Correct localization of epileptogenic zone is important for the successful epilepsy surgery. Both ictal perfusion single photon emission computed tomography (SPECT) and interictal F-18 fluorodeoxyglucose positron emission tomography (FDG-PET) can provide useful information in the presurgical localization of intractable partial epilepsy. These imaging modalities have excellent diagnostic sensitivity in medial temporal lobe epilepsy and provide good presurgical information in neocortical epilepsy. Also provide functional information about cellular functions to better understand the neurobiology of epilepsy and to better define the ictal onset zone, symptomatogenic zone, propagation pathways, functional deficit zone and surround inhibition zones. Multimodality imaging and developments in analysis methods of ictal perfusion SPECT and new PET ligand other than FDG help to better define the localization.
초급성 허혈성 뇌졸중 환자를 대상으로 관류 전산화단층촬영(CT)의 임상적 적용을 평가하였다. 뇌졸중 증상 발현 62명의 환자를 대상으로 하였고, 관류 CT는 소뇌 기저부위에서 8 cm 상방으로 스캔하여 후처리 과정을 거쳐 뇌혈용적(cerebral blood volume, CBV), 뇌혈류량(cerebral blood flow, CBF), 평균 조영제 통과시간(mean transit time, MTT) 및 조영제 최고 도달시간(time to peak, TTP) 등의 네 가지 지도의 영상을 얻었다. 관류 CT의 CBV, CBF, MTT, TTP 지도에서 병변을 평가하였으며, 병변 부위와 정상측 대칭부위에서 MTT와 TTP를 측정하여 차이를 비교하였다. 관류 CT의 네 가지 지도 모두에서 관류결손을 인지할 수 있었고, 관류 결손이 인지되는 부위에서 MTT와 TTP의 현저한 지연이 있었다. 관류 CT의 MTT와 TTP영상이 초급성 허혈성 경계부위의 페넘브라를 반영하였다. 관류 CT의 네 가지 지도를 이용하면 뇌졸중의 조기 진단, 허혈 중심부, 허혈 페넘브라를 알 수 있게 되어 관류결손 부위의 혈류역동학적 평가가 가능함으로써, 허혈성 뇌졸중 환자의 진단 및 효과적인 치료를 위해 관류 CT가 유용하여 임상적 적용이 가능하다.
Myocardial perfusion imaging has been increasingly used to provide prognostic data and guidance on the choice of appropriate management of patients with known or suspected coronary artery disease. The electrocardiogram gated myocardial SPECT program is corning into wide use with an advent of $^{99m}Tc-labeled$ tracers and an improvement of SPECT machines. The gated technique permits measurement of important cardiac prognostic indicators without any further discomforts or radiation burden in patients underwent standard myocardial perfusion SPECT. In addition, gated study significantly improves diagnostic yield by reducing the number of borderline interpretations and could find myocardial stunning and viable myocardium. Gated single photon emission computed tomography (SPECT) imaging allows the automated calculation of end-diastolic volume, end-systolic volume, ejection fraction, myocardial mass and the assessment of regional wall motion and thickening, and it have dramatically improved assessment of coronary artery disease in routine nuclear practice. This allows the simultaneous assessment of both perfusion and function within the same acquisition, and serves as a cost-effective technique for providing more diagnostic data with fewer diagnostic tests. Because the diagnostic and prognostic power derived from knowledge of left ventricular function can be added to that provided by assessing myocardial perfusion, gated SPECT imaging has rapidly gained widespread acceptance and is now used on a routine clinical basis in a growing number of laboratories, including South Korea. The gated SPECT technique for measurement of left ventricular parameters has been validated against a variety of well established techniques. In this work, overview of gated myocardial perfusion SPECT focus on functional parameters is presented.
Patterns of abnormality in regional cerebral perfusion and its relation to clinical severity was evaluated with 32 head injury patients using $^{99m}Tc-HMPAO$ single photon emission tomography (SPECT). The findings were compared with computed tomography (CT) done within 48 hours of each SPECT study. The initial SPECT study was done within 7 days of injury in 16 cases, between 1 week and 2 months in 12, and after over 2 months in 4. Nineteen of the patients underwent followup SPECT and CT after a mean interval of 1 to 2 months. The initial SPECT showed abnormalities in 96% (31/32) of the patients while CT showed abnormal findings in only 81% (26/32). There were a total of 54 supratentorial SPECT lesions in all. Ninity percent (49/54) of these were of regional hypoperfusion, while 5 lesions showed focal hyperperfusion. The lesions were most often localized in the frontal and temporal lobes. Fifty five percent (30/54) were areas not detected as a lesion on CT. Cerebellar diaschisis was observed in 50% (16/32) of the patients. The degree of perfusion abnormality was quantified by the product of differential activity and a size factor. Correlation between the degree of perfusion abnormality and the clinical severity (Glasgow coma scale) failed to show statistical significance (p=0.053). The amount of change in the degree of perfusion abnormality on follow up SPECT was compared to the amount of change in clinical severity. Perfusion abnormality showed a tendancy to improve in most patients, and the degree of improvement showed significant correlation with the amount of clinical improvement (p < 0.01).
Recent progress of technology permits us to assess ventricular function and wall motion as well as myocardial perfusion using electrocardiographic gated myocardial perfusion single photon emission computed tomography (GM-SPECT). It is interesting that echocardiography and magnetic resonance imaging are moving in the same direction with the use of contrast medium to assess myocardial perfusion. A valid fundamental basis for a new technology is essential for a successful competition. Lee et al. report in this issue the reproducibility of serial measurement of left ventricular function including systolic wall thickening using a novel statistical method. It has important implications such as nitroglycerin or dobutamine application during GM-SPECT. The field of nuclear cardiology must continue to strive toward more sophisticated but straightforward evaluation of cardiac diseases.

Coronary computed tomography angiography (CCTA) is routinely used for anatomical assessment of coronary artery disease (CAD). However, invasive measurement of fractional flow reserve (FFR) is the current gold standard for the diagnosis of hemodynamically significant CAD. CT-derived FFRCT and CT perfusion are two emerging techniques that can provide a functional assessment of CAD for risk stratification and clinical decision making. Several clinical studies have shown that the diagnostic performance of concomitant CCTA and functional CT assessment for detecting hemodynamically significant CAD is at least non-inferior to that of other routinely used imaging modalities. This article aims to review the current clinical evidence and recent developments in functional CT techniques.

Objective: To evaluate whether data acquired from perfusion computed tomography (PCT) parameters can aid in the prediction of treatment outcome after palliative chemotherapy in patients with unresectable advanced gastric cancer (AGC). Materials and Methods: Twenty-one patients with unresectable AGCs, who underwent both PCT and palliative chemotherapy, were prospectively included. Treatment response was assessed according to Response Evaluation Criteria in Solid Tumors version 1.1 (i.e., patients who achieved complete or partial response were classified as responders). The relationship between tumor response and PCT parameters was evaluated using the Mann-Whitney test and receiver operating characteristic analysis. One-year survival was estimated using the Kaplan-Meier method. Results: After chemotherapy, six patients exhibited partial response and were allocated to the responder group while the remaining 15 patients were allocated to the non-responder group. Permeability surface (PS) value was shown to be significantly different between the responder and non-responder groups (51.0 mL/100 g/min vs. 23.4 mL/100 g/min, respectively; p = 0.002), whereas other PCT parameters did not demonstrate a significant difference. The area under the curve for prediction in responders was 0.911 (p = 0.004) for PS value, with a sensitivity of 100% (6/6) and specificity of 80% (12/15) at a cut-off value of 29.7 mL/100 g/min. One-year survival in nine patients with PS value > 29.7 mL/100 g/min was 66.7%, which was significantly higher than that in the 12 patients (33.3%) with PS value ≤ 29.7 mL/100 g/min (p = 0.019). Conclusion: Perfusion parameter data acquired from PCT demonstrated predictive value for treatment outcome after palliative chemotherapy, reflected by the significantly higher PS value in the responder group compared with the non-responder group.
Electrocardiogram-gated single photon omission computed tomography (SPECT) provides valuable information in the assessment of both myocardial perfusion and ventricular function. Tl-201 is a suboptimal isotope for gating. Tl-201 images are more blurred compared with Tc-99m tracers due to the increased amount of scattered photons and use of a smooth filter. The average myocardial count densities are approximately one-half those of conventional technetium tracers. However, Tl-201 is still widely used because of its well-established utility for assessing myocardial perfusion, viability and risk stratification. Gated SPECT with Tl-201 enables us to assess both post-stress and rest left ventricular volume and function. Previous studies with gated Tl-201 SPECT measurements of ejection fraction (EF), end-diastolic volume (EDV), end-systolic volume (ESV) have shown high correlation with first-pass radionuclide angiography, gated blood pool scan, Tc-99m-MIBI gated SPECT, contrast ventriculography, echocardiography, and 3-dimensional magnetic resonance imaging. However, problems related to these studies include few agreement data of EDV and ESV, use of a reference method that is likely to have the same systemic errors (gated Tc-99m-MIBI SPECT), and other technical factors related to the count density of gated SPECT. With optimization of gated imaging protocols and more validation studies, gated Tl-201 SPECT would be an accurate method to provide perfusion and function information in patients with coronary artery disease.
Lung perfusion scanning, invariably combined with ventilation studies provides a reliable and non-invasive mean to diagnose lung related pathologies despite the availability of modern techniques such as angiography, magnetic resonance imaging, magnetic resonance angiography, and helical (spiral) computed tomography. The technique involves the generation of images by radiations emitted from radioisotopes introduced in to the lungs. Various radiopharmaceuticals have been proposed and designed to incorporate $Tc^{99m}$ in to macroparticulate form for lung perfusion imaging. However, most of these have associated difficulties such as reproducibility of the product with regards to particle size distribution and poor elimination from the lung capillary bed. $Tc^{99m}$ macroaggregated albumin $(Tc^{99m}-MAA)$ is used extensively for clinical lung perfusion imaging and is considered as the radiopharmaceutical of choice. It is non-toxic, safe, and being biodegradable, is easily eliminated from the lung capillary bed by proteolytic enzyme metabolism and by mechanical forces due to lung movement.

Tourette씨병과 만성틱장애는 7세 전후한 연령에서 가장 흔히 시작되는 소아 또는 청소년기의 행동장애로서 서로 밀접한 관련성을 가지고 있으며, 또한 유전적인 경향이 아주 강한 질환들로 알려져 있다. 원인으로서도 여러가지 가설이 있으나 뇌의 기능 또는 기질적인 장애로 인하여 발병될 가능성도 클 것으로 추정되고 있다. 이에 본 연구는 이 질환들의 생물학적인 원인들 중 일부를 살펴보기 위하여 단일광자방출 전산화단층촬영술을 시행하고 다음과 같은 결과를 얻었다. 1) 31.0%(13/42)에서 대뇌피질의 혈류감소 소견을 보였으며, 전두엽이 가장 흔한 혈류 이상 소견을 보인 부위였다.2) 4.8%(2/42)애서 대뇌기저핵의 혈류에 감소를 나타내었다. 3) 4.8%(2/42)에서 시상부의 혈류감소가 관찰되었다. 4) 16.7%(7/42)에서 소뇌의 혈류감소가 관찰되었다. 5) 주의력결핍과잉운동장애가 동반된 군과 동반되지 않은 군간에 혈류장애의 빈도에 있어서의 차이는 관찰되지 않았다. 6) Tourette씨병과 만성틱장애에 있어서, 혈류장애가 동반된 빈도에 있어서의 차이는 관찰되지 않았다. 이상의 걸과로 미루어 Tourette씨병과 만성틱장애의 생물학적인 원인중의 하나로 뇌혈류장애와 관계가 있을 가능성이 있으며, 뇌혈류의 정도를 정량화시키거나, 또는 자기공명영상과 같은 방법을 병행하여 연구를 시행함으로써, 본 연구의 타당성을 높여줄 수 있으리라 기대된다.
