A Case Report on a Patient of Pain after Core Decompression Surgery on Avascular Necrosis of Femur Head Treated with Korean Medicine Treatments (대퇴골두 무혈성 괴사 감압술 시술 후 발생한 고관절 통증 환자의 치험 1례)

  • Objectives : The aim of this study was to report the improvements in pain that a woman experienced after receiving Korean medicine treatments for the pain associated with core decompression surgery due to avascular necrosis (AVN) of both femur heads. Methods : The patient was diagnosed as having AVN in both femur heads and underwent core decompression surgery. After operation, she had pain on her right inguinal region and her range of motion (ROM) was restricted. She was treated with Korean medicine including, herbal medicine, acupuncture, Chuna Manual Therapy and bee venom acupuncture (BV). This study was measured by using the Visual Analogue Scale (VAS) and the ROM scale. Results : After conservative treatment, the patient's pain was reduced and ROM was increased. Daily living quality had improved. Conclusions : In this case, Korean conservative medicine therapy with Chuna Manual Therapy had a positive effect on the symptoms after core decompression surgery due to avascular necrosis of the femur head.

  • Avascular necrosis (AVN) of the humeral head is a rare, yet detrimental complication. Left untreated, humeral head AVN frequently progresses to subchondral fracturing and articular collapse. Cases of late-stage humeral head AVN commonly require invasive procedures including humeral head resurfacing, hemiarthroplasty, and total shoulder arthroplasty (TSA) to improve clinical outcomes. However, in cases of early-stage AVN, core decompression of the humeral head is a viable and efficacious short-term treatment option for patients with pre-collapse AVN of the humeral head to improve clinical outcomes and prevent disease progression. Several techniques have been described, however, a percutaneous, arthroscopic-assisted technique may allow for accurate staging and concomitant treatment of intraarticular pathology during surgery, although further long-term clinical studies are necessary to assess its overall outcomes compared with standard techniques. Biologic adjunctive treatments, including synthetic bone grafting, autologous mesenchymal stem cell/bone marrow grafts, and bone allografts are viable options for reducing the progression of AVN to further collapse in the short term, although long-term follow-up with sufficient study power is lacking in current clinical studies. Further long-term outcome studies are required to determine the longevity of core decompression as a conservative measure for early-stage AVN of the humeral head.

Arthroscopic assisted Core Decompression of Humeral Head Osteonecrosis - Technical Note - (상완 골두 골괴사증에서의 관절경하 핵심 감압술 - 수술 술기 -)

  • Purpose: We introduce arthroscopic assisted core decompression for humeral head osteonecrosis. Operative technique: After diagnostic shoulder arthroscopy is performed using posterior and anterior portal, we make a 2 cm lateral skin incision approximately 3 cm distal to 1/3 of lateral margin of the acromion and place short gray cannula to prevent adjacent soft tissue injury when insert guide pin. Under C-arm fluoroscopic and arthroscopic guidance, 3 to 4 guide pins are inserted toward the necrotic area. Then we perform drilling using 7.0 mm reamer. Conclusion: We believe this technique is a less invasive approach and avoids the complications comparing to deltopectoral approach. Arthroscopic assisted core decompression is an effective procedure in management of early stage of the humeral head osteonecoris.

  • This paper describes the design and implementation results of JPEG core, based on the ITU-T Recommendation T.81. We designed the RTL circuit in Verilog HDL, making reference to the JPEG program from the Independent JPEG Group. The circuit has been simulated with Verilog-XL, synthesized with Design Compiler and verified using Altera FPGA. Since the synthesized circuit includes a small number of gates, it is expected to be used as a core module in image processing SOC.

  • This paper presents a system-on-chip prototype implementing a full integration for a portable digital audio system. The chip is composed of a audio processor block to implements audio decoding and voice compression or decompression software, a system control block including 8-bit MCU core and Memory Management Unit (MMU) a low power 16-bit ${\Sigma}{\Delta}$ CODEC, two DC-to-BC converter, and a flash memory controller. In order to support other audio algorithms except Mask ROM type's fixed codes, a novel 16-bit fixed-point DSP core with the program-download architecture is proposed. Funker, an efficient power management technique such as task-based clock management is implemented to reduce power consumption for portable application. The proposed chip has been fabricated with a 4 metal 0.25um CMOS technology and the chip area is about 7.1 mm ${\times}$ 7.1mm with 100mW power dissipation at 2.5V power supply.

  • JPEG is a most widely used standard image compression technology. This research introduces the JPEG image compression algorithm and describes each step in the compression and decompression. Image compression is the application of data compression on digital images. The DCT (discrete cosine transform) is a technique for converting a time domain to a frequency domain. First, the image is divided into 8 by 8 pixel blocks. Second, working from top to bottom left to right, the DCT is applied to each block. Third, each block is compressed through quantization. Fourth, the array of compressed blocks that make up the image is stored in a greatly reduced amount of space. Finally if desired, the image is reconstructed through decompression, a process using IDCT (inverse discrete cosine transform).

  • DCT(Discrete Cosine Transform) / IDCT(Inverse DCT) is widely used in various image compression and decompression systems as well as in DSP(Digital Signal Processing) applications. Since DCT/ IDCT is one of the most complicated part of the compression system, the performance of the system can be greatly enchanced by improving the speed of DCT/IDCT operation. In this thesis, we designed a DCT/IDCT core processor using module generator technique. By utilizing the partial sum and DA(Distributed Arithmetic) techniques, the DCT/ IDCT core processor is designed within small area. It is also designed to perform the IDCT(Inverse DCT) operation with little additional circuitry. The pipeline structure of the core processor enables the high performance, and the high accuracy of the DCT/IDCT operation is obtained by having fewer rounding stages. The proposed design is independent of design rules, and the number of the input bits and the accuracy of the internal calculation coa be easily adjusted due to the module generator technique. The accuracy of the processor satisfies the specifications in CCITT recommendation H, 261.

A Biomechanical Analysis of Various Surgical Procedures for Osteonecrosis of the Femoral Head using a Finite Element Method (유한요소법을 이용한 대퇴 골두내 무혈성 괴사증의 다양한 수술적 기법에 대한 생체역학적 분석)

  • Operative procedures such as core drilling with and without fibular bone grafting have been recognized as the treatment methods for osteonecrosis of femoral head(ONFH) by delaying or preventing the collapse of the femoral head. In addition, core drilling with cementation using polymethylmethacrylate (PMMA) has been proposed recently as another surgical method. However, no definite treatment modality has been found yet while operative procedures remain controversial to many clinicians In this study, a finite element method(FEM) was employed to analyze and compare various surgical procedures of ONFH to provide a biomechanical insight. This study was based upon biomechanical findings which suggest stress concentration within the femoral head may facilitate the progression of the necrosis and eventual collapse. For this purpose, five anatomically relevant hip models were constructed in three dimensions : they were (1) intact(Type I), (2) necrotic(Type II), (3) core drilled only(Type III), (4) core drilled with fibular bone graft(Type IV), and (5) core drilled with cementation(Type V). Physiologically relevant loading were simulated. Resulting stresses were calculated. Our results showed that the volumetric percentage subjected to high stress in the necrotic cancellous region was greatest in the core drilled only model(Type III), followed by the necrotic(Type II), the bone graft (Type IV), and the cemented(Type V) models. Von Mises stresses at the tip of the graft(Type IV) was found to be twice more than those of cemented core(Type V) indicating the likelihood of the implant failure. In addition, stresses within the cemented core(Type V) were more evenly distributed and relatively lower than within the fibular bone graft(Type IV). In conclusion, our biomechanical analyses have demonstrated that the bone graft method(Type IV) and the cementation method(Type V) are both superior to the core decompression method(Type III) by reducing the high stress regions within the necrotic cancellous bone. Also it was found that the core region filled with PMMA(Type V) provides far smoother transfer of physiological load without causing the concentration of malignant stresses which may lead to the failure than with the fibular bone graft(Type IV). Therefore, considering the above results along with the degree of difficulties and risk of infection involved with preparation of the fibular bone graft, the cementation method appears to be a promising surgical treatment for the early stage of osteonecrosis of the femoral head.

Work-related Injuries and Diseases of Fisheries in Korea (어업인의 업무관련성 손상 및 질병에 관한 고찰)

  • There are many work-related injuries and diseases of fisheries in Korea. Fishermen have exposed to harmful environment, such as natural disasters, ultraviolet, noise, vibration, organic solvents, ergonomic risk factors, loss of fresh water and food, and psychologic stress. They have suffered from numerous work-related health problems, including musculoskeletal disorders, cardiovascular diseases, infectious diseases, pyschological diseases, decompression sickness, and so on. Sea accidents can cause deaths of fishermen and if sea pollution is occurred, fishermen have a great economic damage. However, the concern of these injuries and diseases has been little because they are small in number and have decreased. Fisheries are important core industries, so the problems of fisheries have to be solved by a government. But work-related injuries and diseases are not investigated properly and accurate diagnoses are not made due to loss of concern. We hope that this article will be a cornerstone to solving the work-related problems of fisheries.

SOC Test Compression Scheme Sharing Free Variables in Embedded Deterministic Test Environment

  • This paper presents a new SOC test compression scheme in Embedded Deterministic Test (EDT) compression environment. Compressed test data is brought over the TAM from the tester to the cores in SOC and decompressed in the cores. The proposed scheme allows cores tested at the same time to share some test channels. By sharing free variables in these channels across test cubes of different cores decompressed at the same time, high encoding efficiency is achieved. Moreover, no excess control data is required in this scheme. The ability to reuse excess free variables eliminates the need for high precision in matching the number of test channels with the number of care bits for every core. Experimental results obtained for some SOC designs illustrate effectiveness of the proposed test application scheme.
