하악 제3대구치와 하악관과의 위치관계에 대한 파노라마 방사선사진과 cone beam형 전산화단층촬영상의 비교 (Comparison of cone beam computed tomography and conventional panoramic radiography in assessing the topographic relationship between the mandibular canal and impacted third molars)

  • Purpose : To assess the diagnostic accuracy and value in an imaging technique field through the comparison of cone beam computed tomography and conventional panoramic radiography in assessing the topographic relationship between the mandibular canal and impacted third molars. Materials and Methods : Participants consisted of 100 patients offered the images through cone beam computed tomography and panoramic radiography. PSR-$9000^{TM}$ Dental CT system (Asahi Roentgen Ind. Co., Ltd, Japan) was used as the unit of cone beam computed tomography. CE-II (Asahi Roentgen Ind. Co., Ltd, Japan) and Pro Max (Planmeca Oy, Finland) were used as the unit of panoramic radiography. The images obtained through panoramic radiography were classified into 3 types according to the distance between mandibular canal and root of mandibular third molar. And they were classified into 4 types according to the proximity of radiographic feature. The images obtained through cone beam computed tomography based on the classification above were classified into 4 types according to the location between the mandibular canal and the root and were analyzed. And they were classified into buccal, inferior, lingual, and between roots, according to the location between mandibular canal and root. The data were statistically analyzed and estimated by $X^2$-test. Results : 1. There was no statistical significance according to 3 types (type I, type II, type III) through CBCT. 2. The results of 4 types (type A, type B, type C, type D) through CBCT were as high prevalence of CBCT 1 in type A, CBCT 2 in type B, CBCT 3 in type C, and CBCT1 in type D and those of which showed statistical significance (P value=0.03). 3. The results according to location between mandibular canal and root through CBCT recorded each 49, 25, 17, 9 as buccal, inferior, lingual, between roots. Conclusion : When estimating the mandibular canal and the roots through the panoramic radiography, it could be difficult to drive the views of which this estimation was considerable. Thus it is required to have an accurate diagnostic approaching through CBCT that could estimate the location between mandibular canal and roots.

Cone-beam computed tomography를 이용한 미맹출 영구치의 계측 (ACCURACY OF CONE-BEAM COMPUTED TOMOGRAPHY IN PREDICTING THE DIAMETER OF UNERUPTED TEETH)

  • 본 연구는 CBCT(Cone-beam computed tomography)영상에서 미맹출 치아 크기 측정의 재현성과 정확성을 평가하기 위해 시행되었다. 매복치를 주소로 부산대학교 치과병원 소아치과에 내원한 혼합치열기 환자 중 진단 목적으로 CBCT 채득에 동의한 환자의 미맹출 견치 및 소구치 69개를 대상으로 하였다. CBCT 영상에서 측정한 미맹출 치아의 최대 근원심 폭경 계측치와 동일한 치아가 구강 내로 완전히 맹출한 후 채득한 석고 모형에서 대상 치아를 digital caliper로 측정한 최대 근원심 폭경 계측치를 비교하여 다음과 같은 결론을 얻었다. 1. CBCT 영상에서 미맹출 치아를 계측하는 방법은 재현성이 높다(ICC=0.91). 2. CBCT 영상에서 측정한 미맹출 치아의 최대 근원심 폭경 계측치와 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치는 높은 상관 관계가 있었다(r=0.91). 3. CBCT 영상에서 측정한 미맹출 치아 최대 근원심 폭경 계측치와 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치는 통계적으로 유의한 차이가 있었다($p$ <0.05). CBCT 영상에서 측정한 미맹출 치아 최대 근원심 폭경의 계측치가 석고 모형에서 측정한 대상 치아의 최대 근원심 폭경 계측치보다 평균 0.2 mm 작게 측정되었다. 그러나 이러한 차이는 임상적으로 수용 가능한 수준이라 생각된다.

Very deep super-resolution for efficient cone-beam computed tomographic image restoration

  • Purpose: As cone-beam computed tomography (CBCT) has become the most widely used 3-dimensional (3D) imaging modality in the dental field, storage space and costs for large-capacity data have become an important issue. Therefore, if 3D data can be stored at a clinically acceptable compression rate, the burden in terms of storage space and cost can be reduced and data can be managed more efficiently. In this study, a deep learning network for super-resolution was tested to restore compressed virtual CBCT images. Materials and Methods: Virtual CBCT image data were created with a publicly available online dataset (CQ500) of multidetector computed tomography images using CBCT reconstruction software (TIGRE). A very deep super-resolution (VDSR) network was trained to restore high-resolution virtual CBCT images from the low-resolution virtual CBCT images. Results: The images reconstructed by VDSR showed better image quality than bicubic interpolation in restored images at various scale ratios. The highest scale ratio with clinically acceptable reconstruction accuracy using VDSR was 2.1. Conclusion: VDSR showed promising restoration accuracy in this study. In the future, it will be necessary to experiment with new deep learning algorithms and large-scale data for clinical application of this technology.

Cone beam computed tomography로 합성된 두부규격 방사선사진에서의 각 계측점의 정확도와 재현성에 관한 연구 (Accuracy and reproducibility of landmark of cone beam computed tomography (CT) synthesized cephalograms)

  • Introduction: Cone beam computed tomography (CBCT) has various advantages and is used favorably in many fields in dentistry. Especially, CBCT is being used as basic diagnostic tool for 3-dimensional analysis in orthognathic patient. Two-dimensional cephalograms can be synthesized from CBCT digital imaging and communications in medicine (DICOM) data. In this study, conventional cephalograms and CBCT were taken simultaneously, and representative landmarks were located and analyzed in its accuracy and reproducibility. Materials and Methods: Ten patients who had orthognathic surgery in Wonkwang University Daejeon Dental Hospital participated in this study. For each patient, CBCT and conventional cephalogram was taken. By using Ondemand (Cybermad, Korea), 2-dimensional cephalograms was established on CBCT. In addition, 19 landmarks were designated and measured by 3 orthodontists twice a week. After these landmarks were transferred to a coordinate, distance of landmark and axis, standard error, distribution degree were measured, compared and analyzed. Results: Comparing the CT ceph group and conventional cephalogram group, CT ceph group had shown shorter distance of landmark and axis in S, Hinge axis, Bpt, Ba, Or, Corpus left. Standard error of the mean shows that CT ceph group has better reproducibility in Or, Corpus left, Hinge axis at X axis and Na, U1R, U1T, Bpt, PNS, Ba Corpus left, Hinge axis at Y axis. In both groups, mean error was less than 1.00 mm, no significant difference were found between CT ceph group and conventional cephalogram group in all measurements. Furthermore, comparing two groups, each 17 landmarks out of 19 had its characteristic in distribution degree. Conclusion: No significant difference were found between CBCT composed cephalographic radiograph and conventional cephalograghic radiograph, clinical application may be possible if improved.

Effect of field-of-view size on gray values derived from cone-beam computed tomography compared with the Hounsfield unit values from multidetector computed tomography scans

  • Purpose: This study aimed to evaluate the effect of field-of-view (FOV) size on the gray values derived from cone-beam computed tomography (CBCT) compared with the Hounsfield unit values from multidetector computed tomography (MDCT) scans as the gold standard. Materials and Methods: A radiographic phantom was designed with 4 acrylic cylinders. One cylinder was filled with distilled water, and the other 3 were filled with 3 types of bone substitute: namely, Nanobone, Cenobone, and Cerabone. The phantom was scanned with 2 CBCT systems using 2 different FOV sizes, and 1 MDCT system was used as the gold standard. The mean gray values(MGVs) of each cylinder were calculated in each imaging protocol. Results: In both CBCT systems, significant differences were noted in the MGVs of all materials between the 2 FOV sizes(P<.05) except for Cerabone in the Cranex3D system. Significant differences were found in the MGVs of each material compared with the others in both FOV sizes for each CBCT system. No significant difference was seen between the Cranex3D CBCT system and the MDCT system in the MGVs of bone substitutes on images obtained with a small FOV. Conclusion: The size of the FOV significantly changed the MGVs of all bone substitutes, except for Cerabone in the Cranex3D system. Both CBCT systems had the ability to distinguish the 3 types of bone substitutes based on a comparison of their MGVs. The Cranex3D CBCT system used with a small FOV had a significant correlation with MDCT results.

  • Purpose: To determine the physical properties of a newly developed cone beam computed tomography (CBCT). Materials and Methods: We measured and compared the imaging properties for the indirect-type flat panel detector (FPD) of a new CBCT and the single detector array (SDA) of conventional helical CT (CHCT). Results: First, the modulation transfer function (MTF) of the CBCT were superior to those of the CHCT. Second, the noise power spectrum (NPS) of the CBCT were worse than those of the CHCT. Third, detective quantum efficiency (DQE) of the indirect-type CBCT were worse than those of the CHCT at lower spatial frequencies, but were better at higher spatial frequencies. Although the comparison of contrast-to-noise ratio (CNR) was estimated in the limited range of tube current, CNR of CBCT were worse than those of CHCT. Conclusion: This study shows that the indirect-type FPD system may be useful as a CBCT detector because of high resolution.

소아환자에서 교정분석을 위한 콘빔CT 촬영시 유효선량에 관한 연구 (Effective dose of cone-beam computed tomography for orthodontic analysis in pediatric patient)

  • Objective: The objectives of this study were to measure pediatric organ and effective doses of cone-beam computed tomography (CBCT) for orthodontic analysis and to compare them to those of panoramic and lateral cephalometric radiography, the conventional radiography for orthodontic analysis. Materials and Methods: Alphard VEGA for CBCT, Planmeca Proline XC for panoramic radiography and Orthophos CD for cephalometric radiography were used for this study. Thermoluminescent dosimeter (TLD) chips were located at 24 anatomic sites of 10-year-old anthropomorphic phantom and exposed during CBCT (C-mode; $200{\times}179mm$ FOV), panoramic and lateral cephalometric radiographic procedures at the clinical exposure settings for 10-year-old patient. Pediatric organ and effective doses were measured and calculated using ICRP 2007 tissue weighting factors. Results: Effective doses of CBCT, panoramic radiography and lateral cephlometric radiography in pediatric clinical exposure settings were $292.5{\mu}Sv$, $19.3{\mu}Sv$, and $4.4{\mu}Sv$ respectively. The thyroid gland contributed most significantly to the effective dose in all the radiographic procedures. Conclusion: Effective dose of CBCT was about 12 times to conventional radiographic procedures for orthodontic analysis in pediatric patient. The use of CBCT for orthodontic analysis should be fully justified over conventional radiography and dose optimization to decrease thyroid dose is needed in pediatric patients.

Correlation between gray values in cone-beam computed tomography and histomorphometric analysis

  • Purpose: The aim of this study was to analyze the relationships between bone density measurements obtained using cone-beam computed tomography (CBCT) and morphometric parameters of bone determined by histomorphometric analysis. Materials and Methods: In this in vivo study, 30 samples from the maxillary bones of 7 sheep were acquired using a trephine. The bone samples were returned to their original sites, and the sheep heads were imaged using CBCT. On the CBCT images, gray values were calculated. In the histomorphometric analysis, the total bone volume, the trabecular bone volume (referred to simply as bone volume), and the trabecular thickness were assessed. Results: Statistical testing showed significant correlations between CBCT gray values and total bone volume (r =0.537, P =0.002), bone volume (r =0.672, P<0.001), and trabecular thickness (r =0.692, P<0.001), as determined via the histomorphometric analysis. Conclusion: The results indicate a significant and acceptable association between CBCT gray values and bone volume, suggesting that CBCT may be used in bone densitometry.

Detection of root perforations using conventional and digital intraoral radiography, multidetector computed tomography and cone beam computed tomography

  • Objectives: This study aimed to compare the accuracy of conventional intraoral (CI) radiography, photostimulable phosphor (PSP) radiography, cone beam computed tomography (CBCT) and multidetector computed tomography (MDCT) for detection of strip and root perforations in endodontically treated teeth. Materials and Methods: Mesial and distal roots of 72 recently extracted molar were endodontically prepared. Perforations were created in 0.2, 0.3, or 0.4 mm diameter around the furcation of 48 roots (strip perforation) and at the external surface of 48 roots (root perforation); 48 roots were not perforated (control group). After root obturation, intraoral radiography, CBCT and MDCT were taken. Discontinuity in the root structure was interpreted as perforation. Two observers examined the images. Data were analyzed using Stata software and Chi-square test. Results: The sensitivity and specificity of CI, PSP, CBCT and MDCT in detection of strip perforations were 81.25% and 93.75%, 85.42% and 91.67%, 97.92% and 85.42%, and 72.92% and 87.50%, respectively. For diagnosis of root perforation, the sensitivity and specificity were 87.50% and 93.75%, 89.58% and 91.67%, 97.92% and 85.42%, and 81.25% and 87.50%, respectively. For detection of strip perforation, the difference between CBCT and all other methods including CI, PSP and MDCT was significant (p < 0.05). For detection of root perforation, only the difference between CBCT and MDCT was significant, and for all the other methods no statistically significant difference was observed. Conclusions: If it is not possible to diagnose the root perforations by periapical radiographs, CBCT is the best radiographic technique while MDCT is not recommended.

  • The most important part of everyday root canal treatment is diagnosis about the morphology of tooth, root and root canal. Usually this procedure is performed by visual examination and radiographic (panoramic/periapical) examination. However, 2-dimentional radiography has several limitations such as imposition of anatomic structures including buccal/lingual root canals and distortion of images. Recently, owing to the increased interest in dental implant and affordable cost of CBCT equipment, CBCT has been introduced widely in local dental clinics. CBCT is characterized by their lower radiation dose and shorter exposure time than conventional CT scan, and ability of 3-dimentional reconstruction of the dento-alveolar structure. Also in endodontic field, the data from CBCT could be very helpful in diagnosing complex root canal anatomy, apical periodontitis, cause of failure and in determining treatment plan. However, there are some limitations such as radiation dose and artifact. Therefore, clinicians should know about indication, advantages and limitations of CBCT, and properly use it for successful root canal treatment to save the natural teeth.
