Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Two forms of pulmonary sequestration occur; intralobar sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We presented on case of intralobar pulmonary sequestration with A-V fistula. The patient was 8 years old male and chief complaints are mild fever and exertional dyspnea. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and its vessel directly communicated pulmonary vein in sequestrated segment. At time of operation, sequestrated lobe measuring 6x5x5cm, well demarcated at the lower lobe was noted. Aberrant, measuring 1.0cm in diameter and 1cm in length and directly connected pulmonary vein. After division and ligation of the aberrant artery, only excision of sequestrated lobe was performed and complication is not during postoperative course.

전폐절제술후 증후군이란 우전폐절제술후 심한 종격동의 이동과 회전에 의하여 기도의 폐쇄와 더불어 심한 호흡부전을 나타내는 질환이다. 유병를은 드물지만, 환자의 증상이 심각하게 나타나므로 유념해야 할 증후군이다. 삼성의료원에서는 1년전 우전폐엽절제술 시행받은 후 발생한 상기 증후군 환자를 대상으로 tissue expander를 이용하여 전위된 종격동 장기를 정상위치로 환원함으로써 좋은 결과를 경험하였기에 이를 보고 하는 바이다.

A 6-year-old female Maltese dog (body weight 2.0 kg) was referred to the Veterinary Teaching Hospital, Kangwon National University with primary complaints including exercise intolerance and heart murmur. Based on clinical and diagnostic findings including grade V/VI left basal continuous murmur, bounding femoral pulsation, left ventricular enlargement pattern in electrocardiogram, cardiomegaly with aortic bulging on the thoracic radiography, and shunt flow between aorta and pulmonary artery on the echocardiography, the dog was diagnosed as the left-to-right patent ductus arteriosus. The patent ductus arteriosus was successfully treated by lodging a single embolization coil with transjugular approach.

A 2-year old castrated male Alaskan malamute was referred with primary complaints of marked anemia, hemeglobinuria and depression. Laboratory tests revealed canine babesiois with severe anemia. The dog was treated by blood transfusion and beneril (diminazene aceturate, 3.5 mg/kg IM). Two days after Beneril injection, the dog suddenly showed ataxia progressing to paresis. MRI revealed irregularly diffused lesions in the cerebellum. The case was tentatively diagnosed as cerebellar encephalopathy caused by diminazene toxicity. The dog successfully recovered following steroid therapy.

Background and Objectives : Singers, the vocal equivalents of Olympic athletes, may present with vocal dysfunction in the singing voice before adverse effects are noted in the speech mode. For the classically trained singers, any imbalance in the respiratory, laryngeal or resonatory subsystems may manifest first in technical compensations. Therefore it is necessary that baseline values be available in order to differentiate technique related dysfunction from abnormalities within the phonatory system. Materials and Methods : Vocal efficiency measures were collected from 55 classically trained singers and 20 untrained adults. All singers was divided into 3 groups : (1) Singers who have normal larynx with out voice symptoms. (2) Singers who have vocal nodule without voice symptoms. (3) Singers who have vocal nodule with voice complaints. Results and Conclusion : Results were compared with each other. Differences in each group wert found, suggesting the need for separate normative data to be used for the evaluation of the classically trained singers.

One four-month-old female shepherd mongrel dog with chief complaints of abdominal distension and dysponea was referred. The authors performed physical examination, hematological examination, X-ray examination, exploratory laparotomy, electrocardiography and angiography as antemortem investigation in addition to postmortem examination. This patient revealed complicated congenital cardiac anormaly including patent ductus arteriosus, both atrial and ventricular hypertrophy, distension in the base of the pulmonary artery, pulmonary stenosis, aortic insufficiency, patent foramen ovate and coronary arteriovenous fistula.

Recently, there is an increasing interest on railway noise and vibration. Accordingly, the importance of technology that can assess the vibration and structural noise caused due to the railway operation is increasing. Additionally, the need to construct a railway noise and vibration database which is a basic data for a reliable railway noise and vibration assessment as well is increasing. However, the problem is that the currently constructed database fails to include the factors influencing the occurrence of noise and vibration sufficiently. Therefore, In this paper, design a railway noise and vibration database that includes those factors sufficiently and which enables the continuous data management and accumulation.

As gastroduodenoscopy performed more frequently, case reports of human echinostomiasis are increasing in Korea. A Korean woman presented at a local clinic with complaints of abdominal pain and discomfort that had persisted for 2 weeks. Under gastroduodenoscopy, two motile flukes were found attached on the duodenal bulb, and retrieved with endoscopic forceps. She had history of eating raw frog meat. The two flukes were identified as Echinostoma hortense by egg morphology, 27 collar spines with 4 end-group spines, and surface ultrastructural characters. This report may prove frogs to be a source of human echinostome infections.

Pulmonary aspergillosis is a relatively rare disease in Korea. Immune suppressive patients, resulting from misuse or long term therapy of antibiotics, steroid and anticancer chemotherapeutics tend to be vulnerable to pulmonary aspergillosis. This study is made to illustrate the clinical features, preoperative diagnosis and surgical role in the management is this diseases. In retrospective study of operative cases from May 1980 through July 1986, 10 cases were analysis. Hemoptysis and blood tinged sputum were the most common chief complaints. Major underlying pathology was cavitary lesion or bronchiectasis caused by pulmonary tuberculosis [5 cases]. 2 cases were disseminated form due to immune suppression. Anatomic location of lesion was in the upper lobe in half case. Systemic antifungal agent is helpful for invasive aspergillosis and Resection is the treatment of choice for localized fungus balls and symptomatic localized forms.

Intralobar pulmonary sequestration is rare congenital lung disease, in which systemic artery supplies a congenitally cystic portion of the lung. Patient was 19 years old male whose complaints were fever, chest pain and sputum. Chest film showed a round homogenous density and air-fluid level at the left lower lung field and on bronchogram, contrast materials did not enter the abscess pocket. By operation multiple lung abscess pockets at the lower lobe were noted. An aberrant artery, measuring 0.4cm. in diameter and 2 cm. in length, arised from the aorta just above the diaphragm and entered the pusterior basal segment of left lower lobe. After division of the artery, left lower lobectomy was done ana postoperative hospital course was uneventful. Pathologic findings were multiple lung cysts which were not connected to the left lower lobe bronchi and an aberrant artery which showed elastic lamillation and mild sclerotic change.
