A Questionnaire Analysis about the Understanding of Allergic Diseases and the Attitude toward Oriental Medical Treatment (알레르기 질환 및 한방치료에 대한 일반인의 인식 조사)

  • Objective : This research was carried out to find out the general understanding of allergic diseases, the frequencies of allergic diseases and the attitude toward their oriental medical treatment(OMT) among common people. Methods : A total of 360 visitors at The 2nd International Exhibition on Oriental Medicine in 2000, participated in this research. We performed this survey with 22 items on the questionnaire. Results : Respondents knew the. fact that the allergic diseases were hereditary and could be affected by the living environment. 41.8% of the respondents had a past history of allergic diseases. Among the people who had allergic diseases, 63.5% had allergic rhinitis, which was the largest portion. Among them, 52.9% had a familial history. 81.5% of respondents with allergic diseases received western medical treatment for their allergies, 16.3% received both western and oriental medical treatment and 2.2% received OMT only. Among those who were treated for allergic disease, 62.0% were unsatisfied with the low effect of the treatment. To the point of excellence of OMT, 69.7%, which was the largest portion of respondents, believed that OMT could improve their constitutions. The problem with OMT was the high price of herbal medicine and this was the problem most frequently stated, by 51.4% of respondents. There was a positive attitude towards the effect of OMT, and respondents had a preference to receive OMT in the future(p<0.01). Conclusions : These days, it is a fact that the OMT for allergic diseases has a lower proportion to that of western treatment In order to increase the role of OMT, we think, oriental medical doctors need to develop an explanatory model of improving $people^{\circ}{\phi}s$ constitutions and should lower the cost of herbal medicine.

The Value of X-ray Compared with Magnetic Resonance Imaging in the Diagnosis of Traumatic Vertebral Fractures

  • Purpose: The purpose of this study was to evaluate the diagnostic accuracy of X-rays in patients with acute traumatic vertebral fractures visiting the emergency department and to analyze the diagnostic value of X-rays for each spine level. Methods: We retrospectively analyzed basal characteristics by reviewing medical records of 363 patients with adult traumatic vertebral fractures, admitted to the emergency center from March 1, 2014 to February 28, 2017. We analyzed spine X-rays and magnetic resonance imaging (MRI) scans to determine distribution according to the vertebral level, and we evaluated the efficacy of X-rays by comparing discrepancies between X-rays and MRI scans. Results: For a total of 363 patients, the mean age was 56.65 (20-93) and 214 (59%) were males. On the basis of X-rays, 67 cases (15.1%) were of the cervical spine, 133 cases (30.0%) were of the thoracic spine, and 243 cases (54.9%) were of the lumbar spine. In particular, the thoracolumbar region (T11-L2) was the most common, with 260 cases (58.7%). In X-rays, fractures were the least in the upper thoracic region (T1-T3), whereas MRI scans revealed fairly uniform distribution across the thoracic spine. Sensitivity of X-rays was lowest in the upper thoracic spine and specificity was almost always greater than 98%, except for 94.7% in L1. Positive predictive value was lower in the mid-thoracic region (T4-T9) and negative predictive value was slightly lower in C6, T2, and T3 than at other sites. Diagnostic accuracy of X-rays by vertebral body, transverse process, and spinous process according to fractured vertebral structures was significantly different according to vertebral level. Conclusions: Diagnostic accuracy of X-rays was lower in the upper thoracic region than in other parts. Further studies are needed to identify better methods for diagnosis considering cost and neurological prognosis.

  • The rahmen bridge is well known common type of bridge in which all members are connected rigidly. The rahmen bridge is built for several situations because it has many advantages such as no need of bridge bearing system, easy of maintenance, reduction of the cross-sectional area of superstructure, and relatively low construction cost compared with other bridge types. Recently, to lengthen the span of rahmen bridge system, steel-concrete composite beam is used for superstructure of rahmen bridge instead of normal concrete girder with slab. However, member forces are increased because of extension of span length of superstructure and substructure is designed and constructed inefficiently when steel-concrete composite rahmen bridge is designed. In this study, new-type steel-concrete composite bridge is suggested. New-type steel-concrete composite rahmen bridge is adopted hinge connection between abutment and foundation for the reduction of the bending momemt at the foundation. In this study, we present the results of experiment conducted to estimate the load carrying capacity of new-type steel-concrete composite rahmen bridge and the structural characteristics of hinge connection.

Hydrodynamic Analysis of Submerged Floating Tunnel Structures by Finite Element Analysis (유한요소해석을 통한 해중터널의 유체동역학 해석)

  • As transportation systems for connecting lands and islands, oversea long-span bridges, underwater tunnels, and immersed tunnels have been mainly used so far. Submerged floating tunnels (SFTs) moored under specific water depth are one of the newest oversea transportation system. Compared to other existing systems, the new system requires relatively less construction cost and time. But, there is still no construction example. For reasonable design of the tunnel and mooring lines the rational structural analysis should be firstly performed. Unlike common transportation structures, the submerged tunnels are mainly affected by the wave, vary irregular excitation component. So, the analysis scheme might be difficult because of the characteristics of the submerged structures. This study aims to suggest the rational global performance analysis methodology for the submerged tunnels. Using ABAQUS the dynamic response of the experimental models studied by KIOST (2013) was investigated considering regular waves. By comparing the simulation results with the experimental results, the feasibility of the numerical simulation was verified. Using the suggested method, the effects of initial inclination of the tethers and draft of the tunnel on the dynamic behavior were studied. In addition, dynamic response of a SFT under the irregular wave was examined.

  • In Korea, many mixed-used development projects(MXD) in mega project size are currently active. As these mega projects require long-term construction and execution and involve by different subjects in different fields, their completion breeds various problems, such as conflicts among project participants due to their complicated interest relationships and inaccuracy in predicting the economic performance of the projects due to inappropriate facility capacity planning. To solve these problems, it is essential to apply value engineering (VE) at the planning phase of the project, which can result in the best possible cost reduction and improvement of project performance. However, not many projects are actually implementing VE because of the lack of available information, as well as the limitations due to uncertainty in the early period of project execution. Therefore, this study aimed at proposing VE Team Building, Quality Model Performance Indexes, Space Model so as to resolve common mega project problems and overcome VE application limitations at planning stage. The result of this study is expected that the data can be utilized as basic data to apply VE at the planning phase of the large complex projects.

  • In these days, internet environment are very quickly development as well on-line service have been using a online for the mission critical business around the world. As the amount of information to be processed by computers has recently been increased there has been cluster computing systems developed by connecting workstations server using high speed networks for high availability. but cluster computing technology are limited for a lot of IT resources. So, grid computing is an expanded technology of distributed computing technology to use low-cost and high-performance computing power in various fields. Although the purpose of Grid computing focuses on large-scale resource sharing, innovative applications, and in some case, high-performance orientation, it has been used as conventional distributed computing environment like clustered computer until now because grid middleware does not have common sharable information system. In order to use grid computing environment efficiently which consists of various grid middleware, it is necessary to have application-independent information system which can share information description and services, and expand them easily. This paper proposed new database architecture and load balancing for high availability through Grid technology.

A study of elementary school pupils using traditional herbal medicines (초등학생에서 한약 이용 실태에 관한 연구)

  • Purpose : Traditional herbal medicines may be overused in Korea. Expenditures and inaccuracies of hospital prescription are increasing gradually. So we investigated the frequency and nature of herbal medicine use among elementary school pupils. Methods : We interviewed students of three elementary schools located in Gwangju with survey papers. We analyzed 905 returned surveys. Results : They consisted of 421 males and 484 females. Five hundred ninety (65.2 percent) of pupils who have taken herbal medicines since birth. First grade made up 74.1 percent, 2nd grade 63.0 percent, 3rd grade 64.8 percent, 4th grade 63.3 percent, 5th grade 65.5 percent, and sixth grade 61.9 percent, respectively. The mean frequency of herbal medicine use was 3.2 times. 6th grade was 3.3 times, 5th grade 3.9 times, 4th grade 3.7 times, 3rd grade 2.7 times, 2nd grade 2.7 times, and 1st grade 2.6 times, respectively. The mean cost of herbal medicine use was from 50,00-100,000 won. The mean age of pupils who took their 1st herbal medicine was 55 months, 6th grade 67 months, 5th grade 58 months, 4th grade 54 months, 3rd grade 55 months, 2nd grade 51 months, and 1st grade 47 months. The most common reason of herbal medicine use was "looking weak, without disease." The rate of elementary pupils who had good effects was 63.7 percent; the rate of side effects was 4.5 percent. Conclusions : The rate of elementary pupils who took herbal medicines was high and gradually higher in lower grades. So, clinicians have to cope directly with this situation and to educate parents about herbal medicines.

Priority Analysis for Infrastructure Recovery from Volcanic Disaster (사회기반시설의 화산재해 복구 우선순위 산정)

  • Recently volcanic eruptions and activities occurring in many parts of world have become a common global concern to many countries. The severity of these Volcanic disasters, such as of Mt. Eyjafjallajokull in Iceland and Mt. Merapi in Indonesia, have caused damages and causalities reaching astronomical levels. The infrastructure is categorized into 18sections that appropriately reflecting the survey data collected from various government agents, current inhabitant and engineers to accumulate a database on the priorities and preferences of restoring and reconstructing many kinds of infrastructure and facilities. The survey data was collect by using the "Likert 5 Scale Method" which emphasized the importance and priority of reconstruction and restoration for the specific facilities and infrastructures. The data was corrugated, organized and used in plotting and planning a strategic recovery agenda. The survey results were analyzed and verified to ensure the validity and reliability of the data by using chi-square test. This paper presents that recovery period and recovery cost to the total damage of infrastructure and facilities were used to make a recovery network with implemented construction management method. The research is expected that a more efficient and prompt recovery protocol and recovery plan can be executed and can be use as a reference and database.

Contingent Valuation of Wildlife-Vehicle Collision Prevention Projects (조건부가치측정법을 이용한 야생동물 교통사고 예방사업의 경제적 가치 추정)

  • With the continuous expansion of highway network and its traffics, neighboring wildlife habitats are splitted into smaller and more isolated patches. The infrastructures contribute to the wildlife-vehicle collision by creating barriers to animal movement. This kinds of traffic accidents are dangerous factors to the drivers' safety and the facilities on the highway as well as to the wildlife themselves. One of the most common ways to prevent habitat fragmentation are fauna crossings and fences. The cost of the mitigation measures to prevent wildlife-vehicle collision could be monetized. However their economic benefits are difficult to be measured. Using contingent valuation method, this study tries to estimate the economic valuation of wildlife collision prevention projects on the Korean highways. The result shows that 43.88% of Korean household had the positive willingness pay to the projects. Moreover, we found that the recognition of the project or the favourable attitude to the environmental issues could raise the willingness-to-pay. Therefore, active public relation on the project could make the friendly public opinion and increase the number of the household which has the positive willingness-to-pay on the project.

The Possibility of Regional Health Insurance Data in Blueprinting the Local Community Health Plan (지역보건의료계획 수립에 있어 지역의료보험자료의 활용가능성)

  • The health center has to play an important role in promoting community health and satisfying a variety of community health needs and demands in the decentralized Korea. The nearly enacted Community Health Act compels every health center to make its own health plans which intend to deal with local health problems and plan its future health care. This obligation is obviously a big burden to most health centers. They do not have experiences in and abilities of making local health care plans. In order to establish a systematic community health plan, health centers have to concentrate their efforts on enhancing the ability of making health care plan through gathering and analysing the local health informations. However, it is very difficult in reality. This is simply because it will take long time to accomplish these activities. It seems natural that various professionals and researchers participate in carrying out the process of making community health plan in the initial stage. No standardized methodology and analysing framework exist even in the health professional society. Nonetheless, it is common to introduce survey research methodologies in analysing consumer's health care utilization and cost, and in identifying factors influencing health behaviors. Many researchers and professionals have applied social survey methodologies in obtaining information on providers and health policy makers as well. The authors have found that few studies have ever utilized local health data stored at the self-employed medical insurance society as the data source of planning activities. The purpose of this study is to illustrate the usefulness of the data stored at the Sung-Dong Gu Self-employed Medical Insurance Society in establishing the community health plan. The major contents of this study are as follows ; 1. frequency of utilization by age, area, sex, type of medical care institutions, and some major diseases 2. Medical treatment by type of medical care institutions, by classification of 21 diseases, by frequency of three-character categories 3. Medical treatment of major neoplasm and some chronic diseases by age, sex, and area. The conclusion of this study is that it is of great potentiality to find out the local health problems and to use them in blueprinting the community health plan through comparing the frequency of medical utilization analyzed by a variety of variables with NHI health data or the health data from survey research.
