  • This study selected Yangpyung as a target site because Yangpyung is a area of high value blessed with well preserved natural resources and beautiful natural scene and where thoughtless development and malformed use of land are in progress under the mask of hotel, accommodations and sales facility which fits the interests of land owner. As for the method used for this study, I inquired into domestic/international instances and the concept of existing environmental indicators and assessment of suitability with a view to establish assessment indicators and pose the concept through theoretical investigation of green-network and to establish green-network. 17 articles of environmental indicators such as aspect analysis, contour, greenbelt, DEM, NDVI, nature conservation area, reservoir and area for the promotion of agriculture were chosen as a actual analysing n data for setting up assessment indicators. From the result of analysis, as anticipated, green zone in Yongmusan areas within Youngmun and Seojong areas were the center of green-network in the wide area green-network in Yangpyung and the restricted area by basin system, road and legal regulation was selected as spot area and finally arable land and reservoir were selected as base which connects core with spot.

  • During the past several years, significant advances have occurred in the utilization of osseointegrated implants for the treatment of partially edentulous patients. One of the biggest purposes for treating of these patients is the high demand for improved esthetics, especially in the anterior region. For this esthetics, the new trend in dental implants is the immediate placement and immediate superstructure fabrication. The refined surgical technique, the skillful soft tissue management, and the proper prosthetic coordination are the main factors to achieve natural looking of implant supported prosthesis. The customized provisional restoration and the customized impression coping are recommended for the optimal peri-implant soft tissue contour. The basic concept of Frialit 2 system was the immediate replacement of a tooth with root-analog fixture after extraction. This system guarantees an ideal result in function and esthetics. The ceramic abutment system offers improved quality in the respect of esthetics, fitness, translucency, and biocompatibility. In this clinical report, the final restoration made with IPS Empress 2 crown on the CeraBase abutmen of Frialit 2 system allowed the reproduction of the natural vitality of tooth and adjacent gingiva.

Clinical Experience of Three-dimensional Reconstruction Using Free TRAM Flap after Total Maxillectomy with Orbital Exenteration (상악골 전절제술 및 안와내용물 적출술 후 횡복직근 유리피판을 이용한 3차원적 재건에 대한 치험례)

  • Purpose: Extensive midface defect following total maxillectomy with orbital exenteration and cheek skin resection should be three dimensionally reconstructed with a large flap that have a sufficient volume of tissue and multiple skin islands. We describe our transverse rectus abdominis myocutaneous(TRAM) free flap with three skin islands which was successfully used in this situation. Methods: A 58-year-old man was performed enbloc total maxillectomy including orbital contents and wide cheek skin because of invasive maxillary squamous cell carcinoma. He was immediately reconstructed with TRAM flap that was designed not vertical but transverse fashion for providing sufficient skin area. Also, deepithelialization procedure making for multiple skin islands was done in flap insetting period when appropriate modification according to the intraoperative situation was possible. Dead space was completely obliterated by bulky muscular tissue, and three skin islands were used for lining of lateral nasal wall, palatal surface, and cheek skin restoration. Results: Postoperative course was satisfying. Maintaining of proper ipsilateral nasal airway, loss of rhinolalia and oronasal regurgitation of food particles, and restoration of cheek contour were successfully obtained. Conclusion: We report clinical experience of threedimensional reconstruction using free TRAM flap after total maxillectomy with orbital exenteration.

Laminate production by manufacturing ceramic casting body and laminating using IPS Empress: a pressure casting method (가압주조방식인 IPS Empress로 세라믹 주조체를 제작 후 적층 형 성한 라미네이트 제작 증례)

  • Purpose: This study aimed to observe the effect of laminate veneer on patient's teeth based on the manufacturing of laminate veneer restorations, which are produced by fabricating a ceramic cast body using IPS Empress, a pressure casting method and then forming the veneer by layering. Subsequently, we assessed the potential of its clinical application. Methods: This study discusses and preserves various treatment plans, such as diagnostic wax-up and treatment room diagnosis, for patients who visit the hospital to improve the appearance of teeth due to diastema of maxillary teeth, inexperienced resin filling, lack of esthetics, and external teeth. A ceramic cast body is constructed using IPS Empress, which is an effective and aesthetic restoration pressure casting method to restore the veneer with a laminate made by layering. Results: Compared with the preoperative state, the frontal view of the patient after the final restoration showed the formation of a natural smile line; the space between the central and lateral incisors was filled in synchronously with the adjacent teeth. In addition, the emergence profile is maintained by reducing the over-contour as much as possible. Conclusion: The patient's quality of life is improved by providing them with a satisfactory natural smile.

A Size Change of Bone Defect Area after Autogenous Calvarial Bone Graft (자가 머리뼈 이식 후 뼈결손부의 면적 변화)

  • Calvarial bone grafting in craniomaxillofacial trauma and facial reconstructive surgery is now widely recognized and accepted as a standard procedure. One of the commonly reported problems of calvarial bone graft is the contour defect caused by partial resorption of the graft. But, there are few reports that discuss the fate of the calvarial bone graft based on the quantitative data. In this article, the changes of grafted calvarial bone were evaluated using 3-dimensional computed tomography(CT). 9 patients were observed with the CT scans at 2mm thickness immediately after operation and at the time of last follow-up. The area of the bone defect was segmented on the 3-dimensional CT image and calculated by AnalyzeDirect 5.0 software. The immediate postoperative bone defect area of the recipient site and the donor site were $612.9mm^2$ and $441.5mm^2$, respectively, which became $1028.1mm^2$ and $268.8mm^2$, respectively at the last follow-up. In conclusion, the bone defect area was less increased on the donor site of calvarial bone graft than on the recipient site. And the CT scan is a valuable imaging method to assess and follow-up the clinical outcome of calvarial bone grafting.

  • $Medpor^{(R)}$(porous polyethylene) Surgical Implants are used for the augmentation or restoration of bony contour in craniofacial defects. The purpose of this study is to evaluate the ingrowth of soft tissue and bone after application in calvaria of rats. The experiment was carried out in 60 rats. The reflected periosteum was resutured after implantation of $Medpor^{(R)}$ as a experimental site, while in the calvarial bone the reflected periosteum resutured without implantation as a control site. The histologic examination was performed after 1-, 2-, 4-, 8-, 12-, 24-weeks implantation in calvaria of rats. I concluded that there was abundant ingrowth of soft tissue and bone without any adverse tissue response and that it shows good stability.

  • Indirect bonding is done by placing the brackets on a model in the laboratory and using a template or tray to transfer the laboratory positioning to the teeth. The advantages of this technique are 1. decreased chair time 2. less patient discomfort 3. accuracy of a attachment placement 4. good adaptation of attachment to tooth contour 5. occlusal relationship of brackets and opposing teeth can be checked The disadvantages of the technique are 1. complex laboratory procedure 2. sometimes difficult on very short clinical crowns 3. teeth with crowns, large buccal restoration will not bond 4. may not be fitted close, if poor adaptation 5. likely to be disturbed setting Several indirect bonding techniques have proved reliable in clinical practice. However, they differ in the way the brackets are attached temporarily to the model, the type of transfer tray or other mechanism used, the adhesive or sealant employed, whether segmented or full bonding used, and the way the transfer is removed so as not to exert excessive force on a still maturing bond.

  • Outcome of esthetic ceramic restorations are affected by tooth size, gingival contour, occlusal relationship, etc. For this reason, demand of orthodontic treatment before esthetic ceramic restoration is increasing. If a Bolton ratio discrepancy, a problem of the maxillary incisor's vertical position, a problem of inclination of anterior teeth, a pathogenic occlusion is existed, a pre-prosthodontic orthodontics should be accomplished. These problems can be satisfactory only after the prosthetic treatment is performed after orthodontic treatment. When orthodontic treatment is given, it should be treated with the following principle. 1. Treat it in the direction of functioning occlusion. 2. Keep the patient's stable occlusal scheme. 3. Treat the teeth by considering the average tooth size and Bolton ratio. Ortho-Prostho combined treatment with optimal treatment plan can lead a patient's function, esthetics, and long-term stability.

  • Panfacial bone fracture is challenging. Even experienced surgeons find restoration of original facial architecture difficult because of the severe degree of fragmentation and loss of reference segments that could guide the start of facial reconstruction. To restore the facial contour, surgeons usually follow a general sequence for panfacial bone reduction. Among the sequences, the bottom-to-top and outside-in sequence is reported to be the most widely used in recent publications. However, a single sequence cannot be applied to all cases of panfacial fractures because of the variations in panfacial bone fracture patterns. In this article, we intend to find the reference and discuss the efficacy of inside-out sequence in facial bone fracture reconstruction.

Three-dimensional Digital Restoration and Surface Depth Modeling for Shape Analysis of Stone Cultural Heritage: Haeundae Stone Inscription (석조문화유산의 형상분석을 위한 3차원 디지털복원과 표면심도 모델링:해운대 석각을 중심으로)

  • This study was focused on digital restoration and surface depth modeling applying the three-dimensional laser scanning system of the Haeundae Stone Inscription. Firstly, the three-dimensional digital restoration carried out acquiring of point cloud using wide range and precision scanner, thereafter registering, merging, filtering, polygon mesh and surveyed map drawing. In particular, stroke of letters, inscribed depth and definition appearing the precision scanning polygon was outstanding compared with ones of the wide range polygon. The surface depth modeling completed through separation from polygon, establishment of datum axis, selection of datum point, contour mapping and polygon merging. Also, relative inscribed depth (5~17mm) and outline by the depth modeling was well-defined compared with photograph and polygon image of the inscription stone. The digital restoration technology merging wide range and precision scanning restored the total and detailed shape of the Stone Inscription quickly and accurately. In addition, the surface depth modeling visibly showed unclear parts from naked eye and photograph. In the future, various deteriorations and surrounding environment change of the Stone Inscription will be numerically analyze by periodic monitoring.
