동결견을 가진 여성에 대한 탄력밴드 저항운동의 영향 (Effect of Self Resistance Exercise in Women with Frozen Shoulder)

  • Background : This study was conducted to know the effect of resistance exercise using thera-band in parallel with conservative therapy in patients with frozen shoulder. Methods : We analyze subjective pain scale, ROM and shoulder functional scale 24 patients diagnosed with adhesive capsulitis were included for study. Among them, 12 patients(experiment group) were managed by resistance exercise as well as conservative physical therapy, another 12 patients(control group) were only treated by conservative therapy alone. All the patients were treated three times a week for 4 weeks, and after, and after each treatment subjective pain scale, ROM, shoulder functional score(SFI) were measured. Results : The VAS were significant reduced between experimental group and control group according to pre-treatment and post-treatment. In experimental group, the ROM and SFI has increased than control group according to pre-treatment and post-treatment. Conclusion : According to the results, we concluded that both conservative therapy and self resistance exercise using thera-band technique are more effective for increasing ROM especially flexion, abduction, internal rotation and SFI than conservative therapy alone.

지간 신경종 보존적 치료에 영향을 미치는 인자 (Factors Affecting on Conservative Treatment of Morton's Neuroma)

  • Purpose: The purpose of this study was to investigate the clinical results of conservative treatment for Morton's neuroma and to analyze the factors which influenced on the results. Materials and Methods: In this retrospective study, 101 cases of 83 patients with interdigital neuroma were conservatively treated with follow-up period of at least 6 months. There was no significant difference in results among different age groups, sxes, and lesion sites. However, the results were significantly better if the treatment was started within 6 months after onset. Results: We had 28 excellent results (28%), and 13 good results (13%). Patients were grouped by sex, age, duration of symptom before treatment, affected location. The result of treatment was evaluated by comparing the subject pain and discomfort score of the first vist and last follow-up. Conclusion: There are excellent results over 41% by conservative treatment. The results of conservative treatment were not related to age, sex, and lesion site, but were related to pre-treatment period. The shorter the pretreatment period was, the better the results.

심한 근관 감염에서의 항생제 선택 (Antimicrobial choice of severe endodontic infection)

  • Objectives : The purpose of our study was to evaluate penicillin as a still drug of choice for severe endodontic infection, by analyzing the antimicrobial susceptibilities from endodontic infections with swelling to figure out appropriate antibiotics as empirical treatment. Materials and methods : This study involved 18 patients who attended for emergency treatment because of facial or periapical swelling associated with root canal infections. Identification and antimicrobial susceptibility test of each pathogen were performed by Vitek2 Systems (bioM$\acute{e}$rieux, Marcy l'Etoile, France). Results : The most frequent bacteria was Streptococcus spp.(77%), and the resistance against penicillin was 35% in overall patients, followed by clindamycin and erythromycin (17%), which was much higher than previous studies. Conclusions : In our study, the higher resistance made penicillin alone not to be chosen as the first antibiotic drug for severe endodontic infections. Combinations with other drug, penicillin with wider spectrum of activity, or changing to other antibiotics was considered while remembering the increased risk of resistant microorganism.

Comparative evaluation of the bond strength of self-adhering and bulk-fill flowable composites to MTA Plus, Dycal, Biodentine, and TheraCal: an in vitro study

  • Objectives: This study aimed to compare the shear bond strength (SBS) of a self-adhering flowable composite (Dyad Flow) and a bulk-fill flowable composite (Smart Dentin Replacement [SDR]) to several pulp-capping materials, including MTA Plus, Dycal, Biodentine, and TheraCal. Materials and Methods: Eighty acrylic blocks with 2-mm-deep central holes that were 4 mm in diameter were prepared and divided into 2 groups (n = 40 each) according to the composite used (Dyad Flow or SDR). They were further divided into 4 sub-groups (n = 10 each) according to the pulp-capping agent used. SBS was tested using a universal testing machine at a crosshead speed of 1 mm/min. Data were analyzed using 2-way analysis of variance. A p value of < 0.05 was considered to indicate statistical significance. Results: A statistically significant difference (p = 0.040) was found between Dyad Flow and SDR in terms of bond strength to MTA Plus, Dycal, Biodentine, and TheraCal. Conclusions: Among the 8 sub-groups, the combination of TheraCal and SDR exhibited the highest SBS.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative treatment for femoral neck stress fracture developed in avascular necrosis of the femoral head. Methods : Patient is hospitalized at Dept. of Oriental Rehabilitation Medicine, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as femoral neck stress fracture developed in avascular necrosis of the femoral head and treated by herbal medicine, acupuncture and moxibustion. This study was measured by Visual Analogue Scale(VAS) score, range of motion and walking time. Results : After conservative treatment, the patient's pain was controlled and VAS score was decreased. Range of motion and walking time were improved. Conclusions : As seen in this one case, Oriental conservative treatment has a positive effect to control pain with femoral neck stress fracture developed in avascular necrosis of the femoral head.

Healing outcomes of root canal treatment for C-shaped mandibular second molars: a retrospective analysis

  • Objectives: This study aimed to evaluate the healing rate of non-surgical endodontic treatment between C-shaped and non-C-shaped mandibular second molars. Materials and Methods: Clinical records and radiological images of patients who had undergone endodontic treatment on mandibular second molars between 2007 and 2014 were screened. The periapical index scoring system was applied to compare healing outcomes. Information about preoperative and postoperative factors as well as the demographic data of the patients was acquired and evaluated using chi-square and multinomial logistic regression tests. Results: The total healing rate was 68.4%. Healing rates for the mandibular second molar were 70.9% in C-shaped canals (n = 79) and 66.6% in non-C-shaped ones (n = 117). The difference was not statistically significant. Conclusions: The presence of a C-shaped canal in the mandibular second molar did not have a significantly negative effect on healing after treatment. Instead, proper pulpal diagnosis and final restoration were indicated as having significantly greater influence on the healing outcomes of C-shaped and non-C-shaped canals, respectively.

  • Objectives : The propose of this study is to find out the clinical application of conservative treatment to the patient who has Lumbar incomplete burst fracture. Methods : We examined 1 patient with lumbar burst fracture who admitted Jaseng Hospital of Oriental Medicine. Results : In this study, the patient showed a certain degree of improvement in gait disturbance. Low back and leg pain were also reduced significantly after conservative oriental medical treatment. Conclusions : Conservative oriental medical treatment can be effective for improving symptoms of Lumbar incomplete burst fracture, decreasing pain.

  • Objectives : The propose of this study is to find out the clinical application of conservative treatment for Failed Back Surgery Syndrome(FBSS) patients. Methods : We examined 3 patients with Failed Back Surgery Syndrome(FBSS) who visited Jaseng Hospital of Oriental Medicine. We used Acupuncture, Bee venom, CHUNA manual therapy, Herbal medication for this patient. And we measured of NRS score to evaluate the conservative treatment effects. Results : In this study, patients sciatica and symptoms has improved. NRS score were also decreased. Conclusions : Conservative oriental medical treatment can be effective for improving symptoms of Failed Back Surgery Syndrome.

Conservative Treatment of Ulnar Nerve Compression at the Elbow: A Systematic Review and Meta-Analysis

  • Background The clinical results of conservative treatment options for ulnar compression at the elbow have not been clearly determined. The aim of this review was to evaluate available conservative treatment options and their effectiveness for ulnar nerve compression at the elbow. Methods In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations, a systematic review and meta-analysis of studies was performed. Literature search was performed using Ovid MEDLINE, Embase, and Cochrane Central Register of Controlled Trials (CENTRAL). Results Of the 1,079 retrieved studies, 20 were eligible for analysis and included 687 cases of ulnar neuropathy at the elbow. Improvement of symptoms was reported in 54% of the cases receiving a steroid/lidocaine injection (95% confidence interval [CI], 41-67) and in 89% of the cases using a splint device (95% CI, 69-99). Conclusions Conservative management seems to be effective. Both lidocaine/steroid injections and splint devices gave a statistically significant improvement of symptoms and are suitable options for patients who refuse an operative procedure or need a bridge to their surgery. Splinting is preferred over injections, as it shows a higher rate of improvement.

Effect of dentin pretreatment and curing mode on the microtensile bond strength of self-adhesive resin cements

  • PURPOSE. The aim was to evaluate the effect of curing mode and different dentin surface pretreatment on microtensile bond strength (${\mu}TBS$) of self-adhesive resin cements. MATERIALS AND METHODS. Thirty-six extracted human permanent molars were sectioned horizontally exposing flat dentin surface. The teeth were divided into 12 groups (3 teeth/group) according to the dentin surface pretreatment methods (control, 18% EDTA, 10% Polyacrylic acid) and curing mode (self-curing vs. light-curing) of cement. After pretreatment, composite resin blocks were cemented with the following: (a) G-CEM LinkAce; (b) RelyX U200, followed by either self-curing or light-curing. After storage, the teeth were sectioned and ${\mu}TBS$ test was performed using a microtensile testing machine. The data was statistically analyzed using one-way ANOVA, Student T-test and Scheffe's post-hoc test at P<.05 level. RESULTS. For G-CEM LinkAce cement groups, polyacrylic acid pretreatment showed the highest ${\mu}TBS$ in the self-cured group. In the light-cured group, no significant improvements were observed according to the dentin surface pretreatment. There were no significant differences between curing modes. Both dentin surface pretreatment methods helped to increase the ${\mu}TBS$ of RelyX U200 resin cement significantly and degree of pretreatment effect was similar. No significant differences were found regarding curing modes except control groups. In the comparisons of two self-adhesive resin cements, all groups within the same pretreatment and curing mode were significantly different excluding self-cured control groups. CONCLUSION. Selecting RelyX U200 used in this study and application of dentin surface pretreatment with EDTA and polyacrylic acid might be recommended to enhance the bond strength of cement to dentin.
