수종의 Engine driven NiTi file과 stainless steel K-file을 이용한 근관형성 후 근관의 조직형태학적 변화에 관한 연구 (A STUDY OF HISTOMORPHOLOGICAL CHANGE OF CURVED ROOT CANAL PREPARATION USING GT ROTARY FILE, PROFILE AND STAINLESS STEEL K-FILE)

  • The purpose of this study was to compare the histomorphological change of curved root canal preparation using GT rotary File, Profile .04 taper and stainless steel K-file. 45 mesial canals(over 20 degree) of extracted human mandibular first molars were mounted in resin using a modified Bramante muffle system and divided into three groups. The roots were cross-sectioned at 2.5mm 5mm and 8mm levels from apical foramen. Tracings of the canals were made from preinstrumentation pictures of the cross section. The canals were prepared using a step-back technique with stainless steel K file(group 1), Profile .04 taper rotary file(group 2) and GT rotary file(group 3). Tracings of the prepared canals were made from postinstrumentation picture. Canal centring ratio. amount of transportation, area of dentin removed and shape of canal were measured and statistically were evaluated with Student-Newman-Keuls test using Sigma Stat(Jandel Scientific Software, USA). The results were as followings : 1 Amount of transportation of group 2 was the lowest at apical part, but there was no statistical difference. The direction of transportation was the outside of curvature at apical part. 2. Centering ratio at the apical part of group 1 was the highest, and there was statistical differences between apical and middle part, apical and coronal part(p<0.05). Centering ratio at the middle part of group 3 was the lowest, and there was statistical difference between apical and middle part(p<0.05). Centering ratio of group 2 was the lowest at apical part, but there was no statistical difference. 3. Amount of dentin removed of group 1 was the highest at coronal, middle and apical part among three groups, and there was statistical difference(p<0.05). 4. The majority of the cross-sectioned canal shape after instrumentation were irregular at coronal, middle and apical part. But there are more number of round shaped canals at group 3 than other group.

  • Dental caries is the most common disease in the maxillofacial area. There are many factors contributing to its development, but complete understanding and prevention is not fully known. Since the structure of the coronal and root portion of the tooth is different, the remineralization and demineralization process is also known to be different. In this study, by using a partially saturated buffer solution, we created artificial enamel and dentin caries and evaluated mineral loss. A remineralization solution with four different degrees of saturation (degree of saturation ; group 1, 0.268, group 2, 0.309, group 3, 0.339, group 4, 0.390, PH 4.3, F-2ppm) was used on a demineralized specimen. The mineral precipitating quantity and depth was evaluated by using microradiography. Using an atomic force microscope (AFM), hydroxyapatite crystals of normal, demineralized, and remineralized enamel and dentin were evaluated. The results were as follows: 1. As the degree of saturation of the remineralizing solution increased, the mineral precipitation in the enamel was increased. In group 4, mineral precipitation was limited near the surface. 2. As the degree of saturation of the remineralizing solution increased, the mineral precipitation in the dentin was decreased and it occurred in a deeper portion. In group 4, however, mineral precipitation occurred on the surface and its quantity increased. 3. There was a statistically significant interaction between enamel and dentin mineral content changes on specimens treated with remineralization and demineralization solution (demineralization r=0.44, remineralization r=0.44, p<0.05). 4. Demineralized hydroxyapatite crystals showed central and peripheral dissolving and widening of intercrystal spaces under the AFM. 5. In dentin remineralization small crystal precipitation occurred between the large crystals. We conclude that by adjusting acidulated buffer solution's degree of saturation, we can control enamel and dentin remineralization. In addition, the AFM is highly useful in evaluating changes in remineralized and demineralized hydroxyapatite crystals.

치과용 지혈제가 상아질 투과도에 미치는 영향 (THE EFFECT OF HEMOSTATIC SOLUTIONS ON DENTIN PERMEABILITY)

  • Before impression making in the fixed restorations or other prosthesis, hemostatic solutions are used for hemostasis and moisture control. Hemostatic solutions effectively control bleeding but their major ingredients, acid removes smear layers which are formed in the tooth preparation, exposes the dentinal tubular orifices which are occluded by smear layers, makes dentinal tubular fluid displace more easily to the various external stimulus, and according to the hydrodynamic theory, consequently causes dentin hypersensitivity. To know the effect of hemostatic solutions on dentin permeability, coronal dentin discs, 1mm in thickness, were prepared from extracted third molars free from decay and wear, and a split chamber device was used. Hydraulic conductance values and SEMs, which were measured before and after treatment with $Astringedentr^{(R)},\;Altract^(R)\;and\;Epri-dent^{(R)}$, were compared and ana-lysed. The following conclusions were drawn: 1. Hydraulic conductance values which were measured after the treatment of hemostatic solutions were increased in all groups(p<0.05). 2. %change values of hydraulic conductance were compared but no significant difference was found among the three hemostatic solutions(p<0.05). 3. On SEM observations of all groups, after treatment smear layers were removed and dentinal tubular orifices were partially exposed. On the basis of these conclusions, the reckless use of hemostatic solutions should be restricted, and when in use, various methods should be considered to protect dentin.

  • 본 연구는 상아질 접착제와 레진 시멘트의 적합성 여부를 알아보고자 시행하였다. 발거한 제3대구치의 상아질 표면을 노출시킨 후 Tescera ATL 복합레진 시편을 상아질 접착제 [All Bond 2 (Bisco), Clearfil SE Bond (Kuraray), Adper Prompt L-POP (3M), One-Up Bond F (Tokuyama)]와 레진시멘트 [Choice (Bisco), Panavia F (Kuraray), RelyX ARC (3M), Bistite II DC (Tokuyama)]로 접착하고 미세인장 결합강도 및 주사전자 현미경 관찰을 시행한 결론은 다음과 같다. 1. Clearfil SE Bond와 All-Bond 2가 Prompt L-Pop과 One-Up Bond F 보다 높은 미세인장 결합강도를 보였다(p<0.05). 2. Clearfil SE Bond와 All-Bond 2 사용시 1-step 상아질 접착제보다 혼성층이 두껍고 레진 tag가 길었다.

지각과민 처치제 도포 후 칫솔질에 의한 마모가 상아질 투과도에 미치는 영향 (THE EFFECTS OF DESENSITIZING AGENTS AND TOOTH BRUSHING ON DENTIN PERMEABILITY, IN VITRO)

  • To study the effect of dentin permeability on a tooth with wear from tooth brushing after application of desensitizing agent, extracted teeth free from caries were chosen. Coronal dentin discs with thickness of 1mm were prepared. Using the split chamber device developed by Pashely, hydraulic conductance, scanning electron microscope images(SEM) and atomic force microscope images(AFM) were compared and contrasted before and immediately after the application of desensitizing agent and after equivalent tooth brushing of 1 week, 2 weeks, and 6 weeks. Four commercially available desensitizing agents were used in this study ; they were Gluma, Seal & Protect, All-Bond 2 and MS Coat. The results of this study are as follows. 1. On all specimens, the hydraulic conductance decreased after the application of tooth desensitizing agent. 2. Except the specimens treated with MS Coat, the remaining specimens had an increase in dentin permeability after tooth brushing for 1 and 2 weeks but a decrease after 6 weeks. 3. The specimens treated with MS Coat had statistically significant increase in the dentin permeability regardless with the duration of tooth brushing. 4. On examination of SEM and AFM, the dentinal tubule diameter had decreased after treatment of desensitizing agents. The specimens other than those treated with MS Coat, smear layers were noted after tooth brushing. It is not always consistant but the hydraulic conductance correlated with the images from SEM and AFM.

Self-Priming Adhesives를 침투시킨 탈회 상아질의 인장강도 (Tensile Strengths of Demineralized Dentin derived from Self-Priming Adhesives)

  • The objectives of this study were to evaluate the tensile strength of resin-infiltrated demineralized dentin according to the demineralization time, and to evaluate the tensile strength of hybrid layer that is formed by infiltrating different priming adhesives or primer/adhesive into demineralizd dentin matrix. Seventy five hour-glass shaped dentin specimens were prepared in mid-coronal dentin from extracted human molars. Thirty specimens were distributed into three groups according to demineralization time - 2 hours, 4 hours and 8 hours. Each specimen was placed in primer/adhesive of All-Bond 2 for 5 hours of infiltration. Another forty-five specimens of them were demineralized in 37% phosphoric acid for 4 hours. They were randomly assigned to three experimental groups - AB, SB and OS - to designate All-Bond 2, Single Bond and One-Step. Each specimen was placed in one of three different adhesives for 5 hours of infiltration. The specimens were visible light-cured for 5 minutes, and then stored for 24 hours in distilled water at $37^{\circ}C$. After that, microtensile bond strength for each specimen was measured, and the fractured surfaces were then observed by SEM. The data were statistically analysed by one-way ANOVA and Tukey's multiple comparison test and Bonferroni's multiple comparison test. The results were as follows; 1. Tensile strength of the group demineralized for 4 hours was significantly higher than that of groups demineralized for 2 hours and 8 hours (P < .01). 3. Tensile strength of the AB group was significantly higher than that of the SB group and OS group (P < .01).

Efficacy of various cleansing techniques on dentin wettability and its influence on shear bond strength of a resin luting agent

  • PURPOSE. To evaluate the shear bond strength of resin luting agent to dentin surfaces cleansed with different agents like pumice, ultrasonic scaler with chlorhexidine gluconate, EDTA and the influence of these cleansing methods on wetting properties of the dentin by Axisymmetric drop Shape Analysis - Contact Diameter technique (ADSA-CD). MATERIALS AND METHODS. Forty coronal portions of human third molar were prepared until dentin was exposed. Specimens were divided into two groups: Group A and Group B. Provisional restorations made with autopolymerizing resin were luted to dentin surface with zinc oxide eugenol in Group A and with freegenol cement in Group B. All specimens were stored in distilled water at room temperature for 24 hrs and provisional cements were mechanically removed with explorer and rinsed with water and cleansed using various methods (Control-air-water spray, Pumice prophylaxis, Ultrasonic scaler with 0.2% Chlorhexidine gluconate, 17% EDTA). Contact angle measurements were performed to assess wettability of various cleansing agents using the ADSA-CD technique. Bond strength of a resin luting agent bonded to the cleansed surface was assessed using Instron testing machine and the mode of failure noted. SEM was done to assess the surface cleanliness. Data were statistically analyzed by one-way analysis of variance with Tukey HSD tests (${\alpha}$=.05). RESULTS. Specimens treated with EDTA showed the highest shear bond strength and the lowest contact angle for both groups. SEM showed that EDTA was the most effective solution to remove the smear layer. Also, mode of failure seen was predominantly cohesive for both EDTA and pumice prophylaxis. CONCLUSION. EDTA was the most effective dentin cleansing agent among the compared groups.

Revascularization of immature permanent teeth with apical periodontitis

  • In the infected immature tooth with periapical involvement, the pulp is considered to hardly exist in the canal and periapical area. Such a tooth receives apexification procedure, because revascularization of the pulp chamber is in principle not expected. Apexification is beneficial to induce further development of an apex to close the foramina, but does not promote the thickness of the entire canal wall dentin. It may be possible for the pulp to be only partially necrotic and infected when an extremely large communication from the pulp space to the periapical tissues exists with a very young tooth. If this were the case, vital pulp in the apical part of the canal could proliferate new pulp into the coronal pulp space by the successful removal and disinfection of the necrotic infected coronal pulp.(omitted)

Nd-YAG레이저에 의한 치수 절단후 잔존 치수 조직의 치유과정에 관한 전자현미경적 연구 (AN ELECTRONMICROSCOPIC STUDY ON THE HEALING PROCESS OF THE REMAINING PULPAL TISSUES AFTER PULPOTOMY BY Nd-YAG LASER)

  • The purpose of this study was to investigate a fragment of possibility of pulpotomy with the Nd-YAG laser by the observation of pulpal healing process and the fine structural changes of the fibroblasts of the remaining pulpal tissues. Class V cavities on !55 teeth from 4 adult dogs were prepared and the pulp chambers were opened with a sterilized round bur. In the control group(19 teeth), the exposed coronal pulps were excised by a sharp excarvator. After bleeding was controlled with the sterilized cotton pellets, calcium hydroxide powder was applied on the remaining pulpal tissues and the cavities were sealed with Z.O.E. In the experimental group 1 : the pulpotomy with laser-calcium hydroxide powder application group(l9 teeth), the exposed coronal pulps were excised by Nd-YAG laser(10 watts power, 2 psi water, 20 psi air) for 2 or 3 seconds and calcium hydroxide powder was applied on the remaining pulpal tissues and the cavities were sealed with Z.O.E. In the experimental group 2 : the pulpotomy with laser-no calcium hydroxide powder application group(17 teeth), after amputating the coronal pulps with Nd-YAG laser as the experimental group 1, the remaining pulpal tissues were covered with stenilized aluminum foil and the cavities were filled with Z.O.E. The animals were sacrificed at the intervals of 1, 2, 3 and 4 weeks. All the teeth were rouutinely processed and the remaining pulpal tissues were observed by the light microscope and electron microscope. The results were as follows : 1. In light microscopic findings, there was no significant difference of the inflammatory response in the remaining pulpal tissues between the control group and the experimental groups. In both of the experimental group 1 : pulpotomy with laser-calcium hydroxide powder application group and the control group, the dentin bridges were observed after 2 weeks and the structure of the dentin bridge was almost same. In the experimental group 2 : pulpotomy with laser-no calcium hydroxide powder application group, the fibrous layers instead of dentin bridge were observed on the superficial portion of the remaining pulpal tissues after 2 weeks and they were consisted with densely crowded active fibroblasts. 2. In the electronmicroscopic findings, the active fibroblasts in the experimental groups were more frequently observed than in the control group at 1 week. But active fibroblasts were found with same frequency after 2 weeks in all of the control group and the experimental groups. 3. General distortions of the cell such as loss of the cell membrane, vaculoization of the cell etc. were observed at the suberficial layer of the remaining pulpal tissues and the carbonization was found in the dentinal wall in 1 week of the experimental groups. 4. In the experimental group 2 : pulpotomy with laser-no calcium hydroxide powder application group, the activity and the density of the fibroblasts in the fibrous layer were more than those in the deep portion of the remaining pulpal tissues after 2 weeks. 5. In the control group, bacteria such as cocci and bacilli were observed frequently, but in the experimntal groups, they could not be observed.

Microtensile bond strength of resin cement primer containing nanoparticles of silver (NAg) and amorphous calcium phosphate (NACP) to human dentin

  • PURPOSE. The purpose of the current study was to evaluate the effect of incorporating nanoparticles of silver (NAg) and amorphous calcium phosphate (NACP) into a self-etching primer of a resin cement on the microtensile bond strength of dentin, regarding the proven antibacterial feature of NAg and remineralizing effect of NACP. MATERIALS AND METHODS. Flat, mid-coronal dentin from 20 intact extracted human third molars were prepared for cementation using Panavia F2.0 cement. The teeth were randomly divided into the four test groups (n=5) according to the experimental cement primer composition: cement primer without change (control group), primer with 1% (wt) of NACP, primer with 1% (wt) of physical mixture of NACP+Nag, and primer with 1% (wt) of chemical mixture of NACP+Nag. The resin cement was used according to the manufacturer's instructions. After storage in distilled water at $37^{\circ}C$ for 24 h, the bonded samples were sectioned longitudinally to produce $1.0{\times}1.0mm$ beams for micro-tensile bond strength testing in a universal testing machine. Failure modes at the dentin-resin interface were observed using a stereomicroscope. The data were analyzed by one-way ANOVA and Tukey's post-hoc tests and the level of significance was set at 0.05. RESULTS. The lowest mean microtensile bond strength was obtained for the NACP group. Tukey's test showed that the bond strength of the control group was significantly higher than those of the other experimental groups, except for group 4 (chemical mixture of NACP and NAg; P=.67). CONCLUSION. Novel chemical incorporation of NAg-NACP into the self-etching primer of resin cement does not compromise the dentin bond strength.
