주민(住民)의 전통의술(傳統醫術) 이용도(利用度) 조사연구(調査硏究) - 민속요법(民俗療法) 이용(利用)을 중심(中心) 으로 - (A Study on the Utilization Level of Traditional Medicine by Residents - On the basis of Use of Folk Medical Techniques -)

  • The general objective of this research is to study behavioral pattern of health care utilization and to measure the level of utilization of the traditional medicine. The specific objective is to study utilization pattern and content of folk medicine which is the indegenous medical technology recognized part of traditional medicine. This research was under taken to generate valid information that will provide basis data for formulating general direction for health education activities and for designing service package for general population. A social survey method was employed to obtain required information for the research activities, The survey field team consisted of 20 surveyors who all participated is an intensive 2 day training course. A total of 3091 households were visited and interviewed by the field team during the period 7 September to 6 October 1987. The major findings obtained from the information collected by the field survey are as follows ; 1) General characteristics of the study households 2562 households out of 3091 households visited were selected for final data process, 80.2 of the selected households were nuclear families ; 17.4%, extended families ; others 2.4%. Only 4.3 percent of the study population in the urban households indicated "no schooling" whereas 14.2% of the rural household members falls within this category. Study population in the urban areas are more protected against diseases by the national medical insurance system than those in rural areas. In their self appraisal of living standard, those who responded with low group are 39.6% and 50.3% respectively by urban and rural households. 2) Morbidity status Period prevalence rate for all diseases during the preceding 15 days before the date of the household interview v as 243,0 per 1,000 study population. For cases with the illness duration of within 15 days, the initial points of medical entry were diversied ; 56.9%, drug stores ; 30.9%, clinics and hospitals ; 4.6% folk medicine ; 1.7% clinics of Korean oriental medicine. Among the chronic case; with illness duration of over 90 days, 34.6% of these people utilized clinics and hospitals of modern medicine ; 31.6%, drug stores ; 18.6% clinics of Korean oriental medicine ; 6.8% folk medical techniques. Noticeable is the almost ten fold increase from the mere 0.9% in the utilization of Korean oriental medicine, whereas in the utilization of folk medicine, it is short of two-fold increase. 3) Folk medicine and its utilization Households that use folk medicine for relief and care of signs and symptoms commonly encountered in daily life, number 1969 households, which accounts for 76.9% of all the study households. This rather high level use of folk medicine is not different from rural to urban areas. The order of frequency of utilizing folk medicine among the study people are : the highest 14.3% for the relief of indigestion ; 8.6% for burns ; 5.1% for common cold ; 4.7% for hiccough ; and 4.2% for hordeolum. A present various procedures of folk medicine is being used to relieve all kinds of symptoms. 192 symptoms are identified at present. The most frequently used procedures of folk medicine appear to be based either on principles of the Korean oriental medicine or of scientific knowledge. Based on these survey findings, proposals for utilizing folk medicine are as follows First, this survey's findings will be feed back to both on the job training and on the spot guidance of community health practitioners, public health nurses and other peripheral work force in the health field, who are in daily contacts with community. This feed back will assure that the health personnel carry out their health education and information activities that are based on the utilization pattern of folk medicine as found in the survey result. Second, studies will be soon implemented that are designed to measure the efficiency and potency of these procedures and to improve these procedures of folk medicine were most frequently used by the community. Third, studies will continue to systematize medicinal plants and skills of Korean oriental medicine that are easily available at minimal cost in daily life for the prevention of diseases and management of emergency cases.

  • Objectives : This study aimed to identify the impact of social responsibility(CSR) activities by the hospital on the organizational commitment and value of the hospital by the internal members, to recognize the importance of staff carry out social responsibility activities effectively in the relevant departments, and use planning as a vital element or establishing a long-term hospital management strategy. Methods : Data were collected from 800 the administrative practioners of the hospitals in 200 hospitals nationwide. A structured questionnaire was used to measure the research variables using a 5-point Likert scale. The final sample consisting of 230 was analyzed using SPSS. Results : It was found that economic, legal, and charitable CSR have a statistically significant effect on organizational commitment and value of hospital Conclusions : The results suggest that hospitals should a fulfilling the obligation to abide by the norms and rules to be strictly observed, social services and charitable activities for the community are a strategy to increase the organizational commitment and hospital value in the long run. These activities should be actively pursued in a way that is sustainable and long-term so that they can improve to management performance.

수중운동 프로그램이 도시주변부 여성의 체력, 체구성과 보행특성에 미치는 영향 (The Effects of an Aquatic Exercise Program on Physical Fitness, Body Composition, and Gait Characteristics in Women)

  • Purpose: The purpose of this study was to identify the effects of an aquatic exercise program on physical fitness, body composition, and gait characteristics using trunk and pelvic angle in women living in urban fringe area. Methods: An aquatic exercise program consists of exercise in a swimming pool and self-help group activity with 16 women living in urban fringe (mean age: 63 years) for 8 weeks (twice a week for 2 hours). Physical fitness, body composition, trunk and pelvic angle using 2D video motion analyzer, and a questionnaire including socio-demographic variables were measured from July to August, 2013. Data were analyzed using a paired t-test with the SPSS/WIN 18.0 program. Results: At the end of 8 weeks intervention, there were significant decreases on body weight (p=.025), body fat (p=.030) and BMI (p=.011). There were significant increases on muscle strength (p=.001) and flexibility (p=.015). Trunk angle was significantly improved, which means participants less moved their body from side to side when they walked (p=.001). Conclusion: From this results, the aquatic exercise program could be an effective nursing intervention to improve physical fitness, body composition, and gait ability for women living in urban fringe area.

지역사회 노인을 대상으로 적용한 라이프스타일 중재의 형태와 효과에 관한 체계적 고찰 (Characteristics and Effects of Lifestyle Interventions for Community Dwelling Older Adults: A Systematic Review)

  • 목적 : 본 연구는 지역사회 노인에게 적용한 라이프스타일 중재의 형태와 효과를 알아보고자 하며, PRISMA 가이드라인를 통해 체계적 고찰을 시행하였다. 연구방법 : 2008년 1월부터 2017년 12월까지 국 내외 학술지에 개제된 논문을 국가과학기술정보센터(National Digital Science Library), RISS, PubMed, CINAHL을 통하여 검색하였다. 선정기준에 따라 최종적으로 20편의 연구를 분석하였으며, PEDro scale(Physiotherapy Evidence Database scale)을 사용하여 문헌의 질 평가를 하였다. 문헌에서 사용된 중재는 참가자들의 질환적 특성에 따라 분류하였다. 결과 : 라이프스타일 중재의 주요 구성요소는 중등도 이상의 운동과 같은 신체활동 증진과 건강한 식습관 형성을 위한 교육 및 훈련으로 확인되었다. 최종 분석된 20편 중 17편의 연구에서는 운동프로그램과 교육 또는 식단관리와 운동과 같이 두 가지 이상의 구성요소를 갖는 형태로 중재가 적용되었다. 노인의 건강 및 삶의 질을 평가하기 위해 다면적인 평가가 이뤄졌으며 그 중 생화학적 요인(biochemical factor)과 건강 및 웰빙(Health and well-being)을 평가하는 도구가 가장 많았다. 연구 분석 결과 지역사회 노인을 대상으로 한 라이프스타일 중재의 효과는 연구별로 상이하게 나타났으나, 14편의 연구에서 중재 직후 실험군과 대조군 간에 건강 및 삶의 질의 유의한 차이가 나타났다. 결론 : 본 연구는 노인의 질환적 특성에 따른 라이프스타일 중재기법을 선택하는데 도움을 줄 수 있을 것으로 사료된다. 향후 국내 지역사회 상황에 부합되는 중재의 개발 및 효과에 관한 연구를 통해, 작업치료적 라이프스타일 중재의 실행가능성에 관한 연구들이 체계적으로 이루어져야 할 것이다.

영국의 일반의약품(Over-the-counter drugs) 관리법의 의료정책적 함의 (The NHS Over-the-Counter Drugs Policy in UK: Its Experiences and Implications)

  • Changes in a drug's availability from prescription only to over-the-counter (OTC) status is of concern to physicians from both public health and individual patient perspectives. Government has generally been supportive of changes in medications from prescription(Rx) to over-the-counter (OTC) status in Korea, however, recognizing that there are both benefits and risks to any health care intervention, health care professionals are conservative in implementing changes to either the process or structure of health care. Changes in status of a drug from Rx to OTC can represent a change in both structure and process. Cost and convenience seem to be major factors in determining whether, given the choice, patients purchase a medicine over the counter or obtain it on prescription. With current arrangements, exemption from prescription charges provides an incentive to continue to obtain products on NHS prescription even when they are available over the counter. There is therefore no simple relation between the availability of over the counter medicines and the level of prescribing of deregulated products. The appropriate use of over the counter medicines-particularly those that have only recently been deregulated-places a burden of care on community pharmacists and calls for closer working relationships with general practitioners. In particular, systems for referral and for recording details of both prescribed and over the counter medicines need to be developed, and a direct route needs to be established for community pharmacists to report adverse drug reactions to over the counter products. Reclassification of prescription medicines-by making them available through pharmacies without a prescription-provides the opportunity for consumers to purchase a wider range of medicinal products without making a demand on NHS resources. There is, however, no simple relation between availability of over the counter medicines and demand for NHS prescriptions. In the late 1980s the UK government fuelled the over the counter market by making it easier to reclassify certain medicines from prescription only status to allow over the counter sale in pharmacies. To explore the influence of deregulation of medicines on NHS prescribing, this article presents analyses of consumer behaviour in using medicines and prescribers' attitudes to over the counter medication and collates findings from research. Policy makers should be aware that patients' expectations in relation to OTC medicines may be in conflict with evidence-based practice.

Progress in the Direct Application of Pharmacogenomics to Patient Care: Sustaining innovation

  • The application of the knowledge from the Human Genome Project to clinical medicine will be through both industrial drug development and the application of pharmacogenomics (PG) to patient care. The slow uptake of clinical innovations into clinical practice can be frustrating, but understanding the history of acceptance and sustaining medical innovation is critically important to position PG to succeed. This primarily means that PG tests must have legitimacy; they must be thoroughly validated, must be cost-effective, must be widely accepted by medical practitioners, must be supported by public policy, and must have a way of being easily incorporated into current medical practice. They must also lead to actionalble decisions by health care providers for their patients. Innovative PG assays should be tested in the best US laboratories, and reimbursement for testing must be accepted at the federal and state level. The companies providing these PG tests should be capable of supporting the interpretation and use of the test throughout medical practice. Advances such as the addition of PG information to drug labeling and the routine use of validated biomarkers to determine choice of cancer chemotherapy have been made. The PG research community must pay attention to the principles that have been previously described for acceptance and sustaining medical innovations in order for PG to be widely accepted in clinical medical practice.

소아.청소년 비만의 예방대책 (Prevention strategies for obesity in children and adolescents)

  • Prevalence of obesity in Korean children and adolescents has dramatically increased since the last 10-20 years. It is important to initiate prevention efforts early in childhood because prevalence of obesity in adolescence is the strongest predictor of its prevalence in adulthood. Intrauterine life, infancy, and preschool years may comprise the critical periods that are essential for the long-term regulation of energy balance therefore, obesity-prevention strategies should be initiated in utero and continued throughout childhood and adolescence. Families with high-risk children should be provided early education about maintaining normal weight. Encouraging physical activity and, especially, avoiding inactivity, are key challenges in the prevention of future obesity. Schools should be primarily involved in educating parents to discourage their children from excessively watching TV or playing computer games and eating unhealthy snacks and food. The involvement of medical practitioners is also important, especially, in the case of obese parents, obesity prevention strategies should be promoted from the first visit of pregnant women to the physicians. Health professionals can also be involved in obesity prevention because they are ideally equipped to identify young children at risk of obesity. Community and nation-wide efforts to increase awareness and promote environments that encourage physical activity and healthy nutrition are required.

  • School bullying has become a major social problem in Korea after the emergence of media reports on children who committed suicide after being victimized by bullies. In this article, we review the characteristics of bullying, and investigate the role of the pediatrician in the prevention of and intervention against bullying and school violence. Bullying can take on many forms such as physical threat, verbal humiliation, malicious rumors, and social ostracism. The prevalence of bullying in various countries is approximately 10% to 20%. In Korea, the prevalence of school violence is similar but seems to be more intense because of the highly competitive environment. From our review of literature, we found that children who were bullied had a significantly higher risk of developing psychosomatic and psychosocial problems such as headache, abdominal pain, anxiety, and depression than those who were not bullied. Hence, it is important for health practitioners to detect these signs in a child who was bullied by questioning and examining the child, and to determine whether bullying plays a contributing role when a child exhibits such signs. Pediatricians can play an important role in the prevention of or intervention against school violence along with school authorities, parents, and community leaders. Moreover, guidelines to prevent school violence, such as the Olweus Bullying Prevention Program, KiVa of the Finish Ministry of Education, and Connected Kids: Safe, Strong, Secure of the American Academy Pediatrics, should be implemented.

Lessons from Developing an Annotated Corpus of Patient Histories

  • We have developed a tool for annotation of electronic health record (EHR) data. Currently we are in the process of manually annotating a corpus of Norwegian general practitioners' EHRs with mainly linguistic information. The purpose of this project is to attain a linguistically annotated corpus of patient histories from general practice. This corpus will be put to future use in medical language processing and information extraction applications. The paper outlines some of our practical experiences from developing such a corpus and, in particular, the effects of semi-automated annotation. We have also done some preliminary experiments with part-of-speech tagging based on our corpus. The results indicated that relevant training data from the clinical domain gives better results for the tagging task in this domain than training the tagger on a corpus form a more general domain. We are planning to expand the corpus annotations with medical information at a later stage.
