BSC 관점에서 SCM 도입 섬유.패션 기업과 미도입 기업의 성과에 대한 사례 연구 (The Case Study on the Performance between SCM Adopted Textile.Fashion Firms and Unadopted Firms in a Viewpoint of BSC)

  • SCM as the important marketing strategy enhance the firm's efficiency and compatibility in global market environment such as global outsourcing. Firms adopted SCM realized the need to evaluate precisely the performance of SCM. In spite of importance of SCM, there was not much intention and research to measure SCM performance in textile fashion industry. Therefore, the purpose of this case study was to measure performance of supply chain management in textile fashion business using BSC(Balanced Score Card) to measure not only financial perspective but also non-financial perspectives such as customer perspective, internal business perspectives, financial perspective, and innovation & learning perspective. The questionnaire developed by the reviews of the literature was adopted for this study. The results of this study showed that SCM performance was enhanced from the point of customer perspective(cost, quality, time, service), financial perspective(cash cycle time, inventory turn over, inventory obsolescence, return on asset, return on investment, capacity utilization), and innovation & learning perspective(cost for human resource management, service for human resources). But there was same performance level regarding internal business perspective(lead time, cost for manufacturing process, product quality control, productive flexibility for time, quantity, and variety). Therefore, we should keep close relationship and two way communication among supply chain members to promote better SCM performance.

중증도 분류에 따른 진료비 차이: 간질환을 중심으로 (Differences of Medical Costs by Classifications of Severity in Patients of Liver Diseases)

  • Background: Diagnosis procedure combination (DPC) has recently been introduced in Korea as a demonstration project and it has aimed the improvement of accuracy in bundled payment instead of Diagnosis related group (DRG). The purpose of this study is to investigate that the model of end-stage liver disease (MELD) score as the severity classification of liver diseases is adequate for improving reimbursement of DPC. Methods: The subjects of this study were 329 patients of liver disease (Korean DRG ver. 3.2 H603) who had discharged from National Health Insurance Corporation Ilsan Hospital which is target hospital of DPC demonstration project, between January 1, 2007 and July 31, 2010. We tested the cost differences by severity classifications which were DRG severity classification and clinical severity classification-MELD score. We used a multiple regression model to find the impacts of severity on total medical cost controlling for demographic factor and characteristics of medical services. The within group homogeneity of cost were measured by calculating the coefficient of variation and extremal quotient. Results: This study investigates the relationship between medical costs and other variables especially severity classifications of liver disease. Length of stay has strong effect on medical costs and other characteristics of patients or episode also effect on medical costs. MELD score for severity classification explained the variation of costs more than DRG severity classification. Conclusion: The accuracy of DRG based payment might be improved by using various clinical data collected by clinical situations but it should have objectivity with considering availability. Adequate compensation for severity should be considered mainly in DRG based payment. Disease specific severity classification would be an alternative like MELD score for liver diseases.

건강보험정책심의위원회의 이익집단 간 대립이 물리치료 수가에 미치는 영향 (Conflict of Interest Groups on the Health Insurance Policy Deliberation Committee Affect the Medical Insurance Cost of Physical Therapy)

  • Purpose: The aim of this study was to demonstrate that non-participation of physical therapists on the political decision-making committee results in invasion of their interests. Methods: To demonstrate the effects, we analyzed the change of medical insurance score decided by the Health Insurance Policy Deliberation Committee between 2001 and 2012 years, focusing on medical examination as the interest of the participation group and physical therapy cost as interest of the non-participation group. Results: Total medical insurance cost increased by 23.72%, on average. Medical examination cost increased by 23.90% and 37.66% in medical examination for new and established patients, respectively. However, physical therapy cost was reduced by 5.01%. The medical examination cost for physical therapy without medical checkup increased by 2.62%. Conclusion: This study shows that the physical therapy cost, related on the interest of the non-participative group in the Health Insurance Policy Deliberation Committee, rather decreased while the total medical insurance cost increased.These findings demonstrate the invasion of the non-participative group on the Health Insurance Policy Deliberation Committee. Thus, aggressive participation in political decision-making committee is necessary in order to protect and increase rights and interests of Korean physical therapists.

불균형데이터의 비용민감학습을 통한 국방분야 이미지 분류 성능 향상에 관한 연구 (A Study on the Improvement of Image Classification Performance in the Defense Field through Cost-Sensitive Learning of Imbalanced Data)

  • With the development of deep learning technology, researchers and technicians keep attempting to apply deep learning in various industrial and academic fields, including the defense. Most of these attempts assume that the data are balanced. In reality, since lots of the data are imbalanced, the classifier is not properly built and the model's performance can be low. Therefore, this study proposes cost-sensitive learning as a solution to the imbalance data problem of image classification in the defense field. In the proposed model, cost-sensitive learning is a method of giving a high weight on the cost function of a minority class. The results of cost-sensitive based model shows the test F1-score is higher when cost-sensitive learning is applied than general learning's through 160 experiments using submarine/non-submarine dataset and warship/non-warship dataset. Furthermore, statistical tests are conducted and the results are shown significantly.

  • The importance of cost estimate in early stage such has been increasing due to market change and severe competition in construction industry. Because the adjustable budget is only 20% after design stage, most of the crucial decisions to influence cost is made in the early stage. However, in the early stage, the project scope is not defined completely so that estimator has inaccurate information to make critical decision. Therefore, this research suggests the cost estimate method using case-based reasoning. Case-based reasoning is appropriate for the early cost estimating, as it has the strength of rapidity and convenience in cost estimation. This research analyzes 84 actual data of public apartment on the scale of 11~15 stories. In order to extract the most similar case, at the first step this research identifies influence factors and calculates attribute similarity. In case-based reasoning, the most challenging task is determining attribute weight. At the third step, this research calculates case similarity which is aggregated attribute similarity multipled by attribute weight. Finally, extracts the most similar case which has the highest score of case similarity.

An Economic Evaluation of Thread Embedding Acupuncture for the Treatment of Lumbar Herniated Intervertebral Disc in a Randomized Controlled Clinical Trial

  • Background: Lumbar herniated intervertebral disc (LHIVD) is a frequently presented condition/disease in Korean medical institutions. In this study, the economics of thread embedding acupuncture (TEA) was evaluated in a randomized controlled trial comparing TEA with sham TEA (STEA). Methods: This economic evaluation was analyzed from a limited social perspective, and the per-protocol set was from a basic analysis perspective. The cost-effectiveness analysis was based on the change in visual analog scale score, and the cost-utility analysis was based on the quality-adjusted life years. The final results were expressed as the average cost-effectiveness ratio and incremental cost-effectiveness ratio, and furthermore sensitivity analysis was performed to confirm the robustness of the results observed. Results: The cost-effectiveness analysis showed that TEA was 9,908 won lower than STEA, while the decrease in 100 mm visual analog scale score was 8.5 mm greater in the TEA group compared with the STEA group (p > 0.05). The cost-utility analysis showed that TEA was 9,908 won lower than STEA, while the quality-adjusted life years of TEA was 0.0026 years higher than STEA (p > 0.05). These results were robust in the sensitivity analysis, but were not statistically significant. Conclusion: In treating LHIVD, TEA appeared to have cost-effectiveness and cost-utility compared with STEA. However, there were no significant differences between the groups in terms of cost, effectiveness, and utility indicators. Therefore, results must be interpreted prudently; this study was the 1st to conduct an economic evaluation of TEA for LHIVD.

가정간호 환자 가족의 부담감과 간호만족도 (Burden and Satisfaction of Family Caregivers under Home Health Nursing Care)

  • This study was conducted to provide the data for the improvement of home health nursing services through the investigation of burden and satisfaction felt by family caregivers under home health nursing care. The study subjects consisted of 200 family caregivers who were enrolled m six university hospital home care services. Data were collected by using constructed questionnaires through mail from March to April of 1999. and analyzed by using t-test. ANOVA, Duncan-test. and Pearson Correlation Coefficients. The results were as follows: 1. The mean score of burden was 2.24. Among the SIX burden dimensions. the highest score was marked in time-dependence dimension. Caregivers were found to have greater burden in the low income families living in flats. In relation to the characteristics of patients. higher scores were shown in the male patients with cerebrospinal diseases. who also revealed higher score of dependency in the Activities of Daily Living. As a whole. there was no significant difference between home nursing care and clinical nursing care in terms of family caregivers' burden. The burden of time-dependence dimension in home nursing care was significantly higher than that of clinical nursing care while the burdens of physical. social. and financial dimensions were significantly lower than those of the clinical nursing care. 2. The mean score of satisfaction was 3.14. Among the six items. the highest score was marked in the nursing care and treatment skill. while the lowest score was marked in the cost containment. Family caregivers with higher academic background and higher income showed higher satisfaction score. The mean score of home care nursmg was significantly higher than that of clinical nursing care.

메타분석을 이용한 임상영양서비스의 비용-효과성 평가 (Evaluation of Cost-Effectiveness of Medical Nutrition Therapy : Meta-Analysis)

  • Objectives: A meta-analysis of the literatures was conducted to evaluate the cost-effectiveness of medical nutrition therapy by dietitians. Methods : The 30 studies were identified from a computerized search of published research on MEDLINE, Science-Direct and the PQD database until May, 2002 and a review of reference lists. The main search terms were“dietitian”,“dietary intervention”,“nutrition intervention”, “cost”,“cost-effectiveness”and“cost-benefit analysis”. The subgroup analysis was performed by publication year, study design, intervention provider, type of patient (in/out-patient) and type of cost (total cost/direct cost). Two reviewers independently selected trials for inclusion, assessed the quality and extracted the data. Results : The 30 studies were identified using the electric database search and bibliographies. The 17 trials were eligible for inclusion criteria, then the systematic review and a meta-analysis were conducted on effectiveness and cost-effectiveness of medical nutrition therapy. The quality of the studies was evaluated using the quality assessment tool for observational studies. The quality score was 0.515 $\pm$ 0.121 (range : 0.279-0.711, median : 0.466). The meta-analysis of 17 studies based on the random effect model showed that medical nutrition therapy was highly effective in treating the diseases (effect size 0.3092 : 95% confidence interval 0.2282-0.3303). The vote-counting method, one of meta-analysis methods, was applied to evaluate the cost-effectiveness of medical nutrition therapy conducted by dietitians. Two criteria (method 1, method 2) for voting were used. The calculated p-values for method 1 (more conservative method) and method 2 (less conservative method) were 0.1250 and 0.0106, respectively. Medical nutrition therapy by dietitians was significantly cost-effective in the method 2. Conclusion. This meta-analysis showed that the effectiveness of medical nutrition therapy was statistically significant in treating disease (effect size 0.3092), and that the cost-effectiveness of medical nutrition therapy was statistically significant in the method 2 (less conservative method) of vote counting. 교량 성능변화를 고려한 유지관리비용 추계분석 방법 개발 (Development of maintenance cost estimation method considering bridge performance changes)

  • 사회기반시설물의 노후화에 따른 교량 유지관리비용의 급격한 증가에 대비하기 위해서는 미래 발생 가능한 유지관리비용의 예측이 선행되어야 한다. 이를 위해 본 논문에서는 교량 관리주체의 유지관리 현황 분석을 통해 교량의 관리수준을 파악하고, 이를 생애주기에 따른 목표관리수준과 성능향상 한계 값으로 모델링하였다. 이와 함께 교량 안전점수에 따른 보수보강비용 예측모델, 성능열화모델, 평균개축비용단가, 평균 개축시기로 구성된 기존의 비용 및 성능 예측모델과 연계하여, 교량 유지관리비용 추계분석을 위한 방법론 및 절차를 제안하였다. 제안된 방법의 활용성 검증을 위해 특정 관리주체가 관리하는 교량의 규모, 노후화 정도, 현재 관리현황을 고려하여 미래 유지관리비용 추계 분석을 수행하였다. 연차별로 개별 교량 수준의 안전등급과 유지관리 조치에 따른 비용 추정 결과와 이들을 관리주체 수준으로 합산하여 연차별 평균 안전점수, 안전등급 구성비, 점검진단비용, 보수보강비용, 개축비용을 합리적으로 추정할 수 있다. 향후 목표성능관리수준의 변경에 따른 생애주기 예산의 변화를 추정하여 최적의 관리수준을 제안할 수 있을 것으로 판단된다.

수익형 민간투자사업(BTO) 입찰평가 분석: 도로사업을 중심으로 (Analysis on Bidding Behavior in Score Auction: Highway BTO Projects)

  • 수익형 민간투자사업의 경우 가격요소와 비가격요소를 동시에 고려하는 다차원 경매제도가 시행되고 있다. 본 논문은 수익형 민간투자사업 중 도로사업의 입찰 평가자료를 분석하여 정책적 시사점 및 개선방안을 제시함을 목적으로 한다. 입찰 평가자료 분석 결과, 평가 1위 사업자들의 득점이 증가하고 있는데, 이는 기본점수의 증가와 경쟁의 심화로 인한 것으로 판단된다. 평가 1위 사업자 선정에는 비가격부문에서 발생하는 점수편차가 결정적인 것으로 제시되었다. 또한 최초제안자 우대점수가 높게 설정된 사업의 경우 경쟁이 발생하지 않음도 확인하였다. 가격부문 중 재정지원부문의 점수별 금액은 평균 7.3억원으로 통행료부문의 점수별 금액 24.3억원과 큰 차이를 보였다. 비가격부문의 점수별 금액은 23.0억원 정도로 추정되어 가격부문과는 다소 차이를 나타내었다. 분석결과를 토대로, 최초제안자 우대점수의 적정 수준 유지, 재정지원부문의 평가방식 개선, 기본점수의 재조정 등이 제언되었다.
