Flexible Background-Texture Analysis for Coronary Artery Extraction Based on Digital Subtraction Angiography (유동적인 배경 텍스쳐 분석을 통한 DSA 기반의 관상동맥 검출)

  • This paper proposes the extraction of coronary arteries based on DSA(Digital Subtraction Angiography) through a texture analysis of background in the angiography. DSA is a well established modality for the visualization of coronary arteries. DSA involves the subtraction of a mask image - an image of the heart before injection of contrast medium - from live image. However, this technique is sensitive to the movement of background and can result to a wrong detection by the variance of background gray-level intensity between two images. Therefore, this paper solves a structural problem resulted from a background movement bV selecting an image which has the least difference of movement through an analysis of the similarity of background texture and proposes a method to extract only the blood vessel efficiently through local gray-level correction of the selected image. Using the coronary angiogram of 5 patients clinical data, we proved that the proposed method has the lower false-detection rate, approximately $2\%$, and the higher accuracy than the existing methods.

Acute left main coronary artery thrombosis as an initial presentation of systemic lupus erythematosus

  • Left main coronary artery (LMCA) thrombosis is rare and the cause should be determined. A previously healthy young man presented with severe chest pain and dyspnea. The electrocardiogram showed typical ST-segment elevation myocardial infarction with clinical instability. Emergency coronary angiography revealed complete LMCA occlusion by thrombosis. After reperfusion, the patient was admitted to the cardiac care unit. He was diagnosed with hemolytic anemia and tested positive for antinuclear antibodies. Systemic lupus erythematosus (SLE) and LMCA disease due to systemic thrombosis were diagnosed. Steroids were started and the patient was discharged without complications. We report this rare case of LMCA thrombosis as an initial presentation of SLE.

Comparison of Methods using Radial and Femoral Arteries in Coronary Angiography (심장동맥 조영술 시 노동맥과 넙다리동맥을 이용한 방법 비교)

  • In this study, 61 patients who underwent coronary angiography from March 2018 to August 2019 were divided into Group I (Radial, n=34 patients) and Group II (Femoral, n=27 patients), and compared and analyzed methods using radial and femoral arteries when coronary angiography was performed. As a result of comparative analysis, Catheter usage was reduced in the examination using radial artery, and in the abnormal shape of blood vessels, Loops arterial abnormalities were more in the radial artery (20.6% vs, 0%, p<0.05), and it was found that the abnormal shape of the radial vessels increased with age. 60s, 70s, 80s, 90s or older (0%, 18.2%, 25.0%, 100%)

  • Coronary artery aneurysm is an uncommon disease. The optimal medical or surgical treatment for this disease remains obscure. The causes of coronary artery aneurysms include atherosclerosis, Kawasaki disease, infectious vascular disease, connective tissue disorder and congenital malformation. A 50 year old man visit our institution for chest pain that had started 3 days previously. After coronary angiography, multiple coronary aneurysms were diagnosed and successful surgical intervention was performed.

  • Recently non-invasive diagnostic imaging replaced the invasive catheter angiography in the diagnosis of vascular disease. Catheter methods are now almost confined to the purpose of intervention. Coronary artery or coronary artery bypass graft still needs catheter technique because of small diameter and the cardiac motion. The last challenge for radiologists in this domain is to obtain a non-invasive imaging. Electron beam tomography(EBT) for high temporal resolution is able to obtain a coronary arteriogram or coronary artery bypass graft (CABG), of which CABG imaging is quite useful for the evaluation of patency. In our experience as well as others, the accuracy of EBT angiogram in evaluating CABG patency revealed that the accuracy of patency of saphenous vein grafts(SVG) is high due to relatively wide lumen, short and straight course and less influence from cardiac motion. The sensitivity and specificity of patency of SVGs were 92%, 97% respectively in the prospective evaluat on and 100% each in the retrospective evaluation. A false positive and a false negative case are rudimentary errors in the initial learing period. In contrast the analysis of left internal mammary artery(LIMA) graft was difficult due to the inherent small size and the adjacent surgical clips provoking beam-hardening artifact; therefore, the method of combining 3 dimensional reconstruction and flow mode study was important in improving the accuracy of LIMA patency. The sensitivity and specificity of LIMA patency were 100% and 80% in both prospective and retrospective evaluation. Therefore, EBT angiography is an accurate non-invasive diagnostic modality for evaluating the patency of CABG, particularly in SVGs. The accuracy can be improved with the improvement of the EBT and the development of the image reconstruction software.

Stomach Cancer Surgery after Coronary Artery Bypass Surgery with in situ Right Gastroepiploic Artery Graft (정위 우위대망동맥을 이용하여 관상동맥우회술을 시행한 환자에서의 위암수술)

  • A 59 year-old male diagnosed as unstable angina underwent off-pump coronary artery bypass surgery using in situ left internal mammary and right gastroepiploic artery grafts. During harvesting the right gastroepiploic artery, there was no abnormal finding in intraabdominal organs including stomach and liver. He was discharged at the 3rd postoperative day without complication. In case of using in situ right gastroepiploic artery, we recommend gastrofberscopic study at regular follow-up, The patient underwent the gastrofiberscopic study at postoperative 3rd month and diagnosed as advanced gastric cancer on the posterior wall of gastric fundus. At 5th postoperative month, total gastrectomy without intraoperative injury of the right gastroepiploic artery was performed at the department of general surgery. He was discharged at the 9th postoperative day. Follow-up coronary angiography performed at the 1st postoperative year demonstrated patent grafts including right gastroepiploic artery.

  • Fistula between coronary artery and pulmonary artery is a type of coronary artery anomalies. It can cause atypical chest pain and fatigue, angina pectoris, endocarditis, finally myocardial steal can result in heart failure and myocardial infarction. But only 0.1-0.2% of coronary angiographic studies reveal the communications between coronary artery and other spaces. (heart chamber, pulmonary artery etc.) It is frequently congenital, but acquired types are increasing because chest and heart manipulations such as opertion of tetralogy of Fallot, endomyocardial biopsy, radiation therapy, or penetrating blunt trauma are increasing. There are reports about repair of fistula using thrombogenic tips, coil embolization and surgical intervention. We report a connection between coronary artery and pulmonary artery in 79 years old female. She was 30 pack-years smoker and suffered from dyspnea several years with chronic obstructive pulmonary disease. She presented with atypical chest pain and palpitation after admission. Electrocardiography showed ST-T wave abnormality. Emergency coronary angiography and chest CT scan revealed coronary-pulmonary artery fistula. Transcatheter embolization was performed and she was relieved from discomforts.

Reconstruction of Injured or Inadquate Left Internal Thoracic Artery in Cornonary Artery bypass Graft (관상동맥우회술시 부적절한 좌내흉동맥의 변형 활용에 대한 경험)

  • Use of the left internal thoracic artery(ITA) to bypass the left anterior descending(LAD) coronary artery has become the standard of care based on its superior graft patency, reduced cardiac events, and enhanced survival. But rarely we encountered with injury to the artery during harvesting which leads to loss of the merits of surgery. We reconstructed inadequate ITAa with other arterial conduits so proximal stump to be a blood source if possible. Maternal and method: Between January 1996 and March 1999, 12 patients received bypass with the reconstructed left internal thoracic artery grafts to left anterior descending artery because of an injury(n=8), short or small(n=4). Right or left ITA was used to LAD as a free graft(n=2). And the other 10 left ITAs were extended with radial artery(n=6), right ITA(n=3), saphenous vein(n=1). Composite "T" graft was made with other arterial conduits in these extended graft(n=5). Result: There was only one morbidity of minor would problem, and no mortality. The patency of extended graft to LAD was complete in 5 patients who received angiography during the period of 2wks to 2 years postoperative, but one of side branch of "T" graft occluded. All of these patients were well. Conclusion: Reconstructive extension with the use of other arterial conduit for the injured proximal ITA is warranted in any patients with acceptable results. acceptable results.

  • Coronary angiography technology is usually used for examining or treating coronary artery stenosis. Especially, when a cardiologist inserts catheter into the heart blood vessel, the catheter path detection system is needed because the cardiologist has difficulty in not damaging vessel. Recently, to reduce this difficulty, many searchers have been working for the various image processing technologies, such as vessel edge detection, optimal threshold method, etc. However the results of these searches are showing different performances depend on the contrast and quality of images. Therefore, this study for the coronary angiography suggests a novel algorithm to avoid these problems. The suggested algorithm consists of multi-sampling, interpolation, threshold method, and fault points elimination. To evaluate the performance of the proposed method, we used several angiographic images in experimentation, and we found that the proposed method is effective for detecting the catheter insertion path.

Effects of a Program Using Video-Audio Media on Knowledge Level and Compliance of Sick Role for Patients Undergoing Coronary Angiography (동영상프로그램이 관상동맥조영술환자의 질병지식과 환자역할이행에 미치는 효과)

  • Purpose: This study was conducted to investigate how a program using video-audio media will affect the knowledge level and compliance of the sick role of patients admitted for coronary angiography. Method: A non-equivalent control group non-synchronized design was used. Subjects were selected from patients admitted to the internal ward of a hospital in B city for coronary angiography between July 1 and September 31, 2010. Twenty subjects were assigned to the control and experimental group, respectively. Video-audio media developed by the authors was used as the experimental tool. The effects of the program were analyzed using a knowledge assessment tool and questionnaire for compliance of the sick role. The SPSS/WIN 14.0 program was used for data analysis. Result: The first hypothesis that the "experimental group receiving the program using video-audio media will report a higher level of knowledge compared to the control group" was supported. The second hypothesis that the "experimental group that received the program using video-audio media will report a higher level of compliance of sick role" was supported. Conclusion: The program using video-audio media was effective in enhancing patients' knowledge about the disease and compliance of the sick role. Thus, it can be an effective nursing intervention for patients with coronary artery disease.
