Preliminary Standard Procedure for Face Lift and Correction of Nasolabial Fold using Thread-Embedding (Maeseon) of Korean Medicine (안면거상 및 팔자주름 개선을 위한 매선 시술 표준안 제안)

  • Objectives : This study aims to suggest preliminary standard procedure for face lift and correction of nasolabial folds using thread-embedding (Maeseon) of Korean medicine(KM). Methods : Three KM practitioners of facial correction and rejuvenation who have over hundred case of practice participated in establishment of standard procedure. Standard procedure contains preprocessing, main procedure for correction, and solution of side effects. Results : Standard procedure is comprised of twelve processes with preprocessing and postprocessing. Preprocessing has position, disinfection, and anesthesia. Main process consists of overall structure correction, face lifting, nasolabial folds correction, and mesh making on cheek. Postprocess covers disinfection, edema prevention. Conclusions : To our knowledge, this is the first work to suggest standard procedure of facial rejuvenation using Maeseon. It would contribute to standardized practice in clinical fields and future study of revealing Maeseon's effectiveness.

A Improved Scene based Non-uniformity Correction Algorithm for Infrared Camera

  • In this paper, we propose an efficient scene based non-uniformity correction algorithm which performs the offset correction using the uniform obtained from input scenes for Infrared camera. In general, pixel outputs of a infrared detector can not be uniform. Therefore, the non-uniformity correction procedure need to be performed to make the image outputs uniform. A typical non-uniformity correction method uses a black body at the laboratory to obtain the output of the infrared detector's pixels for two temperatures, HOT and COLD, and calculates the non-uniformity correction parameters. However, output characteristics of the Infrared detector changes while the Infrared camera is operated, the fixed pattern noise of the Infrared detector and dead pixels are generated. To remove the noise, the offset correction is generally performed. The offset correction procedure usually need the additional device such as a thermo-electric cooler, shutter, or non-uniformity correction lens. Therefore, we introduce a general scene based non-uniformity correction technique without additional equipment, and then we propose an improved non-uniformity correction algorithm based on image to solve the problem of the existing technique.

  • Scoliosis is a complex musculoskeletal dieses requiring 3-D treatment with surgical instrumentation. To investigate the effects of correction surgery, a finite element model of personalized model of the scoliotic spine that will allow the design of clinical test providing optimal estimation of the post-operation results was developed. Three dimensional skeletal parts, such as vertebrae, clavicle and scapular were modeled as rigid bodies with keeping their morphologies. Kinematical joints and spring elements were adapted to represent the inter-vertebral disc and ligaments respectively. With this model, two types of surgery procedure, distraction procedure with Harrington device and rod derotation procedure with pedicle screw and rod system had been carried out. The obtained simulation results were comparatively corresponding to the post operational outcomes and successfully demonstrated qualitative analysis of surgical effectiveness. From this analysis, it has been found that the preparing of appropriate rod curvature and its insertion was more important than just performing the excessive derotation for scoliosis correction.

  • Interventricular Septal Defect is probably the most common congenital cardiac lesion. Despite rapid technical advances and increasing surgical experience the risk of surgical intervention for correction of Ventricular Septal Defect in infants with pulmonary hypertension remains formidable. Since Sirak et al [1959] reported a succesful case of two stage approach to their surgical correction, it has led to a policy of primary palliation,followed by complete correction as a secondary procedure, after age 3 to 4 years. Most surgeon prefer to perform complete correction of Ventricular Septal Defect when body weight exceeds 30 Lbs. and before development of so-called Eisenmengers complex, for the good postoperative results. Authors report 2 cases of Ventricular Septal Defect with pulmonary hupertenslon, who was underwent pulmonary artery banding as a palliative procedure in the Department of Surgery,Severance Hospital Yonsei University. Case 1:4 year old male, initially a complete correction of Ventricular Septal Defect was attempted by the help of mild hypothermia and extracorporeal circulation. During the procedure of a construction of an extracorporeal by- pass, a sudden cardiac arrest developed. After resuscitation of the heart,pulmonary artery banding was performed as a palliation. On the first postoperative day the patient developed generalized tonic convulsion, cyanosis, vomiting and eventually shock. Patient discharged home after a full recovery. Case 2.: 9 month old female, the pulmonary artery constricted with Teflon patch successfully. After the patients first postoperative day several cyanotic spells developed followed by 3 cardiac arrests. This repeated until when she expired with respiratory failure.

A New Approach on the Correction for Compton Escape Component in X-Ray Unfolding Algorithm

  • A new approach on the correction for Compton escape component in X-ray unfolding algorithm was investigated to obtain more accurate X-ray source spectrum. The X-ray detector used in this study was a planar type HPGe detector(EG&G ORTEC, GLP-32340/13-P-LP) whose energy response has been blown and ISO narrow beam series were employed as source spectrum. At lower energy Part of measured X-ray spectrum including the correction for Compton escape component more accurate unfolded spectrum was obtained by letting down the starting energy level of the collection in existing spectrum correction procedure to consider multiple scattering effects. It is, from this study, concluded that accurate correction for Compton escape component is needed in X-ray unfolding procedure since Compton scattering becomes more important as incident X-ray energies increase.

  • Scoliosis is a complex musculoskeletal dieses requiring 3-D treatment with surgical instrumentation. Conventional corrective surgery for scoliosis was done based on empirical knowledge without information of the optimum position and operative procedure. Frequently, post operative change of rib hump increase and shoulder level imbalance caused serious problems in the view of cosmetics. To investigate the effect of correction surgery, a reconstructed 3-D finite element model for King-Moe type V was developed. Vertebrae, clavicle and other bony element were represented using rigid bodies. Kinematic joints and nonlinear bar elements used to represent the intervertebral disc and ligaments according to reported experimental data. With this model, optimization technique was also applied in order to define the optimal magnitudes of correction. The optimization procedure corrected the scoliotic deformities by reducing the objective function by more than 94%. with an associated reduction of the scoliotic descriptors mainly on the frontal thoracic curve.

Long-term Surveillance Comparing Satisfaction between the Early Experience of Nuss Procedure vs. Ravitch Procedure

  • Background: Long-term surveillance comparing satisfaction between the early experience of Nuss procedure vs. Ravitch procedure. Materials and Methods: A total of 100 patients that underwent surgical correction of a pectus excavatum between 2001 and 2004 and were followed for ${\geq}2$ years were included. Surveillance on the degree of satisfaction was performed using five-levels of the Likert scale and self-assessment scoring. Results: Nuss or Ravitch surgery was performed in 63 and 37 patients, respectively. The Nuss procedure required a shorter operation time and shorter hospital stay than the Ravitch procedure (p<0.001). The surveillance demonstrated that 17.6% of the Nuss group and 35.7% of the Ravitch group were not satisfied with the outcome of the surgery (p=0.072). The most common causes of dissatisfaction were redepression in the Nuss group (n=5) and incomplete correction in the Ravitch group (n=7). The multivariate analysis showed that reoperation and a high postoperative pectus index were significant risk factors for a low satisfaction score. Conclusion: The Nuss procedure had several advantages over the Ravitch procedure in the immediate postoperative period. However, the long-term satisfaction was determined by a complete correction without recurrence or need for re-intervention rather than by the operation type.

Photo Epilation with Intense Pulsed Light for Thinning of Anterior Hairline after Hairline Correction Surgery in East Asians

  • Background Thin hairs are critical to achieve natural result in female hairline correction surgery. However, there are few studies on the usefulness of hair thinning by intense pulsed light (IPL) after hairline correction surgery in East Asian females. Methods Hair thinning using IPL was performed in 54 women who had complained about thick hairs along the frontal hairline after hairline correction surgery. Patient mean age was 31.2 years old and patients were an average of 2.1 years post-hairline correction surgery. Initial treatment used 10 J, while second and third sessions were conducted with 10 to 15 J according to responsiveness to treatment. Results Mean thickness of individual hairs assessed before the procedure was $78.86{\mu}m$. The mean number of procedures was 1.6 per patient. Forty of 54 subjects (74%) achieved satisfactory hair thinning with only one procedure from 78.01 to $66.14{\mu}m$ after treatment. The measured thickness was $66.43{\mu}m$ at the end of the first year in patients who were satisfied after one procedure. Thirteen cases achieved satisfactory hair thinning after two sessions. Mean thickness was $74.44{\mu}m$ and $67.51{\mu}m$, before and after the second session. One case required a third session with 15J, thinning from 89.00 to $66.50{\mu}m$. Conclusions Hair thinning by IPL is a very useful method to provide a natural look after hairline correction surgery in East Asians, who have naturally thick hair.

Correction of Lip Canting Using Bioabsorbables during Orthognathic Surgery

  • Lip canting is associated with facial asymmetry, and is one of the most challenging problems in surgical correction of facial deformities. The author corrected lip canting using bioabsorbable devices during orthognathic surgery. Soft tissue suspension procedures were performed on four patients with facial asymmetry. Lip lines improved for all patients. Over an observation period of five years, no complications were noted, nor did any late relapse develop. Furthermore, as time past, the effect of the Endotine suspension procedure increased probably due to induction of fibrosis on surrounding soft tissues.

  • In this study, a new meta-heuristic optimization algorithm, Vision Correction Algorithm (VCA), designed according to the optical properties of glasses was developed. The VCA is a technique applying optometry and vision correction procedure to optimization algorithm through the process of myopic/hyperopic correction-brightness adjustment-compression enforcement-astigmatism adjustment. The proposed VCA unlike the conventional meta-heuristic algorithm is an automatically adjusting global/local search rate and global search direction based on accumulated optimization results. The proposed algorithm was applied to the representative optimization problem (mathematical and engineering problem) and results of the application are compared with that of the present algorithms.
