[Retracted]Assessing Nutritional Status in Outpatients after Gastric Cancer Surgery: A Comparative Study of Five Nutritional Screening Tools ([논문철회]위암 수술 후 외래환자의 영양상태 평가: 5가지 영양검색도구의 비교연구)

  • Objectives: This study aimed to examine the characteristics of patients according to their nutritional status as assessed by five nutritional screening tools: Patient-Generated Subjective Global Assessment (PG-SGA), NUTRISCORE, Nutritional Risk Index (NRI), Prognostic Nutritional Index (PNI), and Controlling Nutritional Status (CONUT) and to compare the agreement, sensitivity, and specificity of these tools. Methods: A total of 952 gastric cancer patients who underwent gastrectomy and chemotherapy from January 2009 to December 2012 at the Samsung Medical Center were included. We categorized patients into malnourished and normal according to the five nutritional screening tools 1 month after surgery and compared their characteristics. We also calculated the Spearman partial correlation, Cohen's Kappa coefficient, the area under the curve (AUC), sensitivity, and specificity of each pair of screening tools. Results: We observed 86.24% malnutrition based on the PG-SGA and 85.82% based on the NUTRISCORE among gastric cancer patients in our study. When we applied NRI or CONUT, however, the malnutrition levels were less than 30%. Patients with malnutrition as assessed by the PG-SGA, NUTRISCORE, or NRI had lower intakes of energy and protein compared to normal patients. When NRI, PNI, or CONUT were used to identify malnutrition, lower levels of albumin, hemoglobin, total lymphocyte count, total cholesterol, and longer postoperative hospital stays were observed among patients with malnutrition compared to those without malnutrition. We found relatively high agreement between PG-SGA and NUTRISCORE; sensitivity was 90.86% and AUC was 0.78. When we compared NRI and PNI, sensitivity was 99.64% and AUC was 0.97. AUC ranged from 0.50 to 0.67 for comparisons between CONUT and each of the other nutritional screening tools. Conclusions: Our study suggests that PG-SGA and NRI have a relatively high agreement with the NUTRISCORE and PNI, respectively. Further cohort studies are needed to examine whether the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

  • Hazard Analysis Critical Control Point (HACCP) systems were developed to ensure a high level of food safety and reduced risk of foodborne illness. This paper focuses on significant issues associated with the implementation of HACCP; it provides an overview on recent literature. The structure of the paper follows six groupings of issues in the international literature of HACCP: (1) comparative studies and unification plan between HACCP and other food safety regulations; (2) verification of the HACCP system's effectiveness in improving food safety; (3) establishment of critical control point (CCP) for various foods HACCP model development; (4) expansion of HACCP application in the various fields and small businesses;(5) the impacts of HACCP on consumer's preferences and firms' financial performance in food industry; (6) HACCP and technological changes. The paper concludes with some suggestions for the future research in order to promote safe food supply chain for global customers.

  • As the 'hyper-connected society' has emerged through the 'Fourth Industrial Revolution, public interests as well as social dangers have increased. Above all, the risk of infringement of information, including confidential personal information, is dramatically increasing. As the hyper-connected society has been realized, even if only one of the internet devices is hacked, there would be a danger that the ripple effect of such a hacking spreads to the whole network. Therefore, the necessity and importance of information security, including cybersecurity, has been increasing. In other words, the stability of cyberspace and internet space is becoming more important. As a result, the Korean government is seeking to build a legal system related to information security, which would be able to cope with the information infringement problem in the hyper-connected society. However, it seems that the government is still struggling with the direction of building such a legal system. In this context, a comparative review examining the legal systems of advanced foreign countries will provide meaningful implications as to what kinds of legal policies we should devise and implement for information security. In particular, the U.S. legislative act that actively responds to the cybersecurity violations is worthy of reference. For this reason, this article systematically analyzes the current status of the U.S. cybersecurity laws. Especially, this article focuses on the "Cybersecurity Information Sharing Act of 2015"(hereinafter "CISA"), that was recently enacted by the U.S. congress. The CISA prescribes the systemic and detailed information-sharing between national and private entities. The CISA, that actively promotes information-sharing, is full of suggestions for us, in that information-sharing is an effective way to properly realize information security in today's hyper-connected society.

  • This study compared policies of elderly abuse of long-term care facilities on WHO, Main countries for improvement of Korean policy. So, This study reviewed policies of elderly abuse of long-term care facilities on WHO, Main countries. So, This study reviewed risk factors that may increase the potential for abuse of an older person can be identified at individual level, care-woker levels, socio-cultural levels and facilities levels. The results of the study were as follows: First, The openness of long-term care facilities are emphasized on many countries. There are a need for a policy considerations such as policies of Ombudsman, Adult guardianship for visiting facilities, external monitoring. Second, There was a difference of facilities infrastructure and workers of treatment condition long-term care facilities each other countries. So It is important to improve facilities infrastructure and workers of treatment condition. Third, It is necessary to consider traits of elderly in long-term care facilities. Especially, it is required to manage dementia elderly and elderly on night time in long-term care facilities. Finally, implications and future directions of policies of elderly abuse of long-term care facilities were discussed based on the finding of the study.

Mid-Term Outcomes and Angiographic Patency of Redo Coronary Artery Bypass Grafting: A Comparison between OffPump and On-Pump Surgery

  • Background: We evaluated the mid-term outcomes and angiographic patency of redo coronary artery bypass grafting (CABG). Methods: Of 2,851 patients who underwent isolated CABG at Seoul National University Hospital from 2000 to 2017, 88 underwent redo CABG. Patients' mean age at redo CABG was 66.0±8.0 years. The mean interval between the first-time and redo CABG was 113.0±62.4 months. The mean follow-up duration was 86 months. Early and mid-term clinical outcomes were evaluated. Angiographic patency rates were evaluated early (1-2 days), 1 year, and 5 years after surgery. Comparative analyses between on-pump and off-pump CABG were also performed. Results: The culprits for reoperation were previous grafts (65.6%), native coronary vessels (17.8%), and both (16.7%). Off-pump CABG was performed in 75 cases (85.2%), and the mean number of distal anastomoses was 1.8±0.8. The saphenous vein (39.7%) was used most frequently, followed by the right internal thoracic artery (28.4%), right gastroepiploic artery (21.3%), left internal thoracic artery (7.8%), and radial artery (2.8%). Operative mortality was 1.1%. The overall survival, cumulative incidence of cardiac death, and cumulative incidence of major adverse cardiac events were 71.3%,12.0%, and 23.3% at 5 years after surgery, respectively. The overall angiographic patency rates were 95.7%, 90.1%, and 92.2% on early, 1-year, and 5-year angiograms, respectively. The angiographic patency rates of saphenous vein grafts were 93.1%, 85.6%, and 91.3% on early, 1-year, and 5-year angiograms, respectively. No significant differences in clinical outcomes or angiographic patency rates were observed between the on-pump (n=13) versus off-pump (n=75) groups. Multivariable analysis revealed that age (hazard ratio [HR], 1.07; p=0.005) and chronic kidney disease (HR, 3.85; p=0.001) were risk factors for all-cause mortality. Conclusion: Redo CABG could mostly be performed using the off-pump technique and did not show increased operative mortality and morbidities.

The Effect of the General Characteristics and Fall Efficacy of the Elderly on the Needs of Health Care Programs (노인의 일반적 특성과 낙상효능감이 건강관리프로그램 요구에 미치는 영향)

  • Objective : This study was to examine the effects of general characteristics and fall efficacy on health care program needs through a comparative analysis of general characteristics and health care program needs for the elderly. Methods : In this study, we analyzed data from 118 elderly people with t-test and ANOVA for comparison of fall effectiveness and health care program requirements and with regression for general characteristics and effects of fall effectiveness on health care program needs. Results : The results were as followed: First, there was a significant difference in gender(p=.00), age (p=.00),educational background(p=.00), and existence of spouse(p=.00), and no significant difference in income(p=.17) based on fall efficiency according to the general characteristics. There was a significant difference in gender(p=.00), age(p=.00), educational background(p=.00), and income(p=.00) and no significant difference in existence of spouse (p=.46) based on health care program needs according to the general characteristics. Second, it was shown that the higher age, the higher the income, the higher the effectiveness of the fall, and in case of woman, the more significant the effect of the exercise program had.. The explanation was 56 percent. Third, the higher the age, the higher the income, and the higher the fall efficiency, the more meaningful the cognitive program demand. The explanation was 48 percent. Conclusion : The data from this study showed the importance of fall efficacy in older people with significantly higher risk of falls than in other age groups, and the need for exercise and cognitive management programs to prevent falls.

Lower levels of albumin, hemoglobin, total lymphocyte count, and total cholesterol and longer postoperative hospital stays were observed among patients with malnutrition compared to normal patients when NRI, PNI, or CONUT were applied. Relatively high agreement for NUTRISCORE relative to PG-SGA was found; the sensitivity was 90.86%, and the AUC was 0.78. When NRI, PNI, and CONUT were compared, the sensitivities were 23.72% for PNI relative to NRI, 44.53% for CONUT relative to NRI, and 90.91% for CONUT relative to PNI. The AUCs were 0.95 for NRI relative to PNI and 0.91 for CONUT relative to PNI. Conclusions: NUTRISCORE had a high sensitivity compared to PG-SGA, and CONUT had a high sensitivity compared to PNI. NRI had a high specificity compared to PNI. This relatively high sensitivity and specificity resulted in 77.00% agreement between PNI and CONUT and 77.94% agreement between NRI and PNI. Further cohort studies will be needed to determine if the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

Meta-analysis of the Effects of Untact Convergence Exercise Programs on Balance, Gait, and Falls Efficacy of Parkinson's Disease Patients (비대면 방식을 융합한 운동 프로그램이 파킨슨 환자의 균형, 보행, 낙상 효능감에 미치는 효과에 관한 메타분석)

  • In this study, a meta-analysis was conducted to find out the effects of convergent "untact" exercise programs on the balance, gait, and falls efficacy of Parkinson's patients. In this study, the PICO were Parkinson's disease, virtual reality program and an e-exercise program intervention, a comparative intervention with the contact interventions applied, and without any interventions applied, outcomes were balance, gait, and fall efficacy. The survey period was between January and February 2021, and five papers were selected and subjected to meta-analysis. Risk of bias, a tool commonly used for randomized control trial studies, was used. Furthermore, RevMan program was used to investigate effect size of untact exercise programs. The result of the meta-analysis showed that the effect size of balance of the untact exercise program group was 1.27 (SMD=1.27; 95% CI 0.72 to 1.83) (Z=4.51, p<0.001), the effect size of fall efficacy was 0.52 (SMD=0.52; 95% CI -0.000 to 1.03) (Z=1.96, p=0.05), and the effect size of gait was -0.40 (SMD=-0.40; 95% CI -1.00 to 0.10)(Z=1.32, p>0.05). A total of 5 literature analysis showed that untact exercise program is more effective in improving balance and falls efficacy than contact exercise program, but no difference in effectiveness was shown on gait.

Comparative Analysis of Survival Period by Technological Capabilities of Innovative SMEs in the Service Industry (기술수준에 따른 서비스업 혁신 중소기업의 생존기간 비교분석)

  • The survival period according to technological capability was analyzed for about 22,500 innovative SMEs in the service industry. The survival period was defined as the occurrence of overdue and default, and the technological capability was divided into two clusters. As a result of estimating the survival period according to technological capability through Kaplan-Meier analysis, it was confirmed that the estimated survival period of T1-T4 grade service innovative SMEs was significantly greater in both overdue and default. As a result of the analysis of the Cox proportional hazard model applying the control variable, it was confirmed that the higher technological capability, the lower the risk in the group of start-up companies. However, in the group of non-start-up companies the technological capability did not significantly affect the survival period, and the influence of the variables related to the size of the company was found to increase. Therefore, the technological capability is meaningful as additional information that has a significant effect on the survival period of innovative SMEs in the start-up companies group of service industry. In addition, it was concluded that it is necessary to reflect the technological capability when establishing the SME support and promotion policy of the start-up companies group in the service industry.

Effects of Investment Behavior Factors and Sub-attributes for Lots Shopping Building on Investment Intention: Comparative Studies between Factor Level and Attribute Level and among Investors Segmented by Investment Intention (분양상가 투자행동요인과 속성들이 투자의도에 미치는 영향: 요인과 속성수준에서의 비교 및 투자의도 세분화집단 간 비교)

  • Real estate investment behavior factors are divided into profitability, risks (stability), liquidity, and regulation (deregulation) factors. The sub-attributes of the investment behavior factors are generally formative indicators. Unlike reflection indicators, formative indicators can identify not only the influence of investment behavior factors on dependent variables, but also the influence of sub-attributes on dependent variables. Therefore, theoretical and practical needs of comparing the influences of factors and sub-attributes on dependent variables has been suggested. In this study, in order to provide information that help marketing for lots shopping building, both the causality between investment behavior factors and investment intention and the causality between sub-attributes and investment intention were comparatively studied for each of the three investor groups: the whole group, the group with high investment intention and the group with low investment intention. For this purpose, a survey and multiple regression analyses were conducted on 237 existing investors in the customer DB of a company that have been developing and selling lots shopping building in the metropolitan area and Sejong City. At the factor level, the effects of profitability and regulation were significant in the whole group and the group with low investment intention, but the effects of risk and liquidity were significant in the group with high investment intention. At the sub-attribute level, all three groups showed different results.
