Objective : The purpose of this study is to review our experience with spontaneous thalamic hemorrhage. Clinical outcome of patients was brought about by comparing stereotactic aspiration and conservative medical therapy. Methods : The study consists of seventy-three cases with spontaneous thalamic hemorrhage which were treated from the period of Jan. 1993 to Dec. 1999. Thirty-eighty patients were treated with computed tomography (CT) guided stereotactic aspiration and thirty-five patients were treated conservatively. We compared the factors affecting treatment and the factors are as follows : age & sex, conscious level on admission, hematoma volume, hematoma sites, presence of ventricular penetration. Results : The results in the thirty eight stereotactic aspirated cases for the 6 months from oneset are as follows : good recovery or moderate disability in 43%, severe disability in 32%, vegitative state in 11%, dead in 13% respectively. The clinical result was more favorable in stereotactic aspiration, with 11-30cc hematoma volume, extend to internal capsule of hematoma, poor conscious level on admission than conservative medical therapy. But age & sex, conscious level on admission, presence of ventricular penetration were not influential in the statistical outcome between stereotactic aspiration and conservative medical therapy. Conclusion : Treatment modality of spontaneous thalamic hemorrhage is still controversial. But stereotactic aspiration is more recommended for improvement therapeutic results than conservative treatment or open craniotomy in case of 11-30cc hematoma volume, extend to internal capsule of hematoma and poor conscious level on admission.
X-linked hypophosphatemia (XLH) is a hereditary metabolic disease caused by the loss of phosphate through the renal tubules into the urine, and an associated decrease in serum calcium and potassium phosphate. Its dental features include spontaneous dental abscesses that occur in the absence of trauma or dental caries. The aim of this case report was to describe the dental problems of XLH patients and to evaluate limitations in their treatment. A 14 year old male and a 38 year old female with XLH were referred to the Department of Conservative Dentistry for endodontic treatment. The dental findings were periapical abscesses without obvious trauma or caries. Conservative endodontic treatment was performed in teeth with pulp necrosis and abscess. In case 1, the treated teeth showed improvements in bone healing, without clinical symptoms. However, in case 2, the implants and the treated tooth showed hypermobility, and the final restoration was therefore postponed. Early diagnosis, periodic examinations, and communication with the patient's pediatrician are important in the dental management of patients with XLH.
Objectives: The purpose of the present study was to evaluate the effects of proanthocyanidin (PAC), a crosslinking agent, on the physical properties of a collagen hydrogel and the behavior of human periodontal ligament cells (hPDLCs) cultured in the scaffold. Materials and Methods: Viability of hPDLCs treated with PAC was measured using the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay. The physical properties of PAC treated collagen hydrogel scaffold were evaluated by the measurement of setting time, surface roughness, and differential scanning calorimetry (DSC). The behavior of the hPDLCs in the collagen scaffold was evaluated by cell morphology observation and cell numbers counting. Results: The setting time of the collagen scaffold was shortened in the presence of PAC (p < 0.05). The surface roughness of the PAC-treated collagen was higher compared to the untreated control group (p < 0.05). The thermogram of the crosslinked collagen exhibited a higher endothermic peak compared to the uncrosslinked one. Cells in the PAC-treated collagen were observed to attach in closer proximity to one another with more cytoplasmic extensions compared to cells in the untreated control group. The number of cells cultured in the PAC-treated collagen scaffolds was significantly increased compared to the untreated control (p < 0.05). Conclusions: Our results showed that PAC enhanced the physical properties of the collagen scaffold. Furthermore, the proliferation of hPDLCs cultured in the collagen scaffold crosslinked with PAC was facilitated. Conclusively, the application of PAC to the collagen scaffold may be beneficial for engineering-based periodontal ligament regeneration in delayed replantation.
In order to determine if GK-101E(N-monochloro-DL-2-aminobutyrate) solution was effective in the removal of carious dentin, this study was conducted on 75 extracted human carious teeth. GK-101E solution was compared in effectiveness to saline solution.75 extracted human carious teeth were randomly assigned to two groups accordingly : (1) 50 teeth were treated with GK-101E solution : and (2) 25 teeth were treated with saline solution. Teeth in each group were divided into the medium or medium-hard consistency. Treatment consisted of directing the pulsating stream of each coded test solution utilizing the Caridex$^{TM}$ Caries Removal System against the carious surface for 4 mimutes. One operator treated all teeth with corded solution, and four investigators scored on the treated teeth based on visual examination and tactile evaluation with a dental explorer in conjunction with staining method with Caries Detector$^{TM}$ The results were as follows : 1. All groups showed various effect of carious dentin removal. 2. GK-101E solution was statistically superior to physiologic saline solution in the removal of carious dentin, regardless of carious consistency. 3. GK-101E solution in the medium-hard consistency was statistically superior to that in the medium consistency in the caries removal(p=0.001). 4. Saline solution in the medium consistency was equivalent to that in the medium-hard consistency in caries removal(p>0.1).
Background : This study was conducted to know the effect of resistance exercise using thera-band in parallel with conservative therapy in patients with frozen shoulder. Methods : We analyze subjective pain scale, ROM and shoulder functional scale 24 patients diagnosed with adhesive capsulitis were included for study. Among them, 12 patients(experiment group) were managed by resistance exercise as well as conservative physical therapy, another 12 patients(control group) were only treated by conservative therapy alone. All the patients were treated three times a week for 4 weeks, and after, and after each treatment subjective pain scale, ROM, shoulder functional score(SFI) were measured. Results : The VAS were significant reduced between experimental group and control group according to pre-treatment and post-treatment. In experimental group, the ROM and SFI has increased than control group according to pre-treatment and post-treatment. Conclusion : According to the results, we concluded that both conservative therapy and self resistance exercise using thera-band technique are more effective for increasing ROM especially flexion, abduction, internal rotation and SFI than conservative therapy alone.
Background and Objectives : Traditionally incision and drainage is considered to be standard treatment of deep neck infection. But antibiotics and diagnostic technique are developed recently, there are reports that conservative therapy could be as successful as open surgical drainage. The purposes of this study has been to assess clinical feature between surgical therapy group and conservative therapy group through statistical analysis. Materials and Methods : A retrospective study was performed on 46 cases of deep neck space abscess, which were confirmed CT, in patients admitted from January 1999 to June 2002. Result : About 80% of all are treated with conservative therapy. Erythrocyte sediment rate, volume of abscess and duration of hospitalization of conservative therapy group are decreased than those of surgical therapy group. Conclusions : Conservative therapy is expected to be effective on treatment of early stage, small sized deep neck infection. But its complication can lead to serious condition of patient it should be done under meticulous observation.
The fractures of lateral end of clavicle can be treated by conservative or operative treatment, but many authors report the higher rate of non-union with conservative treatment and that the operative treatment is more effective for the type II fractures. The authors reviewed and analysed 15 cases of lateral end fracture of clavicle which had been treated at Department of Orthopedic Surgery, Kyungpook National University Hospital from 1991 to 1996. The results were as follows; 1. Among the 15 patients, male was 6,female 9 and the average age was 46.2 years ranged from 21 to 72 years. 2. According to the classification by Rockwood, type II a was 8 cases and II b was 7 cases. 3. We treated operatively 12 cases, 9 cases with tension band wiring and 3 cases with intramedul-Iary K-wire fixation. And we treated conservatively 3 cases with Velpeau cast, but I case of nonunion was treated with tension band wiring. 4. The average follow-up was 3.2 years ranged from 2.1 to 4.7 years. 5. The functional results were evaluated with Weitzman's classification. 6. In the operatively treated cases, there were 4 cases(30.7%) in excellent, 7 cases(53.8%) in good and 2 cases(15.3%) in fair result. But, in the conservatively treated cases, there were 2 cases in good and I case in poor result, and the poor case did not united and had open reduction and internal fixation. 7. In conclusion, the operative treatment revealed good functional results in most cases (10/12) and early open reduction and internal fixation was better method than conservative treatment.
This study was performed to evaluate and compare conservative treatment results by several parameters such as age, sex, symptom duration, type and timing of joint sound, parafunctional habits, splint type, and diagnostic classification. There have been too many articles reporting long term results of conservative treatment but articles related to comparison of treatment results by patients' self-evaluation have been rarely reported. For this study 258 patients with temporomandibular disorders(TMDs) were selected and examined by routine diagnostic procedure for TMDs. The subjects were classified into 5 TMDs subgroups ad treated with conservative treatments involving splints, physical modalities, jaw exercises, and counseling. Visual analogue scale(VAS) about pain, joint sound, and mouth opening limitation was recorded respectively during treatment period. From the VAS data and treatment duration, VAS treatment index(VAS Ti) was calculated. The obtained results were as follows : 1. Pain was the most frequent main symptom in subjects with temporomandibular disorders, and main symptom for mouth opening limitation was comparatively less than for pain or sound in disk displacement with reduction group or in degenerative joint disease group. 2. Degenerative joint disease group had the most poor treatment results and highest occlusal index of Helkim's index. 3. Good prognosis for conservative treatment was observed in acute group, under 6 months than chronic group, 6months over in symptom duration, and subjects with 40 years over in age showed the most poor prognosis. 4. Subjects treated with anterior repositioning splint had better treatment results than subjects treated with centric relation splint, but statistical significance in VAS Ti and treatment duration was not observed. 5. Treatment results according to affected side, types and point of joint sound did not show consistent statistical results. 6. The result for conservative treatment was observed poor in subjects with bruxism and clenching. 7. In studying coincidence between preferred chewing and affected side, frequency of preferred chewing side, in unilateral affection, was higher in ipsilateral than in contralateral side.
Objectives : The purpose of this study is to investigate the efficacy of conservative treatment on Achondroplasia patient who has low back pain with radiation. Methods : The patient was treated by conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. Numeric rating scale(NRS), Oswestry disability index(ODI) were used to evaluate pain reduction. Physical examination also used to compare with admission state and discharge state. Results : After 1 month admission treatment, NRS decreased 9 to 6, ODI dcreased 56 to 42, ROM improved as normal person. Conclusions : It was efficient that Achondroplasia patient who has low back pain with radiation treated as conservative treatment including acupuncture, pharmaco acupuncture, chu-na manipulation, herbal medicine. But It was just one case and treatments were carried out in Integrative Package. So further studies are required to compare that which treatment is efficient.
Successful treatment of a badly broken down tooth with pulpal disease depends not only on good endodontic therapy, but also on good prosthetic reconstruction of the tooth after endodontic therapy is completed. The ideal treatment of endodontically treated teeth has been widely and controversially discussed. Endocrown is a restorative option for endodontically treated teeth. Endocrown design incorporates the core and short post into the crown as a single restoration. The preparation of endocrown consists of a circular equigingival butt-joint margin and central retention cavity of the entire pulp chamber instead of employing intraradicular posts. This design significantly increases the surface area of the preparation available for cementation. It is particularly useful in young patient teeth for long-term provisional restoration and in teeth with short clinical crowns. This technique represents a promising and conservative method for the treatment of endodontically treated teeth that require long-term protection and stability. Endocrown can be considered as a feasible alternative to full crowns or composite overlays for the restoration of non vital teeth.
