Roles of White Blood Cells and Subtypes as Inflammatory Markers in Skin Cancer

  • Objective: Skin tumors are the most commonly seen cancer type worldwide. Regarding pathogenesis, it is thought that disruption of kinetics through T lymphocyte-mediated development of chronic inflammation may be involved. The present study was intended to identify role of inflammatory cells such as neutrophils, monocytes and lymphocytes in the determination of risk for skin cancer. Materials and Methods: We retrospectively reviewed charts of 569 cases diagnosed as having primary skin tumors. Data regarding age, gender and histopathological subtype were recorded. Blood parameters studied on the day before surgery including WBCs, neutrophils, and lymphocyte counts, neutrophil:lymphocyte and neutrophil:monocyte ratios were also recorded. Two-hundred and two healthy individuals presented for check-up in an outpatient clinic were selected as the control group. Parameters studied in cases with skin cancer were compared to those healthy individuals. Findings: Of the cases with skin cancer, 401 were basal cell carcinoma (BCC) while 144 were squamous cell carcinoma (SCC) and 13 were malignant melanoma (MM). WBC, neutrophil and monocyte counts and the neutrophil:lymphocyte ratio were found to be lower in the patient group than in the healthy control group (p<0.001) while no significant difference was found in other parameters reviewed (p>0.05). No significant difference was found in WBC, neutrophil, neutrophil: monocyte ratio according to gender (p>0.05). Monocyte count was found to be $0.68{\pm}0.61$ in men and $0.55{\pm}0.25$ in women, indicating strong statistical significance (p<0.001). WBC, neutrophil and monocyte values were highest in control group while lowest in BCC. When BCC and SCC groups were compared to controls, significant differences found (p<0.001). There were no significant differences in lymphocyte counts among groups (p=0.976). Neutrophil:lymphocyte ratios were 3.24 in BCC, 3.59 in SCC, 3.44 in MM and 5.06 in control group (p<0.001). Conclusions: In our study, it was found that there were significant differences in complete blood count, neutrophil, monocyte and neutrophil:lymphocyte levels among groups. Neutrophil: lymphocyte ratio was found to be lowest in BCC among skin cancers.

Evaluation of valid time for analysis of complete blood cell in pig blood using the Hemavet 950FS (자동혈구분석기 Hemavet 950FS를 이용한 돼지 혈액 내 혈구분석의 유효시간 평가)

  • This study was conducted to determine the valid time for accurate detection of complete blood cell count (CBC) in pig blood using an automatic blood corpuscle analyzer (Hemavet 950FS). Blood samples were collected from 34 pigs (Duroc) with a 60 kg (${\pm}3.5$) body weight. Ten specimens with CBC parameters in normal range and with no hemolysis were selected among 34 samples and used in this study. Regarding leukocytes parameters, white blood cell (WBC), neutrophil (NE), and lymphocyte (LY) counts showed a low daily variation (coefficient of variation, CV), whereas monocyte (MO), eosinophil (EO), and basophil (BA) CVs were significantly high (19.7, 56.9, and 53.3%, respectively). On the other hand, all parameters of erythrocytes and thrombocytes showed stable daily variation. All parameters of leukocytes and thrombocytes were significantly reduced as storage time passed (P<0.01 or 0.001), except for lymphocytes (P=0.535). However, no significant differences were observed in parameters of erythrocytes from blood up to 120 hours. From above results, we assert that Hemavet 950FS is useful in analyzing CBC, except for MO, EO, and BA. For accurate detection of leukocyte and thrombocyte parameters, analysis should be performed within 4 hours after blood collection when using Hemavet 950FS. On the other hand, parameters of erythrocytes could be stably detected for at least 120 hours after blood collection.

Recovery From Chemotherapy Induced Neutropenia Treated with Samul-tanggagambang (항암화학치료 후 발생한 호중구 감소증에 대한 사물탕가감방 증례 보고)

  • The purpose of this study is to report two patients with neutropenia caused by chemotherapy who prescribed Samul-tanggagambang. Samul-tanggagambang was prescribed three times a day to two patients with chemotherapy induced neutropenia. Complete blood cell count is measured before and after prescription. Absolute Neutrophil Count (ANC) was gradually increased after administration of Samul-tanggagambang to the cancer patients. Significantly, no related adverse events were found. Samul-tanggagambang has shown benefit in improving chemotherapy induced neutropenia. It is expected to be a promising treatment for improving chemotherapy induced neutropenia and more clinical research will be required for evidence based using.

Hemato-Biochemical Indices of Hanwoo Cattle Raised at Different Altitudes (서로 다른 고도에서 사육된 비육한우의 혈액-혈청 지수)

  • The health status of three groups of Hanwoo steers (n=157) aged 2-3 years, raised at three different altitudes (600 m, n=50; 200-400 m, n=58; plane land, n=49) and environment with more or less similar management have been evaluated through hematological, biochemical and globulin examinations in order to determine the optimum environment suitable for raising cattle while at the same time minimizing the risk of disease. Five mL of blood samples from each animal were collected by jugular veinipuncture and 2 mL was transferred to a tube containing EDTA for complete blood count (CBC) and 3 mL in lithium heparin for chemistry screening (CS) and immunoassay. Among the CBC parameters a significantly higher white blood cell count (tWBC), total red blood cell count (tRBC), hemoglobin (Hb) and packed cell volume (PCV) were noticed in the high altitude groups, whereas those were lower in the plane land group. In the CS parameters higher levels of aspartate aminotransferase (AST), creatinine, lactate dehydrogenase (LDH) and total bilirubin (TBL) were found in the plane land group, whereas those were lower in the high altitude groups. The total protein (significantly) and globulins were higher in the 600 m group. The results of this study revealed that the overall health status of the Hanwoo cattle based on the hemato-biochemical indices was superior in the highest altitude and inferior in the plane land group but all the parameters were within the reference range in all the groups. Therefore, for recommendation of a suitable environment at an appropriate altitude for raising cattle there need to be further studied along with the hemato-biochemical parameters; considering, breeding, feeding, management, marketing, waste disposal and other factors.

A Study on the Distribution of Tongue Colors and the Correlation of Tongue Colors with the Clinical Laboratory Parameters (설색의 분포 및 설색과 임상병리검사 결과의 연관성에 대한 연구)

  • Objectives : The aim of this study was to evaluate the distribution of tongue color and the correlation of the tongue colors with clinical laboratory parameters. Methods : Statistical analysis on the frequency of each tongue color and the results of the clinical laboratory parameters in each tongue color group. Results : Tongue colors that appeared most frequently were 702c, 695c, 493c, 694c, 701c, 500c, 7432c, 7431c, 7634c, 7419c in that order. Following them, 494c and 507c were rarer colors, and 501c, 673c, 687c, 693c, 696c, 703c, 7418c were equally rarest colors. Among each tongue color group, a significant difference was found in the result of complete blood cell count (CBC). The level of red blood cell (RBC) of 7432c group was significantly higher than that of the other groups except 694c group. The level of hemoglobin (Hb), hematocrit (Hct) of 7432c group were significantly higher than those of all the other groups. The level of RBC of 7431c group was lowest and significantly different from 694c, 7432c group. In results of the other tests were not significantly different. However the level of total cholesterol was higher in 695c, 694c, 500c group, and the level of triglyceride was higher in 500c, 7431c, 694c group. Those four colors have a low chroma and 7431c, 695c take blue violet color. Conclusions : The tongue color is distributed throughout following colors by order of frequency: 702c, 695c, 493c, 694c, 701c, 500c, 7432c, 7431c, 7634c, 7419c, 708c, 494c, 507c, 501c, 673c, 687c, 693c, 696c, 703c, 7418c. Some parameters of CBC has a significant correlation with some tongue colors. Other results including lipid profile, renal function test, and liver function test had no significant difference according to tongue color. However, the colors which have low chroma or take blue violet color tented to be higher in the levels of total cholesterol and triglyceride.

Endocrine Profiles and Blood Chemistry Patterns of Cloned Miniature Pigs in the Post-Puberty Period

  • Although the majority of surviving pigs cloned by somatic cell nuclear transfer (SCNT) appear to be physiologically normal, there is a general lack of detailed hemato-physiologic studies for the period of early adulthood to substantiate this claim. In the present study, we investigated variation in blood chemistry and endocrinological parameters between mesenchymal stem cells (MSCs) derived from cloned and normal age-matched female and male miniature pigs. Cloned females and males showed normal ranges for complete blood count assessments. Biochemical assessments showed that ${\gamma}$-GGT, ALT and cholesterol levels of male and female clones were significantly (P<0.05 or P<0.01, respectively) higher than that of age-matched control miniature pigs. Variations in insulin and IGF-1 were higher in female clones than in male clones and controls. Thus, although female and male cloned miniature pigs may be physiologically similar to normal animals, or at least within normal ranges, a greater degree of physiological and endocrinological variation was found in cloned pigs. The above variation must be taken into account before considering cloned female or male miniature pigs for various biomedical applications.

Prognostic Value of Baseline Neutrophil-Lymphocyte and Platelet-Lymphocyte Ratios in Local and Advanced Gastric Cancer Patients

  • Background: We aimed to investigate the prognostic value of baseline neutrophil, lymphocyte, and platelet counts along with the neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio (PLR) in local and advanced gastric cancer patients. Materials and Methods: In this retrospective cross-sectional study, a total of 103 patients with gastric cancer were included. For all, patient characteristics and overall survival (OS) times were evaluated. Data from a complete blood count test including neutrophil, lymphocyte, monocyte, white blood cell (WBC) and platelet (Plt) count, hemoglobin level (Hb) were recorded, and the NLR and PLR were obtained for every patient prior to pathological diagnosis before any treatment was applied. Results: Of the patients, 53 had local disease, underwent surgery and were administered adjuvant chemoradiotherapy where indicated. The remaining 50 had advanced disease and only received chemotherapy. OS time was $71.6{\pm}6$ months in local gastric cancer patients group and $15{\pm}2$ months in the advanced gastric cancer group. Univariate analysis demonstrated that only high platelet count (p=0.013) was associated with better OS in the local gastric cancer patients. In contrast, both low NLR (p=0.029) and low PLR (p=0.012) were associated with better OS in advanced gastric cancer patients. Conclusions: This study demonstrated that NLR and PLR had no effect on prognosis in patients with local gastric cancer who underwent surgery and received adjuvant chemoradiotherapy. In advanced gastric cancer patients, both NLR and PLR had significant effects on prognosis, so they may find application as easily measured prognostic factors for such patients.

Relationship Between Prognosis and Neutrophil: Lymphocyte and Platelet:Lymphocyte Ratios in Patients with Malignant Pleural Mesotheliomas

  • Background: It has been demonstrated that neutrophil:lymphocyte (NLR) and platelet:lymphocyte (PLR) ratios are associated with prognosis in cancer patients. The aim of this study was to investigate whether pretreatment white blood cell (WBC), neutrophil, lymphocyte, monocyte, platelet, basophil and eosinophil counts, LDH level, NLR and PLR are associated with prognosis in patients with malignant pleural mesothelioma (MPM). Materials and Methods: We retrospectively reviewed files of 50 patients who were managed with a diagnosis of MPM between 2005 and 2010. Demographic and clinical characteristics, treatments, response to treatment and prognostic factors were evaluated, along with relationships between pretreatment blood parameters and prognosis. Results: Overall, 38 men and 12 women were included to the study. Mean age was $61.5{\pm}9.4$ years (range: 39-83 years). There was advanced disease in 86% (n=43) and the histological type was epithelial mesothelioma in the majority (82%). Of the cases, 17 (34%) received radiotherapy, while 42 cases underwent first- and second-line chemotherapy, with cisplatin plus pemetrexed as the most commonly used regimen. In the assessment after therapy, it was found that there was complete response in 4 cases (8%), partial response in 10 cases (20%), stable disease in 17 cases (34%) and progression in 19 cases (38%). Median follow-up was 10 months (range: 10 day-30 months). Median overall survival was found to be 20.7 months while median progression-free survival as 10 months. In univariate and multivariate analyses, it was found that factors significantly affecting overall survival included stage (p=0.030), response to treatment (p=0.026) and monocyte count (p=0.004), while factors affecting disease-free survival included NLR (p=0.018), response to treatment (p=0.001), and PLR score (p=0.003). Conclusions: Overall and disease-free survival was found to be better in cases with a WBC count<8.000, platelet count<300,000, and low NLR and PLR scores in malignant pleural mesothelioma.

Phytolacca Radix Poisoning: A Case Report (자리공 뿌리 섭취로 발생한 급성 중독 1례 : 증례보고)

  • Objectives: This study aims to present the case of poisoning associated with Phytolacca radix. Case Presentation: A 61-year-old male with nausea, vomiting, dizziness, and abdominal pain after ingesting about 10 centimeters of Phytolacca radix visited Korean medicine hospital. He began to show symptoms one hour after eating the plants. Methods: The patient was diagnosed with poisoning by Phytolacca radix. The patient received acupuncture, moxibustion, and herbal medicine treatment including Glycyrrhizae radix. laboratory test including complete blood count, biochemistry was also conducted. Results: Treatment, including acupuncture, moxibustion, and herbal medicine, improved his symptoms. His symptoms subsided within two hours. The laboratory test conducted on the next day show a slight increase in his white blood cell, blood urea nitrogen, urine ketone levels. There was no damage to the liver or kidneys. Conclusions: This report demonstrates the need for caution when consuming wild plants, which could lead to serious adverse effects. Patients should be alerted to the indiscriminate ingestion of wild plants. Further research on how to treat poisoning in Korean medicine needs to be considered to confirm these findings.

Two Cases of Spontaneous Intracranial Hypotension Treated with Epidural Blood Patch (자발성 두개내 저압환자의 경막외 혈액봉합술 치험 2예)

  • Spontaneous intracranial hypotension (SIH) is a rare syndrome which causes postural headache associated with low cerebrospinal fluid (CSF) pressure, without preceding events such as lumbar puncture, back trauma, operative procedure or illness. The headche is usually accompanied by nausea, vomiting, tinnitus, neck stiffness, vertigo, photophobia and in rare cases diplopia, transient visual obscuration. Brain MRI may show subdural effusion, downward displacement of brain and diffuse and continuous meningeal enhancement when enhanced by gadolinium. Low opening pressure was shown through lumbar puncture. The value of protein and cell count of CSF may be slightly elevated. This syndrome usually resolves itself spontaneously or with strict bed rest. When the headache is persistent or incapacitating, more aggressive treatment may be necessary. We treated two cases of SIH in which epidural blood patch produced immediate and complete relief of the symptoms.
