Nodal tumor response according to the count of peripheral blood lymphocyte subpopulations during preoperative chemoradiotherapy in locally advanced rectal cancer

  • Purpose: The objective of this prospective study was to evaluate the relationship between the circulating lymphocyte subpopulation counts during preoperative chemoradiotherapy (CRT) and tumor response in locally advanced rectal cancer. Materials and Methods: From August 2015 to June 2016, 10 patients treated with preoperative CRT followed by surgery were enrolled. Patients received conventional fractionated radiotherapy (50.4 Gy) with fluorouracil-based chemotherapy. Surgical resection was performed at 4 to 8 weeks after the completion of preoperative CRT. The absolute blood lymphocyte subpopulation was obtained prior to and after 4 weeks of CRT. We analyzed the association between a tumor response and change in the lymphocyte subpopulation during CRT. Results: Among 10 patients, 2 (20%) had evidence of pathologic complete response. In 8 patients with clinically node positive, 4 (50%) had nodal tumor response. All lymphocyte subpopulation counts at 4 weeks after CRT were significantly lower than those observed during pretreatment (p < 0.01). A high decrease in natural killer (NK) cell, count during CRT (baseline cell count - cell count at 4 weeks) was associated with node down staging (p = 0.034). Conclusion: Our results suggest that the change of lymphocyte subset to preoperative CRT may be a predictive factor for tumor response in rectal cancer.

Test turnaround Time for Complete Blood Cell Count using Delta and Panic Value Checks and the Q-flag Limit

  • Test turnaround time (TAT) is the lead time from reception to reporting. In the complete blood cell count (CBC), 4 units of the XE-2100 (Sysmex Corp., Japan) processed around 80% of quantity, 1 unit of the LH-780 (Beckman-Coulter Incorp., USA) processed around 10% and 1 unit of ADVIA-2120 (Siemens AG, Munich, Germany) processed around 10%. We analyzed the change in the TAT for the CBC for over 7 years, from January of 2005 to December of 2011. The delta check made alterations of delta to WBC, hemoglobin, hematocrit, platelet and metamyelocyte, however, did not made them to band neutrophil, eosinophil, basophil and monocyte. The panic value check made alterations of panic value to hemoglobin, hematocrit, platelet and monocyte. In the criteria of currently slide review, LH-780 and ADVI-2120 analyzers prepared suspect flags of "Blast, Imm NE2, Immature granulocyte, Imm NE1, Left shift, Variant lymphocyte, Atypical lymphocyte, Platelet clumps and NRBC". The New slide review in the XE-2100 analyzer altered the preparations of a smear slide more than a "Platelet clumps flag(${\geq}200unit$), a single flag excluding the "Platelet clumps flag (${\geq}250unit$) and a multiple flag (${\geq}200unit$)". Also, below the 240 unit, medical technologists prepared manual slides selectively according to their evaluations. The automatic reporting rate was 33.4% without alterations, whereas it was 41.0% without alterations, and was thus improved by 7.6%. The slide review rate was 15.2% before using the Q-flag limit, whereas it was 12.1% for a reduce 3.1%. TAT was 45 minutes without the creation alterations of the delta and panic value checks, whereas it was 35 minutes after making alterations of the delta and panic value checks and thus was shortened by 10 minutes. We came to the conclusion that the establishment and operation of delta and panic value checks and slide review criteria suitable for laboratory environment can reduce unnecessary smear slides, re-checking, re-sampling, re-testing, telephone inquiries and concentrated workloads during specific times of the day.

A Study on the Relationship between Body Composition Analysis and CBC in University Students (대학생의 체성분분석과 혈구산정검사의 관계연구)

  • The tendency of students to become adults in the future to improve the country's public health policy and to determine the socio-economic development of the research is very important. The objectives of this study were to examine the changes in body weight, body mass index (BMI), body composition, and complete blood cell count in a group of male and female university students. Among the body compositions, height, weight, BMI, muscle volume, basal metabolic rate, red blood cells, hemoglobin, and hematocrit showed a correlation between the amount shown both had statistical significance (p<0.01). In particular, platelets showed a rather different result from fat and body fat percentage, and were positively correlated with waist-hip ratio points (p<0.01). This study may be useful as it provides the basic data necessary for students of healthcare. Therefore, developing a sustainable management system of healthcare on a national level for university students is very important.

The effects of Constant Use of Herbal Medicine with Western Medicine On Liver and Kidney Functions (뇌졸중환자에서 한약과 양약의 병용투여가 간장 및 신장에 미치는 영향)

  • Objectives : The purpose of this study was to evaluate whether herbal medication may injure the liver function and kidney function. We clinically studied the changes of biochemistry and complete blood cell count in patients who were hospitalized in Korean hospital. Methods : The subjects were 160 patients admitted in Kangnam Korean hospital, Kyung Hee, whose mean duration of medication was 70 days(min 30, max 252). All patients had been checked biochemistry and complete blood cell count for 30 days since their hospitalization. Results : Aspartate aminotransferase(AST), alanine aminotransferase(ALT), alkaline phosphatase(ALP), blood urea nitrogen(BUN) and Creatinine(Cr) decreased significantly on discharge, compared with the values of admissoin(P<0.05). Total protein significantly elevated on dischrge compared with the values of admisson(P<0.05). Conclusions : This study suggests that even though there are few toxic herbal medicines, in general, herbal medications which are prescribed by oriental medicine doctors should be carefully administered not to hurt liver and kidney function.

Effects on hematology and blood biochemistry profile of intramuscular meloxicam injection in Brahminy kite and Barn owl

  • Background: Meloxicam is used widely for exotic animal analgesia, but its toxicity in common raptor species in Thailand is unclear. Objectives: This study evaluated the single-dose effect of intramuscular meloxicam in common raptor species in Thailand for short-term and long-term periods. Methods: Twenty-two raptors were administered a single 1 mg/kg dose of meloxicam individually via intramuscular injection. The following were evaluated: clinical appearance, body weight, body condition score, body temperature, fecal appearance, complete blood cell count, and biochemistry panel before (day 0) and after the injection (1, 7, and 30 days). The collected samples were categorized into three groups: Brahminy kite (Haliastur indus) (n = 10), adult Barn owl (Tyto javanica) (n = 4), and juvenile Barn owl (n = 8). Results: None of the raptors in the study groups showed any abnormalities. The hematological profiles were significantly different in the short-term period (day 1 and day 7). The creatinine, aspartate aminotransferase, and creatinine kinase increased in several groups. On the other hand, the packed cell volume decreased in the Brahminy kite and juvenile Barn owl groups. According to the findings, an intramuscular injection of 1 mg/kg meloxicam affected the blood biochemistry panel of the muscle, but the affected raptors recovered within one week. Conclusions: An intramuscular injection of meloxicam at a single 1 mg/kg dose in Brahminy kites and Barn owls was not associated with the morbidity, hepatotoxicity, gastrointestinal toxicity, and nephrotoxicity in the short- and long-term periods.

Reference Values of Whole Blood Viscosity and Its Correlation with Hematology and Serum Chemistry in Beagle Dogs

  • The aim of this study was to measure whole blood viscosity (WBV) and then to establish its reference values and identify correlation between WBV and hematology or serum chemistry in beagle. The experiment was made up of 82 healthy beagle dogs. Jugular vein blood samples (10 ml) were collected. WBV (cP) measured within 4 hours after collection by U-shaped scanning capillary-tube viscometer ($BVD-PRO1^{(R)}$) which is capable of measuring yield stress and viscosity of whole blood continuously over a wide range of shear rates from $1s^{-1}$ to $1000s^{-1}$ is new type of capillary tube viscometer and calculates viscosity using Casson fluid model. Measured values of WBV, Complete blood cell count, serum chemistry were analyzed by RM-ANOVA test. Mean diastolic and systolic WBV (cP) were $29.032{\pm}6.137$ and $4.528{\pm}0.865$. Bodyweight (BW), Red blood cell count (RBC), Hemoglobin (HGB), Hematocrit (HCT), Alkaline phosphatase (ALP), Cholestetol (CHOL), Total protein (TP), Globulin, Chloride (Cl), Fibrinogen, were statistically correlated with WBV over whole range of shear rates (p < 0.05). This study newly evaluated reference values of WBV by U-shaped viscometer in beagle. Correlation between WBV and BW, RBC, HGB, HCT, ALP, CHOL, TP, Globulin, Cl, Fibrinogen was presented.

Stem cell therapy for dogs with immune-mediated hemolytic anemia

  • Three different dogs who had immune-mediated hemolytic anemia (IMHA) were treated for more than two weeks with blood transfusion in an animal clinic. Despite this treatment and hospitalization, there was no clinical improvement in clinical signs as well as complete blood cell count (CBC) including hematocrit (HCT) and C-reactive protein (CRP). All cases were then injected two or three times with allogeneic stem cells through an intravenous route for treatment. Upon administrating stem cells to the IMHA dogs, clinical conditions and the indexes of HCT and CRP were clinically improved within or close to normal ranges.

  • Epilepsy is a disorder that causes recurring seizures, and the most objective and useful test for detecting epilepsy is the electroencephalogram (EEG). The subjects of this study are 244 patients who received an EEG after being diagnosed with epilepsy at Seoul National University Hospital in 2014, and who have agreed to the purpose of the study. Based on the EEG results, subjects were divided into normal and abnormal groups with 122 subjects in each group, regardless of their gender and age, to investigate the correlation of EEG and complete blood cell count (CBC) test results. The four significant categories that displayed significant correlation between EEG results and CBC hematological measurements in this study were the white blood cell (WBC), red blood cell (RBC), neutrophil, and lymphocyte tests. The WBC (p<0.05) and neutrophil (p<0.01) showed a positive correlation with EEG results, while RBC (p<0.05) and lymphocyte (p<0.01) showed a negative correlation. One of the limitations of this study is that it is lacking the blood test result analysis according to the types of anti-epilepsy medicine. However, the analysis of EEG results by the same disease has significant meaning. Therefore, further studies are needed to statistically analyze more data in the future.

Inhibitory Effect of Saengangeonbitang-gasamchilgn on Liver Fibrosis in Rat (생간건비탕가삼칠근(生肝健脾湯加三七根)이 흰쥐의 간섬유화 억제에 미치는 영향)

  • The aim of this study was to investigate the inhibitory effect of Saengangeonbitang-gasamchilgn(SGGBTGSCG) on collagen production in rat hepatic stellate cells(HSC) and on the TAA-induced chronic liver injury model in rats. Methods : 1) HSCs were treated with SGGBTGSCG extract powder(50% EtOH SGGBTGSCG, dw SGGBTGSCG). After the treatment, MTT assay, BrdU assay and procollagen assay were done. In addition, gene expressions of collagen type $1{\alpha}2$, ASMA, TIMP1, and TIMP2, all of which are known to be associated with liver fibrosis, were analyzed by RT-PCR. 2) Liver fibrosis was developed in rats by injection of TAA 3 times a week for 5 weeks. After the SGGBTGSCG-treatment, body weight, liver & spleen weight, liver function test, the complete blood cell count and the change of portal pressure were studied. Results : In MTT assay, SGGBTGSCG significantly decreased the viability of HSCs in a dose-dependent manner. In BrdU assay, SGGBTGSCG significantly inhibited the HSC proliferation in a dose-dependant manner. In procollagen assay, SGGBTGSCG decreased procollagen production by HSC. In the change of rats' liver and spleen weight, TAA+SGGBTGSCG groups showed little difference compared with TAA-only group. In the liver function test, SGGBTGSCG decreased the serum level of ALT, AST, and Alp elevated by TAA. In the complete blood cell count, SGGBTGSCG significantly decreased WBC elevated by TAA and increased RBC and Hct lowered by TAA. In the change of portal pressure, SGGBTGSCG decreased portal pressure elevated by TAA. Conclusions : These results suggest that SGGBTGSCG is beneficial in the treatment of cirrhotic patients as well as for patients with chronic hepatitis.

Non-B, Non-T Acute Lymphoblastic Leukemia in a Cat

  • A 7-year-old neutered male, domestic shorthair cat presented anorexia and lethargy. The complete blood cell count revealed severe non-regenerative anemia, lymphocytic leukocytosis, neutropenia, and thrombocytopenia. On the peripheral blood smear examination, medium to large lymphoblastic cells with moderate amounts of basophilic cytoplasm were observed in up to 70% of peripheral leukocytes. Feline leukemia and immunodeficiency viruses were not detected using a commercial diagnostic kit. While splenomegaly and blunt margins of the caudoventral liver were observed in abdominal radiography, changes in the intra-abdominal lymph nodes were not remarkable. Ultimately, flow cytometric immunophenotyping from the peripheral blood revealed a negative for B-cell markers (CD21-/CD79a-) and T-cell markers (CD3-/CD4-/CD5-/CD8-). Based on the hematological examination and the immunophenotyping assay, the cat was diagnosed with non-B, non-T acute lymphoblastic leukemia. Here, we report a rare case of non-B, non-T acute lymphoblastic leukemia to raise awareness and provide information on clinical symptoms and laboratory test and immunophenotyping analysis results.
