  • We investigate the clinical characteristics of odontoma in childhood and adolescence. A retrospective analysis was performed in 105 patients under 15 years old who were diagnosed with odontoma in Kyungpook National University Dental Hospital, the department of pediatric dentistry between 1 January 2008 and 31 December 2013. The ratio of compound odontoma, immature odontoma and complex odontoma were 67.6%, 21.9% and 10.5%, respectively. Odontoma was more prevalent in males and the ratio of males to females was 1.5:1. The age of patients ranged from 3 to 14 years old with an average age of 9.3 years old. The odontoma was more frequently located in the maxilla (73.3%) than in the mandible(26.7%). In 98 cases, it was treated by surgical removal. Among adjacent teeth impacted by odontoma, 26 cases (34.7%) of successional permanent teeth were erupted when only odontoma were removed and 6 cases (8.0%) were erupted when surgical exposure was performed. Orthodontic tractions were performed in 43 cases (57.3%). In conclusion, odontoma could cause complications such as impaction of teeth. If they are found early and treated properly, high eruption success of successional permanent teeth can be obtained. The occurrence frequency of immature odontoma are higher than that of complex odontoma, and they are observed intensively in anterior maxillary teeth. Therefore, it is considered that they are important causes of dental impaction in anterior maxillary teeth.

  • The term odontoma, first introduced by Broca, was originally applied to all odontogenic tumors. Related to the clinical and histological differences, 2 groups of odontoma were defined: compound odontoma and complex odontoma. Complex odontoma is more common in the mandible than in the maxilla, usually occuring in the mandibular premolar-molar area. And this consists of small mass of irregularly arranged enamel, dentine, cementum, and connective tissue, but tooth-like structures are not found. In our case, 34 year old man was diagnosed as the complex odontoma of maxillary sinus by the physical exam, Panex, and C.T scan. We sectioned and removed it, and then confirmed the diagnosis histopathologically. Its uncommon location and size made us report that case and review the literature concerned.

  • Ameloblastic odontoma is a mixed odontogenic tumor, which is characterized as being extremely rare, displaying aggressive clinical behavior, resembling a mixed radiopaque and radiolucent radiographic appearance and containing both ameloblastoma and a composite odontoma. Seven-year-old boy complaining of delayed eruption of the left lower permanent 1st molar came to the department of Oral & Maxillofacial Radiology at Chonbuk National University Hospital. The list of our radiographic differential diagnosis included the dental follicle, compound odontoma, complex odontoma and cystic odontoma. The microscopic analysis showed the lesion with the characteristics of an ameloblastic odontoma. The obtained results were as follows; 1. The area was asymptomatic, but the delayed eruption of the left lower permanent 1st molar was observed. Radiographically, well-defined widened pericoronal radiolucency containing several tooth-like radiopacities involving the impacted left lower permanent 1st molar and well-defined round radiolucency containing irregular radiopaque mass were observed. 3. Histopathologically, several tooth-like structures, odontoma components with the area of typical enamel, dentin and pulp, and ameloblastic components with typical follicular ameloblastoma were observed.

  • Odontomas are nonaggressive, hamartomatous developmental malformations composed of mature tooth substances and may be compound or complex depending on the extent of morphodifferentiation or on their resemblance to normal teeth. Among them, complex odontomas are relatively rare tumors. They are usually asymptomatic in nature. Occasionally, these tumors become large, causing bone expansion followed by facial asymmetry. Odontoma eruptions are uncommon, and thus far, very few cases of erupted complex odontomas have been reported in the literature. Here, we report the case of an unusually large, painless, complex odontoma located in the right posterior mandible.

  • Purpose : The purpose of the present study was to retrospectively evaluate the clinical findings and treatment results for impacted permanent teeth associated with odontomas. Materials and Methods : We retrospectively investigated 73 odontomas in 72 patients who visited Kyungpook National University Dental Hospital from April 2004 through November 2011. The study was performed using medical records, panoramic radiographs, and pathological reports. Data gathered included age, gender, location, chief complaints, effects on dentition, and treatment of odontoma and the impacted tooth associated with odontoma. Results : Most compound odontomas (46.7%) were found in the second decade and complex odontomas were not related to age. Odontomas showed no gender predilection. Fifty-five cases (75.3%) of odontomas were detected on routine dental radiographs. Sixty percent of compound odontomas occurred in the canine area and 57.1% of complex odontomas in the molar areas. Impaction of permanent teeth (61.6%) was the most common complication on the adjacent teeth. Most odontomas (84.9%) were removed surgically and impacted permanent teeth were managed by surgical removal (53.2%), orthodontic treatment (25.5%), or surgical repositioning (6.4%). There was a statistically significant relation between age and preservation of the impacted permanent teeth associated with odontomas (p<0.01). Conclusion : Early detection and treatment of odontomas increase the possibility of preservation of the impacted tooth. Therefore, it would be suggested that periodic panoramic examination during the first and second decade of life might be beneficial for the early detection and better prognosis of odontomas.

  • Odontoma is the most common odontogenic benign tumor, and the treatment of choice is generally surgical removal. After excision, bone grafts may be necessary depending on the need for further treatment, or the size and location of the odontoma. Although the osteogenic capacity of a demineralized tooth was verified as early as 1967 by Urist and many other investigators, the cumbersome procedure, including a long demineralization time, may be less than comfortable for clinicians. A modified ultrasonic technology, with periodic negative pressure and temperature control, facilitated rapid and aseptic preparation of demineralized teeth for bone grafts. This approach reduces the demineralization time dramatically (${\leq}80$ minutes), so that the graft material can be prepared chairside on the same day as the extraction. The purpose of this article is to describe two cases of large compound odonotomas used as graft material prepared chairside for enucleation-induced bony defects. These two clinical cases showed favorable wound healing without complications, and good bony support for future dental implants or orthodontic treatment. Finally, this report will suggest the possibility of recycling the benign pathologic hard tissue as an alternative treatment option for conventional bone grafts in clinics.
