암생존자의 재활을 위한 국내 프로그램 고찰 (Review of Rehabilitation Programs for Cancer survivors)

  • 의료 기술의 발달은 암 환자의 생존율을 크게 향상시켰으며, 암치료는 종료되었지만 암생존자들은 지속되는 신체적, 심리적, 사회적 문제로 고통 받고 있다. 따라서 암생존자들이 경험하는 문제에 총체적으로 접근하기 위해서는 다양한 통합 재활프로그램이 개발되어야 한다. 본 연구는 암생존자를 위한 재활 프로그램의 내용 및 구성을 확인하고 재활의 효과를 분석하여 향후 통합재활프로그램 개발의 자료로 사용하고자 한다. 문헌검토를 통해 검색된 13개의 연구 논문을 고찰한 결과 암생존자를 위한 재활 프로그램은 운동, 심리사회적, 통합 프로그램으로 분류되었으며, 운동 프로그램은 신체 기능 향상과 피로, 이차암 또는 암 재발과 관련된 호르몬의 변화를 통해 재활의 효과를 측정하였다. 심리사회 프로그램은 삶의 질, 우울, 불안 등과 같은 정서의 변화, 긍정적 자기 변화를 표현하는 외상 후 성장으로 효과를 측정하였고 긍정적인 결과를 보여주었다. 통합재활프로그램은 신체적, 심리사회적 측면을 모두 향상시켰다. 향후 암생존자를 위한 재활 프로그램은 신체적, 심리사회적 측면뿐만 아니라 직업 재활 측면까지 고려되어야 하며 암생존자의 다양성에 맞추어 프로그램을 탄력적으로 적용할 수 있도록 구성되어야 한다.

ADHD 치료에서 가족치료캠프의 적용 (Application of Family Treatment Camp in Treatment of ADHD)

  • Objectives : Demands for comprehensive and intensive treatment programs for treatment of children with attention-deficit hyperactivity disorder (ADHD) who suffer from serious impairment in various psychosocial areas are increasing. The aim of this study was to evaluate the possibility of developing new types of camp programs focused on improving social adjustment of children and helping parents effectively manage their children's problem behaviors. Methods : Fifteen children diagnosed as ADHD according to Diagnostic and Statistical Manual of Mental Disorders-IV (mean age 9.5 years) and their parents participated in this program. Eight consecutive camps were held at two-week intervals, from Jun 2007 to Oct 2007 in an adolescent training center located at Chungtaesan, in Korea. Each camp program included social skills training in the classroom and outdoor activities in the forest for children, as well as parent training for mothers. In addition, after the sixth camp, we conducted one parental session for fathers and a three-day booster family camp three months later. Finally, eleven families adhered to the program. Results : All children, parents, and therapists rated questionnaires or checklists at baseline, intra-, and post-treatment. Parenting burden rated using the Parenting Stress Index showed a significant decrease, from 57.89 to 46.22 (p=.019). And, overprotection rated using the Parenting Bonding Instrument showed a significant decrease, from 16.56 to 12.44 (p=.046). However, no significant improvement in children's behavioral and emotional problems was observed. Conclusion : In this study, the consecutive ADHD family camp program was effective for empowerment of parental competency, but not for general improvement of children's behavioral and emotional symptoms.

Comprehensive Approaches to Shoulder Impingement Syndrome: From Diagnosis to Rehabilitation

  • Shoulder impingement syndrome (SIS) is a common musculoskeletal condition characterized by pain and functional limitation due to the impingement of subacromial structures. This comprehensive review elucidates the complex nature of SIS, covering its pathophysiology, diagnostic methodologies, treatment options, and preventive measures. Through an exhaustive examination of current literature and clinical practices, the review highlights the importance of a multifaceted approach to SIS management. Physical therapy plays a pivotal role, focusing on exercises to strengthen shoulder musculature, enhance scapular stability, and improve range of motion. The review also discusses the strategic use of medications such as NSAIDs and corticosteroid injections, emphasizing their effectiveness in pain and inflammation management. Additionally, it advocates for structured rehabilitation programs post-treatment to restore function and prevent recurrence, recommending preventive strategies like ergonomic adjustments, targeted exercises, and proper technique training. This paper underscores the need for personalized and evidence-based treatment strategies, integrating physical therapy and pharmacological management when necessary.

신경계 질환자 대상의 국내 음악중재연구에 대한 체계적 문헌고찰 (Systematic Review of the Literatures on Music Intervention for Neurological Patients in Korea)

  • Purpose: The purpose of this study was to review the literature on music intervention for neurological patients in Korea and to explore the effect of music intervention on functional rehabilitation of neurological patients. Methods: This systematic review examined literature from 2005 to 2010. The existing literature was searched in the electronic databases of RISS and KISS using the following keywords: music, rhythmic movement, stroke, brain injury, dementia, Parkinson, spinal cord injury. Seventy-seven studies were founded through the database. After The exclusion of duplicates, case studies, literature review, and irrelevant studies, nine studies were included for analysis. Results: They included nine quasi-experimental studies. The medical diagnoses of the study participants were stroke, dementia, brain injury, and spinal cord injury. Among nine studies, only two studies were conducted in the nursing area. Eight studies used comprehensive music programs composed of health education, gait training, vocal training, or relaxation therapy, and these comprehensive programs were effective in the areas of physical and psychosocial treatment. Seventeen sessions were provided on average throughout intervention, and the average duration of each session was 49.4 minutes. Conclusion: Comprehensive music intervention can be implemented in nursing care for patients with neurological problem.

학교구강보건실 운영이 아동들의 구강보건지식 및 행동변화에 미치는 영향 (Effects of School-Based Oral Health Programs among Schoolchildren : Focus on the Oral Health Knowledge and Behaviors)

  • Demand for appropriate health care has gradually increased in Korea. In addition, developments of community- and school-based oral health programs have also focused oral health care for the oral health promotion. Especially, school-based oral health programs are the underpinnings of promoting oral health and preventing oral diseases among schoolchildren. School-based oral health programs have had three major components: oral health education, oral health services, and a healthful environments. These included oral health education(one-to-one communication, group communication, and use of mass communication), oral examination, fluoride mouthrinsing, pit-and-fissure sealants, fluoride gel application, mechanical plaque control, and chewing xylitol candy. In this study, we evaluate the effects of oral health programs among primary schoolchildren by comparing the oral health knowledge, oral health behaviors, and perception of caries prevention procedures. Data for this study were obtained from 699 primary schoolchildren at the two primary school in Daegu, Korea. One is experimental group, N primary school, that was established school-based oral health center under supervision of Nam-gu Public Health Center, the other is control group, N' primary school, that was yet to establish school-based oral health center. We surveyed children's oral health knowledge and behaviors, and perception of caries prevention procedures using self-administrated questionnaire and then analyzed differences of each item among two groups. The brief findings of this study were summarized as follows. There are several advantage to a comprehensive school-based oral health program. (1) School-based oral health programs facilitate and increase the effectiveness of teaching oral health subjects. (2) Schoolchildren are available for prevention or treatment procedure. (3) School-based oral health center may be less threating than private dental clinic. (4) With comprehensive school-based oral health programs the decayed, missing, and filled teeth(DMFT) of schoolchildren should demonstrate a substantial and steady decrease over time(Choi et al, 2004). In conclusion, treatment is not the answer to solving children's oral health programs; rather primary prevention is the key. Many countries and communities are focusing on hoe millions of underprivileged children can be provided with health care. Schoolchildren gain the knowledge and behaviors to attain and maintain good oral health in schools. For these reasons, the role of school-based oral health center is not only important but also a necessity.

지역사회 보건교육 (Community Health Education)

  • Health education aims at behavior change rather than just delivering health knowledge to people. In Korea health education activities in public sector began in 1960 and they were included in the primary prevention program in communities. This article reviewed current health education programs in healthy living practice programs provided by local public health centers in Korea and drew implications for the future role of health education in community setting. Health education has been a core function of the National Health Promotion programs in the nation since the enactment of the National Health Promotion Law in 1995. The National Health Promotion programs are funded by the National Health Promotion Fund which are drawn from tobacco tax. The National Health Promotion programs include healthy living practice programs (smoking prevention and cessation programs, moderate alcohol use programs, physical activity promotion programs, and nutrition programs), chronic disease prevention programs, oral health programs and public hygiene programs. Methods of the National Health Promotion programs include health education, health counseling, health class, health information management, survey and research. Smoking prevention and cessation programs include smoking cessation clinic, smoking cessation education, non-smoking environment program, and non-smoking campaign. Moderate alcohol use programs include alcohol use education, moderate alcohol use campaign, alcohol use counseling, and alcohol free environment programs. Physical activity promotion programs include obesity control, targeted exercise program, and exercise civic group programs. Nutrition programs include nutrition management, obesity management, nutrition education, breakfast eating program, and nutrition counseling and treatment programs. The health education programs in community are not efficient today because there are many overlapping contents and short term goals. Community health education programs needs to be more comprehensive. Workforce development is another big issue at the moment because the National credential program will begin in 2009. Variety of community health education programs should be developed and funded by the national health promotion fund.

대학 보건프로그램의 실태 및 분석 (A Study on Current Status of University Health Care Programs)

  • Purpose: To identify current status of university health care program. Methods: Data and information from homepages of 309 colleges or universities in South Korea were collected. The data was analyzed by frequencies, t-test, ${\chi}^2$ test with SPSS Ver. 18.0. Results: 117(37.9%) universities had organization of health care. Whether university had health care program or not had shown significantly depended on number of students, types of school (university or college), region, and existence of medical and nursing course. Medical course was shown as a strong predictor for facilitating university health care program limitedly focusing on diseases treatment. Health promotion programs have been operated in 15 universities, vaccination programs in 10 universities, and health screening in 20 universities. Conclusion: It is strongly recommended to revise the School Health Law for constructing a comprehensive university health care program consolidating health counseling and physical training.

  • While many fertility preservation (FP) options now exist for reproductive-aged cancer patients, access to these services continues to be limited. A comprehensive FP program should be organized to serve oncofertility patients effectively. Also, much effort is needed from various individuals-patients, specialists from various fields, and consultants-to facilitate FP in a timely manner. Various challenges still exist in improving access to FP programs. To improve access to FP treatment, it is important to educate oncologists and patients via electronic tools and to actively navigate patients through the system. Reproductive endocrinology practices that receive oncofertility referrals must be equipped to provide a full range of options on short notice. A multidisciplinary team approach is required, involving physicians, nurses, mental health professionals, office staff, and laboratory personnel. The bottom line of FP patient care is to understand the true nature of each patient's specific situation and to develop a patient flow system that will help build a successful FP program. Expanding the patient flow system to all comprehensive cancer centers will ensure that all patients are provided with adequate information regarding their fertility, regardless of geography.

주의력결핍/과다활동장애(ADHD) 아동에서 사회기술훈련 (THE SOCIAL SKILLS TRAINING IN CHILDREN WITH ATTENTION-DEFICIT HYPERACTIVITY DISORDER(ADHD))

  • 목 적:주의력결핍/과다활동장애(ADHD) 아동들은 또래관계를 영위하는데 필요한 것을 포함한 다양한 사회기술에 결함을 보인다. 이들의 치료에 약물치료를 포함한 부모훈련, 교육적 접근 등이 필요하지만 이와 함께 이들에 대한 사회기술훈련이 시도되고 있다. 이들에게서 사회기술훈련을 적용하여 그 효과를 검증해보고자 한다. 방 법:ADHD로 진단 받은 초등학교 아동 5명과 그들의 부모 5명을 대상으로 Pfiffner와 McBurnett이 제안한 프로그램을 중심으로 총 8회기의 프로그램을 수행하고, 사회기술, 문제행동, 또래의 사회적 수용도, 양육태도 및 양육스트레스, 프로그램 만족도를 비교해 보았다. 결 과:교사와 부모가 평가한 내용이 다소 달랐는데, 교사는 사회기술과 또래의 사회적 수용도에서, 부모는 문제행동의 감소에서 유의미한 호전을 보고하였다. 부모들은 양육태도에서는 별 변화가 없었지만 양육스트레스에서는 현저한 감소를 보고하였으며, 프로그램 만족도는 상당히 높았다. 결 론:ADHD 아동에서 사회기술훈련은 최근에, 주로 보조적이거나 매우 제한적으로 시행되고 있다. 하지만 상당히 많은 아동들이 사회기술의 결함을 호소하고 있기 때문에 이들이 통합적 접근에서 이들의 치료 프로그램에 포함되어 시행하는 것이 바람직하겠다.

미국 브라운필드 관리 프로그램과 정책의 시기별 특성과 함의 (Periodic Characteristics and Implications of Programs and Policies for Brownfield Management in the U.S.A.)

  • 하수처리시설, 군부대 등의 산업이전적지의 공원화 계획이 늘어나면서 브라운필드 재개발은 조경계획 및 디자인에서 하나의 분야로 자리 잡았다. 특히, 주변 커뮤니티 활성화, 지가 상승 등 동반이익에 대한 기대가 높아지면서 선별된 부지들의 종합적 계획 및 관리에 대한 필요성이 인식되고 있다. 본 연구는 미국 브라운 필드 관리 정책의 변화양상을 살펴 본 기초적인 사례연구로서, 1980년 수퍼펀드 프로그램 도입 이후, 약 30 여 년간 확장하여 온 브라운필드 프로그램과 정책의 시기별 특성을 파악하는 것을 주 목적으로 한다. 연구 방법으로 서술적, 해석적 방법에 기반한 미국 환경부 산하 기관들의 웹문서 및 관련 문헌 분석을 통해 정책의 특성을 시계열적으로 분석하였다. 본 연구의 결과, 주요한 변화를 기점으로 하여, 환경규제시기, 오염정화 및 재개발 촉진시기, 종합적 계획시기의 세 가지 시기로 구분되었고, 주요 특성은 규제 완화 및 재조정, 지원 프로그램의 다양화, 하향식과 상향식 방식의 조화, 데이터베이스 구축의 네 가지 측면으로 요약되었다. 본 연구는 대상지 선별기준, 이해 당사자 파악 및 책임분쟁 조정, 오염도 평가비용 및 처리 등 브라운필드만이 가진 공통 문제들이 어떻게 다뤄져 왔는지 선례를 살펴봄으로써, 향후 한국의 체계적 브라운 필드 관리 및 계획에 시사점을 제공하는 것에 의의를 두고 있다.
