Investigation of a pre-clinical mandibular bone notch defect model in miniature pigs: clinical computed tomography, micro-computed tomography, and histological evaluation

  • Objectives: To validate a critical-size mandibular bone defect model in miniature pigs. Materials and Methods: Bilateral notch defects were produced in the mandible of dentally mature miniature pigs. The right mandibular defect remained untreated while the left defect received an autograft. Bone healing was evaluated by computed tomography (CT) at 4 and 16 weeks, and by micro-CT and non-decalcified histology at 16 weeks. Results: In both the untreated and autograft treated groups, mineralized tissue volume was reduced significantly at 4 weeks post-surgery, but was comparable to the pre-surgery levels after 16 weeks. After 16 weeks, CT analysis indicated that significantly greater bone was regenerated in the autograft treated defect than in the untreated defect (P=0.013). Regardless of the treatment, the cortical bone was superior to the defect remodeled over 16 weeks to compensate for the notch defect. Conclusion: The presence of considerable bone healing in both treated and untreated groups suggests that this model is inadequate as a critical-size defect. Despite healing and adaptation, the original bone geometry and quality of the pre-injured mandible was not obtained. On the other hand, this model is justified for evaluating accelerated healing and mitigating the bone remodeling response, which are both important considerations for dental implant restorations.

Bone Regenerative Effects of Biphasic Calcium Phosphate Collagen, Bone Morphogenetic Protein 2, Mesenchymal Stem Cells, and Platelet-Rich Plasma in an Equine Bone Defect Model

  • Fractures in the horse industry are challenging and a common cause of death in racehorses. To accelerate fracture healing, tissue engineering (TE) provides promising ways to regenerate bone tissues. This study aimed to evaluate the osteogenic effects of biphasic calcium phosphate collagen (BCPC) graft, bone morphogenetic protein 2 (BMP2), mesenchymal stem cell (MSC), and platelet-rich plasma (PRP) treatments in horses. Four thoroughbred horses were included in the study, and, in each horse, three cortical defects with a diameter of 5 mm and depth of 10 mm were formed in the third metacarpal bones (MC) and metatarsal bones (MT). The defects were randomly assigned to one of six treatment groups (saline, BCPC, BMP2, MSC, PRP, and control). Injections of saline, BMP2, PRP, or MSCs were made at 1, 3, and 5 weeks after defect surgery. Bone regeneration effects were assessed by radiography, quantitative computed tomography (QCT), micro-computed tomography (μCT), histopathological, and histomorphometric evaluation. The new bone ratio (%) in the histomorphometric evaluation was higher in the BMP2 group than in the control and saline groups. Radiographic and QCT values were significantly higher in the BCPC groups than in the other groups. QCT values of the BMP2 group were significantly higher than in the control and saline groups. The present study demonstrated that BCPC grafts were biologically safe and showed osteoconductivity in horses and the repeated injections of BMP2 without a carrier can be simple and promising TE factors for treating horses with bone fractures.

Incidental occurrence of an unusually large mastoid foramen on cone-beam computed tomography and review of the literature

  • The incidental finding of an enlarged mastoid foramen on the right posterior mastoid region of temporal bone is reported, together with a discussion of its clinical significance. A 67-year-old female underwent the pre-implant assessment of a maxillary left edentulous region. A cone-beam computed tomographic (CBCT) image was acquired and referred for consultation. Axial CBCT slices revealed a unilateral, well-defined, noncorticated, low-attenuation, transosseous defect posterior to the mastoid air cells in the right temporal bone. The borders of the osseous defect were smooth and continuous. No other radiographic signs suggestive of erosion or sclerosis were noted in the vicinity. The density within the defect was homogenous and consistent with a foramen and/or soft tissue. The patient's history and physical examination revealed no significant medical issues, and she was referred to a neuroradiologist for a second opinion. The diagnosis of an enlarged mastoid foramen was made and the patient was reassured.

성견에서 골유착성 타이타늄 임프란트의 기능적 노출 시기가 주위의 골형성에 미치는 영향 (INFLUENCE OF A FUNCTIONAL LOADING TIME ON BONE FORMATION AROUND OSSEOINTEGRATED TITANIUM IMPLANTS IN ADULT DOGS)

  • The purpose of this study was to investigate the influence of early functional load around osseointegrated titanium implants. 24 titanium plasma spray coated implants (ITI HS-type) were placed into the previously extracted site in the mandible of six adult dogs. The implants were divided into three groups : the control group was the implants without abutment during the experimental period; the experimental group I was loaded by connecting the contoured abutment after 6 weeks of healing; the experimental group II was loaded after 12 weeks of healing: and the mandibular second premolar and surrounding tissues were selected for natural tooth group to compare the implanted group. All dogs were injected intravenously tetracycline, alizarin red S, and calcein for bone labeling. After the experimental period of 18 weeks, the dogs were sacrificed and longitudinal sections of the bone-implant interface were cut and observed using light microscope, scanning electron microscope, and fluorescence microscope. The results of the study were as follows: 1. Light and scanning electron microscopically, all implant surfaces were well contact with bone tissue at the cortical layer, but some areas of cancellous bone were not contact directly. 2. Fluorescence microscopically, number and size of the new secondary osteons around the implant were increased than those of the natural tooth. 3. Fluorescence microscopically, linear and concentrical fluorescence was observed at or near the surface of all implants, and the bone formation and remodeling of the implants loaded after 6 week of healing were great, and unloaded implants were worst. 4. Fluorescence microscopically, endosteal bone formation was greater than periosteal bone formation at or near the implants. 5. Fluorescence microscopically, number and size of linear and concentric fluorescence was increased at the lingual side than the buccal side of the loaded implants. The result of the study indicate the possibility of the early load to the implant via a prosthesis.

전외측 대퇴부 천공지 피판을 이용한 만성 경골 골수염에 동반된 하지 전방 연부조직 병변의 재건 (Reconstruction of the Pretibial Soft Tissue Lesion after Chronic Tibia Osteomyelitis using Anterolateral Thigh Perforator Flap)

  • The purpose of this study was to present the clinical result of anterolateral thigh free flap for pretibial soft tissue lesion after chronic tibia osteomyelitis. From December 2006 to September 2008, Five patients were included in our study. 4 of 5 were superficial or localized types of chronic tibia osteomyelitis, based on the classification of Cierny and Mader. Average age at the surgery was 45 years, three were males and two were females. All had a history of chronic tibia osteomyelitis and subsequent pretbial soft tissue lesions coming from previous operations or pus drainage. Pretibial soft tissue defects included small ulcers, fibrotic, bruisable soft tissue and small bony exposures, but not large-sized bony exposures nor active pus discharge. After complete debridement of large sized pretibial soft tissue lesions and decortication of anterior tibial cortical dead bone, anterolateral thigh free flap was applied to cover remained large pretibial soft tissue defect and to prevent the recurrence of infection. All flaps survived and provided satisfactory coverage of soft tissue defect on pretibial region for 16 months' mean follow up period. No patients has had recurrence of osteomyelitis. Anterolateral thigh free flap could be recommend for large sized pretibial soft tissue defect of supreficial or localized types of chronic tibia osteomyelitis after through debridement.

콘빔형 전산화단층영상에서 HU에 의한 두경부 팬텀 경조직의 밀도 측정 (Measurement of hard tissue density of head phantom based on the HU by using CBCT)

  • Purpose : The purpose of this study was to determine a conversion coefficient for Hounsfield Units(HU) to material density ($g\;cm^{-3}$) obtained from cone-beam computed tomography ($CBMercuRay^{TM}$) data and to measure the hard tissue density based on the Hounsfield scale on dental head phantom. Materials and Methods : CT Scanner Phantom (AAPM) equipped with CT Number Insert consists of five cylindrical pins of materials with different densities and teflon ring was scanned by using the $CBMercuRay^{TM}$ (Hitachi, Tokyo, Japan) volume scanner. The raw data were converted into DICOM format and the HU of different areas of CT number insert measured by using $CBWorks^{TM}$. Linear regression analysis and Student t-test were performed statistically. Results : There was no significant difference (P > 0.54) between real densities and measured densities. A linear regression was performed using the density, $\rho$($g\;cm^{-3}$), as the dependent variable in terms of the HU (H). The regression equation obtained was $\rho=0.00072H-0.01588$ with an $R^2$ value of 0.9968. Density values based on the Hounsfield scale was $1697.1{\pm}24.9\;HU$ in cortical bone, $526.5{\pm}44.4\;HU$ in trabecular bone, $2639.1{\pm}48.7\;HU$ in enamel, $1246.1{\pm}39.4\;HU$ in dentin of dental head phantom. Conclusion : CBCT provides an effective option for determination of material density expressed as Hounsfield Units.

Effects and Mechanisms of Bone Marrow Mesenchymal Stem Cell Transplantation for Treatment of Ischemic Stroke in Hypertensive Rats

  • Background and Objectives: Stroke is the most common cause of human death and functional disability, resulting in more than 5 million deaths worldwide each year. Bone marrow mesenchymal stem cells (BMSCs) are a kind of stem cell that are able to self-renew and differentiate into many types of tissues. Therefore, BMSCs have the potential to replace damaged neurons and promote the reconstruction of nerve conduction pathways and connective tissue. However, it remains unknown whether transplanted BMSCs promote angiogenesis or improve the tissue microenvironment directly or indirectly through paracrine interactions. This study aimed to determine the therapeutic effect of BMSCs on ischemic stroke with hypertension in a rodent model and to explore the possible mechanisms underlying any benefits. Methods and Results: Middle cerebral artery occlusion was used to establish the experimental stroke model. The area of cerebral infarction, expression of vascular endothelial growth factor (VEGF) and glial cell line-derived neurotrophic factor (GDNF), and increment of astrocyte were measured by TTC staining, western blot, real-time quantitative polymerase chain reaction (RT-qPCR) and immunocytochemistry. The results showed a smaller area of cerebral infarction and improved neurological function scores in animals treated with BMSCs compared to controls. The results of RT-qPCR and western blot assays showed higher expression of VEGF and GDNF in BMSC-treated animals compared with controls. Our study also showed that one round of BMSCs transplantation significantly promoted the proliferation of subventricular zone and cortical cells, especially astrocytes, on the ischemic side following cerebral ischemia. Conclusions: Above findings support that BMSCs have therapeutic effects for ischemic stroke complicated with hypertension, which may occur via up-regulated expression of VEGF and GDNF and reduction of neuronal apoptosis, thereby promoting the recovery of nerve function.

Alveolar ridge augmentation with the perforated and nonperforated bone grafts

  • Purpose: Autogenous bones are frequently used because of their lack of antigenicity, but good osteoconductive and osteoinductive properties. This study evaluated the biological behavior of perforated and nonperforated cortical block bone grafts. Methods: Ten nonsmoking patients who required treatment due to severe resorption of the alveolar process and subsequent implant installation were included in the study. The inclusion criteria was loss of one or more teeth; the presence of atrophy of the alveolar process with the indication of reconstruction procedures to allow rehabilitation with dental implants; and the absence of systemic disease, local infection, or inflammation. The patients were randomly divided into two groups based on whether they received a perforated (inner surface) or nonperforated graft. After a 6-month healing period, a biopsy was performed and osseointegrated implants were installed in the same procedure. Results: Fibrous connective tissue was evident at the interface in patients who received nonperforated grafts. However, full union between the graft and host bed was visible in those who had received a perforated graft. Conclusions: We found that cortical inner side perforations at donor sites increased the surface area and opened the medullary cavity. Our results indicate an increased rate of graft incorporation in patients who received such perforated grafts.

Finite Element Analysis of Stress Distribution around Patterned Implants

  • Purpose: The purpose of this study was to investigate the effect of patterning on the stress distribution in the bone tissue using the finite element analysis (FEA) model. Materials and Methods: For optimal comparison, it was assumed that the implant was axisymmetric and infinitely long. The implant was assumed to be completely embedded in the infinitely long cortical bone and to have 100% bone apposition. The implant-bone interface had completely fixed boundary conditions and received an infinitely big axial load. von Mises stress and maximal principal stress were analyzed. Conventional thread and 2 or 3 patterns on the upper and lower flank of the thread were compared. Result: The surface areas of patterned implants were increased up to 106~115%. The thread with patterns distributed stress better than conventional thread. Patterning in threads may produce more stress in the implant itself, but reduce stress in the surrounding bone. Stress patterns of von Mises stress were favorable with patterns, while the maximal principal stress was increased with patterns. Patterns in the lower flank showed favorable stress distribution. Conclusion: The patterns in implant thread reduce the stress generated in surrounding bone, but the number and position of patterns were crucial factors in stress distribution.

이중에너지 CT와 같은 시뮬레이션을 이용한 유효원자번호 추출을 통한 췌장 검출 가능성 연구 (A Study on the Possibility of Pancreas Detection through Extraction of Effective Atomic Number using a Simulation such as Dual-energy CT)

  • 본 연구의 목적은 이중에너지 CT를 이용한 유효원자번호 정보를 통한 췌장 검출 가능성 연구이다. 10개의 다양한 인체 등가 물질의 유효원자번호를 Stoichiometric calibration을 통해 추정하였다. Stoichiometric calibration을 위해 저에너지와 고에너지에 해당하는 10개 인체 등가 물질에 대한 HU값을 이용하였다. 이를 바탕으로 반복 알고리즘을 통해 인체 등가 물질에 대한 유효원자번호 영상을 추출하였다. 연구결과에 따르면, 유효원자번호에 따른 감약의 비는 R2값이 0.9999로 추정되었고, Pancreas, water, Liver, Blood, Spongiosa, Cortical bone의 유효원자번호는 이론값과 비교하여 전체적으로 1% 이내의 정확도를 보였다. 췌장암 검사는 조영제를 사용하므로 잠재적인 조영제 부작용 가능성이 있다. 이를 해결하기 위해 조영 증강 없는 이중에너지를 이용한 유효원자번호 추출을 통해 정확하고 안전한 검사에 기여할 수 있을 것으로 사료된다. 본 연구를 바탕으로 향후 연구에서는 임상 영상을 바탕으로 췌장암의 HU값을 이용하여 췌장암 검출에 대한 연구를 수행할 것이다.
