폐 스크리닝 검사로 이용되고 있는 저선량 흉부 CT는 Scan 범위 내에 관상동맥 석회화에 대한 정보도 함께 포함하고 있어 이를 이용한 관상동맥 석회화 판별의 유용성을 알아보고자 한다. 저선량 흉부 CT 검사와 관상동맥 석회화 점수(CACS) 검사를 같은 날 시행 받은 자들을 대상으로 하였다. 관상동맥 석회화 점수 검사 결과를 Coronary artery calcium score categories and risks 분류법을 참고하여 4개 그룹(Low: 1〈CACS〈10, Mild: 10〈CACS〈100, Moderate: 100〈CACS〈400, High: 400〈CACS)으로 각각 30명을 선정한 후 관상동맥 석회화 수치 측정 업무에 종사하고 있는 경력 15년차 이상 5명의 방사선사가 저선량 흉부 CT 영상에서 관상동맥 석회화 유무를 후향적으로 분석하였다. 저선량 흉부 CT 영상에서 5명의 관찰자가 통일되게 판독한 결과가 관상동맥 석회화점수 CT 검사 결과와 일치한 경우는 Low 그룹: 56%, Mild 그룹: 96.6%, Moderate 그룹: 100%, High 그룹: 100%로 나타났다. Low 그룹에서 5명의 관찰자 모두가 석회화를 관찰한 것은 30건 중 17건이었으며, 5명이 모두 판별 불가로 결정한 경우 7건이었다. 무증상 성인을 대상으로 저선량 흉부 CT 검사에서 석회화 점수가 15 이상인 경우에는 관상동맥 석회화를 100% 관찰할 수 있었다. 판별이 가능한 최소 석회화 수치는 1로, 피검자의 체형이 작거나 심장의 움직임이 최소가 되는 시점에서 스캔이 이루어지는 경우 매우 작은 석회화까지도 판별할 수 있다는 것을 알 수 있었다.
The Coronary Artery Calcium Data and Reporting System (CAC-DRS) is a standardized reporting method for calcium scoring on computed tomography. CAC-DRS is applied on a per-patient basis and represents the total calcium score with the number of vessels involved. There are 4 risk categories ranging from CAC-DRS 0 to CAC-DRS 3. CAC-DRS also provides risk prediction and treatment recommendations for each category. The main strengths of CAC-DRS include a detailed and meaningful representation of CAC, improved communication between physicians, risk stratification, appropriate treatment recommendations, and uniform data collection, which provides a framework for education and research. The major limitations of CAC-DRS include a few missing components, an overly simple visual approach without any standard reference, and treatment recommendations lacking a basis in clinical trials. This consistent yet straightforward method has the potential to systemize CAC scoring in both gated and non-gated scans.
Calcific coronary artery disease is an increasingly prevalent entity in the catheterization laboratory which has implications for stenting and expected outcomes. With new interventional techniques and equipment, strategies to favorably modify coronary calcium prior to stenting continue to evolve. This paper sought to review the latest advances in the management of severe coronary artery calcification in the catheterization laboratory and discuss contemporary percutaneous interventional approaches.
관상동맥 CT 조영술은 높은 정확도로 유의한 관상동맥 협착을 발견할 수 있어 관상동맥 질환이 의심되는 환자들에서 침습적 관상동맥 조영술의 문지기로서의 역할을 인정받고 있다. 그러나 관상동맥에 과도한 석회화 경화반이 있으면 내강을 시각화하기 어려워 영상의 해석에 오류를 초래할 수 있다. 이는 주로 CT 스캐너의 제한적인 공간 해상도로 인해 석회화 경화반에 의한 번짐허상이 발생하기 때문이다. 그럼에도 불구하고, 높은 칼슘 점수를 보이는 CT 영상에서 관상동맥 협착을 평가해야 하는 상황을 종종 마주한다. 이러한 한계를 극복하기 위한 기술적인 방법으로 고해상도 CT 스캐너의 도입, 새로운 재구성 기법 및 후처리 기술의 개발, 감산기법 등이 있으며, 판독에 도움이 되는 방법으로 적절한 창너비 및 창높이의 설정, 혈관의 단면 영상에서 석회화 경화반의 범위 및 내강의 잔류 가시성 평가 등이 권고된다.
분진에 노출 되었던 집단을 대상으로 폐질환 선별검사를 위해 시행한 저선량 CT영상의 관상동맥 석회화 소견과 석회화수치 CT검사 결과를 비교 분석하였다. 연구윤리심의위원회의 승인과 연구대상자로부터 동의서를 받은 후, 과거 분진에 노출된 직업력을 갖고 있는 61명의 남자를 대상으로 폐질환의 선별검사를 위해 저선량 CT촬영과 석회화수치 CT검사를 동시에 실시하였다. 저선량 CT영상(Axial image)은 영상의학과 전문의로부터 진폐 소견 및 폐질환과 관상동맥 석회화소견에 대해 판독하였고, 석회화수치 CT검사로부터 얻은 기초 영상(Row image)은 별도의 워크스테이션으로 보내져 상업적인 라피디아 소프트웨어(ver 2.8)를 이용하여 관상동맥 석회화수치를 구하였다. 저선량 CT영상에서 석회화소견을 보이지 않은 그룹(42명, 68.9%)과 보인 그룹(19명, 31.1%)사이에 총 석회화(13.68 vs. 582.93, p=.009), 좌전하행관상동맥 (3.15 vs. 248.95, p=.006)의 석회화수치가 통계학적으로 유의한 차이를 보였고, 좌주관상동맥, 좌회선관상동맥, 우관상동맥에서 석회화소견을 보인 그룹에서 높게 나타났다(p>0.05). 저선량 CT영상의 석회화소견은 석회화수치 CT검사의 석회화수치 100에서 가장 높은 일치도(K-value=0.80, 95% 신뢰구간=0.69-0.91)를 보였다. 폐질환을 조기 발견하기 위해 시행된 저선량 CT영상에서 보여진 석회화소견은 석회화수치 검사결과와 높은 관련성을 보임으로써 폐쇄성 관상동맥질환을 예측할 수 있는 것으로 사료된다.
컴퓨터단층촬영의 관상동맥 칼슘(coronary artery calcium, CAC) 점수에서 Advanced Modeled Iterative Reconstruction (ADMIRE)의 영향을 평가하였다. 89명의 환자를 대상으로 128 slice dual-source CT로 관상동맥 칼슘 영상(348개의 칼슘, 6개 그룹, 총 2088개의 칼슘)을 획득하였다. Filtered back projection(FBP)과 ADMIRE(1-5)로 재구성된 이미지로부터 Volume score, Agatston score를 측정하였다. FBP와 ADMIRE Strength(1-5) 간의 차이는 Kruskal-Wallis 검정을 통해 확인하고, 사후분석은 FBP를 기준으로 Mann-Whitney U 검정을 하였다. Volume score와 Agatston score 모두 FBP와 ADMIRE(1-5)간에 통계적으로 유의한 차이가 있었습니다(P=0.015, P=0.0.38). 추가로 사후분석 한 결과 Volume score는 FBP를 기준으로 ADMIRE 4(Z=-2.359, P=0.018)에서 9.5 %, ADMIRE 5(Z=-3.113, P=0.002)에서 13.2 % 감소하는 것으로 나타났다. Agatston score는 FBP를 기준으로 ADMIRE 4(Z=-2.051, P=0.040)에서 10.4 %, ADMIRE 5(Z=-2.718, P=0.007)에서 14.0 % 감소하는 것으로 나타났다. 높은 ADMIRE strength는 칼슘 면적의 감소로 인하여 Volume score, Agatston score에 영향을 준다. 또한, Maximum HU의 감소로 인한 Density factor 변화는 Agatston score 계산에 영향을 줄 수 있다.
관상동맥 석회화는 심혈관질환 위험인자와 연관이 있으며, 동맥경화반의 양과 밀접한 관계가 있음을 선행 연구에서 보고되고 있다. 이에 본 연구에서는 2020년 12월부터 2021년 2월까지 대구시 소재 S 영상의 학과 건강검진센터를 방문하여 관상동맥 Calcium scoring CT를 시행한 109명을 대상으로 혈액인자가 관상동맥 석회화 수치에 미치는 영향을 알아보고자 하였다. 연구결과 허리둘레 인자에서는 비정상 군이 정상군에 비해 석회화 발생 위험도가 1.113배 증가하였고, 공복혈당 인자에서는 비정상 군이 정상 군에 비해 석회화 발생 위험도가 1.036배 증가하였으며, 중성지방 인자에서는 비정상 군이 정상 군에 비해 석회화 발생 위험도가 1.008배 증가하였으므로 허리둘레 인자와 공복혈당인자, 중성지방 인자가 관상동맥 석회화 수치에 영향을 주는 인자임을 알 수 있었다. 석회화 수치 발생 위험도는 허리둘레 인자와 공복혈당인자, 중성지방 인자와 유의한 관계가 있으므로 석회화 수치와 연관성이 있는 위험 변수들의 수치를 줄이기 위하여 평소 건강관리에 관심을 갖고 건강검진을 통하여 사전에 예방하기 위해 노력한다면 심혈관질환의 발생률을 낮추고, 심혈관질환과 관련된 의료비를 줄이는데 도움이 될 수 있을 것이라 기대한다.
Familial hypercholesterolemia (FH) is a genetic disease that is not well known or diagnosed in Korea. This disease is associated with persistently high levels of low-density lipoprotein cholesterol (LDL-C), which increase the risk of coronary artery disease at a young age. Therefore, early diagnosis and treatment are important; however, there are no global consensus diagnostic criteria. In Korea, the Dutch Lipid Clinic Network diagnostic criteria, and the Simon Broome diagnostic criteria were used for diagnosis of FH according to the agreement announced at the Korean Society of Lipid and Atherosclerosis (KSoLA) in 2022. Recently, the absence of coronary artery calcification has been considered a good prognostic factor, even among patients with very high LDL-C levels who are considered to be at high risk for atherosclerotic cardiovascular disease. We describe throughout this paper the diagnosis and treatment of FH in a young male without coronary artery calcification.

Objective: This study aimed to validate a deep learning-based fully automatic calcium scoring (coronary artery calcium [CAC]_auto) system using previously published cardiac computed tomography (CT) cohort data with the manually segmented coronary calcium scoring (CAC_hand) system as the reference standard. Materials and Methods: We developed the CAC_auto system using 100 co-registered, non-enhanced and contrast-enhanced CT scans. For the validation of the CAC_auto system, three previously published CT cohorts (n = 2985) were chosen to represent different clinical scenarios (i.e., 2647 asymptomatic, 220 symptomatic, 118 valve disease) and four CT models. The performance of the CAC_auto system in detecting coronary calcium was determined. The reliability of the system in measuring the Agatston score as compared with CAC_hand was also evaluated per vessel and per patient using intraclass correlation coefficients (ICCs) and Bland-Altman analysis. The agreement between CAC_auto and CAC_hand based on the cardiovascular risk stratification categories (Agatston score: 0, 1-10, 11-100, 101-400, > 400) was evaluated. Results: In 2985 patients, 6218 coronary calcium lesions were identified using CAC_hand. The per-lesion sensitivity and false-positive rate of the CAC_auto system in detecting coronary calcium were 93.3% (5800 of 6218) and 0.11 false-positive lesions per patient, respectively. The CAC_auto system, in measuring the Agatston score, yielded ICCs of 0.99 for all the vessels (left main 0.91, left anterior descending 0.99, left circumflex 0.96, right coronary 0.99). The limits of agreement between CAC_auto and CAC_hand were 1.6 ± 52.2. The linearly weighted kappa value for the Agatston score categorization was 0.94. The main causes of false-positive results were image noise (29.1%, 97/333 lesions), aortic wall calcification (25.5%, 85/333 lesions), and pericardial calcification (24.3%, 81/333 lesions). Conclusion: The atlas-based CAC_auto empowered by deep learning provided accurate calcium score measurement as compared with manual method and risk category classification, which could potentially streamline CAC imaging workflows.
Risk stratification and assessment of prognosis in patients with known or suspected CAD is of crucial important for the practice of contemporary medicine. Noninvasive testing such as myocardial perfusion scintigraphy, coronary artery calcium scoring or CT coronary angiography is increasingly being used to determine the need for aggressive medical therapy and to select patients for catheterization. The integrated anatomic and functional information may provide more additional information for the cardiologist or other clinician by the improved risk stratification and diagnostic accuracy of integrated techniques. The development of SPECT/CT or PET/CT hybrid systems is therefore of important value for the nuclear cardiology.
