Body Contouring of Breast and Abdomen with Belt Dermolipectomy after Massive Weight Loss: A Case Report (과다 체중 감량 환자에서의 허리띠 피부지방절제술의 치험례)

  • Purpose: Obese proportion is increasing universally, estimating more than a billion. So reducing the weight became one of the topic in medical market. Not only diet, exercise, medication, but also many surgical procedures are being developed, such as sleeve gastrectomy, gastric bypass surgery. After massive weight loss, skin excess and laxity occurs, leading to unsatisfying body contour. Body contouring surgery including abdominoplasty, breast reduction is performed when skin excess is present in abdomen and breast. When skin excess is present circumferentially, belt dermolipectomy is the treatment of choice. Methods: A 23-year-old man had weight gain since he was 12 of age. A year before visiting to our department, his height was 168 cm, weight was 150 kg and body mass index (BMI) was 53.15 kg/$m^2$. The patient lost 55 kg of his weight through exercise and diet control. When he visited again, his weight was 95 kg and BMI was 33.66 kg/$m^2$. In physical examination, skin excess and laxity was seen in both breast and abdomen circumferentially and lateral folds were seen in the back. Abdominal contour deformity (Pitman classification type 6) and pseudogynecomastia (grade 3) were present in both breast. Belt dermolipectomy of abdomen, both breast and lateral folds was performed, resecting 6,400 g of tissue and additive 1,200 g through revisional operation. Results: The patient lost 6,500g of his weight and BMI reduced by 2.3 kg/$m^2$. The patient's hospital course was uneventful during 5 weeks of hospitalization and he was satisfied with his final body contour. Conclusion: Body contouring with belt dermolipectomy in patient who has circumferential skin excess and laxity after massive weight loss can be a treatment of choice.

Classification of Characters out of their Original Cultural Forms to Create Digital Contents : Focusing on Avalokitesvara (觀音菩薩) and Buddhist child (童子) (디지털콘텐츠 제작을 위한 전통문화원형의 캐릭터 분류 : 관음보살(觀音菩薩) 및 동자(童子)를 중심으로)

  • In the midst of the fusion era in digital contents, securing a variety of traditional cultural sources is important. Out of its long history, Korea has extensive spectrum of cultural contents in combination with conventional religions. Buddhist art has been a pillar of the traditional culture for a long time in Korea, by which we could secure new sources for cultural contents and classify them. This is aimed that we could bring the future brands into reality based on the traditional culture through Buddhist cultural contents. This study hired the original cultural forms in the viewpoints of traditional culture. Study on the implication through the symbols of Avalokitesvara (觀音菩薩) and Buddhist child (童子) in Buddhist art might allow to secure a foothold to create own characters. Thus, focusing on Avalokitesvara (觀音菩薩) and Buddhist child (童子) in the Buddhist sacred images as registered in Cultural Heritage Administration for national treasures, those traditional characters were reviewed through their own system of symbol. During the course, Buddhist characters were available to be classified into DB by their roles and materials지물. This study could help to exploit the complicated Buddhist culture of its symbolic meanings and to shape them into original forms of the culture. Also, through the meanings of the Buddhist sacred images, it could provide the digital contents pool with Korean traditional characters.

External and Internal Morphological Standard of original plants and herbal states in Pogostemonis and Agastachis Herba (광곽향(廣藿香)과 토곽향(土藿香)의 외부(外部) 및 내부형태연구)

  • Pogostemonis and Agastachis Herba are the whole of Pogostemon cablin (BLANCO) BENTH or Agastache rugosa (FISCHER et MEYER) O. KUNTZE (family Labiatae) which is produced in all part of Korea and China. This drug is used for removing dampness by means of aromatics in oriental medicine. The standard formula of this drug is important from the viewpoint of the quality control. A characteristic discrimination of internal and external morphological standard in original plants and herbal states of Pogostemonis and Agastachis Herba are as follows. 1. The external characteristics: Pogostemon cablin has hairs and brown-like in stem, elliptical fruit. In the other hand, Agastache rugosa has no hairs and red-like in stem, obovatic trigone fruit. 2. The physical characteristics: Pogostemon cablin is gray in whole, has hairs in stem and numerous hairs of ash in leaf. In the other hand, Agastache rugosa is yellow-green in whole, has no hairs in stem. Specially the latter has deep-green colour and numerous hairs presenting mostly at lower epidermis in leaf. 3. The physical characteristics in currents: Pogostemon cablin is brown, has hairs and round-like stem. In the other hand, Agastache rugosa is green or yellow-green, has no hairs and tetragon in stem. 4. The internal characteristics: Pogostemon cablin has progressed spongy tissue in epidermal cell of leaf and many rank of epidermal cell in stem. In other hand, Agastache rugosa has 1 rank palisade tissue in leaf and few rank of epidermal cell in stem. In the external shape, it was possible that herbs were distinguished according to artificial classification and that same genus-degree of relatedness among herbs could be distinguished by more precise and active observation. In the shape of real herbs, I compared current herbs in market with original herbs which were just collected or were on the course of drying. In addition, it was possible that the internal shape could be identified by using microscope after butanol series. Though it was impossible to make distinction of herbs which are not current in my search contents, this search contents will be a standard for applying herbs in the future. An Additional standard establishment including physiochemical reaction and gene research is required in order to supplement the fault of this search.

  • A thymoma commonly occurs in the superior mediastinum or the upper part of the anterior mediastinum but can be located in other places in rare cases. Cystic degeneration in a thymoma is a relatively common but focal event. In rare cases, the process proceeds to the extent that most if not all of the lesion becomes cystic. We report a case of a patient with a paracardial cystic thymoma in the lower aspect of the anterior mediastinum. A 49-year-old woman was referred to our hospital because of a mass discovered incidentally on a chest X-ray. She showed no symptoms or signs. Contrast-enhanced chest CT scan revealed a $5{\times}5cm$ sized, well-marginated, right paracardial cystic mass with a curvilinear and oval enhancing solid portion. A Surgical resection was performed. The mass was discontinuous with normal thymic tissue. Microscopy revealed a type B1 thymoma with prominent foci of medullary differentiation according to the WHO classification. There was no capsular or local invasion. The postoperative course was uneventful and the patient was discharged in good health.

IL28B rs12979860 Gene Polymorphism in Egyptian Patients with Chronic Liver Disease Infected with HCV

  • Background: Egypt has one of the highest prevalences of hepatitis C virus (HCV) infection worldwide. Although the IL28B gene polymorphism has been shown to modify the course of chronic HCV infection, this has not been properly assessed in the Egyptian population. Materials and Methods: The IL28B rs12979860 single nucleotide polymorphism (SNP) was therefore examined in 256 HCV-infected Egyptian patients (group II) at different stages of disease progression and in 48 healthy volunteers (group I). Group II was subdivided into GII-A (chronic hepatitis patients, n=119), GII-B (post hepatitis cirrhosis, n=66) and GII-C (HCC on top of cirrhosis, n=71). Results: The C/T genotype was the commonest in all groups. It was more frequent in GI (52%) than in GII (48%). There was no significant difference in the frequency of C/T and C/C or T/T genotypes between groups and subgroups (p=0.82). Within the subgroups; the C/C genotype was more common in GII-B while C/T and T/T genotypes were more common in GII-C, though with no significant difference (p=0.59 and p=0.80). There was no significant association between IL28B rs12979860 SNP and viral load, ALT, AFP level, METAVIR scores for necro-inflammation and fibrosis, and Child-Pugh classification. Conclusions: 1) IL28Brs12979860 C/T genotype is the commonest genotype in HCV-associated CH and HCC in Egypt. 2) IL28Brs12979860 polymorphisms are not associated with disease progression or aggression (histological staging, severity of fibrosis in CH or the incidence of post-HCV HCC). 3) Differences in IL28Brs12979860 genotypes could be a consequence of environmental or ethnic variation.

  • The purpose of this study is to clarify the psychological importance of golf player's clothing selection by phenomenological analysis. The study subjects were male and female golf players who participated in the golf tour from October 2017 to March 2018, and a total of 4 male players and 2 female players were selected using the snow ball sampling method. For data analysis, we first coded what we got through in-depth interviews and observations of participation to be organized and analyzed first. Second, classification was conducted based on major themes or phenomena, and meaningful interpretation and research were conducted. In addition, to increase the truth and validity of this study, the true value, applicability, consistency and neutrality of the qualitative research process proposed by Guba & Lincoln (1981). The results of the analysis are as follows. Golf-wear for me, Jinx, My golf wear are my face. Conclusion, Through this study, you can understand the psychological implications of the golf-wear chosen by the golf player and expect positive effects to provide golf player's feedback in the course of making the golf-wear.

Clinical Analysis of Branchial Anomalies : A Review of 72 Cases (새성기형에 관한 임상적 고찰)

  • Background and Objectives : Branchial anomaly is a common neck pathology seen by otolaryngologist. Although there is still controversy regarding the origin, almost all surgeon agree that branchial anomalies result from incomplete involution of the branchial apparatus. Depending on its anatomic location, branchial anomalies can be classified into first, second, third and fourth anomaly. The purpose of this study is to evaluate the incidence of different categories of branchial anomalies and to determine proper method of diagnosis and treatment. Subjects and Method : A retrospective chart review was conducted for 72 patients with branchial anomalies operated on at the Korea University Anam Hospital from 1996 to 2007. The patient's sex, age, presenting symptoms, classification, site of lesion, diagnostic studies, surgical therapy and post-operative surgical complication were reviewed. Results : Patients were commonly seen in the 3rd decades. Eleven patients(15.3%) were first branchial anomalies, 52(72.2%) were second, 1(1.4%) was third, and 8(11.1%) were fourth. Of the types of anomalies, cyst were most commonly seen. In cases of 3rd and 4th branchial anomalies, barium contrast study can delineate the course of sinus or fistula. All cases was operated on for branchial anomalies, there were no major post-operative complication. Conclusion : Cervical mass and recurrent cervical infection or abscess are suspect for branchial anomalies. Especially, clinical history of dysphagia, and recurrent painful swelling in the thyroid region in young patients should raise the suspicion of 3rd and 4th branchial anomalies. Careful history, clinical examination and radiographic study were essential parts in diagnosis of branchial anomalies. Although surgical management of branchial anomalies depend on its type, nature and extent, complete excision is the major surgical procedure.

  • Delirium is an organic psychiatric syndrome characterized by an acute onset, prominent disturbance of consciousness and cognitive impairment with fluctuating course. Although there is not a clear consensus concerning the optimal classification system for delirium subtypes, Lipowski(1983) firstly classified delirium by psychomotor activity, namely hyperactive, hypoactive, and mixed. According results of several following studies, prevalence of hypoactive delirium were not less than that of hyperactive delirium. But a diagnosis of hypoactive delirium often missed, which is most frequently misdiagnosed as depression and dementia. Hyperactive delirium can be caused by alcohol or benzodiazepine withdrawal, would be related with excessive dopamine and cholinergic deficiency, and is more responsive to high-potency antipsychotics therapy. Hypoactive delirium would be caused by metabolic encephalopathy, and tends to present a less responsiveness to antipsychotics and poorer overall prognosis with a prolonged duration of admission than hyperactive delirium. Delirium is not a homogenous syndrome. Because of different subtypes, it may have dissimilar underlying pathogenetic pathways. So different treatment strategies between various subtypes may be needed.

  • In the modern society in which globalization and localization proceed simultaneously, diversified and rapid migration of diaspora makes a new from of boundary off the frame of the state and the nation. This new border accompanies cultural change and racial mixture; retains ethnic conflict, the gap between rich and poor, alienation and discrimination, as well as power conflict; and extends its influence. Nowadays, the countries all over the world including Korea face problem of Diaspora in numerous forms. And each country takes an approach to the problem of the diaspora in the aspects of their society, culture and political technology. This implies that most countries, without understanding the new form of border which is alive and dynamic, define and conceptualize the diaspora in the frame of one state and one nation to carry forward the policies accordingly, resulting in inequal, incomplete and awkward homogenization. This study aimed to explore the identity of the diaspora, the core for the problem solving. Of course, studies about the identity of the diaspora have been continued until today and many great outcomes have been achieved. Nevertheless, this study aimed to explore the identity of the diaspora and the national policies which have a close interrelationship with it. It is because the study ultimately aimed to highlight the interrelationship between the destination countries, Russia and China, and the diaspora, through the definition and the classification of Russian diaspora and Chinese diaspora and the analysis of the national policies about that. However, the intention was not to distinguish superiority through the comparison of the polices about the diaspora between two countries, but to focus on the diversity of the identity of the diaspora through defining each different diaspora and paralleling the policies. Second, the reason for looking into the diaspora policies of these two countries is because it is judged the changes in the diaspora policies of each country is one of the active factors for the changes in the identify of the diaspora of each country and it is the basic research for the study on the identity of the diaspora. New migration of diaspora changes the identity of the state, and the state makes the policies and enforce the policies, resulting in the influence on the diaspora. This interaction acts as the growth factor for the new boundary. The causes of Russian diaspora and Chinese diaspora show apparent 'differences'. In parallel with this, the policies about the diaspora in Russia and China arouse 'differences' to the diaspora. The variation of the identity of the diaspora made by these differences will suggest other viewpoints on the diaspora, and these viewpoints will become the foundation for solving the problem of the diaspora in the present times.

Research on the Modification of Folktales' Motifs (민담 모티프의 변용 양상 연구 - 러시아 요술담의 바바야가를 중심으로)

  • This study explores how folktales' motifs are used in diverse cultural contents such as literature, movies, or comics. More precisely, it is concerned with mysterious characters like Babayaga and Koshchei who appear in Russian fairytales. When the motifs of the reviewed literature are analyzed, the following three criteria are used for classification: New Writing, Rewriting, and New Version. Or course, it is New Writing that is far from the original work. Most of the collected works, including Joan Aiken's The Kingdom Under the Sea, Patricia Polacco's Babushka's Doll, Babushka Baba Yaga, and Tai?-Marc Le Thanh's BABAYAGA, belong to New Writing. The New Writing type mainly focuses on Babayaga's secluded life. In fairytales, Babayaga is depicted as the goddess of Mother Earth, the heroine of the Nature, or the ruler of the Animal Kingdom. That is why she lives in the deep and uninhabited wood. She is related to the Coming of Age ceremony. Her 'Hut on hen's legs' is an obstacle for immature protagonists to face before they come of age. Protagonists are supposed to solve the problems posed by Babayaga. If they succeed, they can get her magical help so as to complete their final mission. Babayaga is likely to appear as their antagonist. Protagonist may be subjected to a terrible ordeal created by Babayaga, and then come of age by recovering from that ordeal. Sometimes, Babayaga helps protagonists to get a grip on reality. That is why she lives in a hut in the deep and dark forest which is on the borderline between life and death. On the other hand, Marianna Mayer's Baba Yaga and Vasilisa the Brave has been classified as a controversial work between New Writing and Rewriting. It is apparently closer to New Version. However, it cannot be classified as a work of New Version because the author incorporates 'probability' into her work. Among the reviewed works, it is Korean Writer Rye, Kami's The Spider Woman's House that best reflects the essence of Babayaga. Babayaga and other characters are amusing in movies because most of the movies are animations for children. In one sense, it is positive that the scary characters in folktales are approachable to children. In other sense, however, it is regrettable that symbolic motifs are completely eliminated. In Mike Mignola's Hellboy, Japanese animations such as Index or Narutaru, on-line games such as Cabal or Vampire: The masquerade, the essence of the supernatural characters in folktales is completely eliminated, and only their 'belligerent power' stands out. It is desirable to put stories into perspective whether they are written or told. The literature property of folktales provides a special opportunity to readers. In this light, a variety of motifs have a good reason to be reborn as modern cultural contents. The bottom line is to maintain its true nature.
