The relationships between panoramic indices and dental implant failure (파노라마방사선사진 지수와 임플란트 실패와의 관계에 관한 연구)

  • Purpose: Several panoramic indices have been suggested to assess bone quality from the morphology and width of mandibular cortex on panoramic radiography. The purpose of this study was to compare dental implant failure group with control group in panoramic mandibular index (PMI), mandibular cortical index (MCI), and gonion index (GI) and to determine the effect of these panoramic indices on dental implant failure. Materials and Methods: A case-control study was designed. Test group (n=42) consisted of the patients who had their implants extracted because of peri-implantitis. Control group (n=139) consisted of the patients who retained their implants over one year without any pathologic changes and had been followed up periodically. They had dental implants installed in their mandibles without bone augmentation surgery from 1991 to 2001. The following measures were collected for each patients: 1) PMI, MCI, and GI were measured twice at one-week interval on preoperative panoramic views; and 2) age, sex, implant length, implant type, installed location, occluding dentition state, and complication were investigated from the chart record. Results: The PMI showed moderate level of repeatability. The intra-observer agreement of MCI and GI were good. There was statistically significant difference in PMI between two groups. There were significant different patterns of distribution of MCI and GI between two groups. Among the panoramic indices, PMI and MCI showed significant correlation with dental implant failure. Conclusion: Panoramic indices can be used as reference data in estimating bone quality of edentulous patients who are to have implants installed in their mandibles.

  • Objectives : The present study attempted to reveal the effects of Morindae Radix extracts on the sciatic nerve neurectomized osteoporotic ddy mice model. Methods : Thickness of hind limbs and their differences, absolute and relative tibia weight and thickness, bone failure load, bone mineral density (BMD), histomorphometrical index, serum osteocalcin level, tibia calcium and phosphorus contents were monitored. Results : In sciatic neurectomized mice, thickness of hind limb, absolute and relative weights, thickness, failure loads, BMD of tibia, trabecular bone volume (TBV), thickness of trabecular bone and cortical bone thickness, length were significantly decreased. However, these changes of those dose-dependently reduced in MR extract-dosing group. Conclusions : it is considered that MR extracts have some favorable effect to prevent, the osteoporosis induced by sciatic neurectomy. However, the exact mechanism and the possibility of MR extract were remains unknown. In addition, the potential toxicity of these MR extracts were also unknown. So the further studies were needed about toxicological and pharmacological aspects.

Clinical and panoramic radiographic features of osteomyelitis of the jaw: A comparison between antiresorptive medication-related and medication-unrelated conditions

  • Purpose: This study was performed to analyze the clinical and imaging features of contemporary osteomyelitis(OM) and to investigate differences in these features on panoramic radiography according to patients' history of use of medication affecting bone metabolism. Materials and Methods: The records of 364 patients(241 female and 123 male, average age 66.8±14.9 years) with OM were retrospectively reviewed. Panoramic imaging features were analyzed and compared between patients with medication-related OM(m-OM) and those with conventional, medication-unrelated OM(c-OM). Results: The age of onset of OM tended to be high, with the largest number of patients experiencing onset in their 70s. The 2 most frequent presumed causes were antiresorptive medication use (44.2%) and odontogenic origin (34.6%). On panoramic radiographs, a mix of osteolysis and sclerosis was the most common lesion pattern observed (68.6%). Sequestrum, extraction socket, and periosteal new bone formation were found in 143(42.1%), 79 (23.2%), and 24 (7.1%) cases, respectively. The m-OM group exhibited sequestrum and extraction socket more frequently and displayed significantly higher mandibular cortical index values than the c-OM group. Conclusion: We observed some differences in imaging features as shown on panoramic radiography according to the history of antiresorptive medication use. This study may help elucidate the predictive imaging features of medication-related osteonecrosis of the jaw.

Computer programme to assess mandibular cortex morphology in cases of medication-related osteonecrosis of the jaw with osteoporosis or bone metastases

  • Purpose: The purpose of this study was to evaluate the morphology of the mandibular cortex in cases of medication-related osteonecrosis of the jaw (MRONJ) in patients with osteoporosis or bone metastases using a computer programme. Materials and Methods: Fifty-four patients with MRONJ (35 with osteoporosis and 19 with bone metastases) were examined using panoramic radiography. The morphology of the mandibular cortex was evaluated using a computer programme that scanned the mandibular inferior cortex and automatically assessed the mandibular cortical index (MCI) according to the thickness and roughness of the mandibular cortex, as follows: normal (class 1), mildly to moderately eroded (class 2), or severely eroded (class 3). The MCI classifications of MRONJ patients with osteoporosis or bone metastases were evaluated with the Pearson chi-square test. In these analyses, a 5% significance level was used. Results: The MCI of MRONJ patients with osteoporosis(class 1: 6, class 2: 15, class 3: 14) tended to be higher than that of patients with bone metastases(class 1: 14, class 2: 5, class 3: 0)(P=0.000). Conclusion: The use of a computer programme to assess mandibular cortex morphology may be an effective technique for the objective and quantitative evaluation of the MCI in MRONJ patients with osteoporosis or bone metastases.

  • The neural bases underlying within or between-language picture naming was investigated by using event-related fMRI. The present suudy explorered the following two goals: The first is to compare cortical activation areas relevant to naming process in native and foreign language, and to decide whether the activation pattern of the foreign word will be the same as native words or not. The next is to find the cerebral areas involved only in alternating language switching between native and foreign language condition. Differential activation patterns were observed for language switching against one-language. Both naming tasks all activated the left inferior frontal gyrus (LIFG) as expected. However the differences in naming between languages were reflected in the activation amount of the LIFG, namely more activation in naming the native language than the foreign language. Especially, naming of the foreign word from English showed the similar area and size in activation with native language suggesting that the process of borrowed noun resembles that of native common noun. And the language switching between languages newly activated the right middle frontal gyrus as well as the left inferior frontal areas. The right middle frontal gyrus engagement in switching conditions obviously identified that right hemisphere is recruited in code switching possibly with respect to meta-cognition controlling language index at a subconscious level.

EXPERIMENTAL STUDY ON THE EFFECT OF CALCITONIN IN THE OSTEOPOROSIS INDUCED BY THE IMMOBILIZATION AND LONG-TERM GLUCOCORTICOID (Glucocorticoid의 장기간 투여 및 운동 억제에 의한 골손실시 calcitonin의 예방효과에 관한 실험적 연구)

  • It is well known that the glucocorticoid induces osteoporosis by suppression of the osteoblast, but its effect on the osteoclast hRS some controversy whether it activates or suppresses the osteoclast. If the calcitonin, which is known to suppress the osteoclast, prevents the osteoporosis by glucocorticoid, then the suppression of the osteoclast by the glucocorticoid is not so significant. And if the calcitonin increases the osteoblastic activity, Tc-99m MDP uptake will be increased in spite of the glucocorticoid effect on the osteoblast. The immobilization operation was performed to the right leg of male Wistar rats weighing about 200gm. each. For 16 weeks after operation, rats were injected glucocorticoid alone or glucocorticoid anci calcitonin. The bone density was measured by means of photodensitometry under reference aluminum step wedge and Tc-99m MDP uptake was available to the index of the osteoblastic activity. 1. The bone density of femora! head was markedly reduced than that of femoral shaft following ration of cancellous and cortical components in both site. 2. Glucocorticoid caused decrease in bone density of spine and femur, md there is significantly increase of it when medication of glucocorticoid and calcitonin injection simultaneously than that of glucocorticoid. 3. Tc-99m MDP uptake was revealed significant reduction in medication of glucocorticoid but increase in gi;.:cocorticoid and calcitonin injection simultaneously in later experimental period. 4. There wail, a slight reduction in plasma osteocalcin in medication of glucocorticoid through experimental periods and an increase in its value in case of giving glucocorticoid and calcitonin simultaneously in later experimental period. From these results, we suggest that osteoporosis by immobilization is more pronounced by glucocorticoid hormone and osteoporosis induced by immobilization and glucocorticoid use is prevented by calcitonin administration with increasing osteoblastic activity.

  • The purpose of this study was to determine the effects of aqua-acupuncture of Laennec N HO (Hominis placenta Cervi pontotrichum cornu Carthami semen). on trabecular area and physiological change in ovariectomized osteoporotic rats. This experiment was performed to .investigate an antiosteoporosis. effects of Laennec N HO(Hominis placenta Cervi pontotrichum cornu Carthmi semen) aqua-acupuncture on the osteoporosis induced by both ovariectomy in Rats. So rats were divided into Sham group and OVX group. OVX groups were divided into control group, Exp. I group, Exp II group and Exp III group). The Ratios of trabecular area seen in ephiphysis and diaphysis of tibia and lumbar vertebral body and physiological change were as follows. 1. In the proximal ephiphysis of tibia, Exp I II III groups were significantly increased compared to that in control group (p<0.05). 2. In the diaphysis of tibia, Exp III group were significantly increased compared to that in control group (p<0.05). 3. In the first lumbar vertebral body, Exp II group were significantly increased compared to that in control group. 4. In the cortical thickness index of tibia, Exp III III groups were significantly increased compared to that in control group. 5. Osteoca1cin was increased in the Exp II group than those of other Experiment groups and control group, and Phosphorus was decreased in the Exp I II group than that in control group, But a significant difference was not seen. GOT, GPT and Alkaline phosphatase were decreased in all Experiment groups than that in control group. But a significant difference was not seen. From the above results, it might be suggested that aqua-acupuncture Laennec N HO(Hominis placenta Cervi pontotrichum cornu Carthami semen) prevented the increase in bone turnover and the decrese in bone mass induced by OVX in rats.

Changes in Sensory Function After Transcranial Direct Current Stimulation on Primary Motor Cortex Area

  • Transcranial direct current stimulation (tDCS) is a neuromodulatory technique that delivers low-intensity direct current to cortical areas, thereby facilitating or inhibiting spontaneous neuronal activity. This study was designed to investigate changes in various sensory functions after tDCS. We conducted a single-center, single-blinded, randomized trial to determine the effect of a single session of tDCS with the current perception threshold (CPT) in 50 healthy volunteers. Nerve conduction studies were performed in relation to the median sensory and motor nerves on the dominant hand to discriminate peripheral nerve lesions. The subjects received anodal tDCS with 1 mA for 15 minutes under two different conditions, with 25 subjects in each groups: the conditions were as follows tDCS on the primary motor cortex (M1) and sham tDCS on M1. We recorded the parameters of the CPT a with Neurometer$^{(R)}$ at frequencies of 2000, 250, and 5 Hz in the dominant index finger to assess the tactile sense, fast pain and slow pain, respectively. In the test to measure CPT values of the M1 in the tDCS group, the values of the distal part of the distal interphalangeal joint of the second finger statistically increased in all of 2000 Hz (p=.000), 250 Hz (p=.002), and 5 Hz (p=.008). However, the values of the sham tDCS group decreased in all of 2000 Hz (p=.285), 250 Hz (p=.552), and 5 Hz (p=.062), and were not statistically significant. These results show that M1 anodal tDCS can modulate sensory perception and pain thresholds in healthy adult volunteers. The study suggests that tDCS may be a useful strategy for treating central neurogenic pain in rehabilitation medicine.

Effect of Korean Red Ginseng on radiation-induced bone loss in C3H/HeN mice

  • This study investigated the effects of Korean Red Ginseng (KRG) on radiation-induced bone loss in C3H/HeN mice. C3H/HeN mice were divided into sham and irradiation (3 Gy, gamma-ray) groups. The irradiated mice were treated for 12 wk with vehicle, KRG (per os, p.o.) or KRG (intraperitoneal). Serum alkaline phosphatase (ALP), tartrate-resistant acid phosphatase, estradiol level, and biomechanical properties were measured. Tibiae were analyzed using micro-computed tomography. Treatment of KRG (p.o., 250 mg/kg of body weight/d) significantly preserved trabecular bone volume, trabecular number, structure model index, and bone mineral density of proximal tibia metaphysic, but did not alter the uterus weight of the mice. Serum ALP level was slightly reduced by KRG treatment. However, grip strength, mechanical property, and cortical bone architecture did not differ among the experimental groups. The results indicate that KRG can prevent radiation-induced bone loss in mice.

Utility of $^{99m}Tc$-MAG3 Perfusion Indices in the Evaluation of Renal Transplant Function During Early Post-transplantation Period (이식 초기 이식신 기능 평가에서 $^{99m}Tc$-MAG3 신관류 지표의 유용성)

  • Purpose: We have examined the utility of $^{99m}Tc$-MAG3 perfusion indices for assessing renal graft function in early post-transplantation period. Materials and Methods: Our study included 80 renal transplant recipients (48 men and 32 women, mean age: 40.3 years). Diagnosis was based on biopsy, laboratory data and clinical course. Renal scintigraphy (RS) was obtained using 100 MBq of $^{99m}Tc$-MAG3 from 11 days to 23 days of kidney transplantation. We measured 5 indices in whole-kidney (WK) and cortical (C) renograms; Hilson's perfusion index (PI), transplant perfusion index (TP) and transplant function index (TF) as perfusion parameter, and the time to peak activity (Tmax) and the ratio of renal counts at 20 min to that at 3 min (K20/3) as functional parameter. Results: The diagnoses at the day of RS were normal graft (NG) in 44, acute rejection (AR) in 14, acute tubular necrosis (ATN) in 10, and Cyclosporine A nephrotoxicity (CsA) in 12. TP and TF were significantly decreased in AR, ATN and CsA, compared to those in NG. K20/3 of AR and ATN wore significantly greater than that of NG. WK-Tmax of AR was significantly longer than that of NG. K20/3 of AR and C-K20/3 of ATN were significantly prolonged relative to those of CsA. There were no statistically significant perfusion indices among complication groups. Conclusion: TP and TF reflecting microperfusion and initial tubular extraction are reliable in assessing graft function. However, it is required to correlate perfusion indices with functional indices and clinical course in differentiating from one another among complication groups.
