  • The best way to care senile dementia patients is understanding the nature of disease. Care-givers can't understand the patients because the patients can't properly communicate with care-givers. Thus, care-givers are supposed to find out ways to understand them in the nature of disease. From this view, there seems no exception in essential attitude of care-givers toward the patients. Without positive understanding the nature of disease and respecting patient's rights as a human even though they are badly disorder in their word and behavior. There seems no expectation for thousands quality of care-giving.

  • Objectives; The purpose of this study was to develop a framework for home care and a Home Care Need Assessment Tool. Method 1. Identifying common domains in the provision of home care. 2. Charts of 253 home care clients were reviewed to obtain a classification of the nursing diagnoses. 3. A focus group methodology was used to develop the domains. 4. The tool was applied to 439 home care clients.(Kappa value=0.460-1.000, sensitivity, 0.444-1.000: specificity, 0.743-1.000). 5. Some refinements and extractions of the defining characteristics and related factors were made based on the results of the focus group. Results Home Care Need Assessment Tool consists of three parts; -Part I : factors related to basic conditions -Part II : a screening component that enables home care nurses to assess 30 multiple domains of 53 nursing diagnoses. -Part III : summative nursing diagnoses and nursing need intensity for the clients. Conclusion This tool provides a comprehensive assessment that helps the recognition of many strengths as well as problems of the clients. It will be usefully utilized in scheduling home care nursing plans and evaluating client outcomes.

  • Purpose: This study was aimed at investigating the recognition and attitude of inpatients for the purpose of facilitating the implementation of the home health care program in relation to the opening of the Home Care Department in P University Hospital in Pusan. Method: Data were collected from the questionnaires returned by 293 patients, who were admitted at P University Hospital. from May 1 to May 15, 2001 and analyzed using descriptive statistics and the Fisher exact test. Results: In regard to the previous information about the home health care program, 55.6% of the inpatients heard about the name through the advertisement in the hospital. the mass communication, and from acquaintances. The percentage of right answers regarding the knowledge of home health care program was 43.9%. In regard to the acceptance of the home health care program, 97.1 % of patients agreed with the implementation of the home care program. The reasons for acceptance were: the maintenance of the continuity of care, the alleviation of the family burden of time. Conclusion: For the stabilization and successful implementation of a home health care system, it should be accompanied not only by advertisement but also education for patients about the home health care program, and a post-evaluative study after the implementation.

A Study on Attitude Toward Oriental Medical Care Treatment Provided by Health Centers in a Designated Rural Areas (군보건소 한방진료실 이용자의 한방진료에 대한 태도분석)

  • Most of oriental medical care resources such as doctors and facilities are distributed in urban areas and approximately ten percent of them is in rural areas. However the aged population of over 60 years old in rural areas is higher than that in urban and these aged population prefer more oriental medical care than the other age group. Therefore, the government planned to carry out the oriental medical care demonstration project in a designated rural areas in 1990. The study was carried out to find out the utilization pattern of medical care and consumers attitude toward oriental medical care treatment provided by health centers. The interview survey was applied to collect the data and 187 patients, who visited to health centers to receive care in 1991, were selected by random sampling. The study results obtained were as follows : 1) Among the 187 respondents, male was 31.6% and female, 68.4%. 2) 73.8% of the respondents were the age of over 45 years old. 3) For the motivation of visiting the health center to receive oriental medical care, 37.4% of the respondents visited purposely according to announcement of oriental medical care and 26.2% of them made a decision by themself 20.3% of them was recommended by the neighbors. 4) The most frequent symptoms surveyed were the disease of the musculoskeletal system and connective tissue. 5) By the subjective judgement of the respondents from the result of the oriental medical treatment, recovered or improved cases represent 69.5%. It is considered that the oriental medical care was acceptable, and also the respondents were satisfied with the oriental medical care in terms of kindness of oriental medical doctors, treatment time and expenses of care.

A Study on Health Service Utilization for the Low Income Elderly in Korea (전국 저소득층노인의 보건의료이용과 영향요인 분석)

  • The purpose of this study was to analyze the relation amongst health status, health care costs. health service utilization among the low income elderly who were 60 years of age or older, earning a half of the average Korean family income. The cross-sectional descriptive survey research we conducted used families randomly sampled nationwide. The data were collected from July 12 to August 7, 1999 and the total sample was 1.259 household members (421 households). These were the major findings. 1. As for the health status. 72.4% of respondents fell ill in the last 1 month; 54% of respondents had chronic disease. 2. As for the health care cost. the cost of hospitalization and the medical treatment were 1.069,000 won and 226.000won. respectively. 59.3% of respondents experienced a burden from the monthly health care expenses. 3. As for the health service utilization for the last 3months. 28.5% of respondents didn't utilize the health service. In addition, 22.2% of respondents gave up a medical treatment because of economic situation (88.8%). 4. The statistically significant determinants of health service utilization are old age, female, living with a spouse, unemployed state, medicare, and more days sick. 5. It is shown conclusively that equity and efficacy of the health care policy are to be considered for lower income older adult.

Needs assessment for maternal health care in Ermera, Timor-Leste (동티모르 에르메라 지역의 모성보건사업 요구 분석)

  • Objectives: The purpose of this study is to obtain the basic data for the development of maternal health care by analyzing the status and needs of service target persons in Timor-Leste. Methods: The subjects were selected through the non-probability sampling method applying the FGI. Researchers interviewed 3 maternal health service managers, 6 midwives at Gleno and Railaco Health Centers and 2 women between 15 and 45 years of age. Results: In the results of on-site visit of the delivery facility and the FGI, we found poor sanitation in delivery room, lack of medical equipment related to antenatal consultation and delivery. In the case of the health center manager, the public health center provides various maternity health services, but the lack of the staff has difficulty in providing the service and managing the subjects. Midwives asked for regular maintenance education. Women in child bearing age living in mountainous areas had poor access to delivery facilities and lack of awareness of delivery services. Conclusions: It is necessary to increase the maternity management rate through regular maternity and maternal health check service and application to maternal management database, to improve the sanitation of the maternity clinic in the public health center, to strengthen the midwife competency program.

Factors Affecting of Long Term Care Hospital Patient's Intention of Transfer to a Nursing Home (요양병원 입원환자의 요양시설 이동의사에 영향을 미치는 요인)

  • Purpose: To examine factors affecting long-term care hospital patients' intention of transfer to a nursing home. Method: A questionnaire survey was conducted in Aug. 2007 that included 655 patients from 49 long-term care hospitals. The survey aimed to assess the patients' health status, family status, cost and intention of transfer to a nursing home. Institutional characteristics were analyzed from the nationwide database of Health Insurance Review & Assessment Service. The affecting factors were examined by employing chi-square test and logistic regression using SAS 8.2. Result: Of the subjects, 32.4% had intention of transfer to a nursing home. The intention of transfer to a nursing home was affected by moderate or severe pain, living together with the primary carer, high cost uncovered by insurance, and recognition of nursing home. Conclusion; For appropriate service utilization. a higher level of care is needed to satisfy patients at nursing homes and a balanced fee schedule is needed between long term care hospitals and nursing homes. It is desirable to encourage transfer to a nursing home at which nurses support patients and their families by giving information, coordination, and to make efforts to establish a reference system.

  • Background: This article reviewed the current status of home-based physical therapy infrastructure in long term care insurance and then solved the problem. Method: We used two forms of data that were acquired from ⅰ) the Ministry of Health & Welfare and the Family and National Health Insurance Corporation, ⅱ) a home-visiting health care program, and ⅲ) evaluation data from the Korea Health Industry Development Institute. The home-based physical therapy program was then analyzed. Results: The role and concept of home-based physical therapy was not clearly established. There were few home-based physical therapy programs in the community. The manpower of home-based physical therapists in the home-visiting health care program was very low. The role between home-visiting nurses (caregivers) and home-based physical therapists was mixed. Research and promotion regarding home-based physical therapy was poor. Conclusion: To establish a system of legal, long-term care insurance, we must increase the manpower of home-based physical therapists and the amount of research pertaining to the demand for home-based physical therapy.

Correlates of Depressive Symptoms and Stress among Korean Women Care-workers for Older Adults Dwelling in Community (재가노인 돌봄 요양보호사의 우울 및 스트레스 관련 요인)

  • Purpose: This study identified the prevalence of depressive symptoms and explored correlates of depressive symptoms among Korean women care-workers caring older adults living in community. Methods: A total of 465 participants were recruited for the study. Depressive symptoms was measured by the CES-D10 and distress was assessed using a single question given 5 Likert scale. Results: The prevalence of depressive symptoms and distress among care-workers were 32.5% and 32.0% respectively. Work environment safety, violence exposure experience, work-family conflict, weekly care work time, and poor health status were significantly associated with depressive symptoms. Only violence exposure experience and poor health status were associated with distress. Conclusion: Our findings suggest more attention on mental health of care-workers and their risky work condition such as violence.

  • In this county, the gap between the urban 'haves' and the rural 'have-nots' continues to be an increasing problem. WHO and UNICEF see primary health care(PHC) as the key to achieving an acceptable level of health throughout the world as a community development. PHC is essential health care made accessible to individuals and families in the community by means acceptable to them. It is the first level of contact of individual, the family, and community with the national health system. It includes at least education on health system. It includes at least education on health problems, promotion of food supply, MCH including family planning, immunization against infectious diseases, control of endemic diseases, treatment of common diseases and injuries, promotion of mental health, and provision of essential drugs. However, of the aboves, education concerning of mental health problems and the methods to identify, prevent, and control them is the principal step of establishment. In Korea, the category of PHC worker includes the physician as public doctor and nurse as primary health care practitioner and community health leader as village health worker. PHC workers of the aboves will thus function best if they are appropriately trained to respond to the health needs of the community. However in this country, since the national PHC service project launched in 1980, the government has not developed and performed appropriate and enough education and training activities. In light of above reasons, several categories of health education activities had been planned and performed being aimed at above specific target groups and the main focus was on the village health workers for about one year from July 1991 to July 1992 in Yeoju Kun of Kyonki Province. At the end of the period, evaluation of education input was carried out to measure the improvement of healthful life of people in terms of awareness, attitude, and practice. The totals of 80 village health workers, 13 public health practitioners and 9 public docters took in the course of health education for a few hours at every month and the evaluation works of educational effect were taken. The results the study were as follows. 1) Number of persons who realized the maxim "health care of the people is a duty of the government" increased after the education course, On the other hand, the rate of satisfaction on the effort of government for health promotion of the people decreased. 2) Public doctors and primary health care practitioners(nurses) liked and enjoyed the education schedule as a meeting of peer group. It provided chances of communication with staffs of Korea University Hospital. It was said that lectures covered great deal of knowledge and technic they urgently needed in the field. 3) After finishing the education course, more of village health workers(VHW) thought they adapted themselves to their roles and functions showing increased number of home visit and contact with primary health care practitioners by month. 4) In case of patient refer, VHW preferred primary health care practitioners to public doctors. 5) Capability of VHWs in most of their functions increased dramatically after when the education course finished except tuberculosis control.
