in the Emergency Department (ED), admission via operating theater, admission to the intensive care unit, transfer to another hospital for emergency surgery, or discharge as hopeless. Results: There were 14,537 patients who met entry criteria. Adverse outcomes occurred for MAPs in range from 90 to 120 mmHg. Adverse outcomes were found, after adjusting for confounding variables, to occur increasingly as the MAP declined below 90 mmHg or rose above 120 mmHg. Conclusion: Not only lower but also higher mean arterial pressure is associated with increased adverse outcomes in adult blunt trauma patients. Thus, patients with a MAP above 120 mmHg should be considered as a special group requiring higher medical attention, just as those with a MAP below 90 mmHg are.

CFD를 이용한 Hybrid 인공습지의 초기침강지 저류판 구조 모의 (Simulation of Various Baffle Types in a Constructed Wetland Sedimentation Tank using CFD)

  • 인공습지는 오염물질 저감기능을 포함하는 여러 가지 생태서비스 기능 때문에 다양하게 적용되고 있다. 그러나 인공습지에 많은 입자상 물질이 유입하게 되면 유지관리의 어려움과 기능의 저하를 초래한다. 따라서 본 연구는 인공습지 내입자상 물질의 제거효율 향상을 위한 침강지내 최적 baffle 설치방안을 도출하고자 수행되었다. 최적 저류판 설치방안은 침강지내 유체와 입자의 흐름을 해석함으로써 도출가능하며, 이를 위해 전산유체역학(Computational Fluid Dynamics, CFD) 이론을 활용하였다. 연구결과는 baffle이 유속을 저하시키고 입자상 물질의 침강효율을 증가시키는 것으로 나타났으며, 경사각을 가진 저류판보다는 수직 저류판이 효과적인 것으로 나타났다. 이러한 연구결과를 활용하여 공공지역 비점오염저감시설로 설치된 소규모 하이브리드 인공습지의 침강지 효율개선 방안을 도출하였다. 수직각을 이용한 저류판을 설치할 경우 침강효율이 1.2~1.3배 정도 증가하는 것으로 나타났다. 이러한 침강지 내 저류판의 설치에 따른 결과는 인공습지의 전반적인 저감 효율을 증가시킬 뿐만 아니라 유지관리 빈도를 줄임으로써 비용효율적 인공습지 설계에 기여할 것으로 평가된다.

충남 일부 지역 병의원의 농약살포 중 중독 사례 분석 및 보건관리방안 연구 (A Study on the Case Analysis and Health Management of Patients with Pesticide Poisoning from Spraying Pesticide in Hospitals in the Chungnam, Korea)

  • Objectives: This study aims to examine patients who developed acute pesticide poisoning while spraying or using pesticide and presented to the emergency department in hospitals in Chungcheongnam-do Province. Based on the findings, this study will provide implications for safety and health management pertaining to the use of pesticides. Methods: Pesticide poisoning data collected by the Chungnam Center for Farmers' Safety and Health from 2014 to 2018 was cross-sectionally analyzed. A total of 331 patients with pesticide poisoning presented to one of hospitals and four of medical centers in the region(Dankuk University Hospital, Gongju and Hongsung, Cheongyang, Cheonan Medical Center). Seventeen of these patients (15 men and two women) developed poisoning while spraying pesticide. The patients' charts were reviewed to collect data on pesticide poisoning, namely currently working in farming-related occupation, means of transportation to the hospital, place of poisoning, symptoms of acute pesticide poisoning, treatment, pesticide used when poisoning occurred, and classification by technical ingredients. Results: Fifteen out of 17 patients who developed poisoning while spraying pesticide were men. Insecticide was used in 35.3% of the cases, and herbicide was used in 29.4% of the cases, which was different from cases of poisoning from ingestion poisoning. The major symptoms were vomiting (35.3%), nausea (29.4%), dizziness (29.4%), and headache (23.5%). A total 11 ingredients were identified in 12 patients, and the most common ingredient was glyphosate, which is an herbicide. Most patients showed a state of mild toxicity, but two patients showed a state of severe toxicity. These patients respectively used glyphosate and paraquat. Conclusions: Our findings can be useful for suggesting the need for a national healthcare system to manage occupational pesticide poisoning among farmers. Further, these findings can be used to increase the awareness of the risk of acute poisoning during pesticide spraying and suggest the need for a safety health education to increase farmers' awareness of pesticide poisoning.

마성분지(麻城盆地)의 지형생성작용(地形生成作用) (Geomorphic Processes of Masung Basin)

  • 경상북도 문경시 마성면 일대에는 고생대의 퇴적암과 변성암 산지로 둘러싸인 석회암 분지가 형성되어 있다. 이 논문에서는 마성분지의 주변에 분포하는 완만한 구릉지의 지형생성작용을 밝히고자 하였다. 이를 위하여 구릉지에 피복된 퇴적층을 분석하였으며, 구릉지의 종단면에서 거리와 고도 사이의 관계를 검토하였고, 구릉지와 지질과의 상관관계를 고찰하였다. 본 논문에서 밝혀진 마성분지의 지형생성작용을 요약하면 다음과 같다. 첫째, 퇴적층은 구릉지의 말단부로 갈수록 두께가 증가한다. 퇴적층내 대부분의 역은 석회암질이며, 배후산지와 구릉지의 접경부에서 침식된 물질이다. 퇴적층은 분급은 양호하나 imbrication이 불량하다. 둘째, 분지 주변의 구릉지(해발고도 $160{\sim}230m$)는 '유로(流路)를 갖는 유수(流水)'의 작용으로 형성되었다. 지속적인 유수의 작용으로, 경암과 연암이 접하는 경계 부분에서는 상대적으로 약한 석회암이 신선한 기반암까지 침식되어 경사급변점이 나타났다. 셋째, 구릉지 형성 이후 배후산지가 협곡으로 개석되면서 생산된 boulder들이 구릉지의 퇴적층을 피복하였다. 결론적으로 마성분지의 지형생성작용은 화강암 분지의 생성작용으로 제시되었던 '암석의 연경차에 따른 차별침식'과 궤를 같이 하고 있으나, '포상류에 의한 풍화물질의 제거'와는 상이한 것으로 밝혀졌다.

  • This study was conducted to investigate health status and nutrient intakes among 5th grade elementary students at Korea Centers for Disease Control and Prevention 2010. This study was cross-sectional study on 1,384 children (687 boys, 697 girls) from nine elementary schools located in Seoul and Gyeonggi province. The average height, weight and BMI were 145 cm, 40.6 kg $19.2\;kg/m^2$ for boys and 145.4 cm, 38.2 kg, $18.0\;kg/m^2$ for girls. The prevalence of overweight and obesity were 11.3%, 5.2%, respectively. Serum AST, ALT, glucose, HDL-Cholesterol and RBC levels were significantly higher, while TG levels was significantly lower for boys than for girls. The average energy intake was 1772.4 kcal, which was 98.7% of Estimated Energy Requirement (EER). The boys consumed more energy intake (1800.4 kcal) than the girls (1744.7 kcal). Also, they took insufficient calcium and folate (69.1% and 83.3% of Recommended Intake (RI)) and excess sodium (297.6% of Adequate Intake (AI)) and 85.1% of the subjects had breakfast everyday. In general, the proportion of the children who consumed fruits and vegetables at least once or more a day was low. There was a higher proportion of children in boys who had ramyun and milk with sufficient physical activity than those in girls. As a result of this study, we can find risk factors on obesity and metabolic disorders, and the results can be used for an evidence of nutrition education program and the intervention program.

어머니의 구강보건관리행태가 초등학교 아동의 치아우식에 미치는 영향 (Effects of Mother's Oral Health Care Behaviors on Dental Caries in Primary School Children)

  • Objectives : By analyzing the affects of the mother's oral care to the caries experiences of the elementary school children, To looking for the way to prevent dental caries in the primary school, to provide baseline data to perform the associated oral health education. Methods : Between the 3,676 children's dental check-up data and the 2,934 survey of parents from 9 elementary schools in the the Gyeonggi Province, the exactly matching data of 2,358 pairs, children versus mothers, were analyzed during the period, 2009 May 4 to July 20, By using the PASW 18(SPSS-PC 18), Frequency analysis of the collected data, and cross-analysis, ANOVA, Bonferroni post-analysis was performed. Results : The findings of the study were as follows. 1. In children, the ratio(DMF rate)of the dental caries experience in permanent teeth, appeared higher for men than women. Divided by grade level, the ratio of caries experience in the permanent teeth(DMF rate) generally appear higher in older than young. Thus between the child's gender and grade level by the ratio of caries experience in the permanent teeth(DMF rate) showed the statistically significant differences (p<0.05). 2. Between the socioeconomic characteristics of mothers and the children's level of dental caries experiences, not showed the statistically significant difference.(p>0.05). 3. The significant difference showed between the dental caries levels of the children and the mother's oral health care behavior - a toothbrush replacement period, mother's broken teeth, mothers guidances of the brushing after snacks for children(p<0.05). Conclusions : The mother's oral health attitudes and behaviors can affect to their children's oral care habits. Therefore The mother themselves should have the right proper habit of oral health care, so that in oral health care mothers should be the model for their children. Institutionally more systematic and detailed oral health educational program in conjunction with the family is needed.

암병동 간호사의 임종간호 스트레스와 인지 및 교육요구도 (Cancer Unit Nurses' End-of-Life Care-Related Stress, Understanding and Training Needs)

  • 목적: 본 연구는 암병동 간호사의 임종간호에 따른 스트레스와 인지정도 및 교육요구도를 파악하고자 수행되었다. 방법: 본 연구는 서울과 경기도에 소재한 4개 종합병원 암병동에 근무하는 간호사 151명이었으며, 자가보고식 설문지로 자료를 수집하였다. 스트레스 측정도구는 40문항, 인지정도와 교육요구도 측정도구는 각각 27문항으로 구성되었으며, 수집된 자료는 서술적 통계방법으로 분석하였다. 결과: 암병동 간호사의 임종간호에 따른 스트레스 정도는 5점 만점에 평균 3.96점으로 높았으며, 영역별 스트레스는 업무량 과중이 가장 높았고, 임종환자에게 시간할애의 어려움, 임종환자와의 인간적 갈등의 순이었다. 임종간호에 대한 인지정도는 5점 만점에 평균 2.64점이었으며, 인지정도가 가장 낮은 문항은 죽음에 대한 아동의 이해였으며, 법적 측면, 진실통고에 대한 윤리적 근거의 순이었다. 임종간호에 대한 교육요구도는 5점 만점에 평균 3.25점으로 높았으며, 스트레스와 소진관리, 임종 전 관리, 의사소통 방법, 임종준비에 대한 의사소통, 마약성 진통제 관리에 대한 교육요구도가 높았다. 간호사의 일반적 특성에 따른 임종간호에 대한 스트레스는 유의한 차이가 없었으며, 임상경력이 길수록, 호스피스 교육기간이 길수록, 임종간호 횟수가 많을수록 임종간호에 대한 인지정도가 높았고, 암병동 근무기간이 길수록 임종간호에 대한 교육요구도가 높음을 알 수 있었다. 결론: 본 연구의 결과에서 임종간호에 대한 스트레스가 높고, 인지정도는 낮으며, 교육요구도가 높았던 내용을 토대로 하여 교육 프로그램을 개발, 적용하는 것이 필요하다.
