Comparison of Radiograpic Contrast Techniques and Contrast Media on Dacryocygtorhinography in Dogs (개에서 누낭비강조영술시 조영방법 및 조영제의 비교)

  • This study was undertaken to evaluate image quality and level of discomfort during examination with various contrast media(meglumine ioxitalamate: Telebrix $30^{\circledR}$ , diatrizoate sodium: Hypaque $50^{\circledR}$, sodium ioxaglate + meglumine ioxaglate: Hexabrix , iopamidol: Iopamiro $300^{\circledR}$ , iodized poppyseed oil: $Lipiodol^{\circledR}$ and barium sulfate: Baritop$^{\circledR}$and various radiographic contrast techniques(eye dropping: group 1, single injection to the superior punctum: group 2 and continuous injection to the superior punctum: group 3) on dacryocystorhinography in dogs. The results were as follows; 1. The Image quality of radiographic contrast technique of continuous Injection of contrast media(group 3) was better than the other techniques. 2. Average scores for dacryocystorhinography quality of sodium ioxaglate + meglumine ioxaglate, iopamidol, barium sulfate, iodized Poppyseed oil, meglumine ioxitalamate and diatrizoats sodium were 3.67${\pm}$0.47, 3.67${\pm}$0.47, 3.50${\pm}$0.50, 3.33${\pm}$0.75, 2.50${\pm}$0.50 and 2.50${\pm}$0.76, respectively in group 3. 3. Diatrizoate sodium produced more discomfort than the other media. It was considered that the best radiographic contrast technique of dacryocystorhinography was continuous injection of contrast media to the superior punctum and sodium ioxaglate + meglumine ioxaglate, iopamidol, iodized Poppyseed oil and barium sulfate were the excellent contrast media for dacryocystorhinography in dogs.

Oral contrast media for computed tomography of canine pancreas

  • Barium suspension, oral iodine contrast medium and water were applied in eight dogs to evaluate (1) distension of gastrointestinal tract, (2) the effect of the oral contrast media on the identification of the pancreas from surrounding organs, and (3) image quality and the presence of artifacts in canine pancreas computed tomography (CT) images. Oral iodine contrast medium, gastrografin, produced significant artifacts that deteriorated the CT images of the pancreas. The use of water did not provide the fullness of the gastrointestinal lumens. Barium suspension was effective for the identification of the pancreas from the surrounding gastrointestinal tract, without significantly increasing image noise. Barium suspension can be used as an optimal contrast medium that will not cause an adverse effect on the pancreatic density and image quality.

  • Purpose: Integrated PET/MRI has been developed recently has become a lot of help to the point oncologic, neological, cardiological nuclear medicine. By using this PET/MRI, a ${\mu}-map$ is created some special MRI sequence which may be divided parts of the body for attenuation correction. However, because an MRI contrast agent is necessary in order to obtain an more MRI information, we will evaluate to see an effect of SUV on PET image that corrected attenuation by MRI with contrast agent. Materials and Methods: As PET/MRI machine, Biograph mMR (Siemens, Germany) was used. For phantom test, 1mCi $^{18}F-FDG$ was injected in cylinderical uniformity phantom, and then acquire PET data about 10 minutes with VIBE-DIXON, UTE MRI sequence image for attenuation correction. T1 weighted contrast media, 4 cc DOTAREM (GUERBET, FRANCE) was injected in a same phatnom, and then PET data, MRI data were acquired by same methodes. Using this PET, non-contrast MRI and contrast MRI, it was reconstructed attenuation correction PET image, in which we evanuated the difference of SUVs. Additionally, for let a high desity of contrast media, 500 cc 2 plastic bottles were used. We injected $^{18}F-FDG$ with 5 cc DOTAREM in first bottle. At second bottle, only $^{18}F-FDG$ was injected. and then we evaluated a SUVs reconstructed by same methods. For clinical patient study, rectal caner-pancreas cancer patients were selected. we evaluated SUVs of PET image corrected attenuastion by contrast weighted MRI and non-contrast MRI. Results: For a phantom study, although VIBE DIXON MRI signal with contrast media is 433% higher than non-contrast media MRI, the signals intensity of ${\mu}-map$, attenuation corrected PET are same together. In case of high contrast media density, image distortion is appeared on ${\mu}-map$ and PET images. For clinical a patient study, VIBE DIXON MRI signal on lesion portion is increased in 495% by using DOTAREM. But there are no significant differences at ${\mu}-map$, non AC PET, AC-PET image whether using contrast media or not. In case of whole body PET/MRI study, %diff between contras and non contrast MRAC at lung, liver, renal cortex, femoral head, myocardium, bladder, muscle are -4.32%, -2.48%, -8.05%, -3.14%, 2.30%, 1.53%, 6.49% at each other. Conclusion: In integrated PET/MRI, a segmentation ${\mu}-map$ method is used for correcting attenuation of PET signal. although MRI signal for attenuation correciton change by using contrast media, ${\mu}-map$ will not change, and then MRAC PET signal will not change too. Therefore, MRI contrast media dose not affect for attenuation correction PET. As well, not only When we make a flow of PET/MRI protocol, order of PET and MRI sequence dose not matter, but It's possible to compare PET images before and after contrast agent injection.

The Spread of Contrast media in Splanchnic Nerve Block (내장신경차단시 조영제확산에 관한 연구)

  • Splanchnic nerve block is performed to relieve intractable upper abdominal pain caused by carcinoma of the pancreas, stomach, liver, or colon; and upper abdominal metastasis of tumors having more distant origins. We have performed splanchnic nerve blocks under control of X-ray fluoroscopy, for all cases of alcohol splanchnic nerve block at $L_1$ vertebra, to determine both the position of the needle tips and the spread of contrast media. During the period from December 1987 to August 1988, this method was used in 40 cases of malignancy at Severance Hospital and we clinically evaluated the location of the needle tip and the spread of contrast media. The results were as follows: 1) Our method was a retrocrural approach, the splanchnic nerve block, in all cases. 2) Most of the inserted needle points were located in the upper and anterolateral part of the $L_1$ vertebra on the antero posterior roentagenogram and in the upper quarter anteriorly on the lateral roentgenogram. 3) There was no specific relationship between the location of the needle and the spread of the contrast media. 4) The contrast media was spread around the needle and then upward along the anterior margin of the vertebral body in most of the cases. 5) Pain relief was obtained immediately in 37 cases (92.5%), but in 3 cases only after a second splanchnic nerve block.

  • The absorption coefficient of contrast media was measured region of diagnostic X-ray. Relative values of absorption coefficient was found the largest peak in the range of $60{\sim}70kVp$ for sodium sulfate and 60kVp for iodine. Increasing the thickness of contrast media and patient, the values of absorption coefficient was rising.

  • Hepatocellular carcinoma (HCC) can be noninvasively diagnosed on the basis of its characteristic imaging findings of arterial phase enhancement and portal/delayed "washout" on computed tomography (CT) and magnetic resonance imaging (MRI) in cirrhotic patients. However, different specific diagnostic criteria have been proposed by several countries and major academic societies. In 2018, major guideline updates were proposed by the Association for the Study of Liver Diseases, European Association for the Study of the Liver (EASL), Korean Liver Cancer Association and National Cancer Center (KLCA-NCC) of Korea. In addition to dynamic CT and MRI using extracellular contrast media, these new guidelines now include magnetic resonance imaging (MRI) using hepatobiliary contrast media as the first-line diagnostic test, while the KLCA-NCC and EASL guidelines also include contrast-enhanced ultrasound (CEUS) as the second-line diagnostic test. Therefore, hepatobiliary MR contrast media and CEUS will be increasingly used for the noninvasive diagnosis and staging of HCC. In this review, we discuss the emerging role of hepatobiliary phase MRI and CEUS for the diagnosis of HCC and also review the changes in the HCC diagnostic criteria in major guidelines, including the KLCA-NCC practice guidelines version 2018. In addition, we aimed to pay particular attention to some remaining issues in the noninvasive diagnosis of HCC.

  • Image enhancement를 하기 위한 영상처리 알고리즘 중의 하나인 contrast enhancement 알고리즘은 화면의 flickering 과 같은 부작용과 조절 가능한 contrast enhancement rate 에 대한 구현의 어려움 등으로 실제 TV와 같은 동영상에 적용하기에 어려움이 있었다. 본 논문에서는 Bin Underflow Bin Overflow(BUBO)를 이용하여 동영상에 적용할 경우에도 flickering 등의 부작용이 생기지 않으며 contrast enhancement rate 을 조절할 수 있는 효율적인 알고리즘을 제안한다. 또한 이와 관련하여 영상의 휘도 레벨에 있어서 어두운 영역의 계조와 밝은 영역의 계조를 향상시킬 수 있는 black/white level stretch 알고리즘과 전체 화면의 출력 휘도 레벨에 대한 dynamic range를 유지하면서 brightness를 조절할 수 있는 알고리즘을 제안한다.

ULTRASTRUCTURAL STUDY ON THE EFFECTS OF RETROGRADE INFUSION OF WATER-SOLUBLE CONTRAST MEDIA ON THE RABBIT SUBMANDIBULAR GLAND (수용성조영제의 주입이 가토악하선조직에 미치는 영향에 관한 전자현미경적 연구)

  • The author observed the effects of retrograde infusion of water soluble contrast media (Tele- brix 30) on the rabbit submandibular glands and compared the effects of different degrees of filling. 26 rabbits were divided into 2 groups of 12 each as experimentals and I group of 2 as normal controls. One experimental group was filled with 0.2㎖ and the other with 0.4㎖. Right submandibular gland of each rabbit was infused with contrast media and left one with physiologic saline as a experimental control, at a constant rate of 0.12㎖/min. using an infusion pump via the main excretory duct. Immediately after the infusion of contrast media, oblique lateral radiographs of the glands were made with occlusal film in order to confirm the glandular filling. The rabbits were sacrificed after varying periods (1, 8, 24 hours and 3, 6, 10 days) and the tissues were prepared for light and electron microscopic examination. The results were as follows: 1. In glands filled with 0.2㎖ contrast media, the initial changes were a few vacuole formation in the acini and slight dilation of the intralobular duct. The moderately severe changes such as vacuole formation in the acini, the abnormal substructure within the secretory granule, dilation of acinar and intercalated duct lumen, scalloping of striated duct lumen and inflammatory cell infiltrate were observed at 3 days. The general appearance was successively recovered, so the tissue had a normal appearance at 10 days. 2. In glands filled with 0.4㎖ contrast media, the most prominent alterations such as severe acinar atrophy, decreased number of secretory granules, proliferation of connective tissue stroma and pronounced inflammatory cell infiltrates appeared at 6 days. Although the general appearance returned to be almost normal at 10 days, acinar cells showed some atrophy and decreased secretory granules. 3. In glands subjected to 0.4㎖ infusion, the alterations were more severe and the recovery was slower than those seen in the glands to 0.2㎖ infusion.

The Optimal Length of Time to Observe Patients with Contrast Media Anaphylaxis in the Emergency Department (조영제 부작용환자의 적절한 병원내 관찰시간에 대한 고찰)

  • Purpose: The purpose of this study was to determine the optimal length of time to observe patients with contrast media anaphylaxis in the Emergency Department. Methods: This study included the patients of all age groups who presented with anaphylaxis due to contrast media to the ED of Pusan National University Hospital from January 2006 to August 2009. The medical records were retrospectively reviewed. We analyzed the 100 patients who met the inclusion criteria. Their median age was 55 (46-62), and 38 out of the 100 patients (38%) were male. Results: The most common symptom of patients was urticaria. Corticosteroid and H1 antihistamine were given to most of the patients in the ED. All were discharged after observation in the ED and none were admitted to the ward. The average time from injection of the contrast media to the onset of symptom was 31 minutes (24-39) and the average time to symptom recovery was 127 minutes (89-188). The mean observation time in the ED was 93 minutes (59-153). Biphasic reactions were reported in only one case (1/100, 1%). Conclusion: These results suggest that patients who present with an anaphylactic reaction after radiologic studies that use intravenous contrast media can be safely discharged early from the ED because the symptom recovery time is not long and a biphasic reaction is rare.
