The development of a community-based medical education program in Korea

  • Purpose: The introduction of community-based medical education would help improve the quality of primary care. This paper suggests learning objectives and an educational program for community-based medical education. Methods: The educational program was developed in a 1-day consensus workshop. Twenty experts, including faculty members from family medicine department of a college of medicine in Seoul and community-based preceptors, participated in the program. A needs-assessment survey was conducted among community-based preceptors before the workshop. Through this workshop, we derived learning objectives and a standardized curriculum for community-based medical education. Results: In the questionnaire before the workshop, community-based preceptors voiced concerns over the program's potential costs and the time required for teaching. The learning objectives and educational programs derived from the workshop's consensus were consistent with the characteristics of the primary care. Based on the results of this workshop, the joint expert team developed a standard educational program on two core topics: clinical teaching and mentoring. Conclusion: From this curriculum development process, participants could construct a more standardized curriculum for community-based medical education. Future studies are needed to evaluate the long-term outcomes of these educational programs, such as the learners' satisfaction and achievement.

Application of Program Theory and Logic Model to Evaluate Immunization Disparity Program for Children under 3 Years

  • With the outbreak of coronavirus disease 2019 (COVID-19) pandemic, health policymakers are adopting new policies regarding the issue of immunization disparities, especially for children in low-income communities of color who lack awareness and thereby access to vaccines. The purpose of this paper is to propose an evaluation framework using program theory-based evaluation approach and logic model to analyze and evaluate the immunization disparities in children aged 19-35 months. Data is collected from New York City department of Health and the U.S. Census Bureau for Northern Manhattan Start Right Coalition program which consists of 19,800 children, and the community-provider partnership includes 26 practices and 20 groups. Program theory is used to evaluate this community-based initiative with the logic model which is a visual depiction that illustrations the program theory to all stakeholders. The logic model highlights the resources, activities, outputs, outcomes, and impacts of the program to guide to planners and evaluators and to call attention to the inadequacies or flaws in the operational, implementation and service delivery process of the program in offering a new perspective on the program. This framework adds to the literature on evaluations of immunization disparities in determining whether evaluators can definitively attribute positive immunization outcomes in the community to the program and conclude whether it has potential in expanding or duplicating it to other similar settings, especially in other rural areas of the United States, and abroad, where routine immunization equity gaps are wide due to income, racial and ethnic diversity, and language barrier.

Needs Assessment of Elderly for Community-based Long-Term Care (요양보호 대상노인의 서비스 요구도 평가)

  • Purpose: Needs of health-welfare-medical service for the elderly is rapidly increasing in Korea. The purpose of this study was to evaluate the needs of health-welfare-medical service for the long-term care elderly in the community and to compare differences by their characteristics. Method: Needs assessment was completed in the homes of 598 persons over 65 years by using the tool of needs assessment, between November and December, 2003. We examined all the health-welfare-medical service of elderly in the community. Data were analyzed using SAS program. Result: The needs of the long-term care elderly in community was largest 'home visiting service of visiting nurse(87.5%)', and then 'religious, psychological and emotional support(73.9%)', 'home visiting therapy of physician(58.5%)', 'social support service(55.7%)', 'health improvement program of public health center and social welfare center(51.8%)', 'health examination(48.8%)' followed. The difference of health-welfare-medical service needs among characteristics(age, medical security, caregiver existence, and regions) was statistically significant by service contents(p<0.05 or p<0.01). Conclusion: We can apply it in the distribution of community resource and the development of service providing programs by figure out the needs assessment for the long-term care elderly in the community, and consequently, through this, realizing the health maintenance and promotion of the long-term care elderly.

Effects of a Community-Based Health Promotion Project for Elderly People (지역사회 중심 노인 건강증진 사업의 효과)

  • Purpose: The purpose of this study was to identify the effect of a community-based health promotion program for elders. Method: The research design was a nonequivalent control group experimental design. The participants were 415 elders who were above 65 years of age (mean age: 80.7, experimental group: 126, control group: 289). For 4 months, the health promotion project which consisted of health education and activity related to smoking, drinking, nutrition and exercise was given to the experimental group. Data were analyzed using descriptive statistics, ${\chi}^2$-test, and t-test with the SAS V8e program. Result: After the intervention, the experimental group showed a higher level of health promotion behavior compared to the control group in health education experience, exercise, smoking, and drinking. There were no differences between the experimental and control groups for nutrition, or high risk drinking and smoking. Conclusion: The results of this study show that the health promoting program for elders provides useful information in developing effective community-based programs and can be recommended as effective interventions to improve the health promoting behavior of elders.

Analysis on the Effects of Community Based Health Education Program for Pregnant Women (보건소 중심의 건강교실 운영효과 분석 -임부 대상 교육을 중심으로-)

  • This descriptive study aimed to determine the effects of community based health education program for pregnant women on their confidence in labor and breastfeeding. Data were obtained using several questionnaires and administered to 174 pregnant women who have participated (n=49) and have not participated(n=125) in health education program, and 244 mothers of infants who lived in one city in Kyunggi province. Summaries of finding were as follows; First, pregnant women who have participated in health education program showed significantly higher score in confidence in labor than those who have not. Also, confidence in labor was different according to the level of education. Second, pregnant women participated in health education program showed higher score in knowledge and attitude on breastfeeding, but the differences were not significant. Knowledge on breastfeeding was higher in multipara. More positive attitude on breastfeeding was found in working mothers and highly educated mothers. Third, $54.1\%$ of mothers continued breastfeeding over six months. Main reasons for discontinuing breastfeeding were lack of breast milk amount, and health problems in infants and mothers. In this study, we could identify the positive effects of health education program for pregnant women provided by public health center, and find the related factors of confidence in labor and breastfeeding. Community based health education program for pregnant women, especially for vulnerable group should be expanded.

The Effectiveness of Mental Health Problems Screening and Treatment Linkage in Children & Adolescents : Community Based Study Focused on ADHD and Depression (소아 청소년 정신건강 선별검사 및 치료 연계 효율성에 대한 연구 : 지역사회 중심 연구)

  • Objectives : This study evaluated the effectiveness of school-based mental health screening and treatment linkage programs, focusing on attention-deficit hyperactivity disorder (ADHD) and adolescent depression. Methods : All 11,158 children and adolescents aged 8-14 years in Seoul received a school-based mental health screening, consisting of the Korean versions of the ADHD rating scale and the Child Behavior Checklist (CBCL), for ADHD, and the Center for Epidemiological Studies Depression Scale (CES-DS) and the Suicidal Ideation Questionnaire-Junior (SIQ-JR), for depression. The high-risk children's and adolescents' diagnoses were confirmed using the Diagnostic Interview Schedule for Children-IV (DISC-IV). Treatment linkage programs were managed by the Seoul Metropolitan Community Mental Health Center (Program 1), the Seoul Metropolitan Office of Education (Program 2), or the Dongjak District Office of Education (Program 3). We estimated and compared the referral rates of the three programs. Results : Program 1 screened and referred 22.9% of ADHD youths to mental health services, Program 2, 68.8%, and Program 3, 40.0%. Program 1 screened and referred 22.8% of depressed youths to mental health services, Program 2, 53.8%, and Program 3, 88.9%. Key elements for successful screening and referral programs were an effective school/community mental health center/Office of Education network, the parents' financial support and perception of their child's mental health status as being problematic, and the teachers' active engagement. Conclusion : This is the first study investigating the effectiveness of school-based mental health screenings' linkage to treatment for primary and middle school students in Korea. An effective network for community mental health and improvements in parents' and teachers' perceptions regarding mental health are needed for more successful treatment linkage.

  • Purpose: Children in migrant villages in Kyrgyzstan have a high incidence of anemia and need effective health promotion programs. The purpose of this study was to evaluate a pilot study of a health promotion project for infants and toddlers based on community participation. Methods: This was a retrospective study conducted to evaluate the effectiveness of the project. The project was carried out by nurses at a public health center, community health workers, and local residents in a migrant village. After the operational system of the project was established, health assessments, child-care education, provision of vouchers for iron supplements, and activities to improve residents' awareness were conducted during 6 months among 100 children, of whom 85 were finally analyzed. Results: In international health projects, close cooperation of the project team with community residents and health workers is important. Access to the community-based program was feasible in the socially and economically poor migrant village, and improvements were shown in children's anemia and awareness of health care. Conclusion: By focusing on the effective aspects of this preliminary project, plans to utilize community health workers and promotion strategies can be added to the main project to improve health promotion among children in this area.

Development of Village Health Worker Training Program in Tuguegarao, Phillipines (필리핀 뚜게가라오 지역에서의 마을보건원 훈련 프로그램 개발 사례)

  • Objectives: The purpose of this study was to develop and execute the village heath worker(VHW) training program which based on short term overseas medical volunteer and medical mission. Methods: Through case studies of previous VHW program and review articles about the state of medical volunteer and medical mission, we developed the VHW training program. Pilot test of the program was carried out in Tugeugarao city, Phillipines on July, 2008. The subject were 32 persons from 15 villages around Tuguegarao city. After training we surveyed regarding the satisfaction of training and the degree of getting knowledge and skills. Results: Through article review, we got the common subjects of previous VHW training program and the limitations of overseas medical volunteer and medical mission. We developed VHW training program aiming to community health promotion through community participation and community change. The main text was "Where There Is No Doctor" of Hesperian Foundation. The program executed through 1 day intensive lecture and practice. The satisfaction of the program was 8.95±1.70 on 10 point rating scale. Conclusions: We strongly expect VHW training program can be available as new model of short overseas medical volunteer as well as medical mission if we could systemize it with the confirming subject before training , management after training and continuous support through connection with the field.

A Theoretical Approach of Social Ecological Model for School Health Promotion Program (학교 건강증진 사업을 위한 사회생태학적 모형의 이론적 접근)

  • Objectives: This study is to draw the design of the program which is improve school health promotion participation by applying the Social Ecological Model based on the literature review on the health promotion. Methods: Literature review was carried out based on 5 factors of social ecological model using computer search engines of Google, ProQuest, and Riss4U. Results; Social Ecological Model is consist of individual, interpersonal, institutional/organizational, community, and policy. Individual sphere is drawn from Health Belief Model, interpersonal sphere is Social Support Theory, institutional/ organizational sphere is institutional resources theory, community sphere is community model, and policy sphere is Social Marketing Theory. The literature review show that the important variables affecting health promotion exist in each sphere. Individual sphere has social economic status, age, sex, sensitivity and specificity of illness, self-efficacy. Interpersonal sphere has support and use of family, friend and neighbor. Institutional/Organizational sphere has environment service reliability and utility. Conclusions: Community sphere has distance, neighborhood safety, interrelationship among institutions. Policy sphere has cost, legislation advertisement, lobby and concern and leadership of Institution.

"Green Harmony" - The Horticultural Therapy Program for Holistic Health of College Students ("그린 하모니" - 대학생의 전인건강을 위한 원예치료 프로그램)

  • This research was to find out the effectiveness of the horticultural therapy program based on the therapeutic factors and the most meaningful experiences of the clients. For this purpose "Green harmony" program focused on harmony through horticulture was implemented. It is based on the preposition that holistic health is promoted with harmonious relationships with oneself, other people, community and nature. The program has three-fold structure: In innermost it deals with the task of the late adolescent, i.e., self identity, secondly the experience of communication and solidarity in the group, and finally extension of interest to the community. For the therapeutic intervention, questions using the metaphor of the activity were given to the clients for the establishment of self-identity, while group activity and the donation of the works let the second and third purpose accomplished. Outdoor activity, 'Tire garden project', has provided the clients with chances of the contact with nature, cooperation with other group members, and a contest to provoke enthusiasm. Also recycling and greening of the community were possible by utilizing old tires for the containers and by donating the final works to the community. For the evaluation of the result, the effect of the program on the stress of the college students was tested, and the most meaningful experiences during the participation to the program were asked to identify the therapeutic factors acknowledged by clients. "Green harmony" program has brought positive effects on the stress of the clients in spite of relatively short period of five weeks. The clients has acknowledged plant/nature contact and interactions between group members for the most meaningful experiences. This research suggests "Green harmony" horticultural therapy program based on the therapeutic factors is highly applicable for the general populations.
