Middle-aged Korean's Ageism Affecting Factors Mediated by Intergroup Anxiety (한국중년의 노인차별에 미치는 영향요인과 집단간불안의 매개효과)

  • The present study empirically confirmed knowledge of ageing and quality of contact were predictors affecting middle-aged Korean's ageism against the elderly and verified mediating role of intergroup anxiety between not only knowledge of ageing but also quality of contact and ageism. To investigate causalities of factors, we purposively collected 400 samples from 20 Dongs evenly located in Jeonju and 393 samples, survived the data cleaning such as missing values, outliers, normality and covariance conditions, were analyzed by frequency, factor analysis, reliability, confirmatory factor analysis and structural model analysis. Followed were the selected contributions of the present study. First, the knowledge of ageing and quality of contact were predictors of ageism mediated by intergroup anxiety. Second, the knowledge of ageing and quality of contact did not directly affect middle-aged Korean's ageism against the elderly. Third, intergroup anxiety had strong effect on ageism. The contributions suggested increasing knowledge of ageing and providing contact experience to middle-aged Korean as combating strategy against ageism.

The Three Dimensional Analysis of the Upper Body's Segments of the Elderly during Walking (보행 시 노인의 상체 움직임에 대한 3차원적 분석)

  • The purpose of this study was to investigate the kinematic variables of the upper part of the body for 8 elderly men during walking. For this study, kinematic data were collected using a six-camera (240Hz) Qualisys ProReflex system. The room coordinate system was right-handed and fixed in space, with righted orthogonal segment coordinate systems defined for the head, trunk, and pelvis. Based on a rigid body model, reflective marker triads were attached on the 3 segments. Three-dimensional Cartesian coordinates for each marker were determined at the time of recording using a nonlinear transformation(NLT) technique with ProReflex software (Qualisys, Inc.). Coordinate data were low-pass filtered using a fourth-order Butterworth with cutoff frequency of 6Hz. Three-dimensional angles of the head, trunk, and pelvis segment were determined using a Cardan method. On the basis of each segment angle, angle-angle plot used to estimated the movement coordinations between segments. The conclusions were as follows; (1) During the support phase of walking, the elderly people generally kept their, head the flexional and abductional posture. Particularly, the elderly displayed little internal/external rotation. (2) The elderly people showed extensional and external rotation postures in the trunk movement. Particularly, It showed the change from adduction into abduction at the heel contact event of the stance phase. (3) The elderly people showed almost same pelvis movement from the flexion into extension, from the abduction into adduction, and from internal rotation into external rotation at the mid stance and toe off of the stance phase.

Current Status of Hip Fracture amongthe Elderly in Pusan (부산지역 노인들의 고관절골절발생실태에 관한 연구)

  • This study was conducted to find out current status and characteristics, so that the result could provide basic data to establish prevention programs for hip fracture among the elderly. The data were coolected from 199 cases hospitalized in 4 university hospitals in Pusan during three years period from January, 1966 to December, 1998. The data were obtained from medical records and by using questionnaires through telephone contact or direct personal interview with the subjects or their family, and were analyzed with x2-test. The results were as follow: The mean age of elderly hip fractured cases was 75.7 years, and 61.8% were female. The average hospital stay of the cases was 30.8 days, and 73.9% survived. Femur neck fracture cases comprised 51.7% of the toral cases, while trochanteric fracture cases comprised 48.3%. The hip fracture occurred more frequently during the day, shown as 66.9%, and the most frequently due to trips(44.2%), followed by slips(322%), c1ash(14.6%), dizziness(7.5%), and 00 on. Femur neck fracture, however, occurred the most frequently due to trips(51.5%) and, on the other hand, trochanteric fracture due to slips(40.6%) and trips(36.5%). About two third of hip fracture(67.8%) occurred inside of the building. The most frequently mentioned location for hip fractures was rooms or floors(32.1%), followed by streets(24.1%), bathrooms(17.6%), stairways(13.1%), and so on. Trochanteric fracture, especially, more frequently occurred inside of the building(71.9%) as compared to femur neck fracture(64.1 %). The rate of independent indoor walking in femur neck fractured cases reduced from 88.3% to 74.8% after fracture incidences, while the rate reduced from 86.0% to 45.8% in trochanteric fracture cases. It indicated that trochanteric fractures affected the walking ability of the elderly more severely that femur neck fractures. In conclusion, hip fracture occurs as a combined result of aging characteristics, environmental factors, and health related characteristics. Future studies that investigaterisk factors of hip fracture in elderly are recommended.

ICT-based Living in the Contact Type Service Model for Self-life Support of the Elderly Living Alone (독거노인 자가생활지원을 위한 ICT기반의 생활밀착형 서비스 모델 개발)

  • In case of current South Korea, the rapid graying is in the progress. Because of it, the increase of elders who live alone causes variety of social, economic and national problems. Especially in the modern city, the health care, safety and maintenance of pleasant and joyful life of elders who live alone are being a big social issue. Even though practical services have been tried at the whole of government approach, the detailed alternatives are insufficient to improve the daily life support and service to approach the information. This research suggests the "ELA Service Model" which actively improves the easy access to information and corresponds to condition and circumstance of elders for providing the ICT-base Elderly Living alone care service. It is a step self-life support service based on fundamental desire of elders which resolves the problem and contributes to establish the active and balanced policy to the elderly.

Qualitative Study on Social Workers' Experiences and Roles during End-of-Life Care in Elderly Long-Term Care Facilities (노인 장기요양기관에 종사하는 사회복지사의 임종 케어 경험과 역할에 관한 질적 연구)

  • This study was to investigate the social workers' psycho·social experiences and roles during the end-of-life care process in elderly long-term care facilities. As a result of data analysis through in-depth interviews, social workers experienced great exhaustion and burnout due to frequent death experiences during the end-of-life care process, and expressed regret for not being able to provide better service for the deceased and longing for loved ones. And in the event of a sudden death, social workers would undergo criticism and complaints. The main roles of social workers during end-of-life care were to contact and communicate with family members when signs of death appeared and support them after the death. The necessity of standardized manuals and education for end-of-life care, a recharging program and support group to prevent burnout of social workers, and a legal safety net for emergency preparedness and emergency measures were suggested. The necessity of death preparation education, hospice care, and advance medical directive was also emphasized for the dignified death of the elderly.

A Case Report of 2 Cases of Severe Sacral Stage IV Pressure Ulcer in the Elderly Who Were Cured by Treatment with Jaungo and Acupuncture and Debridement (자운고와 침 치료 및 변연절제술로 완치된 고령자의 천골부위 4기 중증욕창 2례에 대한 증례보고)

  • The purpose of this study is to report the effect of Jaungo and acupuncture and debridement for severe pressure ulcer in the elderly. Two Elderly patients with stage 4 pressure ulcer in the sacral region were eligible to participate in this study. Dressing using Jaungo was applied twice a day, and acupuncture was applied 4 times a week along the boundary between the normal epidermis and the pressure ulcer granulation tissue in contact with the pressure ulcer interface. Debridement was performed after partial or total removal, if necessary, under the judgment of the attending physician.In both cases, stage 4 pressure sores in the sacrum covered with crusts were cured in 193 days and 223 days, respectively, and no side effects were reported during treatment. This study shows that Jaungo and acupuncture and debridement may represent effective to treat pressure ulcer. but further studies are needed.

The Effects of Grandmaternal Child Care on Intergenerational Contacts: Focusing on Long-Term Reciprocity Relationships (모(母)의 손자녀 돌봄이 성인자녀와의 접촉 수준에 미치는 영향 : 장기적 상호 관계를 중심으로)

  • The purpose of this study was to examine how previous grandmaternal child care affects contacts between older mothers and their adult children. This study especially focused on intergenerational long-term reciprocity relationships. In this study, data from the first wave (in 2006) to the forth wave (in 2012) of Korean Longitudinal Study of Ageing yielded a sample of 1,925 elderly mothers and 7,460 adult children. This study employed multi-level analyses considering hierarchical structures between the mothers and their adult children. In this study, past grandmaternal child care was measured by providing care behavior and unit of time for care. Intergenerational contacts was measured by frequency of face-to-face and non-face-to-face contacts. The results showed that adult children who previously received child care from their mothers were more likely to contact to their mothers. Additionally, the increasing amount of time for elderly mothers to provide care to their grandchildren led to more frequent contacts between elderly mothers and their adult children. The findings proved that grandmaternal child care was in important position in reciprocal relationships between elderly mothers and their adult children. Through these results, this study suggested theoretical, policy, and practical implications.

The Family Network Types and Life Satisfaction of the Rural Elderly (농촌노인의 가족관계망 유형과 생활만족도)

  • This study investigated the family network of elderly and its effects on the subjective life satisfaction in Rural Area. In order to classify the family network, the authors used the analysis technique of social network including to a spouse, children and grandchildren. In addition, the authors described basic characteristics of family network on the family type, interaction frequency, and interaction content. And then family network typified four types by K-means cluster analysis method according to characteristics of family network and examined difference on life satisfaction of the elderly persons according to the type of family network. The major results were as following. First, the elderly did contact his/her children often, emotional support revealed that highest support expectation of elderly. Second, The family network of elderly could be typified four types ; 'relation estranged type', 'children-grandchildren centered type', 'family dependent type', 'couples centered type' and statistically significant difference showed in life satisfaction according to each type. The result, in the 'couples centered type', the life satisfaction was highest; on the contrary, 'relation estranged type', it was lowest. Third, Influencing factors on life satisfaction of the old person were economic conditions, physical conditions, education level, sex, more frequent contacts with grandchild, emotional support expectation of spouses. The results of this study suggest that social welfare political and institutional efforts are needed to improve the relationship between older persons and their children, grandchildren and spouses and life satisfaction of the elderly.

Oral care practices for dementia patients and social support and depression in family caregivers of dementia patients (치매환자 부양가족의 치매환자 구강관리 실태 및 우울과 사회적지지)

  • Objectives: This study aims to contribute to devising systems for family caregivers of dementia patients by examining the state of oral care of dementia patients, and depression and social support among family caregivers of dementia patients. Methods: Family caregivers of dementia patients in the metropolitan area were selected in this study. The inclusion criteria were individuals who have provided care for a dementia patient at home for at least six months and those who come in contact (including phone calls) with the patient at least twice a week. Results: Oral health knowledge of the elderly, caregiving burden, depression, and social support were examined. The mean scores for oral health knowledge of the elderly and caregiving burden were $57.11{\pm}16.94$ out of 100 and $17.33{\pm}8.61$ out of 48, respectively. Further, the mean caregiving behavior score, depression score, and social support score were $8.49{\pm}13.71$ out of 100, $5.11{\pm}3.05$ out of 10, and $72.75{\pm}17.03$ out of 100, respectively. Factors affecting oral health knowledge of the elderly were examined. The results showed that the level of oral health knowledge of the elderly increased with an increasing perception of a need for oral health education (p<0.05), caregiving burden (p<0.01), and social support (p<0.01). Conclusions: These findings suggest that developing and popularizing oral care intervention programs for family caregivers of dementia patients are necessary to ensure systematic oral care for dementia patients.

A Study of the Residential Environment, Mood States, ADL and Nursing Need of the Elderly who Live Alone (독거노인의 거택환경과 기분상태, 일상생활활동 및 간호요구도에 관한 연구)

  • This study was conducted to find out residential environment, mood states, ADL(Activity Daily Living) and nursing need of the elderly who live alone. The questionnaire survey was conducted on 154 single household elderly over 65years who live n D-city, Kangwon Province. The data were collected between September 1. 1999 and October 17, 1999 and analyzed by means of SPSS package. The results were as follows; 1. The percentage of the subjects who owned their house was no were than $13\%$., which showed that they still belonged to the economically vulnerable class. As for their residential environment, the average indoor temperature and humidity level were $16.45^{\circ}C\;and\;44.62\%$ respectively during the daytime between September and October, which proved to range within the standard scope. The average indoor illumination level was, however, 43.61 Lux, which showed that their illumination environment was very poor. 2. The average score of their mood states was 3.31. which turned out to be rather low. 3. The mean score of ADL was 1.69, which showed that they didn't have any serious inconveniences in their ordinary lives. 4. The mean score of their nursing need was 275, which proved to be close to the normal level. 5. The higher the room humidity was, the higher their nursing need increased. The higher the degree of illumination level was, the lower the degree of their nursing need was and the higher the level of ADL was. 6. The major factors influencing their moon status were the frequency of their contact with children and room humidity. These two factors could account for $22\%$ of the total mood status. These results suggest that the residential environment should be necessarily considered in the psychological nursing intervention for the elderly who live alone and they should be urgently supported in terms of health policy so that they can maintain the optimum level of illumination during the daytime.
