The 68.3% of them were employed in an indefinite term, 17.0% were hired in a fixed term, and 11.0% came from outsourcing. They responded as high job-stress level including inadequate compensation (71.22/100) and job demands (71.91/100). They experienced down-talk (63.4%), swearwords (32.9%), being made a dirty face (39.9%), sexual jokes (30.8%), or being likened or evaluated with their appearance sexually (14.3%). Among the causes of job related conflicts and discrimination, deprived salary level was the most frequent reason (83.4%). The conflicts and discrimination were incurred by government officers (52.4%). There were no significant differences in overall job stress, emotional labor, organizational commitment, violence, and discrimination experience based on their employment types. • Purpose: The purpose of this manuscript is to discuss the need for use of evidence based practice (EBP) in LTC, the current use of evidence in long term care facilities and what we know about adoption of the use of EBP in LTC. Methods: Literature review and reporting of findings from the M-TRAIN study that was a quasi-experimental design to test the effectiveness of an intervention to increase the use of EBPs for urinary incontinence and pain in 48 LTC facilities. Results: Barriers to adopting EBPs include lack of available time, lack of access to current research literature, limited critical appraisal skills, excessive literature to review, non-receptive organizational culture, limited resources, and limited decision-making authority of staff to implement change. Strategies to promote adoption of EBP include the commitment of management; the culture of the home; leadership; staff knowledge, time, and reward; and facility size, complexity, the extent that members are involved outside the facility, NH chain membership, and high level of private pay residents. Findings from the M-TRAIN add, stability of nurse leader and congruency between the leaders perception of their leadership and the staff's perception of the leadership. Conclusion: There is clear evidence of the need and the benefits to residents of LTC and to the health care system yet adoption of EBP continues to be slow and sporadic. There is also evidence for the process of establishing best evidence and many resources to find the available EBPs. The urgent need now is finding ways to best get the EBPs implemented in LTC. There is growing evidence about best methods to do this but continued research is needed. Clearly, residents in LTC deserve the best care possible and EBPs represent an important vehicle by which to do this.

  • The objectives of this study were (1) to describe doctors' perception and attitudes toward patient safety culture and medical error reporting in their working unit and hospitals, (2) to examine whether these perception and attitudes differ by doctors' characteristics, such as sex, position, and specialties, and (3) to understand the relationship between overall perception of patient safety in their working unit and each sub domain of patient safety culture. A survey was conducted with 135 doctors working in a university hospital in Korea. After descriptive analyses and chi-square tests of subgroup differences, a multivariate-regression of overall perception of patient safety in their unit with sub-domains of patient safety culture was conducted. Overall, a significant proportion of doctors expressed negative perception of their working units' patient safety culture, many reporting potentials for patient safety problems to occur in their unit. They also negatively viewed their hospital leadership's commitment on patient safety. Regarding the patient safety in their working unit, doctors were most worried about staffing level and observance of safety procedures. Most doctors did not know how and which medical error to report. They also perceived that medical errors would work against them personally and penalize them. About 22 percent of respondents believed that even seriously harmful medical errors were not reported.

  • In general, if employees recognize that their career are tied up, their overall level of job commitment and job satisfaction are decreased which eventually negatively affect individual performance. Especially, career development in the high specialized job like R&D task is more important than other jobs. While the previous studies showed interest in the negative consequences of career plateau, this study aimed at finding out the antecedents of career plateau. By diving the antecedents of career plateau into organizational level and individual one, we conducted direct effect and interaction effect of antecedents variables on career plateau. The empirical results using employees belonging to government funded R&D institute showed that career development practice and work experience in other institutes have negatively related to career plateau, while age has positively related to career plateau. In addition, the interaction effect between career development practice and work experience in other institute was also significant.
