  • The objective of this investigation is to understand the role of hemodynamics in the formation and development of atherosclerosis lesions in the human left coronary artery This study also aims to compare the blood flow characteristics of steady and physiological flows. Three dimensional, steady and physiological flows of blood in the left coronary artery are simulated using the Finite Volume Method. Apparent viscosity of blood is represented as a function of shear rate by the Carreau model. Distributions of velocity, pressure and shear stress in tile left coronary artery bifurcation are presented to compare tile steady and physiological flow characteristics.

  • In this study, We subtracted the calcification blooming artifact from MDCT images of coronary atherosclerosis patients and verified their accuracy and usefulness. We performed coronary artery calcification stenosis phantom and a program to subtract calcification blooming artifact by applying 8 different image segmentation method (Otsu, Sobel, Prewitt, Canny, DoG, Region Growing, Gaussian+K-mean clustering, Otsu+DoG). As a result, In the coronary artery calcification stenosis phantom with the lumen region 5 mm the calcification blooming artifact was subtracted in the application of the mixture of Gaussian filtering and K- Clustering algorithm, and the value was close to the actual calcification region. These results may help to accurately diagnose coronary artery calcification stenosis.

  • The hemodynamic characteristics were compared using commercial CFD code for the stenosed coronary and abdominal arteries. Numerical calculations were carried out in the axisymmetric arteries over the stenotic diameter ratios ranging from 0.25 to 0.875 (6 cases) employing the typical physiological flow conditions. In case of the coronary artery, there was only one recirculation zone observed distal to the stenosis throat during the major portion of the period. However, in case of the abdominal aorta, there were complex recirculation regions found proximal and distal to stenosis throat. For both models, the wall shear stresses(WSS) increased sharply in the converging stenosis, reaching a peak just upstream of the throat, and became negative or low values in the post-stenotic recirculation region. As the results, the oscillatory shear index(OSI) was abruptly increased at the stenosis throat. For the coronary stenosis model, the second peak in the OSI was observed distal to the stenosis. The distance between the first peak and the second peak was increased as the degree of the stenosis was raised. On the orther hand, the abdominal stenosis model showed a complex oscillatory behavior in the OSI index and did not showed such a strong second peak. As the degree of stenosis was increased, recirculation regions of the both arteries were extended much longer and flow pattern became more complex.

  • In an effort to reduce the stroke rate of patients after coronary artery bypass, many authors have studied the prevalence of the extracranial carotid disease and its role in determining neurologic morbidity and mortality rates. From April 1992 to August 1993, Seventy-five patients undergoing coronary artery bypass were preoperatively evaluated for presence of carotid and femoral stenosis by Duplex sono. Among them, fourteen patients was positive by Duplex sono and overall prevalence of carotid or femoral stenosis was 18.7%. And significant carotid stenosis [ > 60% ] had proved to be in 3 patients [ 4.0% ].Prophylatic bilateral carotid endarterectomy was performed in one patient, at 5 months prior to and , 1 week prior to coronary artery bypass respectively. Their mean age was 57.6 years [ ranged from 40 to 70 years] and were composed of 10 males and 4 females. There was no postoperative morbidiry and mortality related to neurologic complications. Our data, although small, suggest that preoperative carotid screening is helpful to determine patients at high risk of stroke, and significant simultaneous carotid and coronary atherosclerosis should be corrected in selected patients by staged operations when feasible.

  • Familial hypercholesterolemia (FH) is a genetic disease that is not well known or diagnosed in Korea. This disease is associated with persistently high levels of low-density lipoprotein cholesterol (LDL-C), which increase the risk of coronary artery disease at a young age. Therefore, early diagnosis and treatment are important; however, there are no global consensus diagnostic criteria. In Korea, the Dutch Lipid Clinic Network diagnostic criteria, and the Simon Broome diagnostic criteria were used for diagnosis of FH according to the agreement announced at the Korean Society of Lipid and Atherosclerosis (KSoLA) in 2022. Recently, the absence of coronary artery calcification has been considered a good prognostic factor, even among patients with very high LDL-C levels who are considered to be at high risk for atherosclerotic cardiovascular disease. We describe throughout this paper the diagnosis and treatment of FH in a young male without coronary artery calcification.

Correlations between Risk Factors for Atherosclerosis and Histopathologic Findings of Radial Artery (동맥경화증 위험인자와 요골동맥 병리소견과의 상관 관계)

  • Patency of the radial artery plays an important role in the survival of radial forearm fasciocutaneous free flap and artery conduit of coronary artery bypass graft procedure. Even though Allens' test has been used for evaluating the patency of radial artery, the studies on the correlations between risk factors for atherosclerosis and histopathologic findings of radial arteries are rare, until now. Therefore, the authors investigated the correlations between these two factors, and tried to estimate the feasibility of the radial artery in high-risk groups for artherosclerosis. The risk factors for atherosclerosis and lipid profiles were investigated in 38 patients by history taking, physical examinations and blood analysis. And 38 cases of segments of vessel were harvested during the elevation of the flap. The degrees of vessel medial sclerosis were estimated by R values(by Kobayashi and colleagues) that is the median value between the thickness of the intima and that of the media. The measured mean R value was $0.210{\pm}0.05$. Thirty one cases belonged to Grade I(R<0.25), 7 cases belong to Grade II(0.25

Expressions of Matrix Metalloproteinase-9 and Tissue Inhibitor of Metalloproteinase-2 with Changes of Interleukin-6 and Interleukin-18 in Atherosclerotic Lesions of Hypercholesterolemic Rabbits (고콜레스테롤혈증 가토의 죽상경화성 병변에서 Interleukin-6와 Interleukin-18의 변화 및 Matrix Metalloproteinase-9과 Tissue Inhibitor of Metalloproteinase-2의 발현)

  • Atherosclerosis is a chronic inflammatory disease of the arterial wall characterized by progressive accumulation of lipids, cells, and extracellular matrix. Matrix metalloproteinases(MMPs) and tissue inhibitor of metalloproteinases(TIMPS) contribute to vascular matrix remodeling in atherosclerosis, and some cytokines may play role in the synthesis or activation of MMPs or TIMPs. Material and Method: We produced experimental atherosclerotic plaques in 9 rabbits by atherogenic hypercholesterol diet for 12 weeks, and 10 other rabbits were used as control group with standard laboratory chow, At that time, 19 rabbits were sacrificed and aorta, coronary arteries and blood specimens were prepared. The expressions of MMP-9, TIMP-2 and interleukin(IL)-18, and the bioactivity of IL-6 were investigated with H&E stain, immunohistochemical stain, immunoblotting(Western blot analysis), and bioassay. Result: Serum cholesterol in the experimental group increased up to 1258$\pm$262 mg/dL(control group: 41$\pm$7 mg/dL). All experimental group showed well-developed atherosclerotic plaques in aorta and coronary artery. The expression of MMP-9 in aorta and coronary artery of the experimental group showed significant increase than that of the control group by immunohistochemistry. Among the experimental group, complicated lesions with intimal rupture or complete luminal occlusion, demonstrated stronger expression of MMP-9. Interestingly, there was no difference in expression of TIMP-2 between the experimental and the control group. These findings were confirmed by Western blot analysis. The bioassay revealed significant up-regulation of serum bioactivity of IL-6 in the experimental group(4819.60$\pm$2021.25 IU/$m\ell$) compared to that of IL-6 in the control group(27.20 $\pm$ 12.19 IU/$m\ell$). IL-18 was expressed in all atherosclerotic plaques, whereas little or no expression was detected in the control group. Conclusion: The increased MMP-9 expression along with the unchanged TIMP-2 expression seem to be contributory factors in extracellular matrix degradation in atherosclerosis. Focal overexpression of MMP-9 may promote plaque destabilization and cause complications of atherosclerotic plaques such as thrombosis with/without acute coronary syndrome. Elevation of IL-6 and IL-18 may be more than just markers of atherosclerosis but actual participants in lesion development. Identification of critical regulatory pathway is important to improve the understanding of the cellular and molecular basis of atherosclerosis and may open the way for novel therapeutic strategies.

Association of carotid atherosclerosis and obesity in type 2 diabetic patients (제2형 당뇨병 환자에서 비만과 경동맥 경화증과의 관계)

In recent years non-invasive high resolution B-mode ultrasound methods have been developed to measure the intima-media thickness(IMT) of the carotid artery as an indicator for early atherosclerosis. Itis known that obesity plays a role in the development of type 2 diabetes and cardiovascular disease, and it has also been reported that not only the amount but also the distribution of body fat is important. This study investigated the relationship between obesity and the development of carotid atherosclerosis in type 2 diabetic patients. Methods: Carotid IMT was measured by ultrasound B-mode imaging in 144 patients with type 2diabetes mellitus. All subjects underwent assessment for the degree and distribution of obesity, the presence of coronary artery disease risk factors, and the presence of diabetic complications. Resuts: Carotid IMT was increased in the abdominal obese group defined by waist circurference. However, there was no significant difference in carotid TMT between the non-obese group and obese group as defined by body mass index, waist to hip ratio, and total body fat percent measured by bio electrical impedance analysis. There were positive correlations between carotid IMT and age, duration of diabetes, systolic blood pressure, and waist circumference. Multiple linear regression analysis revealed the variable that interacted independently with carotid IMT was age in type 2 diabetic patients. Carotid IMT was significantly increased in type 2 diabetic patients with macrovascular complications and microvascvlar complications .Conclusion: This study suggested that abdominal obesity rather than general obesity was associated with carotid atherosclerosis reflected by increment of carotid IMT in type 2 diabetic subjects.

Prediction of Coronary Heart Disease Risk in Korean Patients with Diabetes Mellitus

  • Objective: We developed a new equation for predicting coronary heart disease (CHD) risk in Korean diabetic patients using a hospital-based cohort and compared it with a UK Prospective Diabetes Study (UKPDS) risk engine. Methods: By considering patients with type 2 diabetes aged ${\geq}30years$ visiting the diabetic center in Boramae hospital in 2006, we developed a multivariable equation for predicting CHD events using the Cox proportional hazard model. Those with CHD were excluded. The predictability of CHD events over 6 years was evaluated using area under the receiver operating characteristic (AUROC) curves, which were compared using the DeLong test. Results: A total of 732 participants (304 males and 428 females; mean age, $60{\pm}10years$; mean duration of diabetes, $10{\pm}7years$) were followed up for 76 months (range, 1-99 month). During the study period, 48 patients (6.6%) experienced CHD events. The AUROC of the proposed equation for predicting 6-year CHD events was 0.721 (95% confidence interval [CI], 0.641-0.800), which is significantly larger than that of the UKPDS risk engine (0.578; 95% CI, 0.482-0.675; p from DeLong test=0.001). Among the subjects with <5% of risk based on the proposed equation, 30.6% (121 out of 396) were classified as ${\geq}10%$ of risk based on the UKPDS risk engine, and their event rate was only 3.3% over 6 years. Conclusion: The UKPDS risk engine overestimated CHD risk in type 2 diabetic patients in this cohort, and the proposed equation has superior predictability for CHD risk compared to the UKPDS risk engine.

Association of Clusterin Polymorphisms with Coronary Heart Disease in Koreans

  • Clusterin is an 80 kDa heterodimetric glycosylated protein and it plays diverse biological roles in various tissues and organs. Clusterin is reported to be associated with the pathogenesis of coronary artery disease and atherosclerosis. Therefore, we investigated the genotype for the A/G polymorphism at the position 4,183 of clusterin gene in Koreans and compared genotype of patients with control group. 100 patients (Male 63, Female 37), who previously underwent coronary artery stent due to ischemic heart disease and 100 controls (Male 36, Female 64) participated in this study. There was a significant association between 4,183 A/G polymorphism in clusterin gene and coronary artery disease (CAD). The present study shows that clusterin gene A/G polymorphism may be associated with the pathogenesis of CAD. Further studies with larger population may be needed for the development of diagnostic methods at genetic level such as DNA chip.
