  • Purpose: This study was conducted to provide fundamental information for policy integration of the home health care system. Methods: Focused review was performed from 8 studies out of 48 literature related to home care needs. Results: Females utilized home care based on hospital and public health centers at a higher rate than males. While there had been more rate of the elderly above 60 year-old at hospital and community based home care, on the other hand below 45 year-old at public health center. The most common disease amongst users in all three types was cerebrovascular disease, the second was cancer based at hospitals, hypertension and arthritis at public health centers. There were more needs for those using artificial respiratory equipment, cancer patients, and puerpera. However, the needs for hospital and community based home care were very similar. Conclusions: Due to similar disease characteristics but varied severity at the three home care based settings, they should be managed with flexibility. Home care curriculum should be developed according to common disease type, and home care services should be strengthened for its expertise.

  • Purpose: The aim of this study is to identify factors affecting elders' intention to participate in healthcare programs. Methods: This secondary data analysis used data of 390 elderly people who lived alone and were enrolled in the Visiting Health Care Center of H-city. In 2014, questionnaires were used to collect data about demographics, social activity, depression, frailty, and intention to participate in healthcare programs. The data were analyzed by descriptive statistics and multivariate logistic regressions. Results: Among the participants, 51.8% intended to participate in the offered healthcare programs. The strongest factor affecting the intention was gender. Women were 4.85 times more likely to participate in the programs than men. The women's intention was associated positively with increased levels of frailty. The men tended to participate in the program as they got older and as the level of frailty and the number of chronic diseases increased. Conclusion: Gender-specific public healthcare programs for vulnerable elders living alone should be developed to maximize their participation in the programs and to promote good health. The healthcare programs must be tailored to the levels of the patients' physical functions.

  • Purpose: The purpose of this study was to investigate the ecological factors influencing school adjustment of adolescents from low-income families. Methods: Secondary data analysis was performed using data of 1,321 low-income adolescents in 123 regions found on the Survey on Service Satisfaction with Community Child Care Center. Results: The results of multi-level analysis identified the factors influencing school adjustment of low-income adolescents as follows: individual-level factors were gender, grade in school, and emotional problem; an interpersonal-level factor was family structure; organizational-level factors were length of time attending center and satisfaction with the service of the center; community-level factors were region and perception of community. Conclusion: The results suggest that low-income adolescents' adjustment to school is influenced not only by individual factors but also by diverse environmental factors. Community factors suggest that more education support systems and leisure facilities for adolescents need to be built in small and medium cities. Strategies to enhance positive perception of community are also needed for this population. Further, it is necessary to develop multi-level interventions to improve the school adjustment of adolescents from vulnerable social groups.

The lowest ADL score was reported on walking up to the stairs (5.05), for which the subjects did not need physical assistance, but needed a certain degree of supervision. The highest score was reported on self eating (6.74), followed by bowel management (6.60). 2. The mean IADL score of the subjects was 1.77 out of 4 points. indicating that the subjects were very independent in performing IADLs, without help from others. 3. In relation to general characteristics, the level of ADLs of the subjects was significantly different by age (F=6.65, P=.000), main activities (F=6.36, P=.001), perceived health status (P=4.66, P=.012), educational background (F=4.64, P=.03), marital status(F=4.62, P=.005), and major household income (F=4.15, P=.002). 4. • As this study is the descriptive research study that tries to present the basic data aiming to establish a role of nurses for the Community Welfare Center, by surveying the role expectation and the degree of role performance towards nurses for the Community Welfare Center that nurses for the Community Welfare Center and social workers recognize, it carried out the Questionnaire research targeting nurses who are working for the Community Welfare Center located in Seoul and social workers of institutions where nurses are working, and then analyzed the results, and the study results are as follows. Both the group of nurses and the group of social workers were highly recognizing the necessity and the importance of nurses for the Community Welfare Center, and expected that more effective services will be possible to be offered by means of that nurses do work for the Community Welfare Center. However, compared to the degree that social workers recognize a nurses specialty, the nurses thought that their specialty is not acknowledged in the Community Welfare Center, and even in case of duty friction between groups being felt while social workers and nurses are working together, nurses were experiencing further friction of duties. While nurses are well recognizing their roles, they are statistically and significantly recognizing the degree of role performance lower than expected, thus the difference between the role expectation and the role performance could be seen. Also, the group of nurses showed the higher role expectation towards nurses than the group of social workers, and in relation to roles of a counselor, referral resource, an advocator, an educator, a case manager, a and a coordinator, the degree of role expectation by the group of nurses is statistically and significantly higher than the group of social workers, thus it could be seen that there is difference in the expectation degree between both groups, as to a role of nurses for the Community Welfare Center. In particular, as to a role of universally medical services, the expectations between nurses and social workers showed mutual agreement, but in relation to a role of nurses for community in the more expanded sphere, social workers did not show a receptive attitude, thus it was indicating that there exists an element of role conflict. In relation to the role performance of nurses for the Community Welfare Center, nurses were feeling the disorder degree more than social workers, in all items of disorder factors, and there was difference in recognition between nurses and social workers, as to the priority of disorder factors. Because of, through this study, having been found the difference in recognition of role expectation, the degree of role performance, and a disorder factor between nurses and social workers, as to the nurses for the Community Welfare Center, it is required a study with a more diversified method on a role of nurses for the Community Welfare Center.

  • Purpose: The purpose of this study was to develop and evaluate a self-management program for community dwelling patients with chronic mental illness so that they could improve their daily living skills and interpersonal relationships. Methods: A nonequivalent control group pre-post test design was used. The participants with chronic mental illness were recruited from a community mental health center. Twenty-two participants were assigned to the intervention group and 19 to the control group. The self-management was provided by 8 trained nursing students, and consisted of 8 sessions including education and practice. Each session was composed of strategies to improve self-care of community dwelling patients with chronic mental illness. Baseline and post-intervention measurements were done using a daily living skill performance scale and interpersonal relationship instrument. Results: Scores for daily living skill performance were not statistically significant, but scores for interpersonal relationship were statistically significant. Conclusion: The results of this study show that the self-management program had a positive impact on patients with chronic mental disorders. Therefore, it is suggested that the program could be used as an effective nursing intervention for patients with chronic mental disorders being cared for in community mental health centers.

  • Purpose: The aim of this study was to verify psychosocial issues faced by psychiatric and community mental health nurse practitioners (PCMHNP) working in community mental health centers, and to identify the adaptation processes used to resolve the issues. Methods: Data were collected through in-depth interviews between December 2013 and August 2014. Participants were 11 PCMHNP working in community mental health centers. Analysis was done using the grounded theory methodology. The first question was "How did you start working at a community mental health center; what were the difficulties you faced during your employment and how did you resolve them?" Results: The core category was 'regulating within relationships.' The adaptation process was categorized into three sequential stages: 'nesting,' 'hanging around the nest,' and 'settling into the nest.' Various action/interaction strategies were employed in these stages. The adaptation results from using these strategies were 'psychiatric nursing within life' and 'a long way to go.' Conclusion: The results of this study are significant as they aid in understanding the psychosocial adaptation processes of PCMHNP working in community mental health centers, and indicate areas to be addressed in the future in order for PCMHNP to fulfill their professional role in the local community.
