Robotic versus Laparoscopic Gastrectomy for Gastric Carcinoma: a Meta-Analysis of Efficacy and Safety

  • Purpose: To systematically review efficacyand safety of robotic gastrectomy (RG) compared with conventional laparoscopic gastrectomy (LG) for gastric carcinoma. Materials and Methods: A systematic literature search was carried out using PubMed, Cochrane Library, CBM, CNKI, WanFang, VIP and other sources like relevant references to obtain comparative studies assessing the effectiveness and safety between RG and LG published between 2013 and 2016. Then the literature was screened and the data were extracted by 2 independent reviewers. The quality of the literature was assessed, and the data analyzed using Stata/SE 14 software. Fixed effects or random effects models wereapplied according to heterogeneity. Results: A total of 12 non-randomized observational clinical studies involving 3,580 patients were included, of which 1,096 had undergone RG and 2,484 had received LG. The results of the meta-analysis showed in terms of effectiveness, RG was associated with less blood loss, less time to first flatus and greater number of harvested lymph nodes, but there were no significant differences in proximal and distal resection margins, compared with LG. In terms of efficiency, RG was associated with shorter hospital stay, but longer operative time. In terms of safety, there were no statistically significant differences in complications, mortality and conversions between RG and LG. Conclusions: RG can achieve comparable or better short-term and radical effects than LG, with respect to effectiveness, efficiency and safety in treatment of gastric carcinoma. Future studies involving RG should focus on decreasing operative time and reducing cost. Moreover, there is a need for randomized controlled trials comparing the two techniques with long-term follow-up.

A Control Method of ASMR Contents through Attention and Meditation Detection Based on Internet of Things (사물인터넷 기반의 집중도 및 명상도 검출을 통한 ASMR 콘텐츠 제어 기법)

  • This paper proposes a control method of ASMR(autonomous sensory meridian response) contents to relieve user's stress and improve his attention. The proposed method measures EEG(electroencephalography), attention, meditation, and eyeblink data from an EEG device and sends them to an oneM2M-compliant IoT(internet of things) server platform through an Android IoT Application. Then a SVM(support vector machine) model is built to classify user's mental health status by using EEG, attention and meditation data collected in the server platform. The ASMR contents are controlled by the mental health status classified by a SVM model and the eyeblink data. When comparing the SVM models according to types of data used, the SVM model with attention and meditation data showed accuracy of 85.7%. It was verified that the proposed control algorithm of ASMR contents properly worked as the mental health status from the SVM model and the eyeblink data changed.

Spatial distribution of sediments in the Soyang Lake based on geostatistical analyses (지구통계기법을 이용한 소양호퇴적물 분포연구)

  • To access the volume of sediments deposited after construction of the Soyang Dan and to understand their distribution in the Soyang lake, acoustic profiling using a 10-20 kHz system was conducted along profiles of 227 km length. Profile intervals are approximately 50 and 500 m for longitudinal and cross lines, respectively. The data were gain-controlled and then migrated using the f-k algorithm. After digitization of boundaries of the sediments, the acoustic interpretation was verified through correlating with 38 core samples. Thickness of the sediments averages 0.25 m and reaches to 8.25 m at maximum. Estimated total volume of the sediments based on anisotropic models in geostatistical methods is approximately $5.9{\times}10^6\;m^3$, which is more than twice greater than the earlier estimation based on an isotropic model.

  • In this paper, our goal is to design the controller which operates a manufacturing system, discrete event system, guaranteeing user specification. In specially, based on supervisory control theory W. M. Wonham, our work consists in performing the controller using Petri nets possible parallelism. In addition, we consider the problem of allocation for resources sharing of the issues to consider when designing using Petri net. The controller can be generated by synthesis of user specification model and plant model after giving the management for the resource sharing. This created controller can control the deadlock and starvation which can occur in the case with resource sharing. Previous studies proposed the Constrained Synchronous Reachability graph to synthesis of the two Petri nets models. Therefore, we provide the controlled system which forbids a deadlock and starvation using the result of previous studies.

Intergenerational Contact and Financial Support Between Parents and Married Children : Children's Gender and Birth Order as Correlates (기혼자녀의 성과 출생순위가 부모와의 접촉과 경제적지지에 미치는 영향)

  • This study examined intergenerational contact and financial support exchange between parents and each of their non-coresident married children. Prior qualitative work has suggested that increased contact between parents and their married daughters may indicate a decline in patrilineal norms in contemporary Korean families. Using a nationally representative sample, this study investigated if married daughters engage in similar levels of intergenerational contact and financial support exchange with their parents in contrast to their married brothers (first-born sons in particular). The data were drawn from the first wave of the Korean Longitudinal Study of Ageing (KLoSA, 2006). For analyses, individuals who had at least one non-coresident married child were selected, resulting in the analytic sample of 3,950 parents with 10,947 non-coresident married children. Both regression with robust standard errors and sibling fixed effects regression models were estimated using the reg and xtreg procedures in STATA. Residential proximity and sociodemographic characteristics of both parents and children were controlled in analyses. Findings suggest that, overall, parents report more frequent face-to-face contact with and financial support from their first-born sons in comparison to other sons and daughters. Daughters, on the other hand, were found to engage in more frequent contact via phone call, mail, or email with their parents. In conclusion, we did not find a strong evidence to support the contention that patrilineal norms have softened in contemporary Korean families to the extent that has been suggested in qualitative studies.

A randomized controlled clinical study of periodontal tissue regeneration using an extracellular matrix-based resorbable membrane in combination with a collagenated bovine bone graft in intrabony defects

  • Purpose: The purpose of this study was to investigate the feasibility of regenerative therapy with a collagenated bone graft and resorbable membrane in intrabony defects, and to evaluate the effects of the novel extracellular matrix (ECM)-based membrane clinically and radiologically. Methods: Periodontal tissue regeneration procedure was performed using an ECM-based resorbable membrane in combination with a collagenated bovine bone graft in intrabony defects around the teeth and implants. A novel extracellular matrix membrane (NEM) and a widely-used membrane (WEM) were randomly applied to the test group and the control group, respectively. Cone-beam computed tomography images were obtained on the day of surgery and 6 months after the procedure. Alginate impressions were taken and plaster models were made 1 week and 6 months postoperatively. Results: The quantity of bone tissue, the dimensional changes of the surgically treated intrabony defects, and the changes in width and height below the grafted bone substitutes showed no significant difference between the test and control groups at the 6-month examination. Conclusions: The use of NEM for periodontal regeneration with a collagenated bovine bone graft showed similar clinical and radiologic results to those obtained using WEM.

Digital evaluation of axial displacement by implant-abutment connection type: An in vitro study

  • PURPOSE. To measure axial displacement of different implant-abutment connection types and materials during screw tightening at the recommended torque by using a contact scanner for two-dimensional (2D) and three-dimensional (3D) analyses. MATERIALS AND METHODS. Twenty models of missing mandibular left second premolars were 3D-printed and implant fixtures were placed at the same position by using a surgical guide. External and internal fixtures were used. Three implant-abutment internal connection (INT) types and one implant-abutment external connection (EXT) type were prepared. Two of the INT types used titanium abutment and zirconia abutment; the other INT type was a customized abutment, fabricated by using a computer-controlled milling machine. The EXT type used titanium abutment. Screws were tightened at $10N{\cdot}cm$, simulating hand tightening, and then at the manufacturers' recommended torque ($30N{\cdot}cm$) 10 min later. Abutments and adjacent teeth were subsequently scanned with a contact scanner for 2D and 3D analyses using a 3D inspection software. RESULTS. Significant differences were observed in axial displacement according to the type of implant-abutment connection (P<.001). Vertical displacement of abutments was greater than overall displacement, and significant differences in vertical and overall displacement were observed among the four connection types (P<.05). CONCLUSION. Displacement according to connection type and material should be considered in choosing an implant abutment. When adjusting a prosthesis, tightening the screw at the manufacturers' recommended torque is advisable, rather than the level of hand tightening.

Study on the Institutional Change of Early Childhood Education Quality Assurance in China: From the Perspective of Historical Institutionalism (중국 유아교육의 질 보장제도 변천에 관한 연구: 역사적 제도주의 관점에서의 분석)

  • This study discusses the institutional change of Chinese early childhood education quality assurance institution (ECEQAI) from the three dimensions of the perspective of historical institutionalism of that refer to the external environment, dynamic mechanism containing with nation-market-society, and the invisible ideology of the institutional change. Chinese ECEQAI was divided into five historical periods, which were 17 years after the founding of China (1949-1966), the Great Cultural Revolution (GCR) period (1966-1976), the early stage of Reform and Opening-up (RO) (1976-1995), the social change period (1995-2010) and the new period (2010-now). This study also concludes five separate change models for Chinese ECEQAI that correspond to the five periods. The first was a compulsory and gradual institutional change model influenced by the Soviet model and dominated by national power. The second was compulsory and radical change model controlled by the counter-revolutionary group. The third was a compulsory and gradual institutional change model dominated by national power. The fourth was induced and gradual institutional change model driven by market power and forced by society power. The fifth was a compulsory and gradual institutional change model led by national power and forced by society power. Finally, the future change direction of China's ECEQAI was further pointed out, that was the government-leading and tend to balanced three-pole power model of "Nation-Market-Society", as well as the "three-pole power relationship" of indepth communication and positive interaction.

Comparison of Hoek-Brown and Mohr-Coulomb failure criterion for deep open coal mine slope stability

  • In deep open pit mines, slope stability is very important. Particularly, increasing the depths increase the risks in mines having weak rock mass. Blasting operations in this type of open pits may have a negative impact on slope stability. Several or combination of methods can be used in order to enable better analysis in this type of deep open-pit mines. Numerical modeling is one of these options. Many complex problems can be integrated into numerical methods at the same time and analysis, solutions can be performed on a single model. Rock failure criterions and rock models are used in numerical modeling. Hoek-Brown and Mohr-Coulomb terms are the two most commonly used rock failure conditions. In this study, mine planning and discontinuity conditions of a lignite mine facing two big landslides previously, has been investigated. Moreover, the presence of some damage before starting the study was identified in surrounding structures. The primary research of this study is on slope study. In slope stability analysis, numerical modeling methods with Hoek-Brown and Mohr-Coulomb failure criterions were used separately. Preparing the input data to the numerical model, the outcomes of patented-blast vibration minimization method, developed by co-author was used. The analysis showed that, the model prepared by applying Hoek-Brown failure criterion, failed in the stage of 10. However, the model prepared by using Mohr-Coulomb failure criterion did not fail even in the stage 17. Examining the full research field, there has been ongoing production in this mine without any failure and damage to surface structures.

Attenuation of p-dimethylaminoazobenzene initiated genotoxicity and cytotoxicity in mice by the combined treatment of a traditional homeopathic remedy Chelidonium Majus 200C and vitamin-C

  • The homeopathic remedy Chelidonium majus 200C (Chel-200) is traditionally used by homeopathic practitioners in liver ailments arising out of hepatotoxicity. The present investigation was aimed at examining whether vitamin C (L-ascorbic acid or AA), used in both traditional and orthodox medicines, can show better effects when used in combination with Chel-200, in favorably modifying the toxicological effects induced by the chronic feeding of p-dimethylaminoazobenzene (p-DAB, initiator) and phenobarbital (PB, promoter) in mice for 7 days through 120 days to induce hepatotoxicity and liver tumors. Mice were euthanized at 7, 15, 30, 60, 90, and 120 days of carcinogen feeding to assess various cytogenetical, biochemical and histological changes occurring in them. In a placebo controlled study, Chel-200 or the respective placebo (Alcohol-200C or Alc, "vehicle" of homeopathic drug), was orally administered to toxicant-fed mice. Sub-groups of the mice receiving Chel-200 were also fed either AA or an Alc placebo and their individual and conjoint effects were studied against the respective controls, to evaluate if the combination therapy of Chel-200 with AA can be of additional help in the amelioration of the toxicities generated by the toxicants. The combined feeding of Chel-200 and AA appeared to reduce the cytotoxic and genotoxic effects significantly, when compared to either only the Chel-200 or AA fed group. A similar trend was also obtained in the results of scanning and transmission electron microscopic studies of the livers. Experiments in other mammalian models are warranted to confirm if these drugs in combination could be used in palliative care of human patients with liver diseases including cancer.
