뇌졸중환자의 자가간호 수준과 가족구성원의 간호요구 (A Study on the Care Needs of Family-Caregivers and the Level of Self Care for Patients of Cerebral Vascular Accident(CVA))

  • The purpose of this study was to explore the care needs of family-caregivers caring for patients with a CVA and the level of self care of the patients. The subjects for the study were 112 patients with a CVA and their caregivers. These patients were seen in a hospital or out-patient-department(OPD) at two oriental medical hospital in Jeonbuk province. The survey instruments used in this study were Kang's ADL checklist for self care of patients and Kim's Likert-style checklist for care needs of family-caregivers to patient with CVA. The survey was conducted from July 4 to August 30, 1999. Internal validity by calculation of Cronbach's alpha was 0.95, which was regarded as high. The survey results were analyzed using the SPSS program, with percentages, means, t-test, ANOVA and Pearson's correlation coefficients. The results of this study are as follows : 1. The level of self care for patients with a CVA was : 1) complete dependence(M=14.9, 13.1%), 2) complete independence(M=23.6, 20.9%), 3) incomplete independence(M=23.9, 21.0%), 4) incomplete dependence(M=26.6, 25.0%), 5) dependence and independence(M=23.0, 20.0%). The items for which there was a high level of self care were : 1) drinking(M=3.62), 2) eating (M=3.25), 3) position returning(M=3.18) : and the items for which there was a low level of self care were : 1) ascending and descending stairs(M=2.08), 2) walking(M=2.47). 3) putting on and taking off trousers(M=2.55). 2. The mean score of the sum of the care needs of the family-caregivers was : 1) need for immediate care and help: 2) need of the way to communicate with patient: 3) need for education and assistance related to physical functional level: 4) need to be informed about the disease, treatment and care: 5) need for social support and consultation: 6) need for appreciation: 7) need for management of nursing problems related to immobility. The highest meed factor was the need for immediate care and help(M=3.47): and lowest need factor was the need for management of nursing problems related to immobility(M=2.80). 3. There were significant differences between the level of care need and general characteristic of the caregivers, there were family-caregivers age(P=0.001), marital status (P=0.276), occupation (P=0.006), monthly income(P=0.000), Patient's relationship to caregivers(P=0.004) and health(P=0.000). 4. There were significant differences between the level of self care and general characteristic of the patients, there were patient paralytic condition(P=0.01), blood pressure(P=0.01), and length of suffering(P=0.03). 5 There were significant differences between the level of care need and the general characteristic factors, which were CVA patient's blood pressure (P=0.05), problem of medical fee (P=0.05). 6. There was significantly correlation with the family-caregivers care need and the level of self care in the CVA patient(r=0.300, P=0.000). As a result, need to promote the level of self care in patients and to meet the care need of family-caregivers for more efficient nursing of CVA patients, is emphasized. Therefore more study is needed on an efficient way to provide rehabilitation and quality nursing interventions for family-caregivers and patients with CVA.

고등학생의 건강 및 삶의 질에 대한 진단적 연구 - PRECEDE 모형을 근간으로 - (A Diagnostic Study on High School Students' Health and Quality of Life - Based on the PRECEDE model -)

  • Health education, as the most fundamental concept for national health promotion, alms for developing the self-care ability of the general public. High school days are regarded as the period when most important physical, mental and social developments occur, and most health-related behaviors are formed. School health education is one of the major learning resources influencing health potential in the home and community as well as for the individual student. High school health education in Korea has a fundamental systemic flaw in that health-related subjects are divided and taught under various subjects areas at school. In order to achieve the goal of school health education, it is essential to make a systematic assessment of the learner's concerns connected with his health and life, and the factors affecting them. So far, most of the research projects that had been carried out for improving high school health education were limited in their concerns to a particular aspect of health. Even though some had been done in view of comprehensive school health education, they failed to Include a health assessment of the learner. Therefore, in this study the high school students' concerns related to health and life were investigated in the first place on the basis of the PRECEDE model, developed by Green and others for the purpose of a comprehensive diagnostic research on high school health education. This study was done in two steps : one was the basic study for developing research instrument and the other was the main one. The former was conducted at five high schools in Seoul and Cheongju for 2 months-beginning in March, 1996. The students were asked to respond to questions related to their health and lives in unstructured open-ended question forms. On the basis of analysis of the basic study, the diagnostic instruments for the quality of life, health problems, health behavior and educational factors were constructed to be used for the collection of data for main study. An expert panel and the pilot study were used to improve content validity and reliability of the instruments. The reliability of the instruments was measured at between .7697 and .9611 by the Cronbach $\alpha$. The data for this study were collected from the sample consisted of the junior and senior classes of twenty general and vocational high schools in Seoul and Cheongju for two months period beginning in July, 1996. In analyzing the data, both t-test and $X^2$-test were done by using SAS-$PC^+$ Program to compare data between the sexes of the high school students and the types of high school. A canonical correlation analysis was carried out to determine the relationships among the diagnostic variables, and a multivariate multiple regression analysis was conducted by using LISREL 8.03 to ascertain the influences of variables on the high school students' health and quality of life. The results were as follows : 1) The findings of the hypothesis tests (1) The canonical correlation between the educational diagnosis variables and behavioral, epidemiological, social diagnosis variables was .7221, which was significant at the level of p<.001. (2) The canonical correlation between the educational diagnosis variables and the behavior variables was .6851, which also was significant (p<.001). (3) The canonical correlation between the behavioral diagnosis variables and the epidemiological variables was 4295, which was significant (p<.001). (4) The canonical correlation between the epidemiological diagnosis variables and the social variables was .6005, which was also significant (p<.001). Therefore, the relationship between each diagnosis variable suggested by the PRECEDE model had been experimentally proven to be valid, supporting the conceptual framework of the study as appropriate for assessing the multi-dimensional factors affecting high school students' health and quality of life. Health behavior self-efficacy, the level of parents' interest and knowledge of health, and the level of the perception of school health education, all of which are the educational diagnostic variables, are the most influential variables in students' health and quality of life. In particular, health behavior self-efficacy, a causative factor, was one of the main influential variables in their health and quality of life. Other diagnostic variables suggested in the steps of the PRECEDE model were found to have reciprocal relations rather than a unidirectional causative relationship. The significance of this research is that it has diagnosed the needs of high school health education by the learner-centered assessment of variety of factors related to the health and the life of the students. This research findings suggest an integrated system of school health education to be contrived to enhance the effectiveness of the education by strengthening the influential factors such as self-efficacy to improve the health and quality of the lives of high school students.

중년여성의 삶의 의미와 영향요인 (Factors Influencing the Meaning of Life for Middle-aged Women)

  • The purpose of this study was to examine factors influencing the meaning of life for middle-aged women. The subjects for the study were 190 middle-aged women who live in Busan. Data were collected from May 25 to June 20, 1999. The instruments for this study were as follows : the meaning of life scale developed by Jung D. R(1978) ; the self-esteem scale developed by Ro, E. Y, Kwon, J. H.(1997) ; the volunteer activity scale developed by Park G. J.(1999), the marital satisfaction scale developed by Choi G. Y.(1999) ; and the health status scale developed by Kim S. Y. (1991). SPSS PC+ was utilized for data analysis. Data were analysed according to frequency and percentage, mean and standard deviation, t-test, ANOVA, Sheffe' s test and Pearson' s correlation coefficients and stepwise multiple regression. The reliability of the tools was tested by Cronbach's $\alpha$ : and it showed reliability scores of 0.88(for the meaning of life tool), 0.81(for the self-esteem tool), 0.94(for the marital satisfaction tool) and 0.78(for the health status tool). The results were as follows: 1) The scores showed that middle-aged women had a moderate level of meaning of life, with a mean of 30.31 and standard deviation of 6.24 (with values ranging from 12.00 to 48.00). The scores showed that middle-aged women had a moderate levels of self-esteem, with a mean of 28.92 and standard deviation of 4.67 (with values ranging from 10.00 to 40.00). The scores showed that middle-aged women had a very low level of volunteer activity, with a mean of 0.87 and standard deviation of 0.93(with values ranging from 0.00 to 1.00). The scores showed that middle-aged women had a moderate level of satisfaction in their marriages, with a mean of 31.99 and standard deviation of 7.84(with values ranging from 12.00 to 48.00). The scores showed that middle-aged women had a moderate level of health status, with a mean of 6.63 and standard deviation of 1.57(with values ranging from 3.00 to 9.00). 2) There was a statistically significant difference of the meaning of life of middle-aged women according to their religion(F=8.930, p=.000), christian ($31.94{\pm}5.96$) had more meaning th life than buddhists ($28.40{\pm}6.23$). 3) There was a statistically significant correlations between the subject' s self-esteem and the her meaning of life(r=0.477, p=0.000), the subject' s level of volunteer activity and meaning of life(r=.428, p=.000), her level of marital satisfaction and meaning of life(r=.417, p=.000), and her level of health status and meaning of life(r=.261, p=.000) among these middle-aged women. 4) Self- esteem was the highest factor influencing the level of meaning of life in middle-aged women. 40.0% of the total variance of levels of meaning of life by was dependent on self-esteem, volunteer activity, marital satisfaction and health status. In conclusion, the higher the self-esteem and levels of volunteer activity, marital satisfaction, and health status of middle-aged women, the higher the meaning of life for them. Therefore, it is necessary to elevate levels of the self-esteem and volunteer activity, marital satisfaction and health status in order to help middle-aged women have more meaning of life.

관절염환자의 삶의 질에 영향을 미치는 요인탐색 (Investigation on Factors Influencing the Quality of Life of Arthritis Patients)

  • In this paper, I will examine the variables influencing the Quality of Life of arthritis patients and present basic materials which help arthritis patients have positive thinking in life and ultimately lead a satisfactory life. The subjects for this study are 231 inpatients and outpatients with arthritis living in J and K city in Chonbug Province. For the analysis of collected data I employed the SAS program. The variables for characteristics and the quality of life were analysed by descriptive statistics, T-test and ANOVA, and the relations among variables were analysed through Pearson Correlation; the Regression method was employed to predict the factors affecting quality of life. For the validity of reliance on measuring equipment Cronbach Alpha was used. The results of the study are as follows : (1) The mean score of quality of life of arthritis patients is 3.09(5 in the maximum). The general characteristics which affect the quality of life are age(F=5.13, p=0.0006), standard of education(F=6.49, p=0.0003), marriage status(F=7.77, p=0.0005), monthly pay(F=4.37, p=0.0020), medical benefits (F=4.85, p=0.0087), and supports(F=4.39, p=0.0050). For the disease-related characteristics, there is a significant difference in the 6 items: pain control method(F=5.92, p= 0.0002), physical therapy(F=3.25, p=0.013), whethere or not patients exercise(F=4.62, p=0.0000), regularity of exercise(F=4.79, p=0.0000), frequency of exercise(F=6.29, p=0.0001), and amount of exercise(F=4.62, p=0.0043). Depending on the type of arthritis, there is also a significant difference in the degree of pain felt. The patients with infectious arthritis suffer from pain the most, followed by those with gout, rheumatism and degenerative arthritis, in that order. Although statistics don't show any convincing evidence, those with gout perceive that they are in best health condition, followed by those with rheumatism, degenerative arthritis, and infectious arthritis, in that order(F=2.23, p=0.0669). (2) The quality of life of arthritis patients is correlated positively with perceived health status(r=0.56, p=0.0001), health promoting behavior(r=0.53, p=0.0001), family support (r=0.46, p=0.0001), amount of exercise (r=0.36, p=0.0001), ADL(r=0.36, p=0.0001), HLOC(r=0.32, p=0.0001), frequency of exercise(r=0.32, p=0.0001)in that order, while correlated negatively with the degree of pain felt(r=-0.32, p=0.0001), the number of pain regions(r=-0.19, p= 0.0041), and the duration of pain(r=-0.14, p=0.0279). (3) Regression analysis reveals that the most powerful predictor of the quality of life is perceived health status, which account for 31.11%. The other predictors of the quality of life, which account for 60.22%, are health promoting behavior(16.51%), family support(3.81%), ADL(2.52%), gender(1.86%), the number of family members(1.36%), level of pain(1.24%), duration of pain (1.08%), and level of education(0.67%). The results of the study show that perceived health status and health promoting behavior are the two most important variables. However, considering that the perceived health condition is difficult to control by nursing intervention, it is suggested that the level of expectation for patients, must be decided first, and the health promoting behavior and the family support influencing the quality of life must be taken into account as targets for nursing intervention. As a way of controlling the quality of life, I think that a more comprehensive approach comprising the above important variables along with demographic and general characteristics is needed. I also suggest that we must continue to explore the variables affecting the quality of life and include those variables in nursing intervention.

산업체급식 메뉴 만족도 조사도구의 활용에 대한 사례연구 (Case Study of Menu Satisfaction Index in Business & Industry Food Service)

  • 본 연구에서는 산업체 급식소의 메뉴 만족도를 평가할 수 있는 조사도구를 개발하고 개발된 조사도구를 이용하여 산업체 급식소를 대상으로 고객들의 메뉴 만족도를 조사함으로써 도구의 활용 사례를 제시하였다. 그 내용을 요약하면 다음과 같다. 첫째, 문헌고찰, 전문가 및 고객 대상 면담, 전문가 검토, 신뢰도 및 타당도 검증의 3단계로 메뉴 만족도 조사도구를 개발하였다. 메뉴 품질 속성 15개에 대한 문항과 메뉴에 대한 전반적인 만족도를 측정하는 1개 문항에 대해 고객의 만족도를 매우 불만족, 불만족, 보통, 만족, 매우 만족의 Likert 5점 척도로 자가 평가하도록 하였다. 개발된 도구는 신뢰도와 타당도가 검증되었다. 둘째, 개발된 메뉴 만족도 조사도구를 이용하여 산업체 급식소 1곳을 대상으로 메뉴 만족도를 조사하였다. 15개 속성 모두에 대해 '보통'(3점) 이상의 높은 점수를 보임으로써 고객이 메뉴 품질에 대해 대체로 긍정적인 인식을 하고 있었다. 특히 '적절하게 익혀짐', '적절한 1인분량', '뜨거운 음식은 뜨겁게 제공' 항목에서는 3.53점으로 만족도가 가장 높은 특성으로 규명되었다. 메뉴 만족도 평가 요인 4개 중 '요인 3. 온도 적정성'(3.52점)에 대해 가장 만족도가 높았으며, 그 다음으로 '요인 4. 형식충족성' 3.46점, '요인 1. 음식우수성' 3.35점, '요인 2. 웰빙지향성' 3.31 순이었다. 셋째, 고객 특성에 따른 메뉴 만족도를 분석한 결과, 연령과 소속사에 따라 메뉴 만족도의 일부 요인에 유의적 차이를 보였으며, 음식의 질 평가에 따른 군과 급식전체에 대한 군에 따라서도 유의적 차이를 보였다. 넷째, 메뉴 만족도 요인과 전반적인 메뉴 만족도간에 유의적인 정(+)의 상관관계가 있었다. 다중회귀분석 결과, 4개 요인 중 '요인 1. 음식우수성'(p<.001), '요인 2. 웰빙지향성'(p<.01), '요인 4. 형식충족성'(p<.01) 순으로 '전반적인 메뉴 만족도'에 영향을 미치는 것으로 분석되었다. 이상과 같이 고객의 선택권을 줄 수 있는 푸드코트 형식의 산업체 급식소 1곳을 선택하여 개발된 메뉴 만족도 도구의 활용 가능성을 탐색한 결과, 다음과 같은 메뉴 운영에 대한 총평 및 제언을 제시하고자 한다. 15개 문항 모두 만족도가 3점 이상(리커트 5점 척도 중)으로 조사되어 조사대상 급식소의 메뉴 품질에 대해 고객들이 대체로 만족하고 있으며, 메뉴 관리가 잘 되고 있다고 평가할 수 있다. 이는 새로운 급식소 운영 형태인 푸드코트가 고객에게 만족감을 유도하는데 일조한 것으로 여겨진다. 특히 메뉴의 1인 분량, 온도, 가격에 대해서는 만족하고 있으나 위생이나 신선도, 맛, 건강에의 도움 등에 대해서는 만족도가 상대적으로 떨어지는 것을 알 수 있다. 온도의 적절성은 가장 높은 만족을 보이지만 전반적인 메뉴 만족도에는 유의하게 작용하지 못하는 것으로 나타나 메뉴로서 당연히 갖추어야 할 요소로 사료된다. 또한 어떠한 급식 분야든 상관없이 영원한 과제로 여겨지는 음식의 맛에 대해서는 꾸준한 개선의 노력이 필요하다. 매스컴에서 음식 및 건강관련 보도가 잇따르고 있고, 고객 스스로도 건강에 대한 관심이 고조되고 있는 이때, 산업체 급식소에서 맛있는 음식제공과 더불어 건강과 관련된 급식서비스가 병행된다면 고객의 메뉴 만족도는 증대될 수 있을 것이다. 예를 들어 제공되는 음식별 영양소 함량 혹은 특정 질환 예방에 도움됨을 제시하거나 그날 메뉴의 영양 정보, 건강 정보를 게시하고, 급식 시간 외에 영양사와의 영양 상담을 통해 고객 맞춤식 영양 및 건강 정보를 제공하는 등의 방법이 제안될 수 있다. 본 연구에서는 개발된 메뉴 만족도 도구를 1개 급식소에 시범 적용해 본 것으로 산업체 급식소 전반에 일반화하기 위해서는 무리가 있다. 따라서 향후 연구에서는 다양한 형태의 산업체 급식소를 대상으로 메뉴 만족도를 조사하여 비교함으로써 메뉴만족도 도구를 검증하고, 급식 유형별 만족도의 차이를 분석하여 문제점으로 도출되는 메뉴 품질 속성에 대해 개선방향을 제시해 보는 것이 필요하다.

대형 유통업체 윤리적 리더십의 선행변수에 관한 연구 : 할인점과 공급업체 간 관계를 중심으로 (A Study on Antecedents of Ethical Leadership of Power Retailers, : Focusing on the Relationship between Discount Stores and Their Suppliers)

  • 본 연구는 최근 인사조직 분야에서 집중적인 조명을 받고 있는 윤리적 리더십을 유통경로 연구에 적용함으로써, 그 동안 그 중요성에 비해 덜 조명되었던 유통경로 내 조직 간 리더십에 대한 이해를 증진시키고자 한다. 특히, 본 연구는 비교적 최근 밝혀지기 시작한 윤리적 리더십의 선행변수에 초점을 맞춤으로 유통경로의 구성원 간 관계에서 윤리적 리더십이 형성되기 위해서는 어떻게 해야 하는 지를 규명하고자 한다. 이를 위해 본 연구는 조직의 성격특성과 관계유지전략이라는 두 가지 이론적 차원의 변수들과 실무자들과의 심층면접을 통해 중요하게 언급된 공급업체 관리활동 변수들을 실증하였다. 국내 6대 할인점에 상품을 공급하는 공급업체 295개 사의 납품 담당자를 대상으로 설문조사를 실시하였고, PLS(Partial Least Square) 구조모형 분석을 통해 분석한 결과, 대형 할인점의 조직특성 중 양심이, 관계유지 전략 중 개방전략, 갈등관리전략, 사회망전략이, 공급업체 관리활동 중 교육훈련과 공정한 보상이 대형 할인점의 윤리적 리더십을 강화하는 것으로 나타났다. 반면, 대형 할인점의 정서적 불안정은 윤리적 리더십을 약화시켰고, 친절성과 보장전략, 조직간 커뮤니케이션은 윤리적 리더십에 유의적인 영향을 미치지 않았다.

폐결핵 환자의 치료 순응과 관련된 요인 (Factors Influencing Compliance with Anti-Tuberculosis Therapy)

  • 항결핵치료 시작 후 3개월간의 치료 경과와 치료 순응과의 관련성을 조사하기 위하여 3개 보건소(대구시 달성군, 경주시, 구미시)에 1995년 10월 15일 현재 단기 6개월 표준처방으로 치료 시작 후 3개월 이상 경과된 초치료 결핵환자 중 계속적인 추적관찰에서 누락된 10명을 제외하고 104명의 자료를 분석한 결과는 다음과 같다. 연구 대상자 104명 중 호전군은 80.8%였고, 비호전군은 19.2%였다. 호전군과 비호전군간에 성, 연령, 교육 수준, 직업, 동반 가족유무, 흡연 및 음주습관의 변화 등에 유의한 차이는 없었고, 평균 연령은 비호전군이 51세로 호전군 42세보다 많았으나 유의 한 차이는 아니었다. 호전군에서는 약제의 복용을 식전과 식후 30분에 복용하는 경우(p<0.05), 약의 복용을 잊지 않도록 가족들이 챙겨 주는 경우(p<0.05), 약을 규칙적으로 복용하는 경우(p<0.01), 항결핵치료 도중 의문점을 가족이나 결핵담당자와 상의하는 경우(p<0.01) 등이 유의하게 많았다. 비호전군에서는 당뇨병이나 위장 질환 등을 않고 있는 경우가 유의하게 많았다(p<0.01). 결핵에 대한 지식점수는 호전군이 12.9로 비호전군 10.6보다 유의하게 높았고(p<0.01), 지식정도의 문항에 대한 신뢰도 검증 결과 신뢰성$(Cronbach'\alpha:0.703)$이 있었다. 치료경과를 호전군과 비호전군으로 나누어 종속변수로 놓고 시행한 다중 지수형 회귀분석에서는 약의 복용을 잊지 않도록 가족들이 챙겨주는 경우(p<0.05)가 유의한 변수로 채택되었고, 결핵에 대한 지식이 높은 경우(p=0.054), 약을 규칙적으로 복용하는 경우(p=0.062), 항결핵치료 도중 의문점을 가족이나 결핵담당자와 상의하는 경우(p=0.075)는 경계역의 변수로 채택되었다. 결론적으로 약을 규칙적으로 복용할 수 있도록 항결핵치료에 대한 신념을 불어넣고 경각심을 높임으로써 환자의 치료 결과를 향상시킬 수 있을 것으로 생각되며, 향후 단기 6개월 표준요법에 의한 치료에 있어서 치료시작 후 3개월간의 치료 경과로 치료 결과를 예측하고 환자의 치료순응을 평가하기 위한 변수로 사용하기 위해서는 보다 많은 결핵환자를 대상으로 한 연구가 필요할 것으로 생각된다. 또한 환자의 치료 순응을 높이기 위하여 환자에 대한 교육과 의사 또는 결핵관리자와 환자, 조력자간의 강한 유대 관계의 형성에 도움을 줄 수 있는 방안의 모색이 필요할 것으로 생각된다.

정보서비스품질이 고객로열티에 미치는 영향에 관한 연구: 고객관계관리 관점 (The Effect of Information Service Quality on Customer Loyalty: A Customer Relationship Management Perspective)

  • As managing customer relationship gets more important, companies are strengthening information service using multi-channels to their customers as a part of their customer relationship management (CRM) initiatives. It means companies are now accepting such information services not as simple information -delivering tools, but as strategic initiatives for acquiring and maintaining customer loyalty. In this paper, we attempt to validate whether or not such various information services would impact on organizational performance in terms of CRM strategy. More specifically, our research objective is to answer the next three questions: first, how to construct the instruments to measure not information quality but information service quality?; second, which attributes of information service quality can influence corporate image and customer loyalty?; finally, does each information service type have unique characteristics compared with others in terms of influencing corporate image and customer loyalty? With respect to providing answers to those questions, the previous studies had been limited in that those studies failed to consider the variety of types of information service or restricted the quality of information service to information quality. An appropriate research model answering the above questions should consider the fact that most companies are utilizing multi channels for their information services, and include the recent strategic information service such as customer online community. Moreover, since corporate information service could be regarded as a type of products or services delivered to customer, it is necessary to adopt the criteria for assessing customer's perceived value when to measure the quality of information service. Therefore, considering both multi-channels and multi-traits may enable us to tell the detailed causal routes showing which quality attributes of which information service would affect corporate image and customer loyalty. As information service channels, we include not only homepage and DM (direct mail), which are the most frequently applied information service channels, but also online community, which is getting more strategic importance in recent years. With respect to information service quality, we abstract information quality, convenience of information service, and timeliness of information service through a wide range of relevant literature reviews. As our dependant variables, we consider corporate image and customer loyalty that both of them are the critical determinants of organizational performance, and also attempt to grasp the relationship between the two constructs. We conducted a huge online survey at the homepage of one of representative dairy companies in Korea, and gathered 367 valid samples from 407 customers. The reliability and validity of our measurements were tested by using Cronbach's alpha coefficient and principal factor analysis respectively, and seven hypotheses were tested through performing correlation test and multiple regression analysis. The results from data analysis demonstrated that timeliness and convenience of homepage have positive effects on both corporate image and customer loyalty. In terms of DM, its' information quality was represented to influence both corporate image and customer loyalty, but we found its' convenience have a positive effect only on corporate image. With respect to online community, we found its timeliness contribute significantly both to corporate image and customer loyalty. Finally, as we expected, corporate image was revealed to provide a great influence to customer loyalty. This paper provides several academic and practical implications. Firstly, we think our research reinforces CRM literatures by developing the instruments for measuring information service quality. The previous relevant studies have mainly depended on the measurements of information quality or service quality which were developed independently. Secondly, the fact that we conducted our research in a real situation may enable academics and practitioners to understand the effects of information services more clearly. Finally, since our study involved three different types of information service which are most frequently applied in recent years, the results from our study might provide operational guidelines to the companies that are delivering their customers information by multi-channel. In other words, since we found that, in terms of customer loyalty, the key areas would be different from each other according to the types of information services, our analysis would help to make decisions such as selecting strengthening points or allocating resources by information service channels.

일반인의 통증관리 장애정도 (Patient-related Barriiers to Pain Management in General Population)

  • 목적: 효과적인 통증관리의 장애요인은 크게 제도적 측면, 의료진 측면, 그리고 환자 측면으로 구분되며, 이는 모두 사회적 통념에 영향을 받는다. 따라서 본 연구는 일반인의 통증관리 장애요인을 규명하여 궁극적으로 우리 사회의 통증관리에 대한 이해를 향상시키기 위한 기초 자료를 마련하고자 시도되었다. 방법: 2007년 5월 15일부터 6월 15일까지 자원봉사자, 교사 및 지역사회 주민 163명을 대상으로 Gunnarsdottir, Donovan, Serlin, Voge와 Ward (2002)가 개발한 Barriers Questionnaire II (BQ-II)를 유양숙, 이원희, 조옥희와 이소우(2005)가 번한 보완한 통증관리 장애척도를 사용하여 자료를 수집하였다. 통증관리 장애척도의 Cronbach's $\alpha$는 .88이었다. 결과: 대상자의 평균연령은 53.36세이며, 92.6%가 통증교육을 받은 경험이 없었다. 대상자의 통증장애 정도는 평균 2.51점이었으며, 중독을 포함하는 해로운 영향 영역이 3.03점, 부작용을 포함하는 신체적 효과 영역 2.73점, 숙명론적 태도 영역 2.15점, 그리고 의사소통 영역 1.71점 순이었다. 문항별로는 '진통제를 사용하면 중독의 위험이 있다'의 장애정도가 가장 높았고, 다음은 '진통제를 사용하면 새로운 통증이 생겼을 때 알기가 어렵다', '진통제를 사용하면 몸이 익숙해져 곧 진통효과가 없을 것이다', '진통제로 인한 졸림은 조절하기 어렵다', '진통제는 면역력을 떨어뜨린다' 순이었다. 통증교육을 받은 경우의 통증관리 장애정도는 교육을 받지 않은 경우보다 유의하게 낮았다. 결론: 일반인들의 통증관리 장애요인은 환자나 가족과 유사하게 진통제 중독 및 내성에 대한 염려가 가장 높았으므로 사회적으로 확산되어 있는 통증관리 및 진통제 사용에 대한 올바른 이해를 높이기 위한 교육 및 홍보가 필요하다.

암 환아 가족의 아동 호스피스 요구도 (The Need for Child Hospice Care in Families of Children with Cancer)

  • 목적: 본 연구는 암환아 가족의 호스피스 요구도를 파악하여 앞으로 암 환아 간호시, 대상자의 요구에 적절한 호스피스 간호를 제공하기 위한 목적으로 시도되었다. 방법: 자료수집은 2004년 2월부터 7월까지 서울의 2개 대학병원에서 암으로 진단 받고 입원한 아동의 가족 104명을 대상으로 하였다. 대상자의 아동 호스피스 요구도를 측정하기 위해서는 아동 호스피스에 대한 국내 외 문헌고찰과 아동 호스피스 전문가와 관련자, 암 환아 가족 20명을 대상으로 면담하여 개발한 암 환아 가족의 아동 호스피스 요구 측정도구[10]를 사용하였다. 결과: 1. 대상자의 아동 호스피스 요구 정도는 4점 만점에 $2.77{\sim}4.00$점의 범위로 평균 3.41점(.38)으로 높게 나타났다. 각 요인에 따른 요구정도는 아동의 정서적 간호 말기 주요 신체적 증상의 조절, 2차적인 생리적 문제의 조절, 가족이 어려움 수용, 죽음준비를 위한 영적 돌봄의 순으로 나타났다. 2. 대상자의 일반적 특성에 따른 아동 호스피스의 요구 정도는 아동 어머니의 연령(F==4.980, P=.009), 형제나 친척 중 암 환자의 유무(t=2.423, P=.017)에 따라서만 유의한 차이가 있었다. 즉, 대상자의 아동 호스피스 요구정도는 어머니의 연령이 $36{\sim}40$세인 경우가 요구 정도가 가장 높고 35세 이하인 경우가 가장 낮았으며 형제나 친척 중 암 환자가 있는 경우가 없는 경우보다 요구 정도가 더 높았다. 결론: 아동 호스피스에 대한 암 환아 가족의 요구 정도는 정서적, 신체적, 사회적, 영적 돌봄의 순으로 나타나 가족이 자녀의 죽음을 앞두고 자녀의 신체적 증상관리와 함께 자신의 죽음에 대한 이해가 어려운 자녀의 불안과 두려움을 경감시켜주기 원하는 부모의 특성이 파악되었다. 또한 호스피스에 대한 요구도는 높았으나 자녀에 대한 호스피스 돌봄의향은 낮게 나타나 암 환아 부모들이 적극적인 치료와 호스피스에 대한 양가감정을 가지고 있음을 유추해 볼 수 있었다. 본 연구의 결과는 암 환아 가족들을 위해 간호사가 제공해야 하는 호스피스 돌봄의 방향을 제시하고 있다고 생각되며 이를 위해서는 호스피스 관련 기관뿐만 아니라 국가적 차원의 아동 호스피스에 대한 관심과 지원이 요구된다고 생각한다.
