Effect of the Tai Chi Exercise Program on Physical Function, Cognitive Function, and Quality of Life among Older Adults in the Community: A Preliminary Study (타이치운동 프로그램이 지역사회 거주 노인의 신체기능, 인지기능 및 삶의 질에 미치는 효과: 인지기능을 중심으로-예비조사 연구)

  • Purpose: To assess the feasibility, safety, and preliminary estimates of effectiveness of Tai Chi on the functional outcomes of older adults in the community. Methods: This was a mixed-method study that employed a single-group repeated measure design and in-depth interviews. Nine older adults were recruited from the community were recruited to participate in a Tai Chi program, conducted twice weekly for 6 months. Research outcomes included physical function, cognitive function, and quality of life, measured at intervals of 3 and 6 months. Findings: Tai Chi exercises were gradually conducted based on the health status of the older adults. All participants actively participated in the program with an average attendance of 90%. Consequently, the participants showed significant improvements in mobility and their memory recall ability at both 3 and 6 months. Additionally, the results of the Stroop test exhibited improvement 3 months after the commencement of the study program. Quality of life of the participants improved according to the mild cognitive impairment questionnaire, but it did not show significant improvement in health-related quality of life. Conclusion: The Tai Chi exercise program was a safe and, feasible program to improve the physical function, cognitive function, quality of life among the older adults in the community.

  • Purpose: This study was conducted to examine the effects of the oral care program for improving swallowing function of the elderly using welfare centers on depression, self efficacy, subjective oral health status and swallowing related quality of life. Methods: The intervention in this study was systematically developed through the six stages of Intervention Mapping Protocol (IMP) and was based on Mead's symbolic interaction theory and Bandura's self efficacy theory. A non equivalent control group pre and post-test design was conducted on a total of 37 elderly people (experimental group: 19, control group: 18) from D city. The oral care program was administered to the experimental group once a week for five weeks, totaling five times, and the exact program ran for 50 minutes. The collected data were analyzed using the SPSS/WIN 25.0 program. Results: Following the intervention, subjective oral health score, and swallowing related quality of life were significantly improved in the experimental group. Conclusion: The oral care program presented in this study was found to be effective in increasing subjective oral health status, and swallowing related quality of life for the elderly using welfare centers.

Effects of Korean Medicine Based Arthritis Management Program on Joint Function, Physical Fitness and Self-Care of the Elderly Women with Osteoarthritis. (한의약 관절염관리프로그램이 골관절염 여성노인의 관절기능, 체력 및 자가관리수행에 미치는 효과)

  • Objectives : The purpose of this study was to investigate the effects of Korean medicine based arthritis management program for community dwelling elderly women with osteoarthritis. Methods : A Nonequivalent control group pretest-posttest design was used for this study. Elderly people who agreed to participated in the study were assigned to a experimental group(n = 29) or a control group (n = 30). The Korean medicine based arthritis management program conducted for 12 weeks, 2 times a week. Study outcomes were measured by structured questionnaires from April, 2016 to July. For data analysis, independent t-test, Mann-Whitney U test and ANCOVA were performed using SPSS version 21.0. Results : Pain, stiffness, balance, strength of lower extremity, and self care activity were significant difference between the two groups in pretest and posttest(F = 8.23, p = 006), (F = 9.13, p = .004), (F = 5.74, p = .020), (F = 4.98, p = .030), (F = 8.47, p = .005). Conclusions : The Korean medicine based arthritis management program was effective on decreasing joint pain, stiffness, and increasing balance, strength of lower extremity and self care activity of community dwelling elderly women with osteoarthritis. This program was found to be useful in alleviating and managing joint symptoms of elderly women with osteoarthritis.

Global Health Project for Maternal Child Health in a Developing Country: Case Study in Tigray, Ethiopia (저개발국 모자보건 수준 향상을 위한 국제보건사업 전략 : 에티오피아 티그라이주 사례를 중심으로)

  • Purpose: The purpose of this study was to demonstrate a two-year global health project to improve maternal and child health (MCH) in Ethiopia. Methods: This is a descriptive case study. The target area is Kilte Awlaelo Woreda in Tigray Regional State, Ethiopia. A baseline survey was conducted to identify the needs of community residents and health care professionals. A MCH program was developed according to a project design matrix that included: infrastructure renovation of health centers; continuing education for midwives, nurses, and health extension workers (HEWs); and improvement of residents' MCH awareness. Project evaluation will examine the structure, process, and outcomes of the program. Results: The baseline survey showed low rates of family planning (31%) and antenatal and postnatal care use (36.1% and 69%, respectively). The institutional birth rate was 13.5%. Midwives and nurses received 2~4 educational programs about family planning and perinatal care. HEWs were also given practical education. Water and electrical infrastructure of all five health centers in the Kilte Awlaelo Woreda were renovated. Additionally, medical supplies and equipment were provided. Community health education on perinatal care, family planning, and personal hygiene was presented. Conclusion: This study highlights the role of nursing in global health and provides basic information on the development and outcomes of the global health project.

Effects of Clay and Drawing Art Program on Self-expression and Depression in Elderly Patients at a Long-term Care Hospital (점토 미술 프로그램과 그림그리기 미술 프로그램이 요양병원 노인 환자의 자기표현 및 우울에 미치는 효과 비교)

  • Purpose: Art program has been found to enhance mood such as increasing motivation, self-expression and decreasing depression in older adults. This study was conducted to investigate the effects of clay art and drawing on depression and self-expression among elderly people at a long-term care center. Methods: The research was a pre-test and post-test non-equivalent design. Sixty older patients (clay art program=28. Drawing art program=32) over the age of 65 were recruited from 2 long-term care hospitals to participate in the 5 weeks program. Each group was scheduled with 10,120-minute evaluation sessions, twice per week. Test measures were completed before and after the 5 week intervention period for all participants. The data was collected from April 27 to May 29, 2015. It was analyzed with SPSS 22.0 using chi-square, t-test and paired t-test. Results: There were no significant difference in the levels of depression (t=0.21, p=.830) and total score of self-expression (t=-0.10, p=.919) between the two groups. However, the pre-post scores of self-expression in the clay art program (t=0.22, p=.826) were improved compared to the drawing art program (t=0.80, p=.430). Conclusion: Further studies are needed to evaluate the effectiveness of clay art program for extended senior care applications.

Nurses' Perceived Needs and Barriers Regarding Pediatric Palliative Care: A Mixed-Methods Study

  • Purpose: This study aimed to describe nurses' perceived needs and barriers to pediatric palliative care (PPC). Methods: Mixed methods with an embedded design were applied. An online survey was conducted for nurses who participated in the End-of-Life Nursing Education Consortium- Pediatric Palliative Care (ELNEC-PPC) train-the-trainer program, of whom 63 responded. Quantitative data were collected with a survey questionnaire developed through the Delphi method. The 47 items for needs and 15 items for barriers to PPC were analyzed with descriptive statistics. Qualitative data were collected through open-ended questions and analyzed with topic modeling techniques. Results: The mean scores of most subdomains of the PPC needs were 3.5 or higher out of 4, and those of PPC barriers ranged from 3.22 to 3.56, indicating the items in the questionnaire developed in this study properly reflect each factor. The needs for PPC were divided into 4 categories: "children and adolescents," "families," "PPC management system," and "community-based PPC." Meanwhile, PPC barriers were divided into 3 categories: "healthcare delivery system," "healthcare provider," and "client." The keywords derived from the topic modeling were perception, palliative, children, and education for necessities and lack, perception, medical care, professional care providers, service, and system for barriers to PPC. Conclusion: In this study, by using mixed-methods, items of nurses' perceived needs and barriers to PPC were identified, categorized, and weighted, and their meanings were explored. For the stable establishment of PPC, the priority should be given to improving perceptions of PPC, establishing an appropriate system, and training professional care providers.

Development of a Model of a Day Care Center for Rural Elderly People (농촌형 노인 주간보호시설 모형개발)

  • Purpose: The purpose of this study is to develop a day care center model focused on public health institutions for the elderly residing in their homes. Method: Research design for this study was a mult-level research, which consisted of a related literature review, an Internet search for knowledge of the current situation at home and abroad, on-site interviews, questionnaires collected from a sample of residents in a rural area, and a key-informants approach. Results: 1) The subjects of service - Generalized service should be provided to the elderly, 65 years and older, regardless of their assets. 2) The contents of service - Providing pre-health oriented and post-social welfare service that can integrate and satisfy a wide variety of public health and welfare needs of the elderly would strengthen the health care service of a day care center for the elderly. 3) Delivery system - Basic-level local self-governments should become a central operating body, and establishing a properly adjusted delivery system to a rural area after considering the efficiency and the access of vulnerable rural areas is needed based on modification of 'a Special Law for Agricultural and Fishery Areas' (rural public health center>rural health sub-center ${\rightarrow}$ unified health sub-center ${\rightarrow}$ public health hospital (public health center) ${\rightarrow}$ public welfare office). 4) Facility - Public health facilities such as public health centers and sub-centers should be located in areas that can easily access the facilities. 5) Funding - For day care center for the elderly in local self-government, the central government should modify a relevant implementation of subsidy in and provide some facilities and service regardless of the degree of self reliance of local self-government. 6) Human resources - It is needed to guarantee the period of workers of a day care center for the elderly, at least 3 to 5 years, with considering their specialty on aged care and avoiding circulation based positions. Furthermore, appropriate specially trained personnel such as medical workers and social workers should be placed to take care of both health service and welfare through strengthening of 'rules of law of elderly welfare,' Conclusion: future research is needed to test the model through a demonstration study using a model which may be developed in the future and to standardize the appraisal criteria of people hoping to enter a day care center for the elderly.

A Survey Study on the Improvement Concepts for Community Facilities of Rural Villages Considering Elderly People's Welfare in the Aged Rural Areas (노인복지를 고려한 고령 농촌마을 공동이용시설의 활용 방향에 관한 조사연구)

  • The purpose of this study is to increase the utility in terms of the elderly's welfare. The data introduced in this paper were collected by the constructed interview method, and 192 aged interviewees were selected from 4 rural areas: Namhaegun, Uiryeong-gun Imsil-gun and Sunchang-gun. From the descriptive analysis of the data, the results derived are as follows: 1) The financial supports are needed to remodel rural village community facilities as senior centers. 2) It is expected that the facility be a center for the aged people's living in a village, and that have multiple functions of general care service network health care, meal support and making friendship spaces for them. 3) Share housing function is also added to the facility in emergency conditions.

Job Analysis of Geriatric Visiting Nurses (노인전담 방문간호사의 직무분석)

  • Purpose: The purpose of this study was to identify the duties and tasks of home visiting geriatric nurses using Development A Curriculum (DACUM) method. Method: The sample consisted of 107 geriatric visiting nurses who worked at community service centers in the Seoul metropolitan area. Job analysis was conducted at a DACUM workshop after that a web-based survey was given to participants to verify the accuracy of the duties and tasks of geriatric visiting nurse. Descriptive statistical analysis was conducted using SPSS 23. Results: A total of 8 duties and 56 tasks were identified as part of the job description of geriatric visiting nurses'. A task verification process was conducted. Overall mean ratings of the task importance were high. 'Recording' was identified as the most frequent duty, and 'Community program planning and operating' was identified as the most difficult duty. Conclusion: Duties and tasks that make up the job of geriatric visiting nurses were identified using the DACUM method. The resulting data will serve as the basis for the design of a curriculum development model for nurses involved in geriatric home visiting education programs, and will also be used to identify training needs and establish a standardized job description for geriatric visiting nurses.

Effects of a Fall Prevention Exercise Program on Body Composition, Muscle Strength and Balance, and Frailty in Community-Dwelling Elderly (낙상예방운동프로그램이 재가노인의 신체구성요소, 활동체력 및 허약수준에 미치는 효과)

  • Purpose: To examine the effects of a fall prevention exercise program on the community-dwelling elderly. Methods: The nonequivalent control group pretest-posttest design involved 16. subjects in the experimental group and 17 in the control group. The experimental group received the fall prevention exercise program for 50 minutes, three times each week for 12 weeks. Results: After program participation, the experimental group of subjects showed significantly higher lower limb strength higher endurance, and higher balance than the control group of subjects. The danger of being injured in a fall was also significantly lower in the experimental group. However, there were no significant differences in body constituent factors, agility, and flexibility between the two groups after the intervention. Conclusion: The 12 week fall prevention exercise program was effective in increasing lower limb muscular strength, endurance, balance, and body strength, and in decreasing the danger status of fall injuries. These results suggest that this fall prevention exercise program could be utilized as an effective nursing intervention modality in elderly persons.
