Northeast Asian Energy Corridor Initiative for Regional Collaboration

  • For historical and political reasons, South Korea (hereafter Korea), Japan and China have not achieved much progress in regional energy cooperation for decades. However, the rising importance of Northeast Asia (NEA) in the world energy sphere, especially in the global oil market, is providing an opportunity to create an integrated oil market in the region. This study suggests the Northeast Asian Energy Corridor (NEAEC) Initiative as an effective conduit for raising the possibility of the Northeast Asian oil hub project. The NEAEC Initiative combines the model of Europe's Amsterdam-Rotterdam-Antwerp (ARA) with Singapore's AsiaClear as a form of financial collaboration. The study suggests that an electronically integrated Over-the-Counter (OTC) market clearing mechanism accompanied by other key financial instruments among Korea, Japan and China can be an effective means for promoting financial collaboration in the region.

  • The selection of an appropriate surgical technique for repair of aneurysm of the ascending aortia with aortic insufficiency is unsettled. The etiology of the disease process has been the best indicator for the type of repair. Placement of a supracoronary graft[seperate graft and valve] is a compromise if the coronary ostia are displaced cephalad by the aneurysm, where as insertion of a valved conduit is difficult and unnecessary if the coronary ostia are normally placed. A 53 year old female patient underwent primary repaiar of proximal dissected layer and aortic valve replacement with 24mm carbomedics, The operative findings consisted of a supravalvular intimal tear, cicumferential dissection, dilated aortic annulus and normal position of coronary ostia. She is good physical activity now llmonths posoperatively.

A review of the total correction of tetralogy of Fallot (Fallot 4징증의 근치수술에 대한 임상적 고찰)

  • The experience with operative treatment for total correction of Tetralogy of Fallot at the department of Thoracic and Cardiovascular Surgery, Korea University Hospital from January, 1977, through April, 1983 was reviewed. Of the 29 patients reviewed, male to female occurrence ratio was 22:7 Type of V.S.D. was type II in 26 cases[90%] and total conus defect in 3 cases[10%]. Average size of V.S.D. was 19 mm. Type of Right ventricular outflow tract stenosis was highest frequency with pulmonary valvular and infundibular stenosis combined type in 21 cases[72%], and there were 8 deaths In this group. Type of R.V.O.T. reconstruction contains 2 cases of infundibulectomy only, 1 case of infundibulectomy with valvular commissurotomy, 2 cases of pericardial patch, 23 Cases of pericardial patch with Teflon or Dacron felt reinforced and 1 case of pulmonary valved conduit reconstruction. Operative mortality was higher in outflow patch through pulmonary valve ring. Overall mortality was 31%. Major causes of death and postoperative complications were low output syndrome, complete A-V block acute renal failure, ventricular fibrillation, bleeding brain abscess, and sudden cardiac arrest.

  • Whatever a surgeon choose aortic prosthesis in aortic stenosis, it will always provoke some degree of obstruction to flow due to its smaller effective orifice area to tissue annulus. Occasionally, we meet small aortic annulus to his or her body surface area in aortic valve replacement. The small annulus remains a problem in that both tissue and mechanical prosthesis have significant pressure gradients between LV and aorta in resting or exercising states. In these circumstances, diverse surgical procedures, such as tilting disc prosthesis, supraannular position of aortic prosthesis, and enlargement of aortic root [including aortoventriculoplasty, translocation of aortic valve, & double outlet of LV by valved conduit], were applied. We experienced one case of aortic stenosis with small aortic annulus. Systolic pressure gradients between LV & aorta were 90 mmHg. Diameter of annulus was 19 mm. So we performed patch enlargement of aortic root by Manouguian and AVR with St. Jude medical valve 23 mm.

  • This is a genuine case report of the Ross operation without the use of homografts or heterografts in reconstruction of the right ventricular outflow tract. A 8-year-old boy with congenital aortic stenosis underwent aortic valve replacement with a pulmonary autograft and right ventricular outflow tract reconstruction with a pericardial conduit bearing autologous aortic monocusp. The postoperative echocardiography and cardiac angiography revealed good ventricular function and competent neoaortic valve. He has been followed up for 19 months.

  • The use of the patient`s pulmonary valve for replacement of the patient`s diseased aortic valve was introduced and developed by Mr. Donald Ross. The long term benefits of having a normal, fully viable, trileaflet semilunar valve in aortic position was demonstrated. A 38 year old male had histories of failures of previously implanted aortic prosthetic valves twice and evidence of progressive heart failure. At operation, aortic root abscess was found; the abscess extension to adjacent structures and partial valve dehiscence had occurred. The patient underwent replacement of the aortic root with autologous pulmonary valve, autologous pericardial patch repair of left ventricuar outflow tract and recontruction of the right ventricular outflow tract and pulmonary artery with prosthetic valved conduit. Postoperatively, the patient recovered well. Postoperative doppler echocardiography demonstrated minimal central regurgitation in new aortic valve. Aortic root replacement with pulmonary autograft in a patient of recurrent aortic root abscess and false aneurysm of left ventricuar outflow tract was experienced and reported with follow up echocardiography.

Conduits for Coronary Bypass: Vein Grafts

  • The saphenous vein has been the principal conduit for coronary bypass grafting from the beginning, circa 1970. This report briefly traces this history and concomitantly presents one surgeons experience and personal views on use of the vein graft. As such it is not exhaustive but meant to be practical with a modest number of references. The focus is that of providing guidance and perspective which may be at variance with that of others and recognizing that there may be many ways to accomplish the task at hand. Hopefully the surgeon in training/early career may find this instructive on the journey to surgical maturity.

  • The first successful transthoracic esophagectomy and intrathoracic esophago- gastric anastomosis reported in 1938. Stomach, small intestine, and colon as well as free revascularized grafts have been substituted for excised esophagus. During the past 60 years, there have been substantial advances in preoperative assessment, nutritional support, anesthetic and operative techniques, and postoperative care of patients undergoing esophageal resection and reconstruction. However the hospital mortality and morbidity of esophageal resection and reconstruction is still high and disruption of an intrathoracic esophagogastric anastomosis continues to be the most dreaded complication of esophageal surgery, And the choice of the conduit is still controversial. In this paper, I would like to review the current surgical options available to patients who require esophageal resection and reconstruction as well as the advantages and disadvantages of each technique.

핵융합로용 초전도 전자석 구조재의 파괴역학적 특성에 관한 연구

  • 국제 열 핵융합로 (International Thermonuclear Experimental Reactor) 의 중앙 솔레노이드(solenoid) 초전도 전자석의 피복관 (conduit) 으로서 가장 유력한 후보 재료인 니켈-철 기저 초합금에 대한 개선된 파괴역학적 거동 예측 모형을 개발하기 위하여 피로균열 성장과 파괴 인성 치의 측정이 사용되었다. 유한 요소법을 사용하는 상용 구조해석 코드인 ANSYS 제 5.2판에 의해 초전도 전자석 피복관 재료에 대한 탄소성 파괴역학적 거동을 살펴보았다. 정확한 파괴 기준을 개발하기 위하여 삼차원 J 적분 인자에 의한 결과를 사용하였다. 얇은 피복관재의 경우에 절대온도 4도에서의 적합한 파괴 인성치는 실제 단면효과를 고려한 표면균열 인장시험 결과를 토대로 J 적분으로 도출한 150 MPaㆍm$^{1}$2/ 로 제시되었다.

  • 본 논문에서는 안전한 공장 원격제어를 위한 시스템 프레임 워크를 제안한다. 공장기계와 IT의 융합으로 기술 발전함에 따라 공장 제어 시스템을 공장내부에서는 무선 제어시스템, 공장 외부에서는 원격 시스템의 필요성이 대두되어 많은 연구들이 진행되고 있다. 기존의 공장 자동화 시스템 SCADA(Supervisory Control And Data Acquisition)는 생산 공정 및 플랜트의 상태를 감시하고 제어하기 위한 목적으로 개발되었지만 원격제어 관련 내용이 부족하다. 공장 내부 보안을 위한 표준으로 ISA99 Security Standards에서도 통합 공정 제어 Zone & Conduits 방식 제시 공장의 각 영역을 Zone으로 나누고 허가된 사용자만 Conduit를 통해 다른 Zone의 노드와 통신을 하는 방식을 제안하였다. 하지만, 전체적인 프레임워크는 정의를 하고 있으나 외부 원격 제어 내용 부족하다. 따라서 본 논문에서는 스마트폰을 활용한 공장 외부에서의 안전한 원격 제어를 제공하는 통합관제시스템 프레임워크 구조를 제시하여 향후 관련 기술에 대한 기준점을 제시한다.
