  • The purpose of this study is to introduce alternative cost-effectiveness analysis method of DSM investment programs. This alternative method is Value Test method which consider the effects of DSM investment program on customer value. And this method was applied for actual DSM investment program in natural gas domain. By utilize this method to evaluate cost-effectiveness of DSM investment programs, it is expected to make right decision to enforce and complement those programs.

Analysis of the cost effectiveness according to T-P standard enforcement of public sewage treatment facilities (공공하수처리시설의 총인기준 강화에 따른 비용효과분석)

  • This study was conducted to analyze the cost effectiveness in line with total phosphorus standard enforcement of public sewage treatment facilities. The additional cost for the total phosphorus removal process was calculated to analyze the cost effectiveness of the advanced water purification process. The analysis results showed that the T-P removal by coagulation sedimentation was more efficient than the advanced water purification facilities in terms of facilities investment cost, and if the coagulation filteration was used for T-P removal, the activated carbon process among the advanced water purification techniques was more efficient in terms of facilities investment cost. In this study, the effects of the T-P removal process that will be additionally introduced according to the tightening of the effluent T-P standard were analyzed within a limit. The actual benefits of improved T-P concentration in the water source will provide diverse values, including the leisure water, environment improvement water, eco-system in the water source and industrial water, in addition to the advanced water purification.

Comparing Endoscopy and Upper Gastrointestinal X-ray for Gastric Cancer Screening in South Korea: A Cost-utility Analysis

  • Background: There are limited data evaluating the cost-effectiveness of gastric cancer screening using endoscopy or upper gastrointestinal x-ray in the general population. Objective: To evaluate the cost-effectiveness of population-based screening for gastric cancer in South Korea by decision analysis. Methods: A time-dependent Markov model for gastric cancer was constructed for healthy adults 30 years of age and older, and a deterministic sensitivity analysis was performed. Cost-utility analysis with multiple strategies was conducted to compare the costs and effects of 13 different screening alternatives with respect to the following eligibility criteria: age at the beginning of screening, screening interval, and screening method. The main outcome measurement was the incremental cost-effectiveness ratio. Results: The results revealed that annual endoscopic screening from ages 50-80 was the most cost-effective for the male population. In the females, biennial endoscopy screening from ages 50-80 was calculated as the most cost-effective strategy among the 12 screening alternatives. The most cost-effective screening strategy may be adjustable according to the screening costs and the distribution of cancer stage at screening. The limitation was that effectiveness data were obtained from published sources. Conclusions: Using the threshold of $19,162 per quality-adjusted life year on the basis of the Korean gross domestic product (2008), as suggested by the World Health Organization, endoscopic gastric cancer screening starting at the age of 50 years was highly cost-effective in the Korean population. The national recommendation for gastric cancer screening should consider the starting age of screening, the screening interval, and the screening modality.

An Economic Evaluation of Thread Embedding Acupuncture for the Treatment of Lumbar Herniated Intervertebral Disc in a Randomized Controlled Clinical Trial

  • Background: Lumbar herniated intervertebral disc (LHIVD) is a frequently presented condition/disease in Korean medical institutions. In this study, the economics of thread embedding acupuncture (TEA) was evaluated in a randomized controlled trial comparing TEA with sham TEA (STEA). Methods: This economic evaluation was analyzed from a limited social perspective, and the per-protocol set was from a basic analysis perspective. The cost-effectiveness analysis was based on the change in visual analog scale score, and the cost-utility analysis was based on the quality-adjusted life years. The final results were expressed as the average cost-effectiveness ratio and incremental cost-effectiveness ratio, and furthermore sensitivity analysis was performed to confirm the robustness of the results observed. Results: The cost-effectiveness analysis showed that TEA was 9,908 won lower than STEA, while the decrease in 100 mm visual analog scale score was 8.5 mm greater in the TEA group compared with the STEA group (p > 0.05). The cost-utility analysis showed that TEA was 9,908 won lower than STEA, while the quality-adjusted life years of TEA was 0.0026 years higher than STEA (p > 0.05). These results were robust in the sensitivity analysis, but were not statistically significant. Conclusion: In treating LHIVD, TEA appeared to have cost-effectiveness and cost-utility compared with STEA. However, there were no significant differences between the groups in terms of cost, effectiveness, and utility indicators. Therefore, results must be interpreted prudently; this study was the 1st to conduct an economic evaluation of TEA for LHIVD.

  • Human African Trypanosomiasis (HAT) also known as sleeping sickness, is a neglected tropical vector borne disease caused by trypanosome protozoa transmitted by bites of infected tsetse fly. The basic reproduction number, R0 derived using the next generation matrix method which shows that the disease persists in the population if the value of R0 > 1. The numerical simulations of optimal control model carried out to determine the control strategy that can combat HAT under the minimum cost. The results indicate that, the use of both education campaign, treatment and insecticides are more efficient and effective to eliminate HAT in African community but too costly. Furthermore, the cost-effectiveness of the control measures (education campaign, treatment and insecticides) were determined using incremental cost-effectiveness ratio (ICER) approach and the results show that, the use of education and treatment of infected people as the best cost effective strategy compared to other strategies.

A Study on Use Evaluation of Electronic Journals (전자저널의 이용평가에 관한 연구: Y대 도서관의 IDEAL을 중심으로)

  • This study is to analyze a cost-benefit and a cost-effectiveness on the basis of number full-text downloaded from IDEAL in the electronic journals consortium of Y University Library during 2000. The result of this study shows that Y University Library used more the combined print and electronic journals than only electronic journals, and in the case of the analysis on the cost-benefit, there was no benefit, but the cost-effectiveness was very high. Based upon the result of this study, it suggests that academic libraries request a trial from the information agency and carry out user instruction analysis of varied use statistics, user satisfaction survey so as to promote use of electronic journals.

  • Installing vibration control devices in the structure rises as a solution instead of increasing structural strength considering construction cost. Especially, viscoelastic dampers show excellent vibration control performance at low cost and are easy to install in existing structures compared with other control devices. Therefore, cost-effectiveness of structure with viscoelastic dampers needs to be evaluated. Previous cost-effectiveness evaluation method for the seismically isolated structure(Koh et al., 1999;2000)is applied on the building structure with viscoelastic dampers, which combines optimal design and cost-effectiveness evaluation for seismically isolated structures based on minimum life-cycle cost concept. Input ground motion is modeled in the form of spectral density function to take into account acceleration and site coefficients. Damping of the viscoelastic damper is considered by modal strain energy method. Stiffness of shear building and shear area of viscoelastic damper are adopted as design variables for optimization. For the estimation of failure probability, transfer function of the structure with viscoelastic damper for spectral analysis is derived from the equation of motion. Results reveal that cost-effectiveness of the structure with viscoelastic dampers is relatively high in how seismic region and stiff soil condition.

Cost-Effectiveness Analysis of HPV Vaccination: Comparing the General Population with Socially Vulnerable Individuals

  • Background: After the WHO recommended HPV vaccination of the general population in 2009, government support of HPV vaccination programs was increased in many countries. However, this policy was not implemented in Korea due to perceived low cost-effectiveness. Thus, the aim of this study was to analyze the cost-utility of HPV vaccination programs targeted to high risk populations as compared to vaccination programs for the general population. Materials and Methods: Each study population was set to 100,000 people in a simulation study to determine the incremental cost-utility ratio (ICUR), then standard prevalence rates, cost, vaccination rates, vaccine efficacy, and the Quality-Adjusted Life-Years (QALYs) were applied to the analysis. In addition, sensitivity analysis was performed by assuming discounted vaccination cost. Results: In the socially vulnerable population, QALYs gained through HPV vaccination were higher than that of the general population (General population: 1,019, Socially vulnerable population: 5,582). The results of ICUR showed that the cost of HPV vaccination was higher for the general population than the socially vulnerable population. (General population: 52,279,255 KRW, Socially vulnerable population: 9,547,347 KRW). Compared with 24 million KRW/QALYs as the social threshold, vaccination of the general population was not cost-effective. In contrast, vaccination of the socially vulnerable population was strongly cost-effective. Conclusions: The results suggest the importance and necessity of government support of HPV vaccination programs targeted to socially vulnerable populations because a targeted approach is much more cost-effective. The implementation of government support for such vaccination programs is a critical strategy for decreasing the burden of HPV infection in Korea.

  • Minimum life-cycle cost helps to evaluate cost effectiveness of base-isolated bridges under specific condition. Life-cycle cost mainly consists of the initial construction cost and the expected damage cost. Damage cost estimation needs proper model of input ground motion failure probability evaluation method and limit states definition. We model the input ground motion as spectral density function compatible with the response spectra defined at each seismicity and site condition. Spectrum analysis and crossing theory is suitable for reseating calculation of failure probabilities in the process of cost minimization. Limit states of base-isolated bridges re defined for superstructure isolator and pier respectively The method is applied to both base-isolated bridges and conventional bridges under the same conditions to investigate cost effectiveness of base isolation in low and moderate seismic region. the results show that base-isolation of bridges are more effective in low and moderate seismic region and that the site effects on the economical efficiency may not be negligible in such a region.

Cost-Effectiveness Analysis of Breast Cancer Screening in Rural Iran

  • Background: Although breast cancer is the most common cancer in women, economic evaluation of breast cancer screening is not fully addressed in developing countries. The main objective of the present study was to analyze the cost-effectiveness of breast cancer screening using mammography in 35-69 year old women in an Iranian setting. Materials and Methods: This was an economic evaluation study assessing the cost-effectiveness of a population-based screening program in 35-69 year old women residing in rural areas of South east Iran. The study was conducted from the perspective of policy-makers of insurance. The study population consisted of 35- to 69-year old women in rural areas of Kerman with a population of about 19,651 in 2013. The decision tree modeling and economic evaluation software were used for cost-effectiveness and sensitivity analyses of the interventions. Results: The total cost of the screening program was 7,067.69 US$ and the total effectiveness for screening and no-screening interventions was 0.06171 and 0.00864 disability adjusted life years averted, respectively. The average cost-effectiveness ratio DALY averted US$ for screening intervention was 7,7082.5 US$ per DALY averted and 589,027 US $ for no-screening intervention. The incremental cost-effectiveness ratio DALY averted was 6,264 US$ per DALY averted for screening intervention compared with no-screening intervention. Conclusions: Although the screening intervention is more cost-effective than the alternative (noscreening) strategy, it seems that including breast cancer screening program in health insurance package may not be recommended as long as the target group has a low participation rate.
