Successful treatment of a child with citrullinemia

  • The amino acids formed by degradation of proteins ingested produce ammonia. The ammonia which is broken down and excreted as urea through a process known as the Klebs-Hensleit cycle or the urea cycle (Rezvani, 1995). The urea cycle consists of five enzymes necessary for the synthesis of carbamyl phosphate, citrulline, argininosuccinate, arginine, and urea: carbamyl phosphate synthetase (CPS), ornithine transcarbamylase (OTC), argininosuccinate synthetase (AS), argininosuccinate lyase (AL), and arginase (ARG) (Lloyd, 1992). Congenital deficiencies of the enzymes involved in the urea cycle are diseases that are almost fatal without treatment, showing symptoms like vomiting, lethargy, dyspnea, and coma due to hyperammonemia coming from the accumulation of ammonia and metabolic precursors resulting from the deficiency of one of these enzymes (Batshaw and Brusilow, 1983). Among these, the disease manifested by the congenital deficiency of argininosuccinate synthetase (AS) which is associated with the formation of argininosuccinate in citrulline is called argininosuccinate synthetase deficiency or citrullinemia. There have been two reports on this so far in Korea; one in July 1987 by Kim et al. and the other by Park et al. in 1995. We are to report a case of successful treatment of a child with citrullinemia who was transferred to our hospital due to dyspnea, lethargy, feeding difficulties, convulsions and cyanosis together with some document studies related to this case.

A Case Report of Noonan Syndrome with Mental Retardation and Attention-Deficit Hyperactivity Disorder (정신지체와 주의력결핍 과잉행동장애를 보이는 Noonan 증후군 1예)

  • Noonan syndrome is characterized by short stature, typical facial dysmorphology, and congenital heart defects. The main facial features of Noonan syndrome are hypertelorism with down-slanting palpebral fissures, ptosis, and low-set posteriorly-rotated ears with a thickened helix. The cardiovascular defects most commonly associated with this condition are pulmonary stenosis and hypertrophic cardiomyopathy. Other associated features are webbed neck, chest deformity, mild intellectual deficit, cryptorchidism, poor feeding in infancy, bleeding tendency, and lymphatic dysplasias. The patient is a 10-year-old boy. He had experienced repeated febrile convulsions. He had typical facial features, a short stature, chest deformity, cryptorchidism, vesicoureteral reflux, and mental retardation. His language and motor development were delayed. When he went to school, it was difficult for him to pay attention, follow directions, and organize tasks. He also displayed behavior such as squirming, leaving his seat in class, and running around inappropriately. Clinical observation is important for the diagnosis, so we report a patient who was diagnosed with Noonan syndrome, mental retardation, and attention-deficit hyperactivity disorder.

  • Ictal tachycardia and bradycardia are common arrhythmias; however, ictal sinus pause and asystole are rare. Ictal arrhythmia is mostly reported in adults with temporal lobe epilepsy. Recently, ictal arrhythmia was recognized as a major warning sign of sudden unexpected death in epilepsy. We present an interesting case of a child with ictal sinus pause and asystole. A 27-month-old girl was hospitalized due to 5 episodes of convulsions during the past 2 days. Results of routine electroencephalography (EEG) were normal, but she experienced brief generalized tonic seizure for 3 days. During video-monitored EEG and echocardiography (ECG), she showed multiple myoclonic seizures simultaneously or independently, as well as frequent sinus pauses. After treatment with valproic acid, myoclonus and generalized tonic seizures were well controlled and only 2 sinus pauses were seen on 24-hour Holter ECG monitoring. Sinus dysfunction should be recognized on EEG, and it can sometimes be treated successfully with only antiepileptic medication.

General Pharmacology of G(7-rhEPO\ulcorner a New Recombinant Ruman Erythropoietin (유전자 재조합 사람 erythropoietin, GC-rhEPO의 일반약리작용)

  • To evaluate GC-rhEPO, human erythropoietin produced by recombinant DNA technique, its general pharmacological properties were investigated in experimental animals administering intravenously and in vitro test system. GC-rhEPO at doses of 70,700 and 7,000 IU/kg body weight had no influence on general behavior, spontaneous motor activity, thiopental-inducted sleeping time, writhing syndrome induced by acetic acid, strychnine-induced convulsions, charchoal meal propulsion in mice, and body temperature, gastric juice secretion, urine and electrolyte excretion in rats. In anesthetized rabbits, GC-rhEPO (70, 700 and 7,000 lU/kg, i.v.) did not alter respiratory rate, blood pressure, heat rate. In in vitro experiments, GC-rhEPO did not affect the contractions of the isolated ileum of guinea pigs and the muscle twitchs of isolated neuromuscular junction of the rats. In addition, GC-rhEPO did not affect the blood coagulation time and ADP-induced platelet aggregation in plasma of rabbits. Taken together, these results indicate that GC-rhEPO does not induce any adverse effects in the experimental animals.

Synthesis and Anticonvulsant Evaluation of N-Substituted-Isoindolinedione Derivatives

  • A series of N-substituted-1,3-isoindolinedione derivatives (2-16) were synthesized for the purpose of defining the effect of N-substitution on the anticonvulsant activity of these derivatives. The target compounds (2-16) were obtained by condensation of phthalic anhydride with the corresponding amine derivative. The structures of the synthesized derivatives (2-16) were confirmed by means of IR, $^1$H-NMR, $^{13}$ C-NMR, MS and elemental analyses. The anticonvulsant activity of all compounds (2-16) were evaluated by subcutaneous pentylenetetrazole seizure threshold test at doses of 0.2, 0.4 and 0.8 mmol/kg compared with sodium valproate as a positive control. Their neurotoxicity were determined by the rotorod test. Many of the present series of compounds showed good anticonvulsant activity at the tested doses, as compared to sodium valproate. Three of them (4, 6 and 11) exhibited 100 % protection against convulsions, neurotoxicity and death at all tested doses. Out of the series, two compounds (12 and 13) were completely inactive with 100% mortality. 3-(p-chlorophenyl)-4-(1 ,3-dioxo-2,3-dihydro-1 H-2-isoindolyl) butanoic acid derivative (11) has emerged as the most active compound which is 20 times more active than valproate with ED$_{50}$ 8.7, 169 mg/kg; TD$_{50}$ 413, 406 mg/kg and PI 47.5, 2.4. The results revealed the importance of the combination of baclofenic and phthalimide moieties (compound 11) as a promising anticonvulsant candidate.

Immunohistochemical and Pathological Findings im Mice Inoculated with Encephalomyocarditis Virus (뇌심근염 바이러스 접종 마우스에 대한 병리학적 및 면역조직화학적 소견)

  • 8-10 week old ICR mice were infected intracerebrally and intraperitoneally with different encephalomyocarditis virus(K$_3$, $K_{11}$, ATT-VR 129) to observe histopathological and immunohistochemical change. Results obtained throuh the experiments were summarized as follows : 1. No differences in clinical signs by the virus strains and the inoculation routes were found. Mice infected with EMCV showed clinical signs after 3 days of inoculation. Main clinical signs were tremors, convulsions, circling movement, and uni or bilateral hindleg paralysis followed by death on the 3-8 days. In general, most of the infected animals died or recovered closely on the 8th day of postinoculation. 2. At necropsy, petechial and ecchymotic hemorrhages in lung were observed and no specific findings in other were observed. 3. In histopathological observation, neuroal cell degeneration perivascular mononucear cell in-filtration gliosis were appeared in central nervous system. Myocarditis with myocardial degeneration and necrosis, calcification were observed along with acinar cell necrosis of exocrine glands in pancreas, severe glomerulonephritis in kidney. Also, focal necrosis of hepatocytes and interstitial pneymonia hyperemia, hemorrhages in lungs were observed. 4. By immunohistochemical staining using ABCIT method, the positive cells were recognized in intracytoplasm of acinar cell in pancreas and intracytoplasm of neuronal cells in cerebrum.

Nitrate Poisoning Occurred in Holstein Calves (홀스타인 독우(犢牛)에 발생(發生)한 질산염중독(窒酸鹽中毒))

  • Nitrate poisoning was observed in Holstein calves raising for beef purpose in a dairy-farm of Gyeongbug province. The results observed were summarized as follows: 1. These calves had been fed mainly cornstalk, and a qualitative reaction by "dephenylamine blue test" for nitrate was positive in the serum of the affected calves and in the cornstalk provided. 2. In the clinical signs, the affected calves were suddenly recumbent without premonition and then dyspnea, followed by death. Death also occurred after inappetence, depression, dyspnea, cyanosis of mucous membrane, and terminal anoxic convulsions were observed. 3. In the macroscopical findings, the blood was dark brown (chocolate in color) and more or less poor coagulative. Petechiae of epicardium, severe cloudy swelling of liver, cloudy swelling and hemorrhage of kidney, congestion and hemorrhage of spleen, congestion and edema of lung, and congestion af abomasum and small intestine were observed in these cases. 4. Microscopical changes observed in these cases were peripheral congestion of lobules, centrilobular necrosis and sinusoidal dilatation in liver, hydropic degeneration of hepatic cells, hemorrhage and edema of kidney, necrosis of convoluted-tubular epithelium, and dilatation of Bowman's space and convoluted tubule. There were also congestion and hemorrhage of spleen, hemorrhage and edema of lung, cloudy swelling of myocardium, and congestion and hemorrhage of various organs.

A Case of Acute Glufosinate Ammonium ($BASTA^{(R)}$) Intoxication Associated with Various Neurological Abnormalities (다양한 신경학적 이상을 나타낸 glufosinate ammonium(바스타$^{(R)}$) 급성 경구 중독 1례)

  • BASTA is an herbicide which contains glufosinate ammonium as a main component with an anionic surfactant, polyoxyethylene alkylether sulfate, and nonselectively inhibits glutamine synthetase. It became a wildly used herbicide in Korea and its intoxication is now increasing. A 42-year old woman ingested about 300ml of BASTA in a suicide attempt. She showed unconsciousness and respiratory distress in the beginning, and later developed multiple generalized convulsions, low blood pressure, fever and diabetes insipidus. Although she became alert 12 days after the ingestion, she showed retrograde amnesia for a period of about recent 10 years. A neuropsychological test on day 22 revealed frontal lobe dysfunction, visual memory disturbance and slight decrease in visuospatial function. All these neurological abnormalities that might occur due to glufosinate ammonium were almost improved in the follow-up test performed a month later.

A Case of Intoxication of Ingested Formalin (포르말린에 의한 급성 중독 1례)

  • Formalin is a water-soluble, colorless, pungent, irritating and highly reactive gas. A 40% solution of formaldehyde in water, also known as formalin, is used as a disinfectant, antiseptic, deodorant, tissue fixative and embalming fluid. Ingestion can lead to immediate deleterious effects on almost all systems of the body including gastrointestinal tract, central nervous system, cardiovsacular system and hepato-renal system, causing gastrointestinal hemorrhage, cardiovsacular collapse, unconsciousness or convulsions, severe metabolic acidosis and acute respiratory distress syndrome. We treated a 39-year-old woman who ingested 300 ml formalin in a suicidal attempt. Despite hemodialysis, death occurred after 23 h.

General Pharmacology of CJ-50002, an Oral Vaccine against Vibrio vulnificus Infection (Vibrio vulnificus에 대한 경구용백신 CJ-50002의 일반약리작용)

  • CJ-50002 is an oral vaccine against V.vulnificus infection composed of whole cell lysate of V. vulnificus. The general pharmacological properties of CJ-50002 were evaluated in various animals and in vitro system. CJ-50002 at oral doses of 0.2, 2 and 20 mg/kg had no effect on general behavior in mice, chromo- and electro-convulsions in mice, writhing syndrome induced by acetic acid in mice, body temperature in rats, charcoal meal propulsion in mice and urine and electrolytes excretion in rats. However, oral administration of CJ-50002 at dose of 20 mg/kg prolonged the hexobarbital-inuced sleeping inducing time in mice. In anesthetized dogs, CJ-50002 showed no effect on blood pressure, heart rate and ECG but decreased the respiratory rate and femoral blood flow at dose of 20 mg/kg. p.o. CJ-50002 had no effect on the contractile response of the isolated guinea pig ileum to various spasmogen at concentrations of 0.2, 2 and 20 $\mu\textrm{g}$/ml, respectively. Since these pharmacological effects of CJ-500o2 were observed at dose much greater than those in clinical use (approximately 0.16 mg/kg, p.o.), it is likely that this vaccine may be relatively free of undesirable effects in clinical practice.
