The Effects of Sochongryong-tang on Pulmonary Function and Quality-of-Life in Asthmatic Patients (소청용탕이 기관지천식 환자의 폐기능과 삶의 질에 미치는 영향)

  • Objectives : Asthma is a chronic inflammatory disorder in which the airway smooth muscle undergoes exaggerated constriction and is abnormally responsive to external stimuli and clinically manifests dyspnea, cough, and wheezing. Recently in Korea, the number of asthma patients has tended to increase as air pollution increases and new allergens appear. This study aimed to identify the therapeutic effects of Sochongryong-tang, which has been used on asthmatic patients in Oriental Medicine for a long time. Methods : We studied 10 asthmatic patients who had visited the Division of Respiratory System, Department of Internal Medicine of Kyunghee Oriental Medical Center from February 1, 2001 to June 30, 2001, which examined the patients by the pulmonary function test (PFT) and Quality of Life Questionnaire for adult Korean Asthmatics (QLQAKA) before and after taking Sochongryong-tang for two weeks. The data was analyzed using paired t-test. Results : After treatment with Sochongryong-tang for two weeks, FEV 1.0 showed a significant increase of 14.2%, FVC a significant increase of 9.3% and PEFR a significant increase of 16.2% in results of patients examined asthmatic by PFT. In the results of QLQAKA, the mean of scores increased significantly over 0.5 points in total score, symptom domain, emotion domain, and environmental domain. Conclusions : This study shows that Sochongryong-tang has the effect of improvements of pulmonary function and quality of life in asthmatic patients.

  • Based on the research of cataract, glaucoma, bluish glaucoma, we have abtained the following results. 1. Main etiologies of the cataract are deficiency of the liver and kidneys, main symptoms are no nebula, no pain, no tear and sometimes light blue and white vision appear. 2. Main etiologies of glaucoma are receives heat from the liver, stagnation of the liver and gall bladder etc, main symptoms are both forehead corner pull each ether, pain from eye ball to nasal septum, headache, extending pain of the eyes, sometimes red and white flower vision appear, etc. 3. main etiologies of the bluish glaucoma are melancholy, wrath, stagnation of the liver, internal damage, flaming up fire of the phlegm, deficiency of the liver, deficiency of blood, etc, main symptoms are numbness, visual field constriction, blindness, reduce of visual capacity, blue flower vision sometimes appear, etc. 4. Treatments are mainly uses nourish Um(陰) to reduce pathogenic fire, relieve stagnancy of Ki(氣) of the liver, nourish Um(陰) of the liver and kidneys, etc, acupuncture points are mostly contended arounded eyes such as BL10)(天柱), GV16(風府), LR3)(太衝), HT5)(通里) and in the case of nauses and vomitting PC6)(內關, ST36)(足三里) are included in addition to the above.

Correction of microtia with constriction features using a superficial temporal fascial flap combined with a rib cartilage graft

  • Background Microtia with constricted features is characterized by a short helical length of variable severity, upper antihelical or scaphal deficiency, and a downfolded upper ear. No consensus has been reached regarding the most appropriate surgical method for this condition. In this study, we aimed to introduce a simple and safe surgical method for the correction or reconstruction of upper helix ear deformities. Methods Between February 2011 and June 2014, eight patients with microtia with constricted upper helix ear deformity underwent reconstruction of the ear deformity. The upper ear helical framework was constructed by carving and curving the eighth rib cartilage harvested from the ipsilateral chest wall, covering this cartilage with a superficial temporal fascial flap, and adjusting the skin graft to align with the ear contour. To evaluate their satisfaction, patients were asked to complete a questionnaire regarding ear shape, symmetry, position, color, and overall outcome scored on a 5-point scale at 12 months postoperatively. Results None of the patients experienced severe complications in the reconstructed ear. The preoperative and postoperative vertical ear length ratios were 0.88 and 1.02, respectively. And the mean patient satisfaction scores for shape, symmetry, position, color, and overall outcome were 4.2, 4.5, 4.7, 4.4, and 4.6 out of 5 points, respectively. All patients expressed a high level of satisfaction at 12 months postoperatively. Conclusions Our technique provides a good alternative method for the reconstruction of moderate constricted upper helix ear deformities in patients who meet the surgical indications with satisfactory outcomes and few complications.

Land Use Feature Extraction and Sprawl Development Prediction from Quickbird Satellite Imagery Using Dempster-Shafer and Land Transformation Model

  • Accurate knowledge of land use/land cover (LULC) features and their relative changes over upon the time are essential for sustainable urban management. Urban sprawl growth has been always also a worldwide concern that needs to carefully monitor particularly in a developing country where unplanned building constriction has been expanding at a high rate. Recently, remotely sensed imageries with a very high spatial/spectral resolution and state of the art machine learning approaches sent the urban classification and growth monitoring to a higher level. In this research, we classified the Quickbird satellite imagery by object-based image analysis of Dempster-Shafer (OBIA-DS) for the years of 2002 and 2015 at Karbala-Iraq. The real LULC changes including, residential sprawl expansion, amongst these years, were identified via change detection procedure. In accordance with extracted features of LULC and detected trend of urban pattern, the future LULC dynamic was simulated by using land transformation model (LTM) in geospatial information system (GIS) platform. Both classification and prediction stages were successfully validated using ground control points (GCPs) through accuracy assessment metric of Kappa coefficient that indicated 0.87 and 0.91 for 2002 and 2015 classification as well as 0.79 for prediction part. Detail results revealed a substantial growth in building over fifteen years that mostly replaced by agriculture and orchard field. The prediction scenario of LULC sprawl development for 2030 revealed a substantial decline in green and agriculture land as well as an extensive increment in build-up area especially at the countryside of the city without following the residential pattern standard. The proposed method helps urban decision-makers to identify the detail temporal-spatial growth pattern of highly populated cities like Karbala. Additionally, the results of this study can be considered as a probable future map in order to design enough future social services and amenities for the local inhabitants.

Effect of epidural polydeoxyribonucleotide in a rat model of lumbar foraminal stenosis

  • Background: We aimed to investigate the effect of epidural polydeoxyribonucleotide (PDRN) on mechanical allodynia and motor dysfunction in a rat model of lumbar foraminal stenosis (LFS). Methods: This study was conducted in two stages, using male Sprague-Dawley rats. The rats were randomly divided into eight groups. In the first stage, the groups were as follows: vehicle (V), sham (S), and epidural PDRN at 5 (P5), 8 (P8), and 10 (P10) mg/kg; and in the second stage, they were as follows: intraperitoneal PDRN 8 mg/kg, epidural 3,7-dimethyl-1-propargilxanthine (DMPX) (0.1 mg/kg), and DMPX (0.1 mg/kg). The LFS model was established, except for the S group. After an epidural injection of the test solutions, von Frey and treadmill tests were conducted for 3 weeks. Subsequently, histopathologic examinations were conducted in the V, S, P5, and P10 groups. Results: A total of 65 rats were included. The P8 and P10 groups showed significant recovery from mechanical allodynia and motor dysfunction at all time points after drug administration compared to the V group. These effects were abolished by concomitant administration of DMPX. On histopathological examination, no epineurial inflammation or fibrosis was observed in the epidural PDRN groups. Conclusions: Epidural injection of PDRN significantly improves mechanical allodynia and motor dysfunction in a rat model of LFS, which is mediated by the spinal adenosine A2A receptor. The present data support the need for further research to determine the role of epidural PDRN in spinal stenosis treatment.

IN VITRO EVALUATION OF THE CONSISTENCY OF TWO ELECTRONIC APEX LOCATORS (2종 전자근관장측정기의 일관성에 관한 in vitro 연구)

  • The purpose of this study was to evaluate the consistency of two electronic apex locators in vitro model. Materials consisted of fifty two extracted premolars and two electronic apex locators; Root ZX (J. Morita, Osaka, Japan) and E-Magic Finder Deluxe (S-Denti. Cheonan, Korea). After access preparation, the teeth were embedded in a saline-mixed alginate model. Canal lengths of each tooth were measured at "0.5" and "Apex" mark of the apex locators, respectively so that each tooth had two measurements from 0.5 and Apex points. The file was fixed at final measurement using a glass ionomer cement. The apical 4 mm from the apex was exposed to measure the distance from the file tip to the major apical foramen of each tooth. Average distances and standard deviations were used to evaluate the consistency. Results showed that all measurements of both Root ZX and E-Magic Finder located the major foramen the range of ${\pm}0.5\;mm$ level. Both apex locators showed better consistency at Apex mark than at 0.5 mark. The average distance of file tip-major foramen was - 0.18 mm at 0.5 mark and - 0.07 mm at Apex mark in Root ZX, - 0.25 mm at 0.5 mark and - 0.02 mm at Apex mark in E-Magic Finder. Standard deviation was 0.21 at 0.5 mark and 0.12 at Apex mark in Root ZX, 0.12 at 0.5 mark and 0.09 at Apex mark in E-Magic Finder.
