Comparison of Health Status and Mini Nutritional Assessment according to Self-esteem of Elderly in Care Hospital (요양병원 입원 노인의 자아존중감에 따른 건강상태와 간이영양상태 비교)

  • The purpose of the study was to examine health status and mini nutritional assessment (MNA) according to self-esteem status of elderly in care hospital. The survey was carried out from $13^{th}$ to $31^{st}$ of September, 2014 in five care hospitals. Analysis was performed for 226 subjects. Evaluation criteria included demographics, clinical status, MNA, health-related quality of life (EQ-5D) and self-esteem. The SE I, SE II and SE III groups were divided by self-esteem scores. Data were analyzed one-way ANOVA, Duncan' multiple range test and Pearson's correlation using SPSS 15.0. Education level, religion, dental condition and medical endurance type were significantly different in all groups. Many elderly people were normal BMI ($20.94{\sim}21.67kg/m^2$). Self-esteem significantly increased in order of SE I, SE II and SE III. Mobility, usual activity and anxiety or depression were significantly different in all groups, and EQ-5D of SE II and SE III groups were significantly higher than that of SE I group. Comparison of MNA screening score in elderly patients was as follows : Score for food intake, psychological stress or acute disease, neuropsychological problems, pressure scores or skin ulcer, protein intake, mode of feeding, nutritional status and health status in SE I group were significantly lower than those of SE II and SE III groups. Mobility and drug intake of SE I group were significantly lower than those of SE III groups. Fruit and vegetable intake SE I group were significantly higher than those of SE II and SE III groups. MIS (Malnutrition Indicator Score) was significantly different among the SE I, SE II and SE III groups. We conclude that self-esteem score is positively correlated with protein intake, nutrition status, health status and MIS in elderly care hospital. To improve nutritional status of elderly in care hospitals, systematic nutrition management and self-esteem education program should be implemented.

Evaluation of Habitat Function of National Park Based on Biodiversity and Habitat Value (보호지역의 지정 및 관리를 위한 국립공원의 서식처 기능 평가 -생물종다양성과 서식처 가치에 기반하여-)

  • National parks are designated for the purpose of maintenance, conservation and utilization of different habitats. Therefore, it is necessary to select habitats of a high value as a protected area in order to balance conservation and development. However, the existing national park boundary adjustment and new designation criteria only focused on the endangered species and protected area, without proper evaluation of the habitat value of actual species. Therefore, this study aims to quantitatively evaluate habitat function in terms of biodiversity and habitat value, so that it can be referred to for the designation and boundary adjustment of national parks. We assessed species diversity and habitat values for each of the habitat types, for mammals only, as they are able to choose preferred habitats. In order to evaluate biodiversity, we used Maxent to derive species richness map and used InVEST's Habitat quality model to evaluate habitat value. As a result of evaluation, species richness was high in the national park boundary area. Also, even if the same edge is adjacent to the development area depending on the land cover, the species richness is low. Compared with Wolaksan and Sobaeksan National Park, the species richness and habitat value of the northern area, which is connected with other forests, were higher than those of the southern area where roads were developed. Therefore, it is expected that the use of the result of this study for the national park boundary adjustment and management will enhance the function of the national park as a habitat.

Ecological Assessments of Aquatic Environment using Multi-metric Model in Major Nationwide Stream Watersheds (우리나라 주요하천 수계에서 다변수모델을 이용한 생태학적 수환경 평가)

  • The objective of this research was to develop ecological multi-metric models using natural fish assemblages for a diagnosis of current stream health condition, and apply the model to nationwide lotic ecosystems of the Geum River, the Youngsan River, and the Sumjin River. The ecological stream health model was based on the index of biological integrity (IBI), which was originally developed in North American streams by Karr (1981), and the Rapid Bioassessment Protocol (RBP), which was scientifically established by the US EPA (1999). The metric numbers and metric attributes were partially changed for the regional applications, so the scoring criteria was modified for the assessment. Overall, metric values, based on the IBI calculations, reflected conventional water quality characteristics, based on nutrient regime, and agreed with results of staticeco-toxicity tests. Some stations impaired in terms of stream health were identified by the IBI approach, and also major key stressors affecting the stream health were identified by additional evaluations of physical habitats. Our preliminary results suggested that biological integrity in stream ecosystems was largely disturbed by habitat degradation as well as chemical pollutions. This new approach would be used as a key tool for ecological restorations and species conservations in the degraded aquatic ecosystems in Korea and applied for elucidating major causes of ecological disturbances. Ultimately, this approach provides us an effective management strategy of stream ecosystems through establishments of ecological networks in various watersheds.

Application of Ecosystem Model for Eutrophication Control in Coastal Sea of Saemankeum Area -2. Quantitative Management of Pollutant Loading- (새만금 사업지구의 연안해역에서 부영양화관리를 위한 생태계모델의 적용 -2. 오염부하의 정량적 관리-)

  • One of the most important factors that cause eutrophication is nutrient materials containing nitrogen and phosphorus which stem from excreation of terrestial sources and release from sediment. Therefore, to improve water quality, the reduction of these nutrients loads should be indispensible. At this study, the three-dimensional numerical hydrodynamic and ecosystem model, which was developed by Institute for Resources and Environment of Japan, were applied to analyze the processes affecting the eutrophication. The residual currents, which were obtained by integrating the simulated tidal currents over 1 tidal cycle, showed the presence of a typical counterclockwise eddies between Gyewha and Garyuk island. Density driven currents were generated westward at surface and eastward at the bottom in Saemankeum area where the fresh waters are flowing into, The ecosystem model was calibrated with the data surveyed in the field of the study area in annual average. The simulated results were fairly good coincided with the observed values within relative error of $30\%$. The simulations of DIN and DIP concentrations were performed using ecosystem model under the conditions of $40\~100\%$ pollution load reductions from pollution sources. In study area, concentration of DIN and DIP were reduced to $59\%$ and $28\%$ in case of the $80\%$ reduction of the input loads from fresh water respectively. But pollution loads from sediment had hardly affected DIN and DIP concentration, The $95\%$ input load abatement is necessary to meet the DIN and DIP concentration of second grade of ocean water quality criteria.

Development of an Nursing Assessment Tool for Emergency Patients (응급환자 간호사정 도구개발)

  • Nurses use a nursing process. that is a scientific approach method. in order to understand the clients' condition and to solve their problems. Professional nurses have accepted the nursing process as a standard framework for nursing activities, and the first step of the nursing process is nursing assessment. The purpose of this study was to develop a nursing assessment tool for emergency patients. Assessment is a first step in the nursing process, and scientific assessment helps to better understand the patients. Thus the development of an assessment tool for emergency patients will improve of nursing quality and advance to efficacy of emergency room(ER) management. The study involved the development of a conceptual framework, preliminary tool, content validity verification process. and reliability verification process. The conceptual framework was developed through a review of the literature. and preliminary tool was developed from the conceptual framework that based on care flow sheet' developed James A. Haley. The preliminary tool was evaluated for validity and reliability by seven experts, one ER nurse manager, two ER head nurses, two emergency medicine doctors. and two ER charge nurses and by ten nurses with one to five year careers in ER nursing. The results of this study was as follows 1. The conceptual framework which was developed was composed of three parts. They were triage criteria, first assessment and secondary assessment. 2. The preliminary tool which was developed had 31 items based on the conceptual framework. 3. To test expert validity a 4-point scale was used. items which had gained 3-4 points from six persons of the seven experts were selected. All 31 items from the preliminary tool were selected. 4. The reliability test was done by ten nurses educated in the use of the assessment tool. Two pairs of nurses simultaneously assessed the same patients. A total agreement percentile was calculated and result was 87.3% agreement. 5. After the validity and reliability testing. the final emergency patient nursing assessment tool was finalized with 31 items. and a check-list for the responses. This study concludes that the tool which was developed is both valid and reliable will advance quality care for emergency patients. This emergency nursing assessment tool was also found to be an adequate tool for assessment of emergency patients.

A Case Report of Advanced Non-Small Cell Lung Cancer Patient Treated with Samchilchoongcho-Jung in Conjunction with Alectinib (알렉티닙과 삼칠충초정을 병용하여 호전된 진행성 비소세포성 폐암환자 1례)

  • Objective: The purpose of this study is to report the clinical effectiveness of advanced non-small cell lung cancer (NSCLC) with Samchilchoongcho-Jung (HAD-B1) in conjunction with Alectinib. Methods: The patient was diagnosed with Anaplastic lymphoma kinase (ALK) mutated (2+) non-small cell lung cancer adenocarcinoma stage IV, suffering from edema of lower extremities, dyspnea, pleural effusion, general weakness, insomnia. The patient being treated with Alectinib was treated with Samchilchoongcho-Jung (HAD-B1) for disease control and symptom management. The clinical outcomes were measured by National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Numeral rating scale (NRS) and Eastern Cooperative Oncology Group (ECOG). Results: After treatment, dyspnea and edema of lower extremities was relieved from NRS 7 to 5, and 6 to 1 respectively. And ECOG score of the patient was improved from grade 3 to 2. During and after treatment, we didn't find any severe toxicities on laboratory findings. Conclusion: This case study suggests that Samchilchoongcho-Jung (HAD-B1) may improve symptom relief and life quality of NSCLC patient in conjunction with Alectinib.

Study on Potential Feasibility of Biomethane as a Transport Fuel in Korea (수송용 대체연료로서 바이오메탄의 잠재적 타당성 연구)

  • Biogas production and utilization are an emerging alternative energy technology. Biogas is produced from the biological breakdown of organic matter through anaerobic digestion. Biogas can be utilized for various energy sectors such as space heating, electricity generation and vehicle fuel. Especially, to be utilized as vehicle fuel, raw biogas needs to be upgraded that is mainly the removal of carbon dioxide to increase the methane content up to more than 95 ~ 97 vol% in some cases, similar to the composition of fossil-based natural gas. Usage of Biogas as a fuel of vehicles have an effect of reducing $CO_2$ emission compared to fossil fuels. Biomethane which is produced by upgrading of biogas is regarded as a good alternative energy and usage of clean energy is encouraged to deal with air pollution and waste management as well as production of clean energy. Recently, biogas projects for vehicle fuel are newly being launched and Korea government have also announced a plan for investment to develop biogas as a transport fuel. In this study, it is aimed to examine the potential feasibility of biomethane as a transport fuel. As a results, the status of biomethane, quality standard, quality characteristics, and upgrading technology of biogas were investigated to evaluate of biogas as a vehicle fuel of transportation.

Geochemical Characteristics of Surface Sediments and an Evaluation of Trace Metal Pollution in Gomso Bay, Korea, 2011 (2011년 곰소만 표층퇴적물의 지화학적 특성 및 중금속 오염도 평가)

  • To understand the geochemical characteristics of Gomso Bay, which features extensive Manila clam, we measured various geochemical parameters, organic matter, and trace metals (Cu, Cd, Pb, Zn, Cr, Hg, As and Fe) of intertidal and subtidal surface sediments in 2011. The surface sediments consisted of sedimentary facies including gravel (0.21%), sand (61.1%), silt (32.1%), and clay (6.5%). The chemical oxygen demand (COD) and acid volatile sulfide (AVS) values in most areas were below sediment quality criteria (COD, $20mg/g{\cdot}dry$; AVS, $0.2mg/g{\cdot}dry$). Trace metals in the surface sediments were below pollution thresholds, except for As (morderately polluted). Sediment quality was evaluated using the trace metal pollution load index (PLI) and ecological risk index (ERI), which showed that sediments were generally not polluted and at low risk; however, values along the outer bay were higher. We expect these results will be valuable for sustainable aquaculture prodution and environmental management in Gomso Bay.

Influence of Spa Sewage on the Water and Soil Pollution and Restoration I. Influence of Spa Sewage on the Pollution of Stream Water and Agricultural Land (온배수 유입 소형하천의 수질 및 토양오염과 회복에 관한 연구: I. 온배수가 인근 소하천과 농업 환경에 미치는 영향)

  • This study was carried out to investigate the influence of spa sewage on water quality and chemical contents in the paddy soil along stream from 1997 to 1998. Concentration of $PO_4, SO_4, Cl, NH_{4}, Ca, Na$ and COD in the spa sewage were lower than standard for agricultural usage, and were lowered as the sewage flew to the into stream. The concentration of $SO_4$in spa sewage was over the criteria for agricultural usage in the inlet, but was lower than criteria for agricultural usage by inlet of non polluted stream water. Concentration of pollutants in the sediment of water channel were the highest in the inlet site. There were no pollutants accumulation in the paddy soil where spa sewage was irrigated. It may be resulted from nutrients uptake of rice plant and self purification of paddy soil. On the while, considering electric conductivity and nitrate in spa sewage, this results suggest that long-term irrigation of the spa sewage may be required general management with some decreasing fertilization.

Model Development of Illness Perception and Consequences in Breast Cancer Patients

  • Breast cancer is the first or second malignancy among women worldwide. Illness perception (IP) and quality of life (QoL) are major issues considering breast cancer management. An attempt was here made to inspect the predictive variables influencing IP and their impact on QoL in cancer patients. The key predictors adapted from previous studies including life satisfaction, perceived social support, self-esteem, hope, optimism, and spiritual well being were taken into account. Our sample included 200 female volunteers suffering from breast cancer applying exclusion criteria. The data were collected using various questionnaires and statistically analyzed by means of path analysis and structural equation modeling. The results revealed that of the six predictors, spiritual well being and social support had direct effects on QoL and IP. The only path with significant indirect correlation with IP was social support. Spiritual well being had the second significant direct effect on IP. Self-esteem occupied the third rank in direct effects on both QoL and IP. In conclusion, here in Iran and most likely in other strongly religious communities, spiritual intervention is an effective strategy to raise QoL. Also social support helps women suffering from breast cancer to experience better conception and coping strategies.
