Purpose: Retinoblastoma (RB) is the most common primary malignant intraocular tumor in children. Although systemic chemotherapy has been the primary treatment, intra-arterial chemotherapy (IAC) represents a new treatment option. Here, we performed alternate systemic chemotherapy and IAC and retrospectively reviewed the efficacy and safety of this approach. Methods: Patients diagnosed with intraocular RB between January 2000 and December 2011 at Severance Children's Hospital, Yonsei University, were reviewed. Before February 2010, the primary treatment for RB was chemotherapy (non-IAC/CTX). Since February 2010, the primary treatment for RB has been IAC (IAC/CTX). External beam radiotherapy or high-dose chemotherapy were used as "last resort" treatments just prior to enucleation at the time of progression or recurrence during primary treatment. Enucleation-free survival (EFS) and progression-free survival were assessed. Results: We examined 19 patients (median age, 11.9 months; range, 1.4 to 75.6 months) with a sum of 25 eyes, of which, 60.0% were at advanced Reese Ellsworth (RE) stages. The enucleation rate was 33.3% at early RE stages and 81.8% at advanced RE stages (P=0.028). At 36 months, EFS was significantly higher in the IAC/CTX group than in the non-IAC/CTX group (100% vs. 40.0%, P=0.016). All 5 patients treated with IAC achieved eye preservation, although most patients were at advanced RE stages (IV-V). Conclusion: Despite the limitation of a small sample size, our work shows that an alternative combined approach using IAC and CTX may be safe and effective for eye preservation in advanced RB.
Chronic pain is a multifactorial condition with both physical and psychological symptoms, and it affects around 20% of the population in the developed world. In spite of outstanding advances in pain management over the past decades, chronic pain remains a significant problem. This article provides a mechanism- and evidence-based approach to improve the outcome for pharmacologic management of chronic pain. The usual approach to treat mild to moderate pain is to start with a nonopioid analgesic. If this is inadequate, and if there is an element of sleep deprivation, then it is reasonable to add an antidepressant with analgesic qualities. If there is a component of neuropathic pain or fibromyalgia, then a trial with one of the gabapentinoids is appropriate. If these steps are inadequate, then an opioid analgesic may be added. For moderate to severe pain, one would initiate an earlier trial of a long term opioid. Skeletal muscle relaxants and topicals may also be appropriate as single agents or in combination. Meanwhile, the steps of pharmacologic treatments for neuropathic pain include (1) certain antidepressants (tricyclic antidepressants, serotonin and norepinephrine reuptake inhibitors), calcium channel ${\alpha}2-{\delta}$ ligands (gabapentin and pregabalin) and topical lidocaine, (2) opioid analgesics and tramadol (for first-line use in selected clinical circumstances) and (3) certain other antidepressant and antiepileptic medications (topical capsaicin, mexiletine, and N-methyl-d-aspartate receptor antagonists). It is essential to have a thorough understanding about the different pain mechanisms of chronic pain and evidence-based multi-mechanistic treatment. It is also essential to increase the individualization of treatment.
Collaborative Filtering (CF) suffers from two major problems:sparsity and cold-start recommendation. This paper focuses on the cold-start problem for new customers with no purchase records and the sparsity problem for the customers with very few purchase records. For the purpose, we propose a method for the new customer recommendation by using a combined measure based on three well-used centrality measures to identify the customers who are most likely to become neighbors of the new customer. To alleviate the sparsity problem, we also propose a hybrid approach that applies our method to customers with very few purchase records and CF to the other customers with sufficient purchases. To evaluate the effectiveness of our method, we have conducted several experiments using a data set from a department store in Korea. The experiment results show that the combination of two measures makes better recommendations than not only a single measure but also the best-seller-based method and that the performance is improved when applying the hybrid approach.

Accurate facial expression recognition (FER) requires reliable signal filtering and the effective feature extraction. Considering these requirements, this paper presents a novel approach for FER which is robust to noise. The main contributions of this work are: First, to preserve texture details in facial expression images and remove image noise, we improved the anisotropic diffusion filter by adjusting the diffusion coefficient according to two factors, namely, the gray value difference between the object and the background and the gradient magnitude of object. The improved filter can effectively distinguish facial muscle deformation and facial noise in face images. Second, to further improve robustness, we propose a new feature descriptor based on a combination of the Histogram of Oriented Gradients with the Canny operator (Canny-HOG) which can represent the precise deformation of eyes, eyebrows and lips for FER. Third, Canny-HOG's block and cell sizes are adjusted to reduce feature dimensionality and make the classifier less prone to overfitting. Our method was tested on images from the JAFFE and CK databases. Experimental results in L-O-Sam-O and L-O-Sub-O modes demonstrated the effectiveness of the proposed method. Meanwhile, the recognition rate of this method is not significantly affected in the presence of Gaussian noise and salt-and-pepper noise conditions.

This paper presents an integrated approach to unconstrained face recognition in arbitrary scenes. The front end of the system comprises of a scale and pose tolerant face detector. Scale normalization is achieved through novel combination of a skin color segmentation and log-polar mapping procedure. Principal component analysis is used with the multi-view approach proposed in[10] to handle the pose variations. For a given color input image, the detector encloses a face in a complex scene within a circular boundary and indicates the position of the nose. Next, for recognition, a radial grid mapping centered on the nose yields a feature vector within the circular boundary. As the width of the color segmented region provides an estimated size for the face, the extracted feature vector is scale normalized by the estimated size. The feature vector is input to a trained neural network classifier for face identification. The system was evaluated using a database of 20 person's faces with varying scale and pose obtained on different complex backgrounds. The performance of the face recognizer was also quite good except for sensitivity to small scale face images. The integrated system achieved average recognition rates of 87% to 92%.

The secure access the lighting, Heating, ventilation, and air conditioning (HVAC), fire safety, and security control boxes of building facilities is the primary objective of future smart buildings. This paper proposes an authorized user access to the electrical, lighting, fire safety, and security control boxes in the smart building, by using color grid coded optical camera communication (OCC) with face recognition Technologies. The existing CCTV subsystem can be used as the face recognition security subsystem for the proposed approach. At the same time a smart device attached camera can used as an OCC receiver of color grid code for user access authentication data sent by the control boxes to proceed authorization. This proposed approach allows increasing an authorization control reliability and highly secured authentication on accessing building facility infrastructure. The result of color grid code sequence received by the unauthorized person and his face identification allows getting good results in security and gaining effectiveness of accessing building facility infrastructure. The proposed concept uses the encoded user access authentication information through control box monitor and the smart device application which detect and decode the color grid coded informations combinations and then send user through the smart building network to building management system for authentication verification in combination with the facial features that gives a high protection level. The proposed concept is implemented on testbed model and experiment results verified for the secured user authentication in real-time.
In the present paper routing and wavelength assignment (RWA) in optical WDM networks is considered. Previous techniques based on the combination of integer linear programming and graph coloring are complex and require extensive use of heuristics. Such methods are mostly slow and sometimes impossible to get results due to infeasibility. An alternative approach applied to RWA employs on the greedy algorithm for obtaining the maximum edge disjoint paths. Even though this approach is fast, it produces a solution for any connection request, which is very far from the optimal utilization of wavelengths. We propose a novel algorithm, which is based on the maximum flow technique to obtain the maximum quantity of edge, disjoint paths. Here we compare the offered method with previous maximum edge disjoint paths algorithms ap plied to the RWA.
Objective: The aims of this study are to investigate the trends of intellectual property in order to identify the ergonomic approaches on musculoskeletal disorders, harmful factors of musculoskeletal disorders, and to find the potential applicability of motion capture technology. Background: Ergonomic posture assessment tools often showed interrater variance, though the usage is easy and practical in industrial fields. Moreover new technologies such as motion capture showed the potential applicability in posture assessment. So ergonomists and practitioners became interested in the intellectual properties on musculoskeletal disorder and motion capture technology. Method: Intellectual properties were collected with the combination of keywords such as ergonomic, musculoskeletal disorder, and motion capture using the KIPRIS (Korea Intellectual Property Rights Information Service). Collected intellectual properties were classified into ergonomic area and non-ergonomic area, except unexamined intellectual properties. This study investigated the trend of application of intellectual properties and the probability of using motion capture technology. Results: Few intellectual properties with ergonomic approach on musculoskeletal disorders were founded, despite many products for rehabilitation and sports. One hundred twenty five patents in 1105 patents on musculoskeletal disorders and 138 patents in 1908 patents on motion capture technology were classified into the patents that ergonomic approach can be applied. The patents related to ergonomics area are rapidly increasing after 2010, and there are good opportunities for ergonomists to apply the patents. Conclusion: This study found opportunities on novel methodology in detecting the harmful factors of musculoskeletal disorders, and that the motion capture technology is applicable in ergonomic posture assessment. Application: The results of this study can help ergonomists prepare the ergonomic patents, and can show the potential use of motion capture technology in detecting the harmful posture of musculoskeletal disorders.
The simultaneous occurrence of a gastrointestinal stromal tumor (GIST) and a gastric adenocarcinoma is uncommon, and has rarely been reported in the literature. The present report describes the case of a 74-year-old male patient who initially presented with an adenocarcinoma that had invaded the antral mucosa. Computed tomography then revealed the presence of a suspected GIST, in the form of a $2{\times}2$ cm mass at the hilum of the spleen. In view of the advanced age of the patient, a surgical approach that would minimize risk and maximize quality of life was preferred. The patient therefore underwent simultaneous laparoscopy-assisted distal gastrectomy for the adenocarcinoma and wedge resection for the GIST. This approach was only chosen after confirming that it would be possible to preserve three or more of the short gastric arteries that supply the area below the wedge resection site. This may be considered a feasible apapproach to the management of the simultaneous occurrence of a mid-to-low gastric body adenocarcinoma and a high gastric body GIST.

The effect of multiple process parameters on a set of continuous response variables is, especially in experimental designs, difficult and intricate to determine. Due to the complexity in aeroacoustic and vibroacoustic studies, the often-performed simple one-factor-at-a-time method turns out to be the least effective approach. In contrast, the statistical Design of Experiments is a technique used with the objective to maximize the obtained information while keeping the experimental effort at a minimum. The presented work aims at giving insights on Design of Experiments applied to aeroacoustic and vibroacoustic problems while comparing different experimental designs and approximation models. For this purpose, an experimental rig of a ducted low-pressure fan is developed that allows gathering data of both, aerodynamic and aeroacoustic nature while analysing three independent process parameters. The experimental designs used to sample the design space are a Central Composite design and a Box-Behnken design, both used to model a response surface regression, and Latin Hypercube sampling to model an Artificial Neural network. The results indicate that Latin Hypercube sampling extracts information that is more diverse and, in combination with an Artificial Neural network, outperforms the quadratic response surface regressions. It is shown that the Latin Hypercube sampling, initially developed for computer-aided experiments, can also be used as an experimental design. To further increase the benefit of the presented approach, spectral information of every experimental test point is extracted and Artificial Neural networks are chosen for modelling the spectral information since they show to be the most universal approximators.
