Tracheobronchial Foreign Body in a Shih-tzu Dog; Diagnostic and Therapeutic Trial Using Bronchoscopy (시츄견에서 발생한 기관기관지 이물의 기관지경을 통한 진단적 및 치료적 적용 증례)

  • A 4-year-old, neutered male Shih-tzu dog weighing 5.4 kg was referred due to anorexia and chronic coughing. Based on history, physical examination, laboratory tests, radiographic findings, echocardiography, and bronchoscopic examination, the dog was diagnosed as tracheobronchial foreign body. The foreign body was steamed rice debris, which was removed by bronchoalveolar lavage (BAL) with vacuum suction of bronchoscopy. Bacterial and fungal culture of collected BAL fluid was negative. Baermann test for lungs parasites also was negative. The dog was treated with bronchodilator, antibiotics, anti-inflammatory agent, and mucolytics for 7 days. Appetite increased and coughing sign was clearly improved after removal of foreign body and medical therapy. This case report describes that bronchoscopic techniques are available for the evaluation and management of airway foreign bodies.

Comparison of Quality of Pain Using Patient-Controlled Analgesia (PCA) after Total Abdominal Hysterectomy (TAH) (전자궁적출술후 통증자가조절장치 사용에 따른 통증점수 비교)

  • Background: The purpose of this study was to examine the extent and evaluation of pain after total abdominal hysterectomy (TAH) and to establish correlation between three types of pain; pain at rest, pain with movement and pain with coughing (maximum pain). Methods: The present study compared quality of pain during pain management in 48 patients undergoing TAH. Patients received i.v. meperidine as loading dose in the recovery room and PCA with nalbuphine 90 mg, ketorolac 180 mg, buprenorphine 0.9 mg, droperidol 5 mg, plasma solution A 28 ml for 3 days. The PCA device used was the Baxter infusor$^{(R)}$ (PCA module PC-19-55, 0.5 ml/hr basal rate, 15 minute lockout interval). Patients were then interviewed on Operative Day (OPD), Postoperative Day 1, 2, and 3 (POD 1, 2 and 3) to assess their pain on a visual analogue scale (VAS) of 0 (none) to 10 (worst imaginable). Results: The mean pain score at rest was 2.0 on OPD and decreased to 0.7 on POD 3. The mean pain score with movement was 3.2 on OPD and decreased to 1.6 on POD 3. The mean pain score with coughing was 4.2 on OPD and decreased to 2.2 on POD 3. Conclusions: Patients' experience of three types of postoperative pain emphasizes the need for more effective pain management.

  • We experienced a rare case of trachea diverticula combined with the sequela of tuberculosis and a fungus ball. The patient had complained of coughing and hemoptysis for a long time after experiencing tuberculosis. He was admitted due to hemoptysis and the aggravation of coughing. The CT scan showed a variable sized trachea diverticula combined with tuberculosis sequela and a fungus ball in the right lung fields. The diagnosis was made by bronchoscopy and a CT scan. After bronchial artery embolization and conservative treatment, the patient's symptoms improved and the patient was discharged.

  • Pleural effusion is a state, retention of a mount of liquid in pleural cavity. Main causes of pleural effusion is Congestive Heart Failure that is caused by left ventricular heart failure. And that of Congestive heart failure is caused by increase of pleural capillary pressure or remain of effusion in pleural cavity. Bilateral venous pressure of pleura make worse pleural effusion and one way of venous pressure of that bring out pleural effusion. The purpose of this study is to examine the efficacy of oriental treatment for pleural effusion is caused by heart failure. One woman of 86 years old complained the symptom of general weakness, dyspnea, flank pain, anorexia, insomnia, coughing, secretion mixed blood. The symptom is caused by effusion that is brought out acute pneumonia, heart failure. At the time of Admission, in the diagnosis of Admission, in the diagnosis of Hyuneum(懸飮) she had taken Kungha-tang hap pleurisy-bang,(芎夏湯合助膜炎方), so improved dyspnea, flank pain, insomnia, coughing. In views of examination, decrease of heart failure' s symptom and pleural effusion. After 13days of admission, she had taken palmul-tang.(八物湯). As a conseguence of that, the symptom of general weakness. anorexia is improved and she was discharged.

  • Severe cough may contribute to serious complications such as pneumothorax, pneumomediastinum, rib fracture, subconjunctival hemorrhage, subdural hemorrhage and cough syncope. However abdominal wall hematoma is a rare complication. Because it usually presents with abdomianal pain, abdominal wall hematoma needs to be differentiated from the acute surgical abdomen. A 78-year old woman was admitted with right lower quadrant abdominal pain and a palpable mass for several days. She experienced abdominal pain after violent coughing associated with an upper respiratory tract infection. Abdominal computed tomography revealed an approximately $7{\times}4cm$ sized, ill-defined, soft tissue density lesion in the right lower posterolateral abdominal wall. An abdominal wall hematoma was diagnosed. After admission, she had persistent right lower abdominal pain and an increasing mass. The mass was surgically removed and she was discharged without complications. In summary, when a patient complains of abdominal pain after severe coughing, an abdominal wall hematoma as a differential diagnosis must be considered.

Respiratory symptoms occurring among mushroom farmers (느타리버섯 재배 농민에서 발생하는 호흡기 증상)

  • It has been reported that among mushroom farmers, respiratory diseases such as hypersensitivity pneumonitis can be developed by inhalation of mushroom (Pleurotus ostreatus) spores. For the evaluation of respiratory symptoms among mushroom farmers, a questionnaire was made. The Questionnaire included general characteristics, past occupational histories, durations of the mushroom cultivation and characteristics of the symptoms. Following the questionnaire we interviewed the 72 mushroom farmers (12 males, 10 females) from Kyungbuk Province. We also examined white blood counts, erythrocyte sedimentation rates, eosinophil counts and total IgE counts for the 5 farmers. The results obtained are as follows. 1. The mean age among the 22 mushroom farmers was 46.9 years, and the mean duration of cultivation was 7.5 years. 2. Among the 22 mushroom farmers. 18 farmers (81.8 %) suffered from respiratory symptoms at work. The main symptom was coughing (100.0%), followed by chilling sensation (50.0 %), sputum (38.9 %) and sore throat (27.8 %). 3. Coughing and other associated symptoms occurred during work and disappeared after work or several days later when the exposure had been stopped. 4. Total IgE counts were elevated in all farmers examined the test. With above results, the respiratory symptoms that developed among mushroom farmers were typical patterns of hypersensitivity pneumonitis. Epidemiological studies and preventive measures for mushroom farmers should be established.

  • In healthy adults, diagnosis of aspiration of foreign body into tracheobronchial tree is not difficult because various symptoms such as dyspnea, coughing, or cyanosis develop when foreign body is aspirated into tracheobronchial tree. But unless a clear history of an aspiration event can be obtained, diagnosis will be delayed. Early complications of tracheobronchial foreign body aspiration include asphyxia, cardiac arrest, dyspnea, laryngeal edema, and cyanosis. Late complications include pneumonia, lung abscess, bronchiectasis, hemoptysis, bronchial stenosis, and polyp. Treatment is removal of foreign body by operation or bronchoscopy. Currently, flexible bronchoscopy is preferred in adults than rigid bronchoscopy. A 36-year-old male visited to Dongkang hospital due to productive coughing and dyspnea. On auscultation, focal inspiratory wheezing was heard. On chest PA, mild emphysematous change was seen Flexible bronchoscopy was done. Bronchoscopically, mucoid impaction, surrounding inflammation, foreign body lodged in the right lower lobe bronchus, and interbronchial fistula(between right middle and lower lobe bronchus) were seen Foreign body($2.4\{times}1.3cm$ sized antacid package) was removed by flexible bronchoscopy. Later, history of aspiration of a piece of antacid package was found. We report a case of recurrent bronchitis with interbronchial fistula as a result of occult aspiration of foreign body with review of the literatures.

Foreign bodies in air passage in Children (소아의 기도이물에 관한 통계적 관찰)

  • Foreign bodies in food and air passage, especially the latter in infants and children requires emergency treatment. The frequent symptoms are coughing, wheezing, and dyspnea, So may be confused as asthma, pneumonia and acute Laryngitis. The species and incidence of the foregin bodies are variable according to the age, method of life, environment and economic condition. The authors experienced 35 cases of foreign body in air passage during the period of last 3yrs which were treated by ventilating Bronchoscopy, and thoractomy in this hospital. One of them was lead to death. The authors analysed above cases and report with it's literature consideration. Results: 1. The prevalent foreign bodies were peanut, metal, food particles in order of frequency. 2. In frequent symptoms were dyspnea, coughing and wheezing in the order. 3. In the age incidence, 48% of the foreign bodies were 1∼5yrs. 4. In sex distribution, male to female ratio was 2.1: 1 5. In duration of lodgment, 65% of the foreign body were removed within 24hrs. 6. Most prevalent site of the foreign body was Rt. main bronchus Rt. to Lt. ratis was 2 : 1 was 7. Treatment was done; 25 cases were by ventilating bronchoscopy.

Two Case Reports of Pneumonia in Elderly Patients That Was Improved by Traditional Korean Medicine Treatment (노인성 폐렴 한방 치험 2례)

  • Objective: This study reports on traditional Korean medicine therapy used for pneumonia among elderly patients.Method: Two patients diagnosed with pneumonia were treated with herbal medicine and acupuncture, as well as cupping along the back. We checked chest X-rays, coughing and sputum on the visual analogue scale (VAS), and lab evaluations in order to evaluate the effectiveness of the treatment.Results: Following treatment with traditional Korean medicine therapy, VAS scores for coughing and sputum decreased and the chest X-rays and inflammation markers improved.Conclusion: Traditional Korean medicine therapy treatment appears to be effective for treating pneumonia in elderly patients.

Clinical Importance of Peak Cough Flow in Dysphagia Evaluation of Patients Diagnosed With Ischemic Stroke

  • Objective To investigate the relationship between peak cough flow (PCF), pulmonary function tests (PFT), and severity of dysphagia in patients with ischemic stroke. Methods This study included patients diagnosed with ischemic stroke, who underwent videofluoroscopic swallowing study (VFSS), PCF and PFT from March 2016 to February 2017. The dysphagia severity was assessed using the videofluoroscopic dysphagia scale (VDS). Correlation analysis of VDS, PFT and PCF was performed. Patients were divided into three groups based on VDS score. One-way ANOVA of VDS was performed to analyze PCF, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and age among the different groups. Results The correlation coefficients of VDS and PCF, VDS and FVC, and VDS and FEV1 were -0.836, -0.508, and -0.430, respectively, all of which were statistically significant at the level of p<0.001. The one-way ANOVA indicated statistically significant differences in PCF, FVC, FEV1, and age among the VDS groups. Statistically significant differences in VDS and age were observed between aspiration pneumoia and non-aspiration pneumonia groups. Conclusion Coughing is a useful factor in evaluating the risk of aspiration in dysphagia patients. Evaluation of respiratory and coughing function should be conducted during the swallowing assessment of patients with ischemic stroke.
