Clinical Experience of Finger Tip Amputation of Small Finger in 12-Months-Old: Use of the Technique of Artery-Only Anastomosis (단일동맥연결을 통한 유아 새끼 손가락끝 재접합 경험)

  • Purpose: Recently, replantation of fingertip amputation, Zone I by Yamano classification, is still difficult because digital arteries branch into small arteries and also digital veins are hard to separate from the immobile soft tissue. However the replantation of fingertip in adults is a well-established procedure, but the replantation in infant or child is still uncommon. Therefore we present one case of replantation of the fingertip of the small finger in 12-months-old patient. Methods: We experienced a 12-months-old male amputation of small finger. It had been amputated completely at the level of Zone I by Yamano classification. Replantation was performed using the arteryonly technique with neither vein nor nerve repair. Because the artery has been damaged, it is still possible to make a direct suture by transposing the arterial arch in an inverted Y to I arterial configuration. Venous drainage was provided by an external bleeding method with partial nail excision and repaired margin for approximately 7 days. Results: We were performed replantation in infant with only-arterial anastomosis successfully, result in good recovery of aesthetic and functional outcome. Conclusion: In conclusion, although fingertip injury was difficult to replantation in infant and child, we must try it. Because of its functional and cosmetic advantage.

Polyurethane Foam Template for Simple Design of Digital Island Flap (폴리우레탄 폼 드레싱재료를 이용한 간단한 손가락 섬피판 디자인)

  • Purpose: Various techniques have been attempted for design of the flaps. However, there are some disadvantages. They have thin, pliable, and two dimensional methods. The aim of this study is to report usefulness of polyurethane foam dressing materials for three dimensional design of the digital island flap. Methods: From June of 2007 to september of 2008, 10 patients received digital island flap surgery for soft tissue defect of the finger. After minimal debridement of the wound, size and shape of the defect were measured using polyurethane foam. We used Medifoam-$5^{(R)}$ And then, designed this inset the wound. The flap was designed on the donor site with a arterial pedicle as the central axis according to size and shape. A full thickness skin graft from the groin is applied on the flap donor defect and secured with a tieover bolster dressing. Results: Reviewing sizes of the flaps, the length and width of flaps ranged from 1.5 to 3.3 cm and 1.0 to 2.5 cm. The PACS(Picture Archiving Communication System) program allows identification of the donor depth of finger. The distance for the soft tissue ranged from 4.3 mm to 6.7 mm. Mean depth of donor site was 5.3${\pm}$0.6 mm. Also, the thickness of Medifoam-$5^{(R)}$ ranged nearly 5 mm. On flap insetting, full-thickness skin graft was necessary. We did not experience any problems in the recipient site size either, regardless of the extended flaps. Conclusion: Polyurethane foam has many advantages over the more conventional templates. Refinements in flap design and surgical technique resulted in favorable functional and cosmetic results. Especially, for beginner, Polyurethane foam dressing material is a simple and safe tool and therefore is an excellent choice for design of the island flap.

Make-Up Behavior and Influential Factors - Focusing on Clothing Involvement, Age and Face Satisfaction - (화장행동과 영향 변인 연구 -의복관여도, 연령, 얼굴만족도를 중심으로-)

  • The purpose of this study was to analyze the differences in make-up behavior according to clothing involvement, age, and face satisfaction. Subjects of is study were the females in Seoul and Kyonggi, who were 20s and 40 $.$ 50s. Questionnaire was used as major method of gathering data. The data were collected from Sep. to Oct. in 2003 and analyzed by using SPSS 10.0 with various techniques such as the factor analysis, mean, percentage, cluster analysis, ANOVA, Duncan test, 1-test, Cronbach's $\alpha$, and $\chi$$^2$-test. The results of this study were as follows: 1. The consumers were classified into four categories by clothing involvement; high clothing involvement group, low fashion involvement group, middle clothing involvement group, low clothing involvement group. 2. The differences in make-up behavior according to the clothing involvement showed that make-up behavior was getting more aggressive as clothing involvement was getting higher. And generally Korean females thought the make-up was important. 3. The differences in make-up behavior according to the age revealed that 20s' make-up behavior was fashion oriented more than 40ㆍ50s, and 40ㆍ50s' make-up behavior was that they were taking a serious viewer than 20s' in interpersonal relationship oriented make-up behavior. 4. The result of differences in make-up behavior according to the face satisfaction was that no noticeable difference was found depending on the face satisfaction. This study revealed that the differences in make-up behavior according to clothing involvement and age were found and suggested that the cosmetic market segmentation could depend on clothing involvement market and two age group market such as younger and elder than 40ㆍ50s.

Antimicrobial, Antioxidant and SOD-Like Activity Effect of Jubak Extracts (주박 추출물의 항균활성,항산화 및 SOD 유사활성 효과)

  • The purpose of this study was to investigate antimicrobial, antioxidant, DPPH radical scavenging and SOD-like activity effect of Jubak(AFC) extract using hot water and ethanol solvent. All extracts from Jubak(AFC) had antimicrobial activities in the Bacillus subtilis, Staphylococcus aureus, Pseudomonas aeruginosa and Escherichia coli. Antioxidant activities of the Jubak(AFC) using soybean oil emulsion was confirmed to be higher in the extract using ethanol and hot water than control and the antioxidant was practically effective when concentration of the Jubak was 200 ppm. Scavenging effect of DPPH radical of Jubak extracts showed inhibition effect was above 80%. SOD-like activity were higher in hot water extracts than in ethanol extracts. If keeping up Jubak's study, it could have potential as a cosmetic raw material. Additionally, Jubak could be expected as functional material in food by different extract method.

  • Objectives : The purpose of this study is to report the effectiveness of korean medical treatments on recurrent aphthous stomatitis(RAS) diagnosed as excess pattern(實證) based on symptoms of damum(痰飮), blood stasis(瘀血) and fire-heat(火熱). Methods : We treated a 61-year-old male patient who has RAS by korean medicine, pharmacopuncture and acupuncture treatment. These treatments were performed from 2020.01.22. to 2020.02.29. We have confirmed the improvement of ulcer lesions and pain. We recorded images that include change of symptoms. Results : After the treatments were done, the scores of numeric rating scale(NRS), oral health impact profile-14 (OHIP-14), oral assessment guide (OAG) and world health organization oral toxicity scale (WHO OTS) were decreased. The NRS score was decreased from 7 to 1. The OHIP-14 score was decreased from 30 to 14. The OAG score was decreased from 17 to 9. The WHO OTS score was decreased from 2 to 1. And ulcer lesions were clearly decreased. Conclusions : According to the result, korean medical treatments such as korean medicine, pharmacopuncture and acupuncture treatment, that are targeted at excess pattern(實證) caused by damum(痰飮), blood stasis(瘀血) and fire-heat(火熱) can be effective for the treatment of RAS.

  • The purpose of this study is to determine the effect of the light-emitting-diode (LED) to investigate proliferation of human epidermal keratinocyte and collagen, procollagen expression. In order to determine whether LED irradiation can safely be applied to human skin, the proliferative effects of LED irradiation were determined by MTS assay in Human Epidermal Keratinocytes. Wavelength of 470nm LED irradiation increased mRNA expression of collagen, procollagen without cytotoxity. Our results suggest that 470nm LED irradiation may have a proliferative effects and collagen synthesis property. In order to determine whether LED irradiation can safely be applied to human skin, the cytotoxic effects of LED irradiation were determined by MTS assay in Human Dermal Fibroblasts (HDF). As far as we know, this is the first report demonstrating in vitro collagen synthesis activity of 470nm LED irradiation and being a scientific basis for the cosmetic.

Successful and safe treatment of hemangioma with oral propranolol in a single institution

  • Purpose: Dramatic improvement of hemangioma to propranolol has been recently reported; however, details on dose and duration of treatment, potential risks, and monitoring have not been determined. The objective of this study is to describe and analyze the use of propranolol as a first-line treatment or as a single therapy in management of complicated hemangioma. Methods: A retrospective chart review of eight patients diagnosed with hemangioma and treated with propranolol in Kangbuk Samsung Hospital from February 2010 to April 2011 was performed. Results: Eight patients with hemangioma with functional impairment, cosmetic disfigurement, or rapid growth were treated with propranolol. Five patients had solitary facial hemangioma. The mean age of symptoms at onset was 5 weeks. The median age for starting propranolol treatment was 5.5 months. Propranolol at 2 mg/kg/day was finally administered in divided doses with a gradual increase. Significant regression was observed in seven patients, and shrinkage in size, softening in consistency, and decrease in redness were evident within 4 weeks. Among them, six patients were still taking propranolol, and one patient had stopped after 12 months. Other one patient did not show significant improvement with satisfactory result after 3 months of propranolol use. Treatment with propranolol was well tolerated and had few side effects. No rebound growth was observed in any of the patients. Conclusion: We observed that use of propranolol was very effective in treatment of hemangioma without obvious adverse effects or relapse.

  • The purpose of this study is to analyze the cases of endangered species protection campaigns of domestic and foreign enterprises and to present improvement points and direction. Therefore, this study analyzed the cases and characteristics of the campaign by setting above small and medium sized domestic and foreign companies as subjects for research. The study found that there were mainly campaigns conducted in cooperation with large environmental organizations in Korea, and as an IT powerhouse, there were examples of such campaigns. Foreign companies independently participated in environmental protection, and cases of fashion and cosmetic brand campaigns were easily found. Based on these characteristics, this study aims to present improvements and directions for animal protection campaigns in the future.

Cytotoxicity and Physiological Activity of SunbangHwalmyung-um (선방활명음의 항암 및 항산화효과 검증)

  • Objectives : The purpose of this research was physiological activities and investigate cytotoxicity of Sunbanghwalmyung-um extract. Methods : Physiological activity and a cytotoxicity were examined through the hot water and ethanol extracts from Sunbanghwalmyung-um and its ingredient. Results : The electron donating ability(EDA) was 66.9%, 71.3% in 1000 ppm water extract and ethanol extract. Above 90% from 1000 ppm ethanol extract showed a higher activities and it is Rhei Radix et Rhizoma, Lonicerae Flos, Gleditsiae Spina, Glycyrrhizae Radix, Myrrh. SOD-like activity was weak as 12.24%, 16.62% in 700 ppm. In water and ethanol extracts cytotoxicity were against G361, B16F10, MDA, A549, high cytotoxity over 70%. Rhei Radix et Rhizoma, Gleditsiae Spina, Trichosanthis Radix, Paeoniae Radix Rubra showed high cytotoxicity in water and ethanol extracts. Conclusions : We observed physiological activities and investigated cytotoxicity of Sunbanghwalmyung-um and its ingredients. The results also demonstrated in food or cosmetic industry.

Relationship between Interdental Papilla Existence & Distance from Interdental Alveolar Crest to Contact Point in the Posterior Dentition of Korean adults (한국인 구치열에서 치간유두 존재와 치아접촉점과 치간골 거리와의 관계)

  • The anatomic structure around interproximal area plays an important role not only in the natural teeth, but also in the implant. The loss of papilla can lead to cosmetic deformity, phonetic problem, food impaction on the anterior dentition, and masticatory problem, food impaction and proximal caries on the posterior dentition. The purpose of this study was to evaluate the relationship between interdental papilla existence and distance from contact point to alveolar crest in Korean posteior dentition. 45 Korean adult patients(31males, 14 females) participated in this study. Measurements were carreid out total 126 interproximal areas, 18 first premolar, 31 second premolar, 40 first molar, and 37 second molar areas. Papilla index was recorded as suggested by Jemt. Distance between contact point and alveolar crest measrued by Florida $probe^{R}$, after flap elevation. Each distance was measured 10 times by every 0.1mm unit. The results showed that the mean Papilla index 1.37 and mean distance between contact point and alveolar crest was 7.44mm. The correlation between the Papilla index and distance was high negative correlation(Pearson correlation=-0.47), and it was statistically significant(P=0.000) When the distance between contact point and alveolar crest was 5mm, the loss of papilla was appeared almost in half cases. When the distance was 6mm, the papilla loss was present 95%, when 7mm, the papilla loss was 100%.
