Effect of Acupuncture Combined with Wooden Neck Pillow Self-exercise Program on Cervical Curvature Measurement Methods and Deep Flexor Muscle Strength (경침 자가 운동 프로그램을 병행한 침 치료가 경추 만곡도와 심부 굴곡근 근력 변화에 미치는 영향)

  • Objectives This study was designed to identify the effect of acupuncture combined with wooden neck pillow self-exercise program (WSP). Methods 25 eligible subjects with chronic neck pain were recruited from September 2015 to August 2016. Subjects were treated by acupuncture twice a week for 4 weeks, also were educated WSP and exercised every day for 4 weeks. They were assessed using PI-NRS, NDI and the maximum muscle strength of deep cervical flexor muscles (DCF) before and after treatment. Also the Cobb's angle (C1-C7), Ishihara index, Park method were assessed for evaluating the radiographical changes. Variables were compared using paired t-test. And correlates analysis was used for analyzing relationship between differentials in the maximum muscle strength of DCF and differentials in cervical spine curvature measurement methods. Results Among the 25 subjects, 4 subjects were lost to follow-up or excluded in accordance with the criteria. Significant differences on NRS, NDI and the maximum muscle strength of DCF were seen after treatment (p<0.001). In radiological finding, cervical spine curvature measurement methods were significantly increased (p<0.001, p<0.05, p<0.05). Differentials in the maximum muscle strength of DCF significantly correlates to differentials in Ishihara index (R=0.479, p<0.05). Conclusions Acupuncture combined with WSP may decrease neck pain intensity and associated disability, and also increase the muscle strength of DCF and cervical lordosis, in patients with chronic neck pain. Especially, Improving the muscle strength of DCF has a relationship with improving Ishihara index. However, due to the design of this study, this treatment can not be compared with other treatments. Future randomized project should be needed.

The Effect of Acupotomy on Lumbar Herniated Intervertebral Disc: Report of a Case Series (요추 추간판 탈출증 환자에 대한 도침을 포함한 한방 복합치료 효과: 연속증례보고)

  • Objectives : This is a case series reporting of the improvement in patients with a lumbar herniated intervertebral disc by means of acupotomy combined with oriental medicine. Methods : We treated seven patients with a lumbar herniated intervertebral disc with acupotomy combined with oriental medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and range of motion(ROM). Results : NRS and ODI decreased, and ROM improved in all cases. Conclusions : This study shows acupotomy has a meaningful effect when applied on a lumbar herniated intervertebral disc.

  • Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.

  • Background & Methods: In order to study the effect of electro-lipolysis-acupuncture, 57 outpatients who have had herbal medication in Kirin Oriental Hospital were devided to two groups; acupuncture-treated group(46) vs non-acupuncture-treated group(11) Acupuncture-treated group was given electro-lipolysis-acupuncture at least two times a week and administered with herbal diet medication, whereas non-acupuncture-treated group was administered only with herbal diet medication. Results: Mean age, body weight and BMI of these two group at the start point of the treatment were $27.22({\pm}7.64)\;vs \;29.09({\pm}8.73),\;67.76({\pm}9.34)\;vs\;67.00({\pm}10.69),\;and\;26.20({\pm}3.02)\;vs\;26.14({\pm}4.10)$ in the order of acupuncture-treated and non-acupuncture groups. After one month of treatment, change rate of body weight, BMI, total fat, percentage of fat was significantly higher in acupuncture-treated group. Also the change rate of arm circumference, hip circumference and thigh circumference was significantly higher in acupuncture-treated group. The change rate of abdominal circumference and WHR of acupuncture-treated group was higher than that of non-acupuncture-treated group with no statistical significance. The circumference reduction rate according to herbal dieting program was higher in this order, abdomen, arms, chest, thighs and hips. On the other hand, chest circumference change rate over weight loss rate, which is the portion no acupuncture treatment was given to, and AMC change rate was higher in non-acupuncture-treated group. Conclusion: These results imply that herbal dieting program combined with electro-lipolysis-acupuncture is more effective on reducing body fat, size reduction of localized obesity and preservation of muscle than herbal prescription only dieting program.

Comparative Study on the Effects of Bee Venom Pharmacopuncture According to the Treatment Method for Knee Osteoarthritis

  • Objectives: The purpose of this study is to compare the effects of bee venom pharmacopuncture (BVP) therapy according to the methods used to treat knee osteoarthritis (OA): intra-acupoint combined with intra-articular injection, intra-acupoint injection, and intra-articular injection. Methods: A total of 69 patients were recruited by the Department of Acupuncture & Moxibustion at Dong-Eui Oriental University Hospital from February 1 to July 23, 2012. The patients were assigned to 3 groups: the first group with intra-acupoint combined with intra-articular BVP Injection (the experimental group), the 2nd group with intra-acupoint BVP injection (control group II), and the 3rd group with intra-articular BVP injection (control group II). The participants were assigned in the order in which they were recruited. Treatments were done twice a week, for a total of 9 times. The effectiveness was assessed by using the visual analouge scale (VAS) and the Korea Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC). Results: All three groups exhibited significant VAS and KWOMAC effects. Moreover, the 4 week follow-up after the final treatment showed a persistence of BVP effects. However, when the groups were compared, no statistically significant differences in VAS and KWOMAC were noted, but when improvement was considered, the results showed that intra-articular injection was more effective than intra-acupoint injection. Especially, intra-acupoint combined with intra-articular injection was the most effective among the three treatments. Conclusions: Combining intra-acupoint with intra-articular injection, depending on the patient's symptoms, may produce better results when conservatively treating knee OA.

Long-term Survival of a Hepatocellular Carcinoma Patient Treated by Korean Medicine Combined with Western Therapy: a Case Report

  • Objectives: Hepatocellular carcinoma patients have a poor prognosis and survival beyond 5 years is very rare in spite of western treatment. We report a case of long-term survival of a hepatocellular carcinoma patient treated by Korean medicine combined with western therapy. Methods: The treatment was provided from November 2004 to July 2012. Herbal medicine, acupuncture and moxibustion treatment were used as main treatment methods. We prescribed herbal medicines three times a day and performed acupuncture and moxibustion an average of 2-3 times a week. Clinical laboratory tests were performed each month and a CT scan was carried out 13 times during the treatment period. In order to evaluate the disease status, we checked the patient's abdominal circumference, weight change and subjective symptoms regularly. Results: During treatment, subjective symptoms of the patient and state of ascites due to hepatocellular carcinoma were relatively stable. The progress of hepatocellular carcinoma was gradual without worsening rapidly. Since the initial diagnosis with hepatocellular carcinoma, the patient survived for a further 7 years and 9 months treated by Korean medicine combined with western treatment. Conclusions: According to the above results, this case indicates that Korean medical treatment combined with western therapy for hepatocellular carcinoma patients has synergistic effects of prolonged survival, relief of symptoms and management of complications.

Case of Adductor Spasmodic Dysphonia Patient Complaining of Voice Tremor and Hoarseness Treated with Combined Korean Medical Therapies (음성 떨림과 애성을 호소하는 내전형 연축성 발성장애 환자에 대한 복합 한의치험 1례)

  • Background: Adductor spasmodic dysphonia (ASD) is caused by the involuntary contraction of laryngeal muscles due to dystonia localized to the larynx. In the case of ASD, conventional treatment is mainly performed with a botulinum toxin injection. However, the botulinum toxin injection has a short-lasting effect and requires repeated injections. Alternatives are needed due to concerns over adverse effects, such as general weakness and airway aspiration caused by the botulinum toxin injection. Case report: A 46-year-old female patient with ASD complained of voice tremor and hoarseness. The combined Korean medical treatments-Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and transcutaneous electrical nerve stimulation (TENS)-were administered on the first day the patient was hospitalized. The Voice Handicap Index (VHI) was evaluated during the treatment. The VHI taken on the second day totaled 92 points. On the ninth day, 81 points were recorded. Total score gradually improved, and on the 16th day, 62 points were recorded. Combined Korean medical treatment lasted 19 days. Conclusion: The present case report suggests that a combined Korean medical treatment approach with Ukgan-san-gami, Jakyakgamcho-tang, acupuncture, and TENS might be effective for symptoms such as voice tremors and hoarseness. Combined Korean medical treatment can be a therapeutic option for patients with ASD.

Effect of Korean Medicine Treatment Combined with Conventional Medicine in Patients Diagnosed with Plantar Fasciitis

  • This study examined the effectiveness of Korean-Western cooperative treatment for patients with plantar fasciitis. Fifty patients received Korean medicine treatments (acupuncture, pharmacopuncture, herbal medicine) and Western medicine treatments (polydeoxyribonucleotide, and extracorporeal shock wave therapy). Evaluation methods used were comparison before and after ultrasound (P9), and numeric rating scale scores. Results revealed a significant improvement in the level of pain and evaluation of improvement using ultrasound. Moreover, it was suggested that Korean-Western cooperative medicine treatment may be effective for the treatment of plantar fasciitis.

Clinical Study of Acupucture Effect on Microtraumatic Injuries of the Knee Joint (슬관절 미세손상환자의 침치료에 관한 임상적 고찰)

  • Objectives : This study was performed to evaluate the clinical effect of acupuncture on microtraumatic injuries of the knee joint. Methods : Among the outpatients with knee joint pain who visited to Department of Acupuncture & Moxibustion, National Medical Center from February 2000 to September 2000, we selected 15 cases that showed normal finding in both X-ray and neurological examination, and that were categorized into microtraumatic injuries by the physical examination based on the Cyriax's orthopedic medicine. During acupuncture therapy, the patients were ordered to move involved side actively. The effect was assessed through questionnaires of CNRS(Cincinnati Knee Rating System). Results : We investigated 11 female and 4 male patients. The most common distribution of age was 30's(40%). The most commom duration of symtoms was from 3 to 5 months(40%). The most common microtraumatic injury was pes anserinus bursitis(33.3%), and followed by iliotibial band friction syndrome(20%), and patellar tendinitis(20%). In the CNRS, the mean number of before treatment was 60.6 and after was 66.5. 10 cases among the 15 patients were improved(66.6%). Conclusions : These results suggest that the acupuncture therapy combined with active movements of involved side was effective treatment modality on microtraumatic injuries of the knee joint.
