Pre-existing Immunity to Endemic Human Coronaviruses Does Not Affect the Immune Response to SARS-CoV-2 Spike in a Murine Vaccination Model

  • Endemic human coronaviruses (HCoVs) have been evidenced to be cross-reactive to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Although a correlation exists between the immunological memory to HCoVs and coronavirus disease 2019 (COVID-19) severity, there is little experimental evidence for the effects of HCoV memory on the efficacy of COVID-19 vaccines. Here, we investigated the Ag-specific immune response to COVID-19 vaccines in the presence or absence of immunological memory against HCoV spike Ags in a mouse model. Pre-existing immunity against HCoV did not affect the COVID-19 vaccine-mediated humoral response with regard to Ag-specific total IgG and neutralizing Ab levels. The specific T cell response to the COVID-19 vaccine Ag was also unaltered, regardless of pre-exposure to HCoV spike Ags. Taken together, our data suggest that COVID-19 vaccines elicit comparable immunity regardless of immunological memory to spike of endemic HCoVs in a mouse model.

The COVID-19 Pandemic: Fears and Overprotection in Pediatric Patients with Inflammatory Bowel Disease and Their Families

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has influenced the lives of people worldwide. Little is known about the effects of the COVID-19 pandemic on the behavior and fears of pediatric patients with inflammatory bowel disease (IBD) and their families. We conducted a survey to determine the COVID-19 exposure, related perceptions, and information sources; medication compliance; and patients' and parents' behaviors, fears, and physician contact. Methods: An anonymous cross-sectional survey of pediatric patients with IBD and their parents at one pediatric gastroenterology unit of a university medical center was performed. Results: A total of 46 pediatric patients with IBD and 44 parents completed the survey. Parents of pediatric patients with IBD had high fear of their children becoming infected with severe acute respiratory syndrome coronavirus 2. They perceived schools as the most hazardous environment, whereas the children did not. Half the pediatric patients with IBD feared infection. Patients and parents felt sufficiently informed about COVID-19. The primary source of guidance for pediatric patients was their parents (43%), followed by television and social media, whereas the parents mainly consulted internet news websites (52.2%), television, and public health institutes. Pediatric patients with IBD adhered to their prescribed medication. They also showed cautious behavior by enhancing hand hygiene (84%) and leaving the house less frequently than before. However, in-person medical visits remained favored over video consultations. Conclusion: Although parents expressed overprotective concerns, both parents and pediatric patients with IBD are coping well with the COVID-19 pandemic. IBD-relevant information should be actively conveyed.

A Comparative Analysis of Healthcare-Associated Infection Policy in South Korea and Its Implications in Coronavirus Disease 2019

  • Background: Infection prevention and control (IPC) to manage healthcare-associated infection (HCAI) has emerged as one of the most significant public health issues in Korea. The purpose of this study is to draw implications in IPC policies by analyzing the context, process, and major actors in policy development and comparatively analyzing IPC policy contents of Korea with three other countries. Additionally, IPC policies were analyzed in the context of coronavirus disease 2019 (COVID-19) to provide implications for future pandemics and HCAI events. Methods: This study incorporates a qualitative approach based on document and content analysis, applying codes and thematic categorization. IPC policy contents are comparatively analyzed by adopting the concept model, developed by the World Health Organization, which consists of core components of IPC structure at the national and facility level. Results: National IPC policies were developed within a complex social and political context, through the involvement of various stakeholders. IPC policies in Korea place a high emphasis on establishing IPC programs and built environments in healthcare facilities, whereas there were potentials for improvement in policies involving patients and promoting a safety culture. IPC policies, which currently focus on general hospitals and certain functions of hospitals, should further be expanded to target all healthcare facilities and functions, to ensure more efficient and sustainable IPC responses in the current and future disease outbreaks. Conclusion: IPC is a complex policy arena and lessons learned from the analysis of existing policies in the context of COVID-19 should provide valuable strategic implications for future policies.

1차 확산기 이후 코로나바이러스감염증-2019의 전파가 부모와 아동의 심리에 미치는 영향 (The Effect of the Transmission of Coronavirus Disease-2019 on the Mentality of Parents and Children After the First Wave of Infections)

  • 이 연구의 목적은 코로나바이러스감염증-2019의 전파가 학령기 아동과 부모의 심리에 미친 영향을 수면장애와 우울증의 측면에서 평가하는 것이었다. 2020년 4월 2일부터 2020년 4월 25일까지 강동 경희대학교 치과병원 소아치과를 방문한 123명의 부모와 108명의 학령기 아동을 대상으로 직접 기입 방식의 설문조사를 실시하였다. 참가자들은 Pittsburgh Sleep Quality Index, Generalized Anxiety Disorder (GAD) - 7, Center for Epidemiology Scale for Depression 로 평가되었다. 로지스틱 회기 분석은 유의 수준 5%로 시행되었다. 부모의 GAD, 우울증, 낮은 수면의 질은 각각 34.1%, 17.1%, 44.7%에서 나타났다. 아동의 GAD 유병률은 20.4%였다. 로지스틱 회기분석 결과 코로나바이러스 관련 안전재난문자로 인한 스트레스가 부모의 불안장애, 우울증과 관련이 있었다. 아동의 경우, 코로나바이러스감염증 발생 이후의 감정 변화 정도가 GAD와 연관성이 있었다. 이 연구를 통해 코로나바이러스감염증-2019의 유행으로 인한 어린이와 보호자들의 심리적 상태 변화가 있음을 확인하였고, 치과진료 시 이들의 심리상태를 고려해야 하겠다.

인간 코로나 바이러스를 중심으로 2017-2019년 소아청소년 호흡기 바이러스 감염증의 역학 및 특성: 경기 북서부지역 단일기관의 후향적 연구 (Epidemiology and Characteristics of Pediatric Respiratory Virus Infection From 2017 to 2019 Focusing on Human Coronavirus: A Retrospective Study of a Single Center in Northwestern Gyeonggi-do)

  • 목적: Coronavirus disease 2019 (COVID-19) 발생 이후 인간 코로나바이러스를 포함한 호흡기 바이러스의 패턴에 변화가 있을 것으로 생각되어, COVID-19 발생 이전 경기도 명지병원에 급성 호흡기 감염증으로 입원한 소아청소년을 대상으로 바이러스 감염의 패턴을 보고하였다. 방법: 2017년 1월부터 2019년 12월까지 급성 호흡기 감염증으로 명지병원에 입원한 18세 이하 소아청소년을 대상으로 하였고 real-time reverse transcription polymerase chain reaction (RT-PCR) 결과를 바탕으로 후향적으로 의무기록을 고찰하였다. 결과: 총 3,557명 중에서 중복감염 포함하여 3,686건의 바이러스가 검출되었고 평균 양성률은 78.6%이었으며 여름에 비해 겨울에 PCR 양성률이 높았다. 파라인플루엔자바이러스, 메타뉴모바이러스, 보카바이러스는 4-6월에 주로 검출되고, 인간엔테로바이러스는 7-9월에, 호흡기세포융합바이러스와 인플루엔자바이러스는 겨울철에 주로 검출되었다. 진단별로 인두편도염에서는 아데노바이러스가 가장 많았고 폐렴에서는 호흡기세포융합바이러스가 가장 많이 검출되었다. 인간 코로나 바이러스는 겨울철에 주로 검출되었으며, 크루프 환자 중에서 3번째 높은 빈도로 나타났다. 결론: 본 연구결과가 이전에 보고되었던 소아청소년의 호흡기 바이러스 역학과 크게 다르지 않았다. 또한 COVID-19 발생 이후 호흡기 바이러스 패턴 비교 연구에 도움이 되고자 COVID-19 발생 이전 호흡기 바이러스 역학정보를 보고하는 바이다.

COVID-19 and veterinarians for one health, zoonotic- and reverse-zoonotic transmissions

  • A novel coronavirus emerged in human populations and spread rapidly to cause the global coronavirus disease 2019 pandemic. Although the origin of the associated virus (severe acute respiratory syndrome coronavirus 2 [SARS-CoV-2]) remains unclear, genetic evidence suggests that bats are a reservoir host of the virus, and pangolins are a probable intermediate. SARS-CoV-2 has crossed the species barrier to infect humans and other animal species, and infected humans can facilitate reverse-zoonotic transmission to animals. Considering the rapidly changing interconnections among people, animals, and ecosystems, traditional roles of veterinarians should evolve to include transdisciplinary roles.
