Percutaneous Thrombin Injection Based on Computational Fluid Dynamics of Femoral Artery Pseudoaneurysms

  • Objective: To analyze the computational fluid dynamics (CFD) of femoral artery pseudoaneurysm (FAP), identify a suitable location and timing for percutaneous thrombin injection (PTI) based on this analysis, and report our clinical experience with the procedure. Materials and Methods: CFD can be used to analyze the hemodynamics of the human body. An analysis using CFD recommended that the suitable location of the needle tip for PTI is at the center of the aneurysm sac and the optimal timing for starting PTI is during the early inflow phase of blood into the sac. Since 2011, seven patients (three male and four female; median age, 60 years [range, 43-75 years]) with FAP were treated with PTI based on the devised suitable location and time. Prior to the procedure, color Doppler ultrasonography was performed to determine the location and timing of the thrombin injection. Results: The technical success rate of the PTI was 100%. The amount of thrombin used for the procedure ranged from 200 IU to 1000 IU (median, 500 IU). None of the patients experienced any symptoms or signs of embolic complications during the procedure. Follow-up CT images did not reveal any embolism in the lower extremities and showed complete thrombosis of the pseudoaneurysm. Conclusion: Based on our study of CFD, PTI administered centrally in the FAP during early inflow, as seen on color Doppler, can be an effective technique.

Miniature Schunauzer Dog에서 발생한 심방중격 결손 증례 (A Case of Atrial Septal Defect (ASD) in a Miniature Schunauzer Dog)

  • An atrial septal defect (ASD) is congenital heart disease with a communication between the atria, which allows blood to shoot from the atrium with pressure. A 3-month-old female Miniature Schunauzer was referred to the Veterinary Teaching Hospital of Konkuk University for the evaluation of systolic heart murmur. At presentation, the mucous membrane was cyanotic. On physical examination, an ejection-type systolic murmur was auscultated at the pulmonic area. In addition, thoracic radiography showed enlargement of main pulmonary artery and right atrial/ventricular enlargement. Echocardiography revealed dilated right atrium and atrial septal defect. However, mitral and tricuspid valve were still intact and well tolerating. The presence of an ASD was confirmed by identifying flow across the defect with color Doppler imaging. Doppler echocardiography provides a means of non-invasive documentation and quantification of ASD. Complete blood count and serum chemistry were not remarkable. Although large defect was confirmed between the two atrium, the patient did not show any obvious clinical signs of heart failure at this time.

Echocardiographic Diagnosis of Subaortic Stenosis with Severe Deformation of Mitral Valve Apparatus in a Dog

  • A 1-year-old castrated male Schnauzer dog was presented with heart murmur. Auscultation revealed systolic murmur located at the left heart base (grade 5/6). There were no remarkable findings on thoracic radiographs. Two-dimensional echocardiography revealed subaortic tunnel-like obstruction at the entrance to the left ventricular outflow tract. Anterior mitral valve leaflet appeared to be tethered to septum with minimal motion. Chordae tendineae was abnormally thickened. Color Doppler analysis revealed turbulent flow starting below the aortic valve. Mitral regurgitation was presented during systole. Spectral Doppler recordings revealed high velocity flow through the aorta and mitral regurgitation. Based on echocardiographic examination, the dog was diagnosed with subaortic stenosis concurrent with mitral dysplasia. The patient was medicated with ${\beta}-blocker$ and diuretics. It has been doing well without apparent clinical signs at 2 year after the diagnosis.

IHb 색상 정보를 이용한 색조 도플러 초음파 영상에서 상완 동맥의 고혈압 혈류 속도 분석 (Velocities Analysis of Hypertension Blood Flow of Brachial Artery on Color Doppler Ultrasonography using IHb Color Information)

  • 본 논문에서는 이러한 문제점을 개선하기 위해 상완 동맥 영역에 대한 RGB 채널을 HSV 채널로 변환한다. 변환된 HSV 채널에 대해 고혈압 영역의 특징을 강조하게 하기 위해 밝기 값을 나타내는 V값을 조절한다. 조절된 HSV 채널을 다시 RGB 채널로 변환한 후, Fuzzy C_Means 기반 무게중심과 Possibilistc C_Means 기반 무게 중심을 기반으로 새로운 무게 중심을 구하여 픽셀들을 클러스터링하여 상완동맥 영역의 고혈압 영역을 추출한다. 추출된 상완 동맥의 고혈압 영역에 대해 헤모글로빈 색소 정보를 나타내는 IHb 값을 이용하여 상완 동맥의 고혈압 영역에서 유사한 헤모글로빈 색소 정보를 가지는 영역을 분할한다. 분할된 영역들을 혈류의 속도를 나타내는 색상표와 대조하여 고혈압의 진행에 대해 분석하는 방법을 제안한다. 제안된 방법을 색조 도플러 초음파 영상을 대상으로 실험한 결과, 제안된 방법이 고혈압의 진행에 대한 분석 결과와 색조 도플러 초음파 영상 장비에 나타난 고혈압 진행 결과와 거의 일치하는 것을 확인할 수 있었다.

박리전단층이 축대칭 하향단흐름에 미치는 영향 (Effect Of The Separating Shear Layer on the Flow Over an Axisymmetric Backward-Facing Step)

  • An experimental study on the flow over the axisymmetric backward-facing step was carried out. The purpose of the present study is to investigate the effect of the boundary layer thickness at the separation point on the reattachment length and to understand the structure of the recirculating flows. Local mean and fluctuating velocity components were measured in the separating and reattaching axisymmetric turbulent boundary layer over the wall of convex cylinder placed in a water tunnel by using 2-color 4-beam fiber optics laser Doppler velocimetry. The study demonstrated that the reattachment length increases with increasing boundary layer thickness. It was also observed that the reverse flow velocity and turbulent kinetic energy decrease with an increase in the momentum thickness at the separation point. The measured velocity field suggests that the boundary layer thickness at the separation can affect definitely on the formation of corner eddy.

  • Acute idiopathic scrotal edema (AISE) is a self-limiting condition that is characterized by acute scrotal swelling and erythema. AISE is a very rare cause of acute scrotum, especially in neonates. We report a case of AISE in a 26-day-old infant who was admitted to the outpatient clinic with swelling and erythema of the penis and scrotum for a week. His vital signs were stable, and laboratory findings were non-specific. A diagnosis of AISE was made using scrotal ultrasonography with color Doppler. His symptoms resolved within four days after the onset of supportive treatment, and he was discharged from the hospital. In neonates with an acute scrotum, AISE should be considered to prevent unnecessary surgical exploration.

Application of Light Collecting Probe with High Spatial Resolution to Spark-Ignited Spherical Spray Flames

  • A light collecting probe named Multi-color Integrated Cassegrain Receiving Optics (MICRO) is applied to spark-ignited spherical spray flames to obtain the flame propagation speed in freely falling droplet suspension produced by an ultrasonic atomizer. Two MICRO probes are used to monitor time-series signals of OH chemiluminescence from two different locations in the flame. By detecting the arrival time difference of the propagating flame front, the flame propagation speed is calculated with a two-point delay-time method. In addition, time-series images of OH chemiluminescence are simultaneously obtained by a high-speed digital CCD camera to ensure the validity of the two-point delay-time method by the MICRO system. Furthermore, the relationship between the spray properties measured by phase Doppler anemometer (PDA) and the flame propagation speed are discussed with three different experimental conditions by changing the fuel injection rate. It was confirmed that the two-point delay-time method with two MICRO probes is useful and convenient to obtain the flame propagation speed and that the flame propagation speed depends on the spray properties.

상피성 난소암의 선별 검사 (Screening Tests for Epithelial Cancer of the Ovary)

  • 이상에서 고찰하였듯이 현재까지는 어느 하나로 결정할 만한 선별 검사 방법이 없지만, 그 중에서 경질 초음파 검사와 color-flow Doppler 초음파 검사가 시행하기가 쉬우면서 민감하고 비교적 특이도가 높은 방법이라고 할 수 있다. 그러나 역학적인 면에서 조기 진단 혹은 선별 검사의 효율은 검사의 시행과 그에 따른 처치에 의해 사망률이 실제로 감소되었을 때 유의하다고 할 수 있으며, 이런 면에서 아직까지 난소암으로 인한 사망률을 감소시킬 만한 결정적인 선별 검사 방법은 알려져 있지 않다. 사망률이 1/3 감소되었음을 확인하는데 100,000명의 선별 검사자와 100,000명의 대조군이 필요하므로 앞으로 보다 많은 인구를 대상으로 한 역학적인 연구가 필요하다. 앞으로의 선별 검사에는 보다 특이도가 높은 종양 표지 물질의 개발, 초음파를 비롯한 진단 기기의 혁신적인 발달이 필요하며 이는 현재까지의 발전 상황으로 보아 실제로 가능할 것으로 생각된다. 이와 더불어 돌연변이를 일으킨 난소 상피 세포가 수 차례의 분열만 일으키더라도 그 유전자 산물을 검색해 낼 수 있고, 나아가서는 DNA 진단까지 가능한 분자 생물학적 혹은 세포 유전학전 진단 방법의 개발과 이용도 기대된다.

  • A numerical technique is employed to simulate the flow patterns in the human carotid artery and a phantom of the carotid artery made of acrylic material is used to observe the flow phenomena in the carotid artery. For numerical analysis the idealized geometric shape of the carotid artery is constructed to portray the phantom. Steady momentum equation is solved by the finite element method and the numerical results are compared with the results of MRA and color Doppler images.

Siberian Husky 견에서 팔로사징 진단 증례 (A Case of Tetralogy of Fallot in a Siberian Husky Dog)

  • A 7-month-old 8.2 kg female Siberian Husky with history of exercise intolerance, delayed growth, and tachypnea was presented to Veterinary Medical Teaching Hospital, Kyungpook National University. A holosystolic murmur was identified in cardiac ausculation. In electrocardiography, thoracic radiographs, and ultrasonography, a right axis deviation, enlarged right ventricle, stenotic main pulmonary artery, large ventricular septal defect, marked right ventricle hypertrophy, and overriding aorta with dilation were identified. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect and the blood flow streams converging from the right and left ventricles into the aorta. The dog was diagnosed as tetralogy of Fallot. The current state of the dog is well-tolerated without any specific medication.
